| The rising tide of the school caseload and medical field burnout | 01 Jul 2021 | 00:58:51 | |
Matt, Michael, and Michelle look at the rising tidal wave of increased caseloads that may happen during the next school year. The Informed SLP: “In the outpatient setting, patients with unilateral vocal fold immobility (UVFI) don’t typically have dysphagia (the aspiration rate may be as low as 3%). One might expect—like several researchers have—that the reason some people with UVFI aspirate is because of impaired laryngeal adduction. Well, it turns out we’ve been lookin’ at the wrong structure, and analysis of swallow physiology shows a different story.”

| Trauma Informed Care and Parkinson's Disease and Dysphagia | 08 Jun 2021 | 01:07:33 | |
This week Michael has taken the week off but Speech Science is lucky to have Ruchi Kapila step in as a cohost. Ruchi is the Co-Creater and Co-Host of The Hyndsyte Project Podcast. Michelle continues to re-pack for her move back to the state of Texas! Ruchi just returned from presenting at the Northwest Art and Science of the Performing Voice Conference and is looking forward to the next few weeks of being in Texas. Matt finished at the school district for the summer and took his boys camping over the weekend. The crew discuss the need to address trauma and the return to in person therapy and the affect medication may have on dysphagia and patients with Parkinson’s Disease. The Informed SLP looks at recommendations for Oral Care with patients with Parkinson’s Disease. The shoutout this week goes to MarissaMM and other grad students who have recently passed their Praxis and are preparing for the world of being a CF. The Due Process asks the crew if they would be okay with wearing identification on their name badge identifying if they have received the vaccine or not. The Discord is up and ready for people to interact with the crew 24/7 with a new website, www.discord.speechsciencepodcast.com. SSPOD Shoutout: Do you know an SLP who deserves a digital fist bump or shout-out? We want to know your #SSPODSHOUTOUT, which is recognition for someone doing something awesome somewhere. SSPOD Due Process: Your opportunity to message us about a problem in your area that you would like us to talk about, use the #SSPODDUEPROCESS. Topic #1: As the world of therapy continues to move to in person, the crew discuss the importance of addressing and acknowledging the unspoken trauma that our clients and students have been through. Topic #2: A recent student looked at the impact medications may have on dysphagia in patients with Parkinson’s Disease. Medication may lead to poorer oral health conditions and when paired with poor self-recognition of dysphagia symptoms may be putting our patients at risk.
The Informed SLP: You’re seeing a patient with Parkinson’s Disease (PD) who’s complaining of difficulty swallowing. Your exam reveals that his overall oral health is in need of some attention. We know the state of our mouths has a lot to do with how we chew and swallow. Maybe this is contributing to his difficulty swallowing? | What do you need in your speech bag? | 02 May 2021 | 01:27:52 | |
this week, Michael, Matt, and Michelle were joined by Learn With Less’ Ayelet Marinovich! Ayelet’s kids have returned to in person school this week. Michelle received her second shot of the vaccine. Michael celebrated Earth Day. Matt took his son’s Cub Scout pack to the Reds Stadium for a tour and goes off on the hospital that mis-charted his daughter’s hearing loss. On today’s show, Ayelet joins the crew to talk about what is important to have in your speech bag and what’s worth it and what’s worth skipping, a look at ASHA Elections and the lack of diversity, and the Informed SLP dives into the link between posture and your voice. The crew breaks down the over-reliance school districts have on standardized assessments and ensuring students meet a certain threshold.

