84. Part-Time Work Options for Doctors Approaching Retirement - John Jurica MD30 Apr 202400:23:14

Follow our show wherever you get your podcasts: https://link.chtbl.com/JoH0AMKR

Interested in exploring non-clinical careers and unique side gigs in medicine?

During the peri-retirement period, doctors may find themselves contemplating retirement or looking for ways to reduce their clinical workload. Engaging in part-time or side gigs can provide a gradual off-ramp from full-time clinical practice, allowing individuals to adjust to a new lifestyle without the sudden loss of income.

Dr. John Jurica, a Medical Director, blogger, podcaster, and Certified Physician Executive with expertise in non-clinical careers and side gigs, described various part-time and side gig opportunities physicians can explore during this time. These options range from remote work in fields like medical writing, chart reviews, and medical device regulatory consulting to more hands-on roles like medical legal pre-litigation consulting. These opportunities not only offer financial stability but also allow individuals to continue utilizing their medical expertise in different capacities.

“Even people, as they approach retirement, some of them have been half burned out or mostly burned out and just haven't really done anything about it. To me, we have a bunch of people that just leave medicine and do things, but there's a lot of jobs that they go to, at least a subset of those, that they're part-time. They can be done part-time at the beginning and become full-time. I think there's an option to just do the in-between and try something and see if you like it.”

- John Jurica MD

Episode highlights:

  • Challenges of transitioning from active practice
  • Rise of virtual platforms for part-time work
  • Social security disability chart reviews
  • Collaborative roles with non-physician providers
  • Medical device regulatory consulting
  • Medical-legal pre-litigation consulting
  • Legacy and life purpose post-retirement

Follow our show wherever you get your podcasts: https://link.chtbl.com/JoH0AMKR

Are you ready to break free from the cycle of people-pleasing and perfectionism in your medical career?

Setting boundaries and prioritizing self-care are essential for physician wellness, as discussed in this episode featuring Megan Melo, MD. She is a dual board-certified Family and Obesity Medicine Physician, a Certified Life Coach for Physicians, and a Certified Daring Way™ Facilitator.

Dr. Melo highlighted the societal pressure on women in the medical field to always prioritize others and never say no. This pressure stems from early socialization as "good girls" who are expected to be accommodating, hardworking, and always pleasing others. This ingrained behavior can lead to burnout and dissatisfaction in one's career as individuals struggle to meet unrealistic expectations and constantly prioritize others over themselves.

Tune in for a candid discussion on the realities of healthcare, the importance of setting healthy boundaries, and the need for individual action to prioritize wellness in a challenging industry.

"You're going to need to decide where are you going to have boundaries because you can't do it all. You can't show up this way. You can't always say yes and never say no. You will drown in resentment and frustration and overwhelm and burnout. It's just not possible to do all of these things at this high level and, succeed, or be happy." - Megan Melo MD

Episode highlights:

  • Gender and generational differences in healthcare

  • The impact of leadership and vulnerability on physician well-being

  • The good girl syndrome in medicine

82. Moral Injury vs. Physician Burnout with Jennie Byrne MD06 Mar 202400:29:23

Follow our show wherever you get your podcasts: https://link.chtbl.com/JoH0AMKR

Are moral injury and burnout the same?

Dr. Byrne and I delve into the nuances of moral injury and burnout, exploring whether they are the same or different concepts. We share stories, insights, and strategies to help healthcare professionals recognize and prevent burnout and understand the impact of moral injury in the workplace.

Dr. Jennie Byrne, a psychiatrist, neurophysiologist, and author of the book “Work Smart: Use Your Brain and Behavior to Master the Future of Work,"  brings a unique perspective to the conversation, emphasizing the importance of addressing moral injury as a wound that requires healing.

On the other hand, I shared my perspective that burnout is an energy balance challenge, where a physician's physical, emotional, and spiritual energy accounts are depleted, yet they continue to push through.

Our debate sheds light on the complexities of these issues and offers valuable insights for healthcare professionals navigating these challenges.

