There are ways of navigating the maternity care system and curating your twin birth so it still represents your preferences, even if new restraints mean you have to compromise. In this episode we use a case study to unfold the story of strategies that you can use to put as many options on your twin menu as possible. In this episode we speak to Midwife Hannah Muller and Doula Louise Gorham about their role in Natasha’s twin birth with her two breech babies. | Episode 222 - How to get hot safely in pregnancy | 27 sept. 2026 | 00:31:03
You CAN have hot baths, heat packs, sauna and do intense exercise in pregnancy. Feeling hot during pregnancy is not a risk to your unborn baby, but hyperthermia for a prolonged period could be. You may have been told by your care provider to avoid hot baths in pregnancy, however, the Finnish and Swedish women are being told by their care providers that it’s safe to sauna during pregnancy. I have a look at the research on getting hot when pregnant and how to do it safely. It’s time to enjoy heat again and I’ll explain how.

| Episode 217 - Surviving and Thriving in Midwifery | 23 août 2026 | 00:59:04 | |
We have to be honest about the reality that working in midwifery is an unsustainable role. The lucky ones land a job that doesn’t leave them mentally and morally exhausted after a day of working amongst a toxic workplace that doesn’t care for the needs of midwives. It sounds harsh, but Lynnelle Moran has been researching this exact situation and has identified the factors that contribute to unsustainable circumstances for the midwifery workforce. Fortunately, she also researched the solutions, and the good news is, you can make your work as a midwife a little more manageable while we wait for the system to change. Listen to this episode to understand the midwifery context and how to survive or thrive in it.

| Episode 213 - What if I have an epidural | 26 juil. 2026 | 01:16:42 | |
You might be one of the 60% of women who have an epidural during their labour or birth. Whatever birth you’re planning, knowing about epidurals is an absolute must. Epidurals are the most effective form of pain relief available in labour, but they are much more than just a quick injection—they come with a complete "package" of interventions, altered physiology, and downstream effects that women are rarely fully informed about.

| Episode 212 - It’s Not ‘False’ Labour, It’s Prodromal Labour | 19 juil. 2026 | 01:36:16 | |
Around 25% of women will experience a longer than anticipated early labour phase. Some maternity care providers might explain this as ‘false’ labour… but I can assure you it’s not false, it’s called ‘prodromal labour’ or prolonged early labour. You are in labour, but for some reason your body is not making visible signs of progress towards birth. Why does this happen? There are lots of reasons why you could be having a long early phase of labour… it can be completely normal and there is lots you can do to resource yourself for this labour marathon. In this episode I explain prodromal labour, what causes it and how to manage it. As an extra special treat, Poppy Child from @popthatmumma shares her story of prodromal labour. This episode is full of tips and strategies if you are sustaining yourself through a long early labour phase.

This episode gives a snapshot of the main global challenges that face women in their childbearing years and the midwives that care for them. In this episode, Mel covers what she learned from the International Confederation of Midwives conference in Portugal June 2026. | Episode 208 - Beyond 40 weeks pregnant | 21 juin 2026 | 00:59:35
The concept of a rigid 40-week due date is one of the most deeply entrenched myths in modern maternity care. Today I cast a critical eye over the historical and mathematical flaws behind how Estimated Due Dates (EDDs) are calculated, and why treating day 280 as a strict deadline creates unnecessary interventions.

| Episode 206 - The First 2 Weeks Postpartum (Part 1) | 07 juin 2026 | 01:31:29 | |
The first two weeks after birth have a lot going on… So I'm doing 2 episodes on it. Here you will learn what recovery can look like for both mother and baby. I discuss the wide range of postpartum experiences, the care gap that can exist after birth, and how support from midwives, nurses, GPs, partners, family, friends, and the wider community can help with feeding, recovery, and emotional wellbeing.
I’m also going to explain matrescence, the transition into motherhood, and the physical, emotional, and identity changes that can happen after birth. This episode covers practical early postpartum advice, including skin-to-skin contact, feeding, rest, wound healing, bleeding, swelling, afterbirth pains, and what to watch for if there are signs of infection or heavier-than-expected blood loss.
I give guidance on the first wee and first poo postpartum, as well as the importance of food, fluids, and support in the early days. The first days can feel intense, but we adjust and things usually become more manageable.