Topic #1: As new SLPs get ready to graduate, the crew talk about the materials every SLP should or should not have in their bag. If you are seasoned SLP what things should you take out of your therapy bag?
The Informed SLP: we know that good posture improves respiratory support, which in turn can optimize vocal performance and speech production. Now, think about those patients who have difficulty with postural control. But what more can we do to achieve better posture for our patients?
ASHA Spotlight: We take a deep dive into the lack of diversity available in the ASHA Elections and why that is.

| Changing Special Education, ASHA PAC and You! | 07 Apr 2021 | 00:56:00 | |
Michael, Matt, and Michelle are back together for the show. Michelle returns from her trip to Texas and returns as a homeowner! Michael is loving being a new father, except for his sleep pattern. Matt took advantage of his Spring Break and got work done around the home he needed to complete.
On today’s show, we reimagine what special education should look like, a look at the ASHA PAC, the Informed SLP looks at Social Media and TBI, and the Speech News Headlines.
The Discord is up and ready for people to interact with the crew 24/7 with a new website, www.discord.speechsciencepodcast.com. ----more---- SSPOD Shoutout: Do you know an SLP who deserves a digital fist bump or shout out? We want to know your #SSPODSHOUTOUT, which is recognition for someone doing something awesome somewhere. SSPOD Due Process: Your opportunity to message us about a problem in your area that you would like us to talk about, use the #SSPODDUEPROCESS.
Topic #1: We have witnessed a dramatic shift in Special Education over the course of the last 12 months. Should we take this opportunity to reimagine and change the way we look at our services?
Topic 2: Why doesn’t ASHA do more for us? Legally they can only do so much, and this is where the ASHA PAC comes into the conversation. How much did the PAC raise last year? The number will be surprising and disappointing.
The Informed SLP: we know that written communication can be easily misinterpreted because it’s lacking tone, body language, and context. Add in social communication deficits (like people with Traumatic Brain Injuries (TBI) often have, for example) and social media can become a source of stress instead of connection.

| One Year Ago… A look back at how therapy has changed during the year of the Pandemic | 29 Mar 2021 | 00:48:30 | |
Matt and Michael are on the show this week while Michelle is in the Great State of Texas. Michael is celebrating his new 1-month old child in his home and is coming to terms with balancing being a father and a therapist. Matt and Michael discuss the must needs for a new parent such as the Nose Freeda! Matt talks about his plans for spring break and working with more adult patients while also handling a young gamer in his home.
On today’s show, we look back on the year that was and how everything changed since March of 2020, a new study that shows how valid language samples are online versus in person, the Informed SLP checks in with how modified diets and thickened liquids affect the nutritional status of patients, and the Speech News Headlines.
Topic #1: What does therapy look like one year into the pandemic? The way we look at pragmatic and social language may have to shift as students have been home and are now transitioning back into the world. What therapy materials have you created or modified that you wont be changing back?
Topic 2: With the increase in telepractice, how effective are our language samples if done online versus in person? A recent study states that there is in fact a high reliability and did not differ from in person. What does this mean for the future of Speech and Language Therapy in hard to get to places?
The Informed SLP: If you’re a medical SLP, you’ve probably wondered how thickened liquids (TL) and texture modified diets (TMD) affect overall nutritional status. What do you need to know to create a positive change in your setting.

| What do you do when the team changes your IEP goals without your input? | 07 Dec 2020 | 01:07:00 | |
Matt, Michael, and Michelle check in following Turkey day. Michelle is prepping for the arrival baby speech science 2.0. Michelle and her family have jumped into Christmas as early as possible. Michael is moving back to online therapy and finding ways to engage families more effectively. Matt moved online for a week and he looks forward to move back to face to face therapy.
On today’s show, we recognize the folks over at Cognixion for their new breakthrough in AAC. The team breakdown the requirements and what to do when an evaluation team changes your goals on an IEP. A listener writes in asking Michael about the link of meaningful interactions and brain growth. This week’s Informed SLP looks at the importance of evaluating students in both languages if they are bi-lingual. The weekly news headlines return and the ASHA spotlight looks at their resources for screen time with children.

SSPOD Due Process: Your opportunity to message us about a problem in your area that you would like us to talk about, use the #SSPODDUEPROCESS. What do you do when the school based team changes your IEP goals?
The Informed SLP: Most SLPs probably already had a sneaking suspicion that it is best to screen children who are dual language learners (DLLs) for language disorders in both languages. Now there are studies to support this. ASHA Spotlight: How much tech time is too much tech time?