74. Stop Physician Suicide Part 2: Reaching Out to a Colleague in Distress12 Sep 202300:35:12

Part 3 of this Series "Coaching a Suicidal Physician Client" is AT THIS LINK

Behind the white coats and stethoscopes, there lies a profound truth - doctors, too, are human.

Joining me again are I.C.F.-Certified coaches on our team at TheHappyMD.com - Pam Pappas MD and Penelope Hsu MD, to continue the three-part series on recognizing and preventing physician suicide. In part one, Dr. Pappas and Dr. Hsu bravely shared their own experiences with suicidal ideation. In part two, we discuss what to do if you suspect a colleague is having suicidal thoughts and how to reach out to express our concern. We also explore the barriers to seeking help including imposter syndrome, shame, and guilt.

While we may not be able to prevent every instance of physician suicide because of their never-show-weakness programming, we have the power to make a difference through our compassion. Whether we're colleagues, friends, family, or just part of a tribe, we can reach out and give support to someone who might be in a tough spot. Our job is to look out for each other and make sure we have each other's backs even if they tell you to go away and leave them alone. You’ll never know when a simple yet genuine gesture of empathy can save someone’s life.

“If you do reach out, you're not going to get rebuffed. If you can hold on somehow through all that darkness, the love that you would give is right there for you. They're going to be there. You just have to say something.” - Penelope Hsu MD

“An antidote to shame is the self-compassion. If we're the safe place, then people feel safe to tell their story and somehow the shame starts to ebb away.” - Pam Pappas MD

73. Christopher Habig and the Future of the US Direct Primary Care (DPC) Industry05 Sep 202300:28:09

Follow our show wherever you get your podcasts: https://link.chtbl.com/JoH0AMKR

The son of two physicians, Christopher Habig has seen first-hand the impact they made on their community. However, as he shadowed doctors in hospital systems, he noticed their dissatisfaction. This led him to trailblaze the Direct Primary Care (DPC) industry. Since 2016, his companies have launched and supported 130 DPC practices across 37 states in the US. After launch, clients have the option of becoming an employee, owning their own business with full assistance, or having an entirely independent practice.

Chris and I discuss the Concierge/DPC business model. He explains how DPC allows physicians to make twice as much money while seeing fewer patients, leading to a better work-life balance. He also gives the benefits of DPC for rural healthcare and the potential for specialists to adopt this model. Christopher stresses the importance of providing options and enabling physicians to practice medicine as they were trained.

As more physicians embrace DPC, the future of medicine looks promising, with improved patient care and a return to the values that inspired many doctors to enter the field in the first place.

“Just making physicians their own boss, that is the best way that we have seen time and time again to help solve burnout.” - Christopher Habig

Episode Highlights:

  • The inspiration behind DPC

  • DPC 101: Make more money, see fewer patients

  • Platform to provide support and guidance to start a successful practice

  • A practice model for rural healthcare and the future of medicine in the USA

Many people are taught to believe ambition is a selfish, greedy, or overly competitive drive that nice people don’t utilize or admit to.

Ambition is the driving force to strive for success and accomplishment. It pushes us forward beyond our limits to reach new heights. In the world of medicine, ambition is essential for doctors to survive the rigorous training process and thrive in their careers. However, there is a lot of misunderstanding about ambition and its negative associations.

Together with Harjot Singh MD, we will look at what is ambition and how it can be a positive motivator. Dr. Singh is a child and adolescent psychiatrist, keynote speaker, and a physician leadership and communication coach here at TheHappyMD.com

In this conversation, we’ll discuss ambition as a positive force when used to build your ideal practice or work for the greater good.

"How fired up are you? That's a very key question I always ask people who come do the first session with me. How fired up are your aspirations for your goals?" - Harjot Singh MD

Episode Highlights:

  • The concept of ambition and the fields of study

  • Ambition and patient-centered care

70. Preventing Physician Suicide Part 1: Recognizing You Are At Risk - The First-Person Experience of Suicidal Ideation15 Aug 202300:35:07

PART TWO of this series "Reaching out to a Suicidal Colleague" is at THIS LINK

We kick off a seriously important three-part series that tackles something we all need to talk about more - preventing physician suicide. This week, I'm sitting down with two remarkable guests and I.C.F.-certified coaches on our team at TheHappyMD.com, Dr. Penelope Hsu and Dr. Pam Pappas.