| Episode 203 - Swollen cervix in labour | 17 mai 2026 | 00:50:12 | |
If you get a swollen cervix during labour, your care provider might recommend an epidural or caesarean section. There are many other things you can try before resorting to such extreme measures and this episode covers them all. Interestingly, there are no research papers AT ALL to substantiate any of the strategies to remedy a swollen cervix so it’s worth trying them all to see what works, if you want to avoid an epidural or caesarean just because of your swollen cervix

| Episode 192 - Mel's mastitis protocol | 01 mars 2026 | 00:50:22 | |
I came up with this Mastitis protocol early in my career and it’s just always worked. For this reason I have had no inclination to update or change it, because it has proved it to be effective. In this episode I explain my protocol and provide a detailed list of what to do if you think you might have mastitis. My protocol has elements within it that are not aligned with some of the current guidelines and I explain where it diverges and why I persist in using it despite the change in the guidelines

| Episode 190 - When do I NEED an obstetrician? | 15 févr. 2026 | 01:17:07 | |
Often women are provided with too much medical intervention during pregnancy, birth and postpartum. How do we know when we actually need it? Or when it’s just part of the usual over-medicalised care routine? In this episode Mel offers some suggestions about which circumstances warrant obstetric care and looks at the research about which circumstances midwifery-led care would allow for more favourable outcomes than obstetric care.

| Episode 189 - Midwives Save Lives | 08 févr. 2026 | 00:45:46 | |
The International Confederation of Midwives (ICM) shares that if 1 million more midwives provided care to women through their childbearing years, they would be healthier, happier and fewer women and babies would be damaged or die during their childbearing years. It’s time to stop pretending that Midwives don’t make a difference to outcomes for women and babies. Help us tell governments, regulators and people in power that we want 1 million more midwives in order to save women and babies. Listen to the research and sign the petition for one million more midwives… your signature will make a difference.

| Episode 187 - Midwifery without borders with Diane Lockhart | 25 janv. 2026 | 01:09:56 | |
A rebellion can take many forms, for Diane Lockhart her brand of rebellion involved her reluctantly and somewhat accidentally opening a birth center in a Ugandan slum. Amani Birth Center has now been running for 10 years and Diane shares how they have managed to care for women without the loss of a single mother or baby at the birth center. Diane is a true midwifery rebel and she shares what you can do for Amani - there are some epic opportunities in this episode for any midwife or doula looking to innovate within midwifery or to expand your understanding of physiological birth in service of women. This is a must listen for every midwife, doula and obstetrician.
In this episode Mel spoke about the assembly of rebellious midwives. A place for woman centered midwives to come together, become better midwives together and survive the system in solidarity. Join the assembly here, Mel is in there every week to meet you.
You can learn more about Amani here: https://www.amanimamas.org/ and here. and get in touch with Diane via the website if you want to visit Amani
Diane has a BIG dream to purchase the Amani birth centre building and expand their services, click here to help make that happen with a donation

| Episode 184 - Orgasmic birth | 04 janv. 2026 | 00:40:29 | |
Orgasmic birth doesn’t mean you are going to orgasm at birth… but that certainly is a possibility when you realise how involved your clitoris and oxytocin (the orgasm hormone) are during labour and birth. In this episode Mel speaks with Deborah Pascali Bonaro about Orgasmic birth and how we are wired to feel joy, bliss and euphoria during birth. We talk about how women can tap into the possibility of joy and how to avoid things that will inhibit your ability to feel joy during birth. This episode is for anyone who wants to have a great birth.

| Episode 177 - Defending birth and midwifery in the UK | 16 nov. 2025 | 01:13:09 | |
The UK used to be held up as an idyllic maternity care system worth replicating, but in the past 10 years, the caesarean section rate has increased to over 45% and poor maternal outcomes are rising as a result. Midwives are leaving in droves, birth services that once facilitate physiological birth are closing and the whole system is becoming hostile toward normal birth. There are people petitioning to put a stop to midwifery education from focusing on physiological birth which will fundamentally change midwifery in the UK relegating midwives to obstetric nursing roles. In this episode Mel speaks to Dr Sheena Byrom and Prof Soo Downe who explain the situation and offer practical solutions.
The easiest way to help save physiological birth in the UK is to sign this open letter from @thepeoplesbirthtribunal People power = change… sign the letter to help enact change. Together we can save physiological birth in the UK.
