This weeks episode of Ask Dr Jessica is joined by a pediatric hematologist, Dr Gavin Roach.  Hematologist are doctors that specialize in disorders of the blood.  In this episode, Dr Roach explains what are the most common reasons why people visit a hematologist.  We discuss iron deficiency anemia @4:53 (signs to look for and how to treat), blood clots @24:49 (signs to look for and how to manage), bruises  @33:13 (what bruising is normal and when to think about seeing a hematologist) and bleeding issues @38:20  (such as bloody noses) .  Great episode to know what is normal and what would benefit from seeing a hematologist. 

Gavin D. Roach, MD, MS, is an attending physician at Seattle Children’s Hospital, Associate Professor of Pediatrics at the University of Washington School of Medicine, and the Medical Director of the Thrombosis Program at Seattle Children’s. Prior to working in Seattle in 2021, he had previously been on faculty at UCLA (2014-2021).  While at UCLA, Dr. Roach was the Director of Pediatric Hematology and oversaw the treatment of all patients with benign hematologic disorders in inpatient and outpatient settings.  He served as the Program Director of the Fellowship Training Program in pediatric hematology/oncology and was involved with medical education at all levels of medical training.

Ep 88: When to worry about lumps, bumps, hernias, & abdominal pain? with Cathy Shin MD, pediatric surgeon29 May 202300:36:23

Today's episode of Ask Dr Jessica features Dr Cathy Shin, a pediatric surgeon at Children's Hospital of Los Angeles.  In this episode, we talk about the most common reasons why children visit a pediatric surgeon--including skin bumps (6:58), lymph nodes (9:25),  appendicitis (13:28) and hernias (22:58).  She gives great advice on what to look for--what are reassuring signs and what are the signs to visit a doctor.  Dr Shin is also the director of the trauma program at Children's Hospital of Los Angeles---so we discuss ways that parents can help prevent common pediatric trauma--such as car safety (29:57),  swim safety (31:33), gun safety (33:23) and the importance of helmets (34:53).  

Dr Shin is a very accomplished surgeon and it was an honor to have her speak on Ask Dr Jessica! Dr Shin is an Associate Professor of Surgery at USC and she is also the director of the Trauma Program.   She completed her Surgery Internship at the University of Chicago Medical Center, and her Surgery Residency at Children's Hospital of Los Angeles.   She then completed two fellowships---her ECMO Fellowship at Children's Hospital Cincinnati and her Pediatric Surgery fellowship at the Children's Hospital of Los Angeles.

Dr. Anya Trumler-Sebring was awarded her Doctorate of Medicine from the University of Nebraska Medical Center, where she subsequently completed an Internship in Internal Medicine and a Residency in Ophthalmology. She then completed a Fellowship in Pediatric Ophthalmology and Adult Strabismus at the University of California San Diego, followed by a second Fellowship in Pediatric Glaucoma and Anterior Segment at the Wills Eye Hospital in Philadelphia, PA.

She spent 5 years practicing Pediatric Ophthalmology and Adult Strabismus as an Assistant Professor of Ophthalmology at the Johns Hopkins Wilmer Eye Institute in Baltimore, MD.

Her specialty includes the diagnosis and treatment of pediatric and adult strabismus, pediatric cataracts, pediatric glaucoma and children with ocular disease. She also practices comprehensive pediatric and adolescent eye care.

She is a Board Certified Ophthalmologist and is a member of the American Academy of Ophthalmology as well as the American Academy of Pediatric Ophthalmology and Strabismus. Dr. Trumler-Sebring has authored and co-authored numerous textbook chapters and research articles regarding pediatric cataracts, glaucoma and strabismus.

Dermatologist Dr David Reid joins Ask Dr Jessica to talk about common skin conditions that are chronic (stay around for a long time), like dandruff, Keratosis Pilaris and Moles.   Dr Reid explains the best treatment options for dandruff--like using selenium sulfide, Ketaconazole, or zinc pyrithione (or a combination thereof). He also gives tips on how to treat keratosis pilaris -- a common, harmless skin condition that causes dry, tiny bumps found often on the upper arms, thighs and cheeks.  And lastly, we talk about moles!  He gives a great overview on what moles are, and signs when to see a dermatologist. 

Dr David Reid is currently the Chief of Dermatology at Rush Medical Center in Chicago.  He completed his medical school degree at Harvard Medical School and his dermatology residency at Northwestern University.   He has been well published and has lectured internationally.