We discuss the often-ignored topic of recognizing your own suicidal ideations and the immense implications it can bring along. Both Dr. Hsu and Dr. Pappas hold nothing back as they share their personal experiences with suicidal ideation and burnout. Their stories remind us why it's crucial for wellness leaders to show vulnerability, and to use their platforms as stages to empower others in their struggles.

"I think It is far more common than we think. Isolation is the thing that keeps us from acknowledging it within ourselves, of getting support, of sharing, of leaning on anyone. My hope would be that people can hear our stories and realize it doesn't mean you're weak. It doesn't mean you're broken. It just means you're human." - Penelope Hsu MD"Things are going on inside us all the time that may look very different on the surface and how other people see us. We know that physicians are extremely well-versed in trying to look more together than they actually feel. And the code of silence around depression and suicidality is extremely strong in our culture at large, but I think even more so in physicians because there are a lot of implications." - Pam Pappas MD

69. Full Risk and the Transformation of Primary Care - The ChenMed Experience with Faisel Syed MD and Daniel McCarter MD25 Jul 202301:02:52

Follow our show wherever you get your podcasts: https://link.chtbl.com/JoH0AMKR

In the dynamic landscape of healthcare, the payment method is a primary driver of both the patient and the physician experience.

This episode takes us on a journey that explores the profound difference when operating within a payment system that prioritizes comprehensive care over transactional interactions.

Join Dike Drummond MD and two leaders in the ChenMed system, Dr. Faisel Syed, National Director of Primary Care, and Dr. Daniel McCarter, National Director of Primary Care Advancement, to know more about the transformation of your practice that can occur when you are in a full-risk, single-payor setting.

Fee-for-service model is where physicians are caught in a hamster wheel, constantly focused on addressing the patient's chief complaint within a limited time. The assembly-line nature of this system with multiple payors and insurers creates a production-driven, disease-care grind naturally and automatically and has transformed the physician-patient relationship into a transaction.

In contrast, ChenMed operates under a full-risk model. This allows physicians to focus on comprehensive care for medically complex and underserved populations. They prioritize long-term outcomes and patient well-being by taking full responsibility for the patient's total cost of care. This unlimited access ensures that physicians have the time and opportunity to develop meaningful relationships with their patients.

Listen for a detailed compare and contrast of FFS and Full Risk as it applies to both the patient and the physician experience.

Follow our show wherever you get your podcasts: https://link.chtbl.com/JoH0AMKR

Burnout among doctors is a growing concern in healthcare systems around the world. In the United States and the United Kingdom, the number of physicians suffering from burnout is on the rise.

In this cross-continental conversation, Dr. Rachel Morris shares her perspective on this global issue and her experiences of burnout. Dr. Morris is a former GP, keynote speaker, a specialist in resilience in the workplace, executive and team coach, and host of You Are Not a Frog Podcast.

From the unrealistic expectations placed on GPs in the UK to the barriers created by the multi-payer system in the US, we uncover the many issues plaguing the healthcare industry. But it's not all doom and gloom. We also share practical solutions for overcoming burnout and finding balance in medical practice.

"I always think it's a bit like a big sponge. Because, as healthcare professionals, you just absorb the extra work over the years. But now, our sponge is just completely sodden. We can't absorb anything more and then it overspills." - Dr. Rachel Morris

Here's a sneak peek at some of the key topics we cover in this episode:

  • The pressures of healthcare work - The canary and coal mine analogy, symbolizing healthcare workers in high-stress environments.

  • US vs UK challenges in healthcare delivery systems - tackling the issues of staffing crisis, multi-payer system, and political system

  • Overcoming burnout and building support networks - fostering collaboration within the tribe of lightworkers

Follow our show wherever you get your podcasts: https://link.chtbl.com/JoH0AMKR

Have you ever questioned your own sanity or judgment in a healthcare setting? Do you constantly feel self-doubt?

Today, we discuss gaslighting, a form of psychological manipulation that can leave you doubting your self-worth and reality, particularly in healthcare.

