Hey Mama! Welcome back to Pregnancy Unpacked with Trish, your pregnancy bestie. We’re diving into weeks 22-24, where your baby’s hitting papaya size and your bump’s stealing the show! Trish breaks down baby’s big developments, your body’s changes, and the emotional weight of this in-between season. You’re carrying a lot, but you’re not alone - let’s unpack it together!

What’s Happening with Baby

  • Size: Baby’s about the size of a papaya (~11-12 inches, ~1.5 pounds).
  • Development: Lungs are producing surfactant, key for breathing at birth. Eyelids are opening, eyelashes are forming, and baby’s hearing is sharp—reacting to your voice or loud noises. Movements are getting stronger, with a unique rhythm starting to emerge.
  • Cool Moment: Trish shares a sweet memory of her son Grayson startling during an ultrasound—proof of that magical connection!

What’s Happening with You

  • Physical Changes: Your uterus is ~2 cm above your belly button, stretching ligaments and causing round ligament pain, lower back aches, or sciatica. Braxton Hicks might be noticeable (see Episode 105 for details). Shortness of breath, increased discharge (normal unless foul-smelling), and insane hunger are common.
  • Digestive Issues: Bloating, gas, or reflux? Try papaya enzymes, stay hydrated, and eat balanced meals. Check with your provider for gas pain relief.
  • Sleep Struggles: Feeling tired but not sleeping? Grab a pregnancy pillow (linked in Trish’s Amazon shop below).
  • Emotional & Mental Load: Feeling resentful, touched out, or like you’re carrying it all? It’s normal. Have those hard convos with your partner or support system

Appointments & What to Expect

  • Monthly provider visits include fundal height checks, Doppler heartbeat monitoring, blood pressure, urine tests, and swelling checks. Discuss cramps, movements, or preterm labor signs.
  • If you have cramping, backaches, or pelvic pressure, your provider may run a fetal fibronectin test or refer you to a maternal-fetal medicine specialist. Always call for fluid leaks, bleeding, or intense pressure - you’re advocating, not annoying!

Action Steps for Weeks 22-24:

  1. Prep questions for your next provider visit.
  2. Track anything weird and call your provider if needed.
  3. Observe baby’s movements - get curious, no formal kick counts yet.
  4. Start your postpartum support plan (it’s not too early!).
  5. Check in with your partner or support person about your emotional and physical needs.

You’re growing a human and showing up for life—that’s superhero stuff, mama!

  6. Understanding Your Interventions & Your Birth Rights: Like I’ve said before, we don’t just refuse willy nilly. You need to know what this intervention is for, why it is used, and when it is appropriate. Should I say yes? Should I say no? You need to know your rights to navigate this.

But there is a side to birth that many do not talk about, which has to be honored because many parents experience it, and that is the loss of their little one.

This episode is dedicated to the many babies who are born sleeping, and as I write this, I am crying because my entire life has been affected by the loss of a newborn.

Let’s sort through the myths and facts about Big Babies (aka fetal macrosomia) so you can have an informed and empowered birth experience!

Fetal macrosomia is a condition where the fetus weighs more than 8 pounds 13 ounces (4,000 grams) at birth.

Risk factors for fetal macrosomia include:

maternal age

  • gestational diabetes
  • obesity in the mother
  • family history of large babies

Diagnosis is usually made through ultrasounds or other prenatal tests, although they are often WRONG!

Hey Mama! Welcome to Pregnancy Unpacked with Trish, Labor Nurse Mama. In this episode, we’re diving into weeks 20-22 of pregnancy - the halfway mark! We’re unpacking what’s happening with your baby, your body, and those emotional shifts no one warns you about. From belly pops to Braxton Hicks, Trish is here to guide you through it all with real talk and big support. You’re not alone, and you’ve got this!

What’s Happening with Baby

  • Size: Your little one is about the size of a banana (~10.5 inches, ~1 pound).
  • Development: Taste buds are forming, baby’s tasting your meals through amniotic fluid, and hair, eyebrows, and lashes are sprouting. Their heartbeat is strong, and brain connections are helping control those tiny limbs.
  • Cool Fact: Baby’s skin is coated in vernix caseosa, a protective creamy layer you’ll see at birth. Plus, they’re starting a sleep-wake cycle (which might not match yours!).

What’s Happening with You

  • Physical Changes: Expect a sudden belly pop, round ligament pains, or painless Braxton Hicks contractions (check out Episode 105 for more on these vs. real labor). Constipation, back pain, or varicose veins might pop up too.
  • Emotional Rollercoaster: Feeling off, disconnected, or guilty for not “glowing”? That’s normal. Pregnancy is a huge identity shift. Trish shares tips to process these feelings and lean on your support system.
  • Stay Hydrated: Hydrate like crazy, mama! Water helps with constipation and keeps you out of triage.

  • It’s okay to feel overwhelmed, touched out, or out of sync with your partner. Communicate openly - weekly check-ins can build resilience.