Paul A. Offit, MD, is Director of the Vaccine Education Center and professor of pediatrics in the Division of Infectious Diseases at Children’s Hospital of Philadelphia. He is the Maurice R. Hilleman Professor of Vaccinology at the Perelman School of Medicine at the University of Pennsylvania.
Dr. Offit is an internationally recognized expert in the fields of virology and immunology, and was a member of the Advisory Committee on Immunization Practices to the Centers for Disease Control and Prevention. He is a member of the Food and Drug Administration Vaccines and Related Biological Products Advisory Committee, and a founding advisory board member of the Autism Science Foundation and the Foundation for Vaccine Research, a member of the Institute of Medicine and co-editor of the foremost vaccine text, Vaccines.
He is a recipient of many awards including the J. Edmund Bradley Prize for Excellence in Pediatrics from the University of Maryland Medical School, the Young Investigator Award in Vaccine Development from the Infectious Disease Society of America, a Research Career Development Award from the National Institutes of Health, and the Sabin Vaccine Institute Gold Medal.
Dr. Offit has published more than 160 papers in medical and scientific journals in the areas of rotavirus-specific immune responses and vaccine safety. He is also the co-inventor of the rotavirus vaccine, RotaTeq, recommended for universal use in infants by the CDC. For this achievement, Dr. Offit received the Luigi Mastroianni and William Osler Awards from the University of Pennsylvania School of Medicine, the Charles Mérieux Award from the National Foundation for Infectious Diseases, and he was honored by Bill and Melinda Gates during the launch of their Foundation’s Living Proof Project for global health.

Episode 75 of Ask Dr Jessica we welcome Dr Jeff Gelfand to discuss autoimmune disease, specifically Multiple Sclerosis.  About 1 million people in the United States are affected by Multiple Sclerosis (MS), and often it is diagnosed in adolescence or early adult years.  In this episode, our goal is to help listeners understand what MS is and to promote awareness about the illness.  What are the signs and symptoms? What are the available treatments? How does one get MS --is due to genetic factors, environmental factors, or both? Is MS preventable?  One of Dr Gelfand's many gifts is being able to talk about complicated medical topics into simplified, easier to understand verbage.

About Dr Gelfand:
 Dr. Jeffrey Gelfand is a neurologist who specializes in caring for patients with autoimmune, inflammatory and neurodegenerative neurological conditions. His expertise encompasses multiple sclerosis, neurosarcoidosis, encephalitis, meningitis, transverse myelitis, optic neuritis, neuromyelitis optica, central nervous system vasculitis, autoimmune dementias and encephalopathies, autoimmune cerebellar ataxia and paraneoplastic neurological syndromes. He also treats neurological problems in people with rheumatic diseases or other systemic illnesses and cares for people with general neurological disorders.

Gelfand received his medical degree from Harvard Medical School. At UCSF, he completed a residency in neurology, followed by fellowship training in multiple sclerosis and neuroimmunology (the study of how the nervous and immune systems interact). He also has a master of advanced study degree in clinical research from UCSF.

As director of the UCSF Division of Neuroinflammation and Glial Biology clinical fellowship program, Gelfand trains specialists in multiple sclerosis and neuroimmunology after they have completed a residency in neurology. UCSF recognized his excellence in teaching in 2014 with the Robert B. Layzer Award and in 2015 with an award from the Haile T. Debas Academy of Medical Educators.

Gelfand is an elected fellow of the American Academy of Neurology. In 2019 he was elected vice-chair of the Autoimmune Neurology Section of the American Academy of Neurology.

Ask Dr Jessica Episode 65 discussing the use of ketamine for treatment of depression, under the care of a psychiatrist.  In recent years, there has been an increase in the number of teenagers who are experiencing major depression.  Randomized trials have demonstrated that  ketamine (given through the IV) and  esketamine (the nasal form) can rapidly improve treatment-resistant depression, including suicidal ideation.   I think it is important to stay up to date on available therapy, because with this knowledge, you never know whose life you may impact for the better.

Episode 62 of Ask Dr Jessica discusses the fentanyl crisis with anesthesiologist and pain specialist, Dr Andy Leitner.   There has been a growing problem with street drugs laced with fentanyl, often in lethal doses.  Fentanyl poisoning is now the leading cause of death in adults 18-49.  With all of this news,  educating our children is more important than ever.  In this episode, Dr Leitner explains what fentanyl is exactly.  When it is used safely in the medical setting?  What is the fentanyl crisis exactly and why are we finding it more frequently on the street drug market?  What does a fentanyl overdose look like and what can parents do about it? 

Andrew Leitner, M.D., earned his undergraduate degree magna cum laude in biology from Tufts University in Medford, Massachusetts. He went on to receive his medical doctorate from the Washington University School of Medicine in St. Louis, Missouri. He completed his residency in the Department of Anesthesiology at University of California Los Angeles, where he also pursued a fellowship in interventional pain management. 