Our guest, Dr. Tracy O’Connell, is a radiologist, coach, educator, Certified Expressive Writing™ Facilitator, and Certified Daring Way™ Facilitator. Together with Dr. O'Connell, we examine the power dynamics and moral injury that can lead to gaslighting in healthcare relationships, and how it is often used as a form of covert domination. We also talk about the tactics used by psychopaths to gain the upper hand in conversations and share practical advice on how to address and overcome gaslighting in medical training. Learn to build social and emotional intelligence, trust, and psychological safety to establish boundaries and walk away when you feel gaslighted.

“Gaslighting is a dance that requires two people or two entities. It requires the gaslighter, who needs to be right at all costs. And it requires a gaslightee, somebody who absolutely needs the approval of the gaslighter in order to have the dance.” - Tracy O’Connell MD

What we talked about:

  • Definition and the three stages of gaslighting

  • Psychopaths and their domination techniques

  • Power dynamics and moral injury in medical culture

  • Overcoming the inner critic and building self-worth

62. Teresa Cardador PhD - Status Leveling Gender Bias for Women Physicians23 May 202300:34:28

Women in male-dominated professions still face unique challenges in today's society. One such profession is Medicine, where female physicians often struggle to navigate the complex gender dynamics and hierarchies in the workplace.

In this fascinating episode, I had the pleasure of speaking with Dr. Teresa Cardador, an  Associate Professor of Labor and Employment Relations and Center for Social and Behavioral Science Affiliate at the University of Illinois, Urbana-Champaign. She is also an award-winning educator focusing on human resource management, motivation, performance, employee relationships, and negotiation.

We discussed her groundbreaking research on the status-leveling burden experienced by women physicians and its physical, emotional, and psychological toll. Additionally, we delve into Dr. Cardador's follow-up research, revealing the nurses' perspective on the interactions between female nurses and female physicians.

By understanding these issues, we can work towards creating more inclusive work environments that empower and support women in medicine. So, join us and be a part of the change!

“This is why we use the term burden. There's certainly a lot of evidence that this is associated with a lot of physical demands on women and a lot of psychological demands that make their work more challenging in a context that is already extremely challenging.” - Teresa Cardador PhD

Episode highlights:

  • Naming the phenomenon: What is status-leveling burden for women physicians?

  • Case study setup: An interview with 29 female and 16 male surgeons

  • The institutional responses and the recommendations for organizations

  • Nurses' perspectives and status hierarchy in Medicine

60. Elizabeth Hughes MD: Doctors Emotionally Abusive Relationship With Our Careers09 May 202300:29:48

Are you ready to redefine your relationship with your career?

In this insightful episode, I had the pleasure of speaking with Dr. Elizabeth Hughes, a dermatologist, coach, speaker, and creator of her own skincare line. We delved into the fascinating topic of physicians' relationship with their career, specifically exploring the parallels between our professional lives (Medicine, the job) and dysfunctional romantic relationships often seen in movies. Dr. Hughes reveals her personal journey of how she transformed her life and career to create meaningful change. She also shares her wisdom on recognizing when it's time to make a change, stepping out of the whirlwind, and setting boundaries to protect yourself from toxic relationships.

Staying in that bad relationship was not doing the right thing for my children. And so for medicine, it was staying in medicine as I was practicing was actually not doing the right thing for my patients. It was time for me to make a change. That became my driving force.” - Dr. Elizabeth Hughes

“So that's why burnout is inevitable for an employee physician. There will come a time where you say, I can't take it anymore. And that's when burnout puts you on a path with more purpose. Just like the day you realize, I can't go on in this relationship anymore." - Dr. Dike Drummond

Episode highlights:

  • Realizing the power of choice and the possibility of making a difference

  • Five signs you’re in a toxic personal life & career relationship:

  1. An abuser never admits to being wrong.

  2. The abuser never actually trusts you.

  3. An abuser does whatever he wants and expects you to roll with the punches.

  4. An abuser expects you the the victim to support all of his freeloader friends.

  5. Abusers make their victims feel wholly dependent on them.

  • Navigating job descriptions and burnout

  • The various levels of detachment from one's professional life

54. The Origins of Your Practice Overwhelm - the Drivers of the Whirlwind15 Feb 202300:10:17

Choosing to become a doctor is an honorable decision that can bring meaning, fulfillment, and joy to your life, but the reality of the profession can be overwhelming. For many physicians, burnout has become a common struggle. It can be difficult to stay motivated when you’re feeling overburdened by the demands of the organization’s business goals. You end up trying to fit your job description into someone else's.

