180. Becoming Empowered and Getting Vulnerable with Dr. Casperson09 Oct 202200:49:24
Listen to this very deep and vulnerable conversation I had with my friend Dr. Kathy Zhang on her podcast, The Purpose Filter. There are so many aspects to living a life of fulfillment, but what if there was one area of your life you could improve that would spill over into all others? Sounds like a bit of a life hack. Well, it is, and we want to share it with you on today’s episode. Is your sex life fulfilling, or are you just going through the motions? The empowerment you get from knowing what you want in the bedroom and being able to speak up and ask for it, or the ability to say no, when no means no, is a crucial element to all aspects of your life. If you can stand up for yourself in these very vulnerable and intimate situations, the empowerment that happens outside of the bedroom will come naturally. This all starts with you and your willingness to make a decision to uplevel your life and stop living on autopilot. “Should we care about the people who are sleeping with the people we are giving Viagra to?” “My husband died so now I need to take care of myself”. “It is easy to be average” “She just wants the damn thing to end – which is another reason that women fake orgasm” On faking orgasms - “He thinks it is the end of the party and you were never at the party.” “You can’t just give somebody desire. It is an inside job” “I think working on your sex life is the final frontier of personal growth.” “Everything you do to have a better sex life spills over to other aspects of your life” “What if my whole purpose in life is to tell people to use vaginal estrogen cream?” Tune in to hear how you can create more joy and abundance in your life! Topics discussed in this episode: • Dr. Casperson’s experience in a largely male dominated field • What is it like becoming the first doctor in the family • Bad advice Dr. Casperson received when she was becoming a doctor • Giving yourself permission to step into your power • Why your voice is important and unique • What was Dr. Casperson’s turning point to change the way she lived her life • Applying dying lessons now while we are living • Why women feel their pleasure and themselves are not a priority • How to move past shame and guilt surrounding sex and beyond • Going from being pulled along for the ride to getting into the driver seat • How young people should re-frame purpose • Sharing the gift of awareness and presence Connect with Dr. Kathy Zhang on Intagram https://www.instagram.com/purposefilter/ And on her website: https://luyikathyzhang.com/ Did you get the You Are Not Broken Book Yet? https://amzn.to/3p18DfK Join my membership to get these episodes ASAP when they are created and without advertisement and even listen live to the interviews and episodes. www.kellycaspersonmd.com/membership --- Send in a voice message: https://podcasters.spotify.com/pod/show/kj-casperson/message

179. Roommate Syndrome, Giving Couples Hope, and How To Be a Man Worth Desiring with Coach Glen Lovelace02 Oct 202201:04:58

Meet Glen Lovelace! I love his generosity toward helping men in relationships and his masculine energy on this podcast! He shares his story from desperate (even suicidal) plans to living a life of helping others live their best relationships.

When women learn about their sexuality and get empowered, often they feel stuck because of a spouse who they fear is not open to change.  I wanted to bring on someone who literally deals with this as his job.  Glen helps men who's wives have been transformed in one way or another, usually by coaching education.

Glenn Lovelace is the wingman for men whose wives have transformed from coaching. He is a certified coach through the life coach school. Glenn has coached over 3,000 clients 1 on 1 since 2008.

His transformation started with bodybuilding after being in a very low place in his life. He decided he needed to prioritize himself to be the best spouse and father he could be.

"It’s okay to be uncomfortable."

How to “gently” change your partner (and remember you can’t change anyone who doesn’t want to be changed).

1) Show them how you have changed

2) Bring in a third party influencer

3) Learn how to speak the same language

4) Always remember you two are on the same team

Roomate Syndrome – just existing together and losing touch why you love and are attracted to each other.

If one person does their work, the marriage can get better.

Your spouse is just there to love.

Can couples start speaking the same language? Glen says yes!

Intimacy not in the bedroom is important!

Wanting your spouse to change for you to be happy is a toxic plan.

Happy Wife Happy Life - it’s a lie and Glen explains how it wasn’t good for him.

Each person is in charge of their own pleasure.

Don’t assume what is needed in a situation - - Ask

How to be a man worth desiring? Start dating and desiring your wife. Approach her how she wants to be approached. Date every week. Keep your commitments.

There are seasons to your sexuality.

