**Reframing Sex Post-Prostatectomy**

I’m not going to sugar coat it: things are different than they were, but your capacity for pleasure is limitless! Connect with your pleasure rather than the performance. While using something new to help you get erect may seem unfamiliar, getting comfortable with it is only a temporary hurdle. Once you’re comfortable, you’re free to keep swinging.

**Strategies For Getting Hard**

At this phase, you want to focus on blood flow. Tools like the penis pump, medications like Cialis or Viagra, and techniques like physical touch can help you get and maintain an erection. But remember: it’s not just starting in body, you also need to get your brain involved and retrain your neural pathways with mental arousal.

**Your Concerns Are Interconnected**

Recovery is a package deal. There’s research that supports using a penis pump can improve urinary incontinence. Focusing on all things that have changed will bring your recovery faster and farther.

**Intake and Output**

Start by taking note of how much water you currently drink a day: the goal you should aim for is ~half your body weight in ounces of water. Also note how often you go to the bathroom: the goal you should aim for is creating a bladder schedule where you pee every 2-3 hours. If you’re nowhere close to that 2-3 hours, don’t panic and don’t force yourself to hold it. A pelvic floor physical therapist can plan out a treatment path to get you where you’re at to where you want to be.

**Pelvic Floor Contractions**

Pelvic floor contractions are a coordinated effort with key three areas: simultaneously pulling in and lifting back of the pelvic floor, lifting the testicles upwards, and lifting the rectum towards your belly button. If you’re struggling with these movements, you’re not alone. Many men feel more confident with contraction exercises after working with a physical therapist.

Dr. Rachel Worman is a licensed physical therapist (PT) who has experience in acute care physical therapy and extensive dual training, clinical and teaching experience in musculoskeletal and pelvic health physical therapy. After 13 years of clinical practice, Dr. Rachel earned her PhD with a focus on pelvic health outcome measures and male pelvic health.

Pelvic floor physical therapists and patients have to be mindful of the influence that media (including social media and speakers at conferences) has on trending treatment ideas and how common it is for a 10-second video on social media to hijack a patient’s treatment plan, simply because it wasn’t a good fit.

There’s no one-size-fits-all treatment strategy for pelvic pain, which is why there many treatment strategies. Through critical evaluation, clinicians must determine the most appropriate and effective treatment strategy, or combination of strategies, for that single patient. Dr. Rachel shares how important it is for clinicians to be flexible, individualized, and science-based, and to avoid getting caught up in clinical habits of using one hallmark treatment for all patients.

In such a difficult field, where all the measures we have for tone are problematic, these  generalizations and trending treatment ideas, while good for marketing, ultimately delays effective treatment and harms patients. It’s necessary to prioritize the knowledge of evidence-based practices and personalized care to ensure optimal outcomes for patients.