Matt is coming to terms with his reports being too long checking in at over 12 pages. Michelle and her family were enjoying the unseasonable warm weather in the beginning of November. Michael has noticed an increase in virtual cases as Covid numbers increase near his clinic.
On today’s show, we discuss the link between diverse social connections and cognition, how to handle contentious IEP meetings, and we sit down with the Dysphagia Outreach Program. | ASHA Interstate Compact, Dementia and Voting, Smiles for Speech | 30 Oct 2020 | 00:49:27 | |
On this week’s episode the group has their struggles. Michael is unable to join the group as he is stuck in traffic, Michelle’s computer is updating and joining the group via iPad, and Matt didn’t have water for the first 5 minutes of the show! The group responds to a text message about a recent ABA episode.
On today’s show, we discuss the impact of the ASHA Interstate Compact and should people with Dementia be allowed to vote. Michelle had the opportunity to sit down with Sandy Dorsey of Smiles for Speech to talk about their virtual 5k to raise money.
This week, the shoutout goes to the SLPs who are working on creating the ASHA Interstate Compact.
Topic #1: The threshold for an Interstate Compact is 10. ASHA is working hard to get to the minimum 10 states required. What does this mean on your states rules? What if you work in Ohio but live in Kentucky and your school goes digital, can you work from home in this situation? Your state may say no.
Topic 2: Are people with dementia allowed to vote? With almost 2.5% of the American population with dementia, what roles do we serve in educating families about voting? The Advisory board states that anyone should be able to vote.
Smiles for Speech: Sandy Dorsey is a certified Speech-Language Pathologist and the founder of Smiles for Speech. She is the owner of All About Speech, LLC, a private practice based in New York City that offers speech-language services to school aged children with speech and language disorders. Sandy talks about the 5K virtual race and their goal to raise funds to provide resources for the most vulnerable children with special needs living in impoverished communities in rural areas of Kenya and Ghana.

| Language and Spelling and Delays in Evaluations due to Covid | 23 Oct 2020 | 00:53:15 | |
The whole group is back together this week. Matt has a ton to unpack involving Cub Scout fundraisers (selfish self-promotion link below for Popcorn sales) and hikes, mandatory high school coach meetings, meeting Help Me Grow as a parent, and calling his local senators to fight for hearing aid insurance changes. He was told a hearing aid for a child is considered cosmetic. Michelle celebrates her eternal 24th birthday 10 years later by hiking across state lines and she is an aunt again for her 5th niece. Michael spent the weekend with his nephews for the weekend. His private practice has moved to about 50% in person and 50% tele- practice. The group also responds to the news that Charlie Brown won’t be airing on television this Halloween.
On today’s show, we discuss the backlog of educational evaluations and waiting on legal rulings and the link between language and spelling. This week’s Informed SLP looks at. We also look at our SSPOD Shoutouts and Due Process. The weekly news headlines return and the ASHA spotlight looks at what they have put out for stuttering.

They are offering up a virtual 5K marathon with the goal to raise funds that will be used to provide resources for the most vulnerable children with special needs living in impoverished communities in rural areas of Kenya and Ghana.
Topic #1: Due to the immediate closures of schools in the springtime, many children went without evaluations. The crew discuss the impact of not completing an evaluation has on timelines and the student’s success.
Topic 2: What should SLPs be doing with spelling in the classroom? A new study looks at the indepth link between spelling and language.
The Informed SLP: We all know the struggle of figuring out the “active ingredients” for aphasia therapy. We consider dosage, number of trials and sessions, home programs, aphasia type, treatment approach, stimuli, types of cues…but it’s still hard to know what makes aphasia therapy work.
ASHA Spotlight: How do we educate someone on the impact of stuttering? ASHA has a tool-kit for that.