Foundations of trust in a relationship

- Security, trust, safety, stability

The trouble with saying “divorce isn’t an option”

Talk about your religion – what does your religion say about sex and marriage and how have you navigated any challenges?

177. Me and My Body Are A Team - Teaching Reconnection with Katrina Ubell, MD18 Sep 202200:32:44

A mindwork episode!

Dr. Ubell is one of the first physician life coaches!

She gained and lost the same 40 lbs at least 10 times since puberty and now helps women become a team with their body and stop obsessing over food using the power of mindwork.

She reached out to me to be on the podcast and at first I didn’t know how to connect the dots besides life coaching. I have a podcast episode called “sex while fat” – we are okay at any size. I was worried she just wanted to talk about losing weight.

So I said Okay if we can talk about how mind work in her arena can also relate to a positive body image and sex life.

Like my older episodes talking about money and sex – these topics are more related than we think because of our thoughts about them.

“I am interested in people stopping hating themselves” – Dr. Ubell

What is diet culture?

The concept of mutual respect between you and your body.

Can weight loss goals exist with body positivity?

Shame is useless.

“I haven’t had success so there must be something wrong with me” Dr. Ubell

Why do we get numb or disconnected from our bodies?

Over eating as a trauma response – sexual assault or childhood trauma – I got big to not feel or too attractive.

Changing one’s identity requires changing our thoughts.

Menopause and weight gain/loss – watch what you believe about this.

Acceptance of our bodies as we get older.

Entitlement thoughts! I feel so seen! I am entitled to my latte!

How to stop invalidating your emotions

Relationship with yourself and thoughts about yourself are important for both weight loss and sexuality.

174. Vibrators, Princess Marie Bonaparte, and Motorcycle Coils28 Aug 202200:34:03

First I read from the recently published article, "The Anatomy of Female Sexual Arousal: Princess Marie Bonaparte, the Halban-Narjani Procedure, and the Art of Constantin Brancusi." Then I talk about seven different types of vibrators. Then I finish with a tragic case of motorcycle spring meets penis. Enjoy!

1. The OG - Hitachi Magic Wand. external, powerful and marketed as personal massagers

2. 𝗖𝗹𝗮𝘀𝘀𝗶𝗰 𝗽𝗲𝗻𝗲𝘁𝗿𝗮𝘁𝗶𝘃𝗲 𝘃𝗶𝗯𝗿𝗮𝘁𝗼𝗿𝘀 - put curve on them, call them G’Spot’ and raise the price

3. 𝗖𝗹𝗶𝘁𝗼𝗿𝗮𝗹 𝘃𝗶𝗯𝗿𝗮𝘁𝗼𝗿𝘀 - these focus on external clitoral stimulation. Air pulse – Wevibe Melt – Best in class IMO.

4. 𝗗𝘂𝗮𝗹 𝘃𝗶𝗯𝗿𝗮𝘁𝗼𝗿𝘀 𝗮𝗸𝗮 "𝗥𝗮𝗯𝗯𝗶𝘁𝘀" - Work in and out at the same time.

5. 𝗕𝘂𝗹𝗹𝗲𝘁 𝘃𝗶𝗯𝗿𝗮𝘁𝗼𝗿𝘀 - usually a "starter" vibrator for most folks.

6. 𝗖𝗼𝘂𝗽𝗹𝗲𝘀 𝘃𝗶𝗯𝗿𝗮𝘁𝗼𝗿𝘀 - many many different types here. Eve by Dame is one that is "hands free", many strap onto a penis.

7. 𝗔𝗻𝗮𝗹 𝘃𝗶𝗯𝗿𝗮𝘁𝗼𝗿𝘀 - kinda self explanatory.

Vaginal Lasers

How a male can introduce my book to his female partner

Unopposed Estrogen can cause harm

Q&A from listeners:

Genetic testing in 2017 and was positive for Lynch Syndrome. I was just diagnosed with uterine cancer yesterday. 43 years old and total hysterectomy in my very near future. Terrified about trying to get my hormone levels correct post surgery!! Any suggestions or recommendations?

I’m a 38 y/old breast cancer survivor. I’m on estradiol, got all the fancy lubes, and now I’m curious about testosterone… People only seem to talk about estrogen for women, but my libido is nonexistent and penetrative sex is excruciating. Curious if testosterone also takes a hit while on hormone blocking meds post-cancer. I discuss this stuff with the oncologist but it feels trivial compared to the work they did to cure my cancer. However, my PCP won’t touch the hormone topic with me and defers to the oncologist because of my cancer history… thoughts/suggestions?

