She became a member of the Royal College of Physicians in 1998 and a Fellow of the Royal College of GPs in 2016. She is also a menopause specialist and a member of the UK Government’s Menopause Taskforce. This mini-series explores hormone replacement therapy (HRT) after breast cancer, a controversial and emotive subject. The conversation focuses on the further understanding of risks and benefits of HRT (hormone replacement therapy) or also MHT (menopause hormone therapy), the emotional impact on patients, and the different views among medical professionals. She is a colleague of Dr. Nick Panay, whom she met while working at Chelsea and Westminster Hospital, seeing complex menopause cases. Dietician and board-certified specialist in oncology nutrition Krystle Zuniga discusses what these lifestyle recommendations are with us on the show today. Have you been told what they are? Herbal medicine is one of the most used complementary therapy by people with cancer. Yet, I feel the majority of us don’t actually know what it really is, beyond what you can pick up in a little bottle from your local health food store - how it is used and hence it becomes an under-effective therapy. I am very grateful to be joined by Anita Ralph who is a consultant medical herbalist MSc MNIMH MCPP, and Melinda McDougall, who also holds a Master of Science degree in herbal medicine and specialises in women’s health. Both of them are super passionate about helping us understand how we can use herbal medicine in the best way to support us going through menopause after cancer. Anita began full-time training to become a medical herbalist in 1987 and on graduating, became a member of the National Institute of Medical Herbalists (NIMH). She is a member of the College of Practitioners of Phytotherapy (CPP) a professional association dedicated to exploring and promoting scientific advances in herbal medicine. Anita runs a busy, full-time, multidisciplinary clinic with herbal dispensary in Kent, and she also collaborates with consultant gynaecologist, Anne Henderson, offering an integrated service to those with gynaecological problems. Did you know that exercise can reduce your risk of a cancer recurrence by up to 30%?! In a recent post in my Facebook group I asked my community of women how much exercise had been discussed with them by their health care professionals and what their biggest hurdles were. And we had a flurry of messages within a few minutes. Many echoing the same sentiment: - It’s been very difficult to get support advice - nothing from my healthcare team. - I’m struggling with fatigue and muscle pain, should I push through it? - What kind of exercises help joint pain when you're on aromatase inhibitors after breast cancer? - I have been told absolutely nothing other than given a leaflet with about four exercises on it for after surgery, and perhaps even discouraged from exercising as I need to “rest”. - Not much support at all where I am. No exercise referral programmes, no real advice given and if it wasn’t for me being a cardiac nurse I wouldn’t know anything about exercise rehab. I am delighted to speak to two fantastic experts on the podcast today. Professor Anna Campbell and Sarah Newman. Anna will talk us through the research her and her team conducted and exactly what that means for us in everyday life. Sarah will talk us through her own cancer diagnosis and how she became a CanRehab exercise specialist, now helping others become active. They both discuss how we can access some of these fantastic services and what it is we can each do to reap some of these benefits.

This meditation is for anyone, regardless of what has happened to you until now. Although you are in the home of The Menopause And Cancer podcast, I was told that many people listen to my meditations, even if they have not had a cancer diagnosis or are not in menopause. And that’s totally ok! You are so welcome! Many of us face ill health in so many different ways and this can be the most challenging time in our lives. We feel broken somehow, it comes with worries and fears and it can make us feel hopeless. Many people also have the perception that we must function at 100% to be worthy of love and kindness. That being ill somehow makes us feel lesser than. This mediation welcomes in all our feelings and pains - Allows us to notice that everything always changes - and that we are good enough just the way we are. We are worthy of love.