5-Step Action Plan:

  1. Write down anatomy scan questions.
  2. Check in with your partner or support system about your emotions.
  3. Start a list for your next appointment (pregnancy brain is real!).
  4. Notice baby’s movements and enjoy them.
  5. Protect your energy - write down what drains vs. restores you.

We chatted about the postpartum routine she recommends for her patients and their support partners when they enter their postpartum hospital room.

Here are the main tips:

  • Baby eats every 2-4 hours.
  • When the baby stirs, the partner/support person cares for the baby, and Mama goes and takes care of her peri-care needs.
  • Baby gets fed
  • Everyone rests
  • You do you during postpartum (you must listen, it's too much to write out!!) But basically, you decide what you are comfortable with during postpartum recovery and do it.

Mom guilt is a common feeling experienced by many mothers. It can arise for various reasons, such as feeling like you're not doing enough for your child or feeling like you're not giving them enough attention. Here are some tips that can help you overcome mom guilt:

  1. Talk about roles, responsibilities, and expectations with your partner from the start (pregnancy).
  2. Recognize that it's normal: It's important to realize that mom guilt is a normal feeling that many mothers experience. You're not alone in this, and it's okay to feel this way.
  3. Be kind to yourself: Don't be too hard on yourself. Remember that you're doing your best, and that's all that matters. Give yourself credit for the things you are doing right.
  4. Prioritize self-care: Taking care of yourself is just as important as taking care of your child. Make sure to take some time for yourself each day, whether it's to exercise, read a book, or take a relaxing bath.
  5. Talk to other moms: Talking to other moms who are going through similar experiences can help you feel less alone. You can also get some helpful tips and advice from other moms. (Join us inside Calm Mama Society!)
  6. Focus on the positives: Instead of dwelling on the things you're not doing, focus on what you are doing. Make a list of your accomplishments, big or small, and remind yourself of them when you're feeling down.

From Dr. Tran: "Chiropractic care during pregnancy looks at the power, passage, and passenger.

The power is nervous system health and making sure power to the uterus is nice and strong.

That way, the uterus can function at 100% and contract/relax properly when it's time.

The passage is about Mom's pelvis and its surrounding structures. It’s so important that the spine and pelvis are in good alignment during pregnancy.

A misalignment in the pelvis can cause instability and surrounding muscles/ligaments to be pulled in an uncomfortable direction.

This can cause discomfort during pregnancy. Babies are super smart and will go wherever and in whatever position is comfortable for the baby.

Baby is the passenger. We always want to ensure mom is aligned so the baby is in the best position for birth. 

Then after birth, it's always good to get the baby checked as well. Baby has been in utero in awkward positions for months. During the check, I’m looking to see if there are any Torticollis, primitive reflexes intact and strong, oral restrictions, cranial shape, range of motion, and misalignments.

Checking babies is super gentle; any adjustments are with no more pressure than you’d check a ripe avocado or tomato. Getting babies checked during the first year of life is great! I like making sure babies are developing well and hitting all developmental milestones. Then if they’re not, what’s going on there and how to address it?"

A Very Simple Explanation Of The Four Stages Of Labor During Childbirth | 5507 Apr 202300:32:42

Listen in on this recording from an IG Live where Trish breaks down the four stages of labor.

The four stages of labor are:

  1. Stage One: This stage is the longest and can last up to 20 hours or more for first-time mothers. During this stage, the cervix gradually opens (dilates) to allow the baby to pass through the birth canal. Stage one has three sub-stages: early labor, active labor, and transition.
  2. Stage Two is the pushing stage when the baby is born. During this stage, mama pushes to help the baby move through the birth canal and out of the body.
  3. Stage Three: This stage begins after the baby is born and ends with the delivery of the placenta (also known as the afterbirth).
  4. Stage Four is the period immediately after the delivery of the placenta, where the mother's body changes to return to its pre-pregnancy state and the mother and baby bond. This stage typically lasts about one to two hours.

How to Prepare Your Fur Baby for Your New Baby: Dog Meets Baby | 5431 Mar 202300:32:30

Introducing a baby to a dog can be a delicate process, and it's important to take it slow and ensure both the baby and the dog are comfortable and safe. Today we talk with Dominika here are some tips to help introduce your baby to your dog:

Tip 1: Start to prepare the dog for new items coming into the home and experiences before the baby comes home.

Tip 2: Prepare the dog for alone time and separation from you at specific times, work from different rooms, and install a baby gate to separate them, so they don't blame the baby, start small, teaching them it's okay; I always come back, creating a new routine for off-limit spaces like a nursery.

Tip 3: Planning the introduction after the baby is born; having the baby on the floor is unsafe, and keeping the baby in your arms or a crib and the dog on a leash or behind a gate is best.

Tip 4: Prepare for future car rides: think about how you safely have a dog and a baby in the car.

Tip 5: Dog training is priceless, now is the time to brush up on training.

It is a powerful way to relieve tension, gastrointestinal issues, breastfeeding struggles, and many other body functions.

We recently talked to another mama, Ronnie, about her experience bonding and realized that so many mamas struggle with guilt when the bond isn't magical.