I’ve gathered so much info from you about the estradiol cream and so much more than I got my gyn to prescribe the cream for me. I’d like to note that I am 59 and I have never had any problems with painful sex or any vaginal issues in the past. I applied the first dose both inside and a bit on the outside areas, it was fine with no noticeable changes. But 4 days later I applied the second dose, this time just inside and within a day or so I felt changes. My clitoral area was sensitive with discomfort and I experienced dryness in the outer labia like never before - not really painful but noticeable discomfort. I have not taken a third dose but I really want to be able to use this so I don’t shrivel up and go away LOL. I also was getting a few hot flashes and they really decreased after the first dose but are back now, although not as much. Is it possible that the discomfort will diminish after a while if I keep taking it? Would it be effective if I only applied it once weekly?

I’m 58 and 2 years in menopause. I use an estrogen patch, progesterone, and a testosterone cream - thanks to you - but it’s not clear to me if my transdermal estrogen will protect the vulva from atrophy. Should I also be using an estrogen cream on my vulva in addition to my HRT regimen? I asked my doctor and she said creams for the vulva are only from lichen sclerosus, but that doesn’t sound right.

169. His Porn Her Pain - Sex Therapist Marty Klein, PhD24 Jul 202200:45:42

Dr. Marty Klein has been a Licensed Marriage & Family Therapist and Certified Sex Therapist for over 35 years. He has focused his entire career toward a single set of goals: telling the truth about sexuality, helping people feel sexually adequate & powerful, and supporting the healthy sexual & intimate expression and exploration of women & men. He practices in Palo Alto.

He has written 7 books, translated into 11 languages and has given over 1,000 lectures.

Why are people so against porn? Or why not be against porn?

The Ice Cream/Broccoli hypothesis

‘When people have low desire for lousy sex, that is not surprising” – Dr. Klein

Should we worry about porn dehumanizing women?

What is the number one fantasy by women?

Should you actually live our your sexual fantasies?

“Sexual fantasies do not have meaning” – Dr. Marty Klein

Porn addiction – define?

What about porn users who aren’t functionally sexually at home?

Is porn infidelity? What about sexting someone else? Only Fans?

Talk about the power struggles couples get in and think it is about porn

“The ultimate issue in many couples in conflict about pornography is the decline or disappearance of their once enjoyable sex life. While it is common to say that the porn user is selfish and rejecting of their partners, a lot of porn use is actually a desperate attempt to stay in a sexually unfulfilling couple.”

What to do if no longer attracted to your spouse? How is this not porn’s fault if it makes you get off to perfect bodies?

“Desire in middle age does not feel like desire at 23 years old” – Dr. Marty Klein

What he wishes women knew about their male partner’s porn.

What would you like to tell the men?

Why are there not enough sex therapists and what should I do about it?

155. Talk to Kids About Sex: 5 Common Mistakes01 May 202200:25:44

Dr. Shannon Widlansky was trained as an Ob/Gyn who is now a life coach.
She has been a gender expert for 8 years. She helps institutions use more gender-inclusive language.
She is a facilitator of the Our Whole Lives curriculum which teaches sexuality to all ages.

She talks about what was wrong with past sexuality education.

The definition of “man” or “woman” is wrapped up in the function of a body part. Which gets shaken up when there are problems with these organs.

You don’t have to talk about the clitoris when your education is only a “disease and pregnancy prevention plan” – Dr. Casperson

5 common mistakes adults make in talking to kids about sexuality

1) Not planning ahead

2) That it is more than sex and puberty

    a. 5 circles of sexuality

           i. Body image and appreciation, sensualization

           ii. Intimacy

           iii. Identity

iv. Repro health – puberty changes, infection

           v. Sexualization – consent, how media portrays sexuality

3) One and done “the talks” – a lifelong conversation.

4) They have to get it right – don’t expect to get it “right”

5) Inclusive language “most but not all”

Consent – ideally an enthusiastic yes, not something that is coerced.

What country does it well - Netherlands

Other tips – what would your ideal conversations look like? Lead from confidence

154. April Live Podcast27 Apr 202200:43:08

April Live Podcast!