As time passes, it gets exhausting until you finally say, "I cannot bear it anymore." As a result, some doctors find themselves deciding whether they should continue their career as a physician or pursue another path. But for those who choose to stay in medicine, it’s important to remember that you are a lightworker. You chose this career because of the purpose it provides. Don't lose sight of that!

“There's a difference between a job and a practice. Practice is what you want to do. Job is how it fits into your employer's business model.” - Dike Drummond MD

In this episode, we talk about:

  • The three classical symptoms of burnout

  • Components of the whirlwind

  • Drivers of your job description and the revenue model

  • Reclaiming control over your practice

51. What To Do When a Patient Says Thank You21 Dec 202200:11:19

Thank yous are opportunities to tap into spiritual energy. Among the three energy accounts in a physician, this is the most powerful, and when it is filled, the physical and emotional accounts are filled as well.

In order to do this, the physician must take a moment to be mindful of the patient's words, acknowledge their gratitude, and appreciate the connection between them. This will help the physician to really feel the spiritual energy infusion and embrace it, allowing them to take this energy and use it to improve their practice.

In this episode, Dr. Dike teaches us a technique on how to simultaneously fill up a physician’s three energy accounts, something simple yet powerful. He also shares tips on impacting positive influence among team members just with the words ‘thank you’.

Reflecting on the patient's gratefulness can be used to create a more encouraging and supportive atmosphere in the medical office that promotes healthier physician well-being and ultimately can help to improve the quality of care.

“It's the thank you that makes it feel like you made the difference, that you set out to make when you first chose to be a doctor back at the light worker's fork in the road.” - Dike Drummond MD

50. Physician's 3 Energy Accounts14 Dec 202200:08:56

Why is it crucial to recognize how burnout affects all aspects of a physician's life?

Energy management is not just about getting enough sleep or taking breaks when you're tired. As a doctor, it's also about figuring out how to balance your physical, emotional, and spiritual needs so that you can be your best self at work and in life.

Recap: In part 1 of this mini-training series, we discussed the common physician burnout myths and revisited healthcare’s two prime directives. Also, using the bank account model, we explained the concept of energy inside the physiology of a doctor and how they are trained to continue to perform their duties even below the empty mark.

This episode is a continuation that dives deeper into the three energy accounts with their corresponding burnout symptom that occurs when each tank is in a negative balance and the strategies for refilling these tanks. Taking the time to understand this model can help physicians take action to prevent burnout and safeguard their own well-being.

Episode Highlights:

  • (0:36) Intro

  • (2:55) Physical energy account; burnout symptom: exhaustion

  • (3:56) Emotional energy account; burnout symptom: cynical and sarcastic

  • (5:46) Spiritual energy account; burnout symptom: what’s the use

49. Burnout Is Not About Batteries Run Down07 Dec 202200:10:39

Physicians, it's time to address the common misperceptions about burnout and the false ideas that it’s simply a matter of batteries being run down.

In this episode, we'll talk about the common myths and provide a model of the way energy works in a doctor that is accurate. Instead of thinking of your energy as a battery, think of it as an energetic bank account so that you can feel empowered to tackle your work challenges head-on.

In this series of mini trainings, Dr. Drummond will outline exactly how to approach burnout differently than trying to solve it and provide detailed solutions for maintaining a positive energy balance, so be sure to tune in to the next two episodes.

48. Nisha Mehta MD - Healthy Boundaries and Saying "NO" for Physicians16 Nov 202200:40:41

The impact of physician burnout on the US healthcare system is staggering. According to a recent study, 45% of American doctors are over the age of 55, and 40% of female physicians will reduce their clinical activities or retire completely within their first six years of practice.