| Over Reliance on Standardized Assessment and ABA Encroachment | 15 Oct 2020 | 01:22:04 | |
After taking a month off in celebration of Matt’s new daughter the crew returns with episode 127. Therapy conversations are happening in the group’s discord, jump in and let your voice be heard. Matt talks about bringing his daughter home from the NICU and jumps into the Hot take about failures in paperwork that happened in the NICU. He also found the magic word when discussing charting mistakes in the hospital that will get you answers, a root cause analysis. Michael has more mask to mask therapy and has seen his teletherapy numbers begin to decline. He also discusses the evolution of his speech therapy room from a multi-use room to a dedicated space. Michelle has completed her move to a new place .7 miles away from the previous place. Michelle enjoyed the weekend by taking the family across stateliness to visit a local farm.
On today’s show, we discuss ethical or unethical encroachment of ABA into the realm of the SLP and we discuss the over reliance on standardized assessments. This week’s Informed SLP looks at how daily cognition training can keep the doctor away. We also look at our SSPOD Shoutouts and Due Process.
Dr. Heidi Alaskary has been named the lead co-chair for the T20 Task Force 6 on economy, employment and education in the digital age.
SSPOD Due Process: Do you have a complaint or need to vent, then you want to participate in the #SSPODDUEPROCESS. This week, an anonymous listener asked why are therapists only using standardized assessment in decision making? Matt, Michael, and Michelle dive into the diagnostic triangle and the importance or lack of importance of using only a standardized score to make decisions. Matt talks about how he has added more in depth reporting and multiple standardized and not-standardized assessments which has caused his reports to swell to 10-16 page reports for and ETR (3 year re-evaluation).
Topic #1: Researchers look to have identified a possible genetic link for stuttering and a potential link to dopamine receptors. The next step is to trial a synthetic dopamine drug. What impact does this have on therapy and does this mean we are just treating the symptoms of dysfluency? The Informed SLP: Engaging in cognitive training every day may keep the doctor away!. Basak and colleagues found that cognitive training was equally effective in enhancing cognitive functioning in healthy adults and adults with mild cognitive impairment (MCI).
ASHA Spotlight: With ASHA Canceling its convention this year, the crew discuss future locations and the impact location will have on attendance.

| Genetics, Dopamine, and Stuttering, Poetry in Therapy, and Is Social Thinking Abelist? | 17 Sep 2020 | 01:01:27 | |
After taking a week off for Labor Day the group returns to the airwaves. Therapy conversations are happening in the group's discord, jump in and let your voice be heard. Michael continues to transition to face to face therapy in his clinic has proven to be great for him as much as it has been for his clients. Michelle must move, again, less than a month in at her current location. However, on the positive side, they hadn’t unpacked everything yet. Matt has completed his last CEU hour needed to get his ACE award and he prepares to enter the delivery room for the latest edition to his family. On today’s show, we discuss the impact genetics and dopamine have on stuttering, the use of poetry in the therapy setting, and we look at Social Thinking and break down if it’s too ableist. We also look at the news headlines including Big Brother and Blackout Drinking. This week’s Informed SLP looks at the why and how of working on writing. We also look at our SSPOD Shoutouts and Due Process.
Sandy Bennett is most deserving of this week’s SSPODshoutout. Sandy retires from speech and language therapy after contributing to it for 50 years.
SSPOD Due Process: Do you have a complaint or need to vent, then you want to participate in the #SSPODDUEPROCESS. This week, an anonymous listener sends in the following situation. A high school student working on /r/ and /l/ says the do not bother them, but teachers report that the student does not volunteer information and has cried during her presentations in March because someone asked her to repeat a phrase. The therapist states the student is unmotivated in therapy. Should therapy be continued or move to dismissal? Topic #1: Researchers look to have identified a possible genetic link for stuttering and a potential link to dopamine receptors. The next step is to trial a synthetic dopamine drug. What impact does this have on therapy and does this mean we are just treating the symptoms of dysfluency?
Topic #2: Inferential and figurative language can be difficult to target in a therapy setting. One SLP looks at adding Poetry to her therapy lessons.
Topic #3: A study from an ABA group has come up again questioning the validity of the Social Thinking Curriculum. Could there be some truth to the ABA concerns about evidence? Should we be using Social Theory to cover up communication deficits or should we follow a less ableist look at Pragmatic Language?
The Informed SLP: The Why and How of Working on Writing. Working on writing is an essential part of supporting our clients’ literacy development and research to support it.