Board-certified in both anesthesiology and pain management, Dr. Leitner is fluent in Spanish, Hebrew and Hungarian. He is an active member in numerous professional associations including the American Society of Anesthesiology, the American Society of Regional Anesthesia, the California Society of Anesthesiologists and the North American Neuromodulation Society. He has also written several articles and abstracts.

This week, our guest is pediatric ENT Dr Ali Strocker and we discuss choking hazards.  We talk about the most common foods that lead to choking, and what parents need to know to help keep their children safe.  We also hear Dr Strocker's take on baby led weaning, on practice this method in a safe way.  Thank you to Dr Strocker for coming back on the podcast!  (she also did a previous episode on Swimmer's ear!)

Dr Stroker is an ENT, specializing in children of all ages with ear, nose and throat problems. Dr Strocker received her medical doctorate with high honors from the UCLA School of Medicine in 2001 and was inducted into the Alpha Omega Alpha honor society .  She completed a residency in Otolaryngology-Head and Neck Surgery at UCLA.  Dr. Strocker was an assistant clinical professor of Otolaryngology-Head and Neck Surgery at USC-Keck School of Medicine and has been an attending surgeon at Children’s Hospital Los Angeles.

Coughs are often so difficult for families; they can be so uncomfortable and persistent, and many families don't know how to best help their children.  In this episode, with pediatric pulmonologist, Dr Cesar Chavarria, we discuss what is a normal length of time for coughs to last, what medications can be offered, and also when to see a doctor.  We also talk about asthma (starting @18:55). We discuss common signs of asthma, how to know the difference between asthma and a regular cold virus cough, how to treat asthma and what demographic it typically affects. 

Dr. Chavarria is a pediatric pulmonologist with more than 30 years of experience in treating pediatric respiratory conditions. He attended undergraduate school at Madrid College and medical school at the National Autonomous University of Mexico, both in Mexico City. He completed his pediatric internship at the University of Colorado Health Sciences Center in Denver, and his residency in pediatrics and fellowship in pediatric pulmonology and critical care at the Long Beach Memorial Medical Center.  Dr. Chavarria is board certified in Pediatrics and in Pediatric Critical Care.

Episode 55: Tongue-tie "clipping" (aka frenotomy); are we over-doing it or have we discovered an under-diagnosed problem? with Pediatric ENT Matthew Brigger MD, MPH19 Sep 202200:31:16

On this weeks episode of Ask Dr Jessica, Dr Matthew Brigger joins to discuss frenulotomy, also known as tongue-tie "clipping" or tongue-tie "release".  In the past 20 years, the number of newborns receiving the  tongue-tie releases has doubled at the very least.  The number of babies born with tongue ties has been unchanged, so as a pediatrician, one has to wonder why this procedure has increased so much---  Has the condition been under-diagnosed and now social media has helped bring this issue to light? Or are we  doctors doing too many in the hopes that it will help mothers with breast feeding?  What is the evidence that this procedure is beneficial? Dr Brigger offers much clarity to the  benefits and  draw backs of this procedure.

Dr Brigger has a very impressive resume—he is currently the division chief of pediatric ENT at Rady Children’s hospital in San Diego.  He is also a professor of Surgery at UC San Diego School of Medicine.  He also served as a pediatric ENT in the NAVY, he received his masters in public health at Harvard, and he has authored over 70 peer reviewed studies.

About Carson:

Carson Meyer is a doula, nutrition consultant, and advocate for holistic maternal care.  She graduated from NYU in 2016 with a degree in child development and holistic health. Now based in North Carolina, Carson has spent the past seven years guiding hundreds of parents through pregnancy, birth, and postpartum. As a doula, she offers education, resources, and compassionate support—whether her clients choose a home or hospital birth.  In 2020, Carson expanded her scope of care by becoming a certified nutrition consultant through the Oh Baby Academy. On today's episode of Ask Dr Jessica, I am thrilled to have as my guest Dr. Joan Rosenberg.  In this episode we discuss how to raise our children with resilience, self-esteem, and how to help guide them through disappointment.  We also talk about it is ultimately helpful to learn to "sit with" unpleasant feelings. Bestselling author, consultant, master clinician, and media host, Dr. Joan Rosenberg is a cutting-edge psychologist who is known as an innovative thinker, acclaimed speaker, and trainer. As a three-time TEDx speaker and member of the Association of Transformational Leaders, she has been recognized for her thought leadership and influence on personal development.