  • How do you transition from BioT pellets to systemic estrogen and vaginal cream?
  • Just started vaginal estrogen and both times had lower abdominal discomfort the next day. Is this a potential side effect?  Is there another route besides the cream or ring?
  • I have been seeing a PFT on and off for about 6 yrs since having a partial hysterectomy.  Recently I've had a lot of pressure and feel as though something is stuck in the rectal area. I have seen my GP and I have had a pelvic and rectal exam. He's sending me for an ultrasound this Saturday. I have also seen my PFT. I will see her again in a month. I guess my question is, if I have a prolapse going on would the PFT pick that up with the pelvic exam? and would an ultrasound show a prolapse?
  • Hormone problems related to COVID? Any insight?
  • Bartholin Cysts
  • I am 57 and still having a pretty regular period.  Question about Bio_T and hormone labs.
  • My periods have always been heavy, 7 days… and have gotten even heavier as I’ve gotten older ......I have to say having a pretty regular period at 57 sucks.  Based on my labs I can infer I’m not menopausal and all I can say is WTF!
  • AND MORE!

Join me and Palliative care physician Dr. Faryal Michaud.

When your view of sexuality is narrow, it is threatened by health challenges.

The effects of cancer diagnosis on sexuality.

“You are still a man if your penis doesn’t work” – Dr. Casperson

Where does intimacy come from?

Tips for communication

- Talk about sex outside the bedroom

- Curiosity  “curiosity is the opposite of judgement” Dr. Faryal

- This is not just one conversation

- Don’t think one person is the problem – it is a couple issue

We discuss sexless marriage

We discuss how common it is to struggle with sexuality after a baby comes into a family

New relationship brains are akin to taking cocaine – we neglect other things besides our new love. This wears off after about 6-18 months.

“You can’t think an orgasm” – Dr. Casperson

“How are you going to be the person worth having sex with?” – Dr. Casperson

“We are so ill prepared to go into long term relationships.” – Dr. Casperson

Anxiety kills sex drive

Awareness, acceptance and acknowledge – three components of dealing with what comes up in life.

Living life on purpose, a view of life as fleeting as a way to make friends with your life and this moment.

Death is certain, suffering is optional.

How sexual function is biopsychosocial and the meds that affect the “bio” part of this

There are two FDA approved meds for women with low desire. They work in 50-60%of people that take them. They work by boosting dopamine.

Addyi - filbanserin - came out in 2015. It is an oral nightly medication. Side effects are sleepiness and may include weight loss.
It is safe to use if you are on antidepressants.

Vyleesi – bremelanotide – an on demand injection – likely works by increasing dopamine.

Do we fix hormones first or use these meds first?

How to navigate prescribing these meds as a provider?

Dr. Rubin talks about her work changing national guidelines.

23% of women have clitoral adhesions

147. Author Interview - Dr. Van Anders - The Heteronormativity Theory of Low Sexual Desire23 Mar 202200:41:06

Desire is Shaped by our Lived Experiences – The Heteronormative Theory Paper

Dr. Sari van Anders is the Canada 150 Research Chair in Social Neuroendocrinology, Sexuality, and Gender/Sex, and Professor of Psychology, Gender Studies, and Neuroscience, at Queen's University.

Dr. van Anders has published over 80 papers with research that sets out new ways to conceptualize, understand, and map gender/sex, sexual diversity, and sexuality, and also provides unique tools and theories for feminist and queer bioscience, especially within social neuroendocrinology and studies of testosterone.

Can we talk about Gendered behavior for a fast minute – a lot of people – majority people – might not know or believe that they are “socially gendered” – she explains this to us.

How does being gendered as a female affect our desire?

how some aspects of sexuality widely assumed to be hormonal, might actually be better understood with or alongside a sociocultural lens.

Okay – on to the mind blowing paper on “the heteronormativity theory of low sexual desire in women partnered with men” – Archives of Sexual behavior 2021.

- Why did you write this?

- You propose the heteronormativity theory of low sexual desire in women partnered with men, arguing that heteronormative gender inequities are contributing factors.

- Four Hypothesis

o Inequitable divisions of household labor

Women have less free time

o Blurring of partner and mother roles – its not that mothers aren’t sexual – it is that the relationship between mothers and those they mother is not a sexual one.