This is why I am adding some of the tips I have gathered throughout my career and from the mamas I hang with every week inside our private membership.

Tips for Bonding Foundation Before Baby is Born:

  • Talk to Baby
  • Call The Baby By Name
  • Keep a Pregnancy Journal

Here are some tips for bonding with a baby:

  1. Physical touch: Holding, hugging, cuddling, and kissing the baby helps to create a strong bond. Skin-to-skin contact is crucial for newborns, as it can help regulate their body temperature, breathing, and heart rate.
  2. Eye contact: Eye contact with the baby during feedings, diaper changes, and playtime help build a connection and allows the baby to feel seen and heard.
  3. Talking and singing: Talking and singing to the baby helps to establish a connection and encourages language development.
  4. Responding to needs: Being responsive to the baby's needs, such as feeding, changing, and soothing, helps the baby to feel secure and loved.
  5. Spending time together: Spending time with the baby, such as going for walks, playing games, and reading books, helps to build a bond and create positive experiences.
  6. Schedule Self-care for YOU!!!

Remember, bonding takes time and may not happen immediately. It is important to be patient and consistent in your efforts to bond with the baby.

Back in the day, we instructed everyone to lay on their backs, grab behind their knees (I affectionately call your knee-pit) and hold their breath while I (your labor nurse) counted to ten.

What we used to do during pushing isn't bad; it just might not be best for you or all women. And we did it the same way for everyone.

Pushing can involve either an open or closed glottis. The glottis is the opening between the vocal cords in the larynx.

Open glottis pushing involves taking a deep breath and exhaling forcefully while keeping the vocal cords open. This technique allows for more air to be expelled and can provide a greater amount of pressure to push the baby through the birth canal. Open glottis pushing may also reduce the risk of pelvic floor injury.

Closed glottis pushing, on the other hand, involves holding one's breath and contracting the abdominal muscles while closing the vocal cords. This technique can create more pressure, but it also reduces the amount of oxygen getting to the baby and may increase the risk of pelvic floor injury or damage to the perineum.

Some studies suggest that open glottis pushing may be associated with shorter pushing times, less perineal trauma, and a lower risk of instrumental delivery than closed glottis pushing. However, the optimal pushing technique may depend on various factors, such as maternal fatigue, the baby's size, and the baby's position in the birth canal.

It's important to note that both open and closed glottis-pushing techniques have potential benefits and risks.

You should do your due diligence by taking a full comprehensive birth class and then listening to your gorgeous body during birth.

Bonding with Baby isn't Always Easy: Ronnie's Birth Story | 4924 Feb 202300:37:05

Today I am chatting with a new mama who found me at the end of her second pregnancy. Ronnie is married to her high school sweetheart and mama to a son and a daughter.

Ronnie had a traumatic cesarean in 2020, leading her to seek a VBAC birth when she was pregnant again.

Ronnie sent me this lovely email, "You were the only birth-related blogger/podcaster that didn’t make me feel ashamed for being so uneducated about and traumatized by my first birth. I felt so broken after my traumatic c-section. I thought my body was a failure, I was weak, and my babies were just too big. Then…you. You and my (new) very supportive doctor gave me the confidence to jump in head first and pursue a VBAC. Thank you. I feel whole again. I found you, and you gave me the confidence I needed to push through! I had a very redemptive VBAC last November…and I feel like a new woman!"

We chatted about her VBAC birth experience and how she was surprised that her bond with the baby didn't happen off the bat. Mama, this is normal and doesn't mean you are a bad mom.

When you first bring your newborn home, it can be hard to figure out what to do. Here are some tips to help you and your new baby bond more easily.

It's normal to feel like you're not bonding with your newborn.

You're exhausted, and it can feel like your baby is just an extra thing you have to care for in addition to your other responsibilities. The most important thing you can do is give them a safe environment, so they feel secure and loved.

Make sure that you and your partner have time together with the baby. This will help you feel connected to each other and ensure everyone meets their needs.

Don’t be afraid to ask for help. It’s important to remember that this is a big change for everyone—for you, your partner, and your family members—and there are many ways people can get support when they need it.

If something isn’t working for you or someone else, don’t hesitate to reach out!

Here are some tips and tricks to help you overcome the initial hurdles and enjoy this incredible experience.

1. Take time for yourself.

It's crucial that you set aside time for yourself so that you can recharge your batteries and come back to parenting with a fresh perspective.

2. Have realistic expectations about what kind of parent you will be.

Don't expect to be perfect at first—or ever! You won't have all the answers, and it's okay not to know exactly what you're doing. If your baby is safe and healthy, everything else will eventually fall into place!

3. Accept help from others when it's offered (and don't feel guilty about taking advantage).

Accept offers of help when your friends and family suggest them--especially when you are so tired that you feel you can't do anything right. People who know you well will not be offended if you ask them to hold your baby while you shower or if they take the baby while you go to the store for diapers. People who love you want to help, and this is an excellent opportunity for all of you to get to know each other better!