| The Pitfalls of Teletherapy, Caregivers Perspectives, and Inclusive Halloween | 02 Sep 2020 | 01:10:19 | |
Matt announces on air that he would officially like to go back to school to get his principal’s licensure to pursue his dream job in the schools. He took his boys to learn tennis and is putting the speech schedule together. Michelle continues to unpack and has joined an outdoor socially distant workout group. On today’s show, we discuss the telehealth company that was fined for not being truthful in its ads and we talk about the importance of caregivers perceptions of SLPs, what we do in therapy, and we react to the headlines in the news involving a Gucci’s new model with Down Syndrom, dysfluency and Gabby Giffords at the DNC, and Target has new accessible Halloween costumes. Matt sat down Dr. Givona Sandiford to talk about her Health and Fitness Journey. This week’s Informed SLP looks at the role parents play in ASD diagnosis. We also look at our SSPOD Shoutouts and Due Process. The ASHA spotlight looks at what ASHA is doing to improve funding and prep for SLPs in the schools.
The ESY services provided to one of her students was done by a non-SLP and the district does not want to inform the family. What should they do?
Topic #1: PresenseLearning settled with the California Government for $600,000 related to making false claims about the effectiveness of their therapy. Families across the country have already raised concerns about their students not being served correctly; how do we ensure that our students that receive teletherapy would benefit from it.
Topic #2: Caregivers value clear and direct information from their SLPs, but we as a field are not always clear. What is the importance in using clear and defined terms when speaking to parents.
Interview: Matt spoke with Dr. Givona Sandiford about her health journey following the birth of her child. She is in the running for Ms Health and Fitness and can be on the cover of Muscle and Fitness: Hers.
The Informed SLP: Parents can effectively participate in early screenings for ASD via telepractice. The Informed SLP looks at the research associated with this.
ASHA Spotlight: We look at what ASHA is doing to advocate for safety, telepractice, increased funding, and flexibility for the upcoming 2020–2021 school year.

| Returning to Schools, Covid and Your Voice, and Med Students Performing Speech | 29 Jul 2020 | 00:54:57 | |
Michelle’s trip to Kansas was side-lined as she faces quarantine. Michael has been working with students and their families to ensure language skills continue to generalize and he found a way to end up on the Golf Course. Matt continues to work home health care and has found a new love for some clear liquors while he has been tested every 2 weeks for COVID.
On today’s show, we discuss Houston medical students being asked to be SLPs, the tell tale signs in your voice of COVID, and the return to schools for SLPS. This week’s Informed SLP Dr. Meredith Poore Harold stops by to talk about why we shouldn’t buy into the Headlines. We also look at our SSPOD Shoutouts and Due Process.
The Discord is up and ready for people to interact with the crew 24/7 with a new website, www.discord.speechsciencepodcast.com.
SSPOD Shoutout: Do you know an SLP who deserves a digital fist bump or shout out? We want to know your #SSPODSHOUTOUT, which is recognition for someone doing something awesome somewhere. Tera Sumpter submitted a Shoutout to Tina Moreno the co-founder of Camp Alec.
SSPOD Due Process: Do you have a complaint or need to vent, then you want to participate in the #SSPODDUEPROCESS. This week’s due process was submitted anonymously: How do we balance returning to schools and the safety of everyone involved?
Topic #1: Families in several states are suing to have their students IEP minutes met during the change in schooling. Tied into the Due Process for this week, how do we balance the needs of our students and also the safety of the caregivers?
Topic #2: A recent article highlighted Med-Students performing SLP services, what should have happened?
Topic #3: COVID may change your voice and may be detectable
The Informed SLP: Dr. Meredith Poore Harold stops by and talks with Matt about the importance of diving deep into the articles and not falling for just the headlines.