Nurturant experiences decrease testosterone and increase peptides like oxytocin and vasopressin.

Thinking sexual thoughts can change testosterone levels

How Cortisol affects our sexual response

o Objectification of women

Proximity to the norm matters for their desire

Women’s focus on appearance and being sexy instead of sexual

Ignorance about the vulva and clitoris negatively impacts desire in women partnered with men

o Gender norms surrounding sexual initiation.

Discomfort initiating sex contributes to low desire.

If women have to say yes everytime a man wants sex – this contributes to low desire.

“women’s sexual pleasure within heteronormative framings is fraught.”

Women’s duty for sex or inability to say no.

- What do we do now that we know this?

- Orgasm coercion is her future research

143. Brains and Pains - Healing Chronic Pain with Deb Malkin09 Mar 202200:49:22
Worldwide, the rates of chronic pelvic pain for women of childbearing age range from 14% to 32% Deb Malkin is a plus size, body positive, LCS certified life coach, massage therapist, movement enthusiast, and client zero of her pain recovery coaching work. She had only been taught that pain is related to issues in the tissues and was never taught the link between the brain, nervous system and the experience of chronic pain. Her mission has always been for people of all sizes, abilities and genders to feel at home in their bodies. She shares her story of chronic pain. The brain can continue and perpetuate pain long after the body has healed. Why is chronic pain everywhere? Does pain tell us lies or is it in how we interpret the pain that gets us in trouble? The role of fear and being able to feel. We talk about pain with sex and pelvic pain and tips for that issue. The “perfectionist personality” and suppressed rage and how it is involved with pain. 134. Stop Gaslighting My Vagina06 Feb 202200:18:33
On social media A woman asked – my vagina is so dry that sex is painful, how can I fix this? Wikipedia: Gaslighting is a colloquialism, loosely defined as making someone question their own reality. The answer provided on social media to this question: - Regular sexual activity, self stimulation and pelvic floor exercise can help keep your vagina health. - IS THIS ACTUALLY TRUE? - This doctor said “I tell women to avoid sugar and use probiotics to make sure their entire ecosystem is “humming along” – NO evidence that oral probiotics affect vaginal microbiome – then she plugs her diet and her probiotic products - She then goes on to say – two additional ways to counteract vaginal dryness is to strengthen the pelvic floor by doing kegel exdrcises and using a “holistic formulation” that helps maintain optimal levels of natural moisture, and “supports a healthy vulva and vagina”. – this is her product – which states it “prevents leaks” and costs 70 dollars. Also claims to “reignight desire” – WTF I searched in J sex med - does intercourse delay or treat vaginal atrophy? – can’t find a damn thing I found on an internet site - “Regular sexual activity, with or without a partner, can help maintain healthy vaginal tissues through increased blood flow and avoid or lessen the effects of vaginal atrophy.” - No citations, so again I went to J Sex med – can’t find a thing On menopause.org - Regular vaginal sexual activity is important for vaginal health after menopause because it stimulates blood flow, helps keep your vaginal muscles toned, and maintains your vagina’s length and stretchiness. - No citations, and furthermore, the vaginal muscles are smooth muscles controlled by the autonomic nervous system – you can’t tone them with sexual activity - I will buy that stretching the vagina frequently probably allows it to be stretched frequently without pain – but is there evidence and why sex, why not just dilation? Current treatment options for postmenopausal vaginal atrophy Iuliia Naumova1 and Camil Castelo-Branco2 Int J Womens Health. 2018; 10: 387–395. It is important not to forget about the positive effect of sexual intercourse on the improvement of vaginal health – no citations (JAMA 1983;249:2195-2198) Vaginal Atrophy in the Postmenopausal WomanThe Importance of Sexual Activity and Hormones - But this study was not randomized – perhaps the less atrophy allowed the women to be sexually active and not vice versa? How do we know the sexual activity caused less atrophy? We don’t have any studies. Another paper on vaginal atrophy gave this recommendation - Regular sexual activity should be encouraged to maintain vaginal health. (II-2B) - Usefulness/efficacy is less well established by evidence/opinion Why is level 2-2B evidence cited as scientific fact? Pro-tip – Billionaire Marc Cuban just started an online pharmacy for low cost generic drugs https://costplusdrugs.com/ - vaginal estrogen tabs for < 10 bucks, cream for 54 dollars.