47. Jane Bridges MD - “Docs and Cops” Gun Safety Training for Physicians09 Nov 202200:26:25

A key component to preventing burnout in healthcare professionals is to engage in fun educational social events. It provides an excellent opportunity for older and newer physicians to interact, as well as encourage residents to stay in medicine after their residency.

With her experience in physician wellness and social planning, Dr. Jane Bridges, a diabetologist at Good Samaritan, in Vincennes, Indiana, came up with a program called “Docs and Cops”. This is a yearly event that provides training on gun education, gun safety at home and in the workplace, and wound care techniques. Both doctors and police officers eagerly await this gathering, which gives them the opportunity to be out in nature, enjoy shooting, and most importantly, be informed and prepared because the number of gun violence cases is on the rise.

As a gun enthusiast herself, Dr. Bridges wants to give physicians and the public a different perspective on guns, not only as a means of self-defense but also as a form of sport.

“It’s just great being out in nature and shooting guns and doing something in an organized, controlled way, an educational way. The gun isn’t bad. It’s whoever is having the gun.” - Jane Bridges MD

Episode Highlights:

  • (02:15) How the program "Docs and Cops" was conceived

  • (05:33) Gun education and gun safety at home and in the workplace

  • (08:18) Nurses' wound care training and stopping bleeding

  • (12:21) Using the phone app Guard911 for sending distress signal

  • (15:18) Doctors' positive response to yearly hospital program

44: Malpractice and Burnout; Lessons from a Mock Trial with Dr. Heath Jolliff D.O.10 May 202200:18:32

The possibility of being sued following a medical error is a major source of anxiety for physicians. While there are numerous educational items devoted to medical malpractice risk, these are sometimes limited in scope. Moreover, medicolegal training in residency is frequently insufficient.

Our special guest today is Heath Jolliff, DO, Board Certified in Emergency Medicine and Medical Toxicology, national speaker, educator, and certified executive coach.  Over the course of his career, he has observed many physicians grappling with the stress and burnout of clinical documentation, administration overload, and the threat of malpractice.

In 2020, Dr. Jolliff, along with his colleagues, conducted a mock trial, recreating an actual chest pain malpractice case and revealing insights and burnout prevention lessons for all physicians. The major purpose of this study was to provide participants with practical advice on how to enhance patient safety and documentation while also reducing their medicolegal risk. To learn more about this, the video of the Mock Trial and the Study of this learning experience is available at the links below.

Episode Highlights:

  • (01:54) The objective of the mock trial

  • (03:26) Methodology; Availability for public viewing

  • (07:20) Change in medical practice and documentation

43. Organizing a Resident Union with Hannah Porter MD MBA26 Apr 202200:31:36

Unions have been a significant force for change in many industries, and in recent years even in healthcare.  They can implement policies - better pay, benefits, and working conditions, and push for more transparency and accountability. With unions, employees can make their voices heard and if successful, it could improve the healthcare system as a whole.

In this episode, we talk to Hannah Porter MD, a 2nd-year Dermatology Resident at the University of Vermont Medical Center. She and her team organized the UVMC residents and fellows as the latest union shop for the Committee of Interns and Residents/SEIU. Her story progresses from leadership's disrespect and lack of transparency on issues as simple as PPE to the face-to-face conversation required to obtain a final tally of 209 for the union and 59 against. It even includes Union Busting Consultants making $2500/day to lobby the Residency Program Directors into threatening withdrawal of funding for attending conferences, and a cameo from none other than Senator Bernie Sanders.

After a year, we'll follow up with Dr. Porter to see if they’ve successfully negotiated better pay and if work hour restrictions have been enforced.

“I know that there are several ways to approach how we can fix this healthcare system, and this is one of the ways from my perspective.” Hannah Porter MD MBA

Episode Highlights:

  • (01:39) How it all started and recognizing the need for a union

  • (7:27) Spreading the word to colleagues and Senator Bernie Sanders’ participation

  • (12:37) The biggest benefit of having a union shop

  • (16:48) Change in the treatment by senior leaders after being unionized