So I thought it would be fab to get some signposting from an expert on this subject. For many years Kate has worked within a large IVF clinic. She completed her master's in science in Reproductive medicine which specifically focused on fertility and menopause. Managing menopause after a cancer diagnosis brings with it a heap of physical and psychological symptoms, many of which can be discussed much more openly than others. Our sexual desires, loss of libido, difficulty becoming aroused, painful sex and so much more are often not spoken about. What is normal and what can I do if I don’t like that ‘new normal’? In this episode, we will focus less on all the things we can do to support our vaginal health, but we will explore sexual health and what desire means to us. What does sexuality mean to you? Hannah describes how hard that journey was and how her sudden menopause was really tricky to manage. Today's episode is different. I have no expert or wonderful survivor story for you - all you are getting is moi! Over the next few weeks and months, I will share with you some of the other bits and pieces that have helped me heal over the last few years. As a cancer survivor, I have found much light and hope in meditation and mindfulness. As a teacher, I share poems, readings and I have decided that I will share some of those with you on the podcast. So that together we can practice tapping into the present moment, away from solely focussing on wanting to find solutions. Helen Munro from last week’s podcast (listen here) has shared today’s poem with me - and it’s so wonderful I want to share it with you. WELCOME TO HOLLAND by Emily Perl Kingsley I am often asked to describe the experience of raising a child with a disability - to try to help people who have not shared that unique experience to understand it, to imagine how it would feel. It's like this…… When you're going to have a baby, it’s like planning a fabulous vacation trip - to Italy. You buy a bunch of guidebooks and make your wonderful plans. The Coliseum. The Michelangelo David. The gondolas in Venice. You may learn some handy phrases in Italian. It's all very exciting. After months of eager anticipation, the day finally arrives. You pack your bags and off you go. Several hours later, the plane lands. The stewardess comes in and says, "Welcome to Holland.” "Holland?!?" you say. "What do you mean Holland?? I signed up for Italy! I'm supposed to be in Italy. All my life I've dreamed of going to Italy.” But there's been a change in the flight plan. They've landed in Holland and there you must stay. The important thing is that they haven't taken you to a horrible, disgusting, filthy place, full of pestilence, famine and disease. It's just a different place. So you must go out and buy new guidebooks. And you must learn a whole new language. And you will meet a whole new group of people you would never have met. It's just a different place. It's slower-paced than Italy, less flashy than Italy. But after you've been there for a while and you catch your breath, you look around.... and you begin to notice that Holland has windmills....and Holland has tulips. Holland even has Rembrandts. But everyone you know is busy coming and going from Italy... and they're all bragging about what a wonderful time they had there. And for the rest of your life, you will say "Yes, that's where I was supposed to go. That's what I had planned.” And the pain of that will never, ever, ever, ever go away... because the loss of that dream is a very very significant loss. In today’s episode, I am speaking to the incredible Stacey Owen. Stacey and I met on the navigating menopause program last year which I co-curated and co-hosted with charity Trekstock. Together with a team of incredible experts and co-host Jemima Reynolds, head of health at Trekstock, we helped guide young adults in their 20s and 30s to work out a way forward, that’s right for them. Stacey’s breast cancer treatment saw her go into a medically induced early menopause, aged 30, a common but often unspoken side-effect of hormone-receptive breast cancer treatments. At first, just grateful to be out of active cancer treatment, she didn’t realise how much the menopause and its symptoms were impacting her life. She says: “I felt like I was the only person going through this – I felt very alone, it was very isolating. I hardly spoke about it to anyone.” Fast forward a year and a bit and Stacey was seen in Davina McCall’s second documentary, Sex, Mind and the Menopause. What happened in the time between a very closed and hidden experience until showing up and speaking up on national TV is what I want to get to the bottom of in today's episode. What an incredible transformation! I share Stacey’s frustration with the entire situation and the lack of discourse around the topic as a whole. She says: “I was kind of so annoyed at all women. For the fact that the menopause is so common, it’s everywhere. I am very excited to welcome Dr. Lindsey Thomas onto today’s episode of the podcast. Lindsey is a registered British Menopause Society Specialist, with the highest level of training. She can be found at the NHS specialist menopause clinic in Leeds and she is also a menopause specialist at Menopause Care. She has worked in a large inner city practice in Sheffield as a GP for almost 20 years, where much of her time has been spent working in women’s and sexual health. So I could not think of a better guest to help us understand: How to get help if we need it? Who to ask for help? And when to ask for it? Each week I get emails from women who say they have had absolutely no help. They might have been discharged after their active treatment for cancer and they feel just left. Many echo what I have been experiencing myself - and that is total confusion of what’s what? What is menopause? What is cancer? What’s lingering from the pandemic if you have been diagnosed throughout? And then we wonder who is the right person to help us? Our oncologist? GP? I have made it my mission to help empower each and every one of my listeners and clients. So that we are all more educated, more informed and hence, can make better choices for us now and our long-term health. This week I am joined by Carly Moosah, writer, coach and fellow breast cancer and BRCA1 survivor. And we are discussing one of the most asked questions I always get: “How do I befriend the new me? I don’t like who I’ve become. I just want my old pre-cancer, pre-menopausal self back!” Carly and I share a very similar history with cancer and we both arrived in menopause through surgery. But we have both (for now at least) decided to go down very different paths in managing our situations. The reason why I want to highlight this is to say: It’s OK! And to celebrate all of our differences. There is no right or wrong in managing your menopause after cancer. So much of our past and experiences muddles into our present moment and influences our decisions too. So in today's episode, I want to celebrate that it’s ok that we are all doing whatever we can so that it works for us. It is my aim for this podcast to be a judgment-free and inclusive space - whatever your choices, whatever you decide to do. There is no right or wrong. The only wrong is when women are left unsupported and undereducated - but we’ll discuss more of that in the next episode.

Annice gives so much hope and insight into the long-term treatment of menopause after cancer. She also shares with us that a new non-hormonal treatment is going to be licensed as early as 2023. This will target the actual hot flush mechanism in our brain and looks to be so promising. It is also thought it will help with mood, sleep and even weight loss. Although the license will be for women in natural menopause at first, there is hope and things are changing. Joint pain can significantly affect daily activities and diminish overall quality of life, making it challenging for you to do the things you once enjoyed doing. You might feel very frustrated that you have completed cancer treatment and now to be left with what sometimes can be a debilitating pain. We know a proportion of women even stop their endocrine treatment as the joint pain is too much to bear. Most of the time when you look for information about which herbal supplements might be useful in helping with symptoms for menopause after cancer, the advice is “Caution - there might be contraindications”! Additionally, 100% of all the people who responded to our survey on social media said they were worried about contraindications and the safety of herbal supplements. In today's conversation, we want to find out what is safe and what is not safe for those affected by cancer. Turmeric, ashwagandha, red clover and soy supplements can have a positive effect but we want to know who needs to stay away from them. I am delighted to get to the bottom of this with Dr. Vera Martins who is a naturopath and herbalist with a PhD in cancer biology and Rebekah Brown, founder of co-community MPowder, a company dedicated to providing insight as well as innovation to ensure we can all ‘meno-well’.