| Dysphagia Tools, IDEA Failures, and Counseling | 15 Jul 2020 | 01:02:00 | |
Michael and Michelle have the week off, so Leigh Ann Porter, host of Speech Uncensored settles into the co-host chair this week with Matt. Leigh Ann recounts her week in the clinical setting and the joy of seeing a patient returning to your caseload. Matt’s home health caseload continues to rise which means his free time continues to decrease. Matt and Leigh Ann discuss the tests they enjoy when working with adults. In the middle of the show, Matt had to run down a mouse in his house!
On today’s show, we look at tools for dysphagia, the importance of counseling during therapy, and which states are failing IDEA. This week’s Informed SLP looks at perceptual ratings for dysarthric speech. We also look at our SSPOD Shoutouts and Due Process. The ASHA spotlight looks at the ASHA Connect Conference.
Dr. Anthony Koutsoftas from Seton Hall University received a 1.4 million dollar federal grant for research in improving writing skills in special education.
SSPOD Due Process: Do you have a complaint or need to vent, then you want to participate in the #SSPODDUEPROCESS.
Topic #1: With the rise of 3d printing companies have found ways to create new assistive tools for feeding therapy. Obi a robot who can feed a patient to trays specially designed can help change our approach to feeding therapies.
Topic #2: A majority of states are failing to meet the minimum of IDEA. Why are schools failing? Is it directly linked to funding and will it improve anytime soon?
Topic #3: Counseling should be a bigger part of our therapy sessions, or so says a recent study. Do you find yourself as an information counselor or one that helps identify our patients’ wants and needs?
The Informed SLP: A downside to relying on perceptual ratings is that perceptual ratings alone don’t usually qualify a child for speech services or provide a normative benchmark (other than normal/abnormal). But the authors of this study have created age-specific norms and cut-off scores for perceptual ratings that can be used in dysarthria assessments of school-aged children.
ASHA Spotlight: The ASHA Connect Conference is up and online till July 20th.

| Vocabulary and Recall, Disney and Disability Lawsuit, and Patriotic Music and Memory | 08 Jul 2020 | 00:57:23 | |
Its episode 118, even if they keep calling it episode 117. During this week’s show, they plan for a longer Fourth of July Weekend. Michelle’s child turns two on the fourth this week as she wraps up her last few days before her move. Michael looks forward to some social distancing family get togethers. Matt talks about how he is terrible at using the holidays in his speech and language therapy and he will be attending a reverse parade.
On today’s show, we discuss recent research showing why some words are easier to remember than others, Disney World has won a disability lawsuit, and how music impacts memory. This week’s Informed SLP looks at tongue ties. We also look at our SSPOD Shoutouts and Due Process. The ASHA spotlight looks at what ASHA is doing to spotlight SLPs of color.
Jennifer Jayme and those who got the Military Spouse Community page up and running for ASHA receive this week’s SSPOD Shoutout.
SSPOD Due Process: Do you have a complaint or need to vent, then you want to participate in the #SSPODDUEPROCESS.
Topic #1: The national institute of heal looks at the frequency of vocabulary and why some are easier to remember. If a word has a higher frequency will they recall it easier on a screener? Does this impact the way and the vocabulary we target in therapy based on the patient?
Topic #2: Disney previously changed the way they handled customers and guests with disabilities by issuing a return time instead of allowing the guest to go to the front of the line immediately. A guest sued Disney, but a court found Disney to not be at fault. The group talks about the impact of learning to wait and how this can help.
Topic #3: Patriotic music may be linked to certain memories. How does this translate into other areas of therapy?
The Informed SLP: Talking Tongue Ties: You won’t find many hotter topics in our little corner of the world right now than tongue-ties AKA ankyloglossia AKA tethered oral tissues AKA a very short and/or tight sublingual frenulum that restricts lingual mobility.
ASHA Spotlight: ASHA is printing and spotlighting SLPs of color.

Michael’s clinic has begun to open up to in person live therapy and he continues to balance the role teletherapy has on his current caseload as well. Michelle continues to prep for leave to Kansas and looks to transition patients to new therapists. Matt is now being tested every other week for Covid-19 as part of keeping the home health staff and patients safe.

The study showed similar results with both groups.
