Back Pain in Kids, Spondys, Spondylolysis & Stress Fractures With Angela Jackson06 Jul 202300:40:15

Angela Jackson, specialist paediatric muscular skeletal physiotherapist joins the podcast again to talk about all things spondy
Spondylolysis is one of the most common back pain injuries seen in child athletes and although common it's vitally important that these injuries are picked up and The Back Pain Podcast is helping to raise awareness with Angela explaining how a stress fracture is very different to a full fracture and how they can be picked up along with the actions that need to be taken

KEY TAKEAWAYS

  • Lower back pain in a young athlete is an inconvenience that needs sorting out but the majority go back to sport without any problems
  • These injuries don’t occur in the non-athlete population. Whatever level it’s at, it needs checking
  • If you have an experienced clinician they should be able to manage it without imaging
  • A graduated return to activities is what’s required, we want to have an adult athlete who can withstand the demands of adult sport
  • We need to help them to see good credible role models who have come back from this injury
  • Working in phases helps young people see what the journey is back to health
  • It’s about getting them back into sport layer by layer, making them stronger and educating them

BEST MOMENTS
‘It’s not that kids are doing too much, it’s that they are doing too much too soon’
‘We only get this opportunity to make you stronger once’
‘Find a hero in their sport that you know has come back from this injury, loads of people have gone through this’

Relative Energy Deficiency In Sport (RED-S) With Dr Clodagh Dugdale03 Mar 202300:54:30

Dr Clodagh Dugdale Consultant in Sports and Exercise Medicine working at the Spire hospital in Cambridge discusses relative energy deficiency in sport

Learn what RED-S actually is, the signs to look out for and how it’s a syndrome that affects not only elite athletes but also recreational athletes and other cohorts

This is a must-listen for anyone from professionals to weekend warriors in understanding the symptoms of RED-S and raising awareness

KEY TAKEAWAYS

  • It’s a mismatch of energy intake and energy expended, it’s a low energy availability status
  • There is a normalisation of certain symptoms such as a lack of periods and rates of depression and anxiety are much higher in those who have RED-S
  • Coaches and trainers are the key because they see people every week and would see the changes
  • A lack of periods can be an early sign of RED-S and should be investigated
  • It’s can be about both the workload and the dietary intake and a multi-disciplinary approach is the most effective way to treat RED-S
  • People with RED-S are often post viral most of the time and low in iron
  • Currently, it's about raising awareness about it with training and education
  • Any treatment must be bespoke as the time and types of food are also factors in taking in enough energy
  • There are many external factors as well as internal factors that need to be taken into account

BEST MOMENTS

‘We need to raise awareness of all the symptoms throughout all levels of sport’

‘If someone has reduced periods they should go and see someone as its most likely  a result of low energy availability’

‘There are a lot of doctors who don’t know about this’

Do I Need an MRI for My Back Pain?20 Nov 202001:00:17

Dave and Rob are with physiotherapist Adam Dobson discussing scans and in particular how MRI scans can and are used in connection with back pain.

Scans are important but only for a small number of patients with back pain and Adam explains in detail the pathway and context that may lead to a scan being done along with reasons why a detailed clinical diagnosis is so important in this episode packed with great information and content.

Its ultimately about the right scan at the right time and this is a great opportunity to find out more.

KEY TAKEAWAYS

  • 90% of back problems are not diagnosable on a scan and a scan will only be important for a small number of patients with back-related problems.
  • Scans can be used to rule something out such as a fracture, cancer or an infection and may open up a change of treatment plan.
  • If there is a good quality working diagnosis then imaging can create a potential additional step before treatment begins.
  • Patients should know what a scan is likely to show, the implications of the findings and what it might change.
  • Scans are highly detailed and show lots of things that may not be relevant and are common in lots of the population. These things are not predictors for pain or function in the future.
  • Patients should be prepared and educated to help them understand imaging.
  • If you’ve had a problem for less than 6 weeks, the nerves are working and there are no indicators for serious pathology there is a high chance your problem will resolve itself over time.
  • It’s about the right scan for the right person at the right time
  • The picture can change - If you are told you don’t need a scan it doesn’t mean you will never need one.
  • If you have persistent and disabling pain there is something wrong but it can be about the factors involved and where best to focus effort with therapy and support powerful options.

BEST MOMENTS

‘If it’s is unlikely to change the treatment plan and there is no evidence for serious pathology then there is a strong argument that imaging is not required’

‘An MRI scan can be used as a reassurance about what’s not there’

‘In collaborative working with the patient imaging is a malleable decision’

Discs Don’t Slip!06 Nov 202000:51:13

David Poulter is a physical therapist working out of Minnesota in the United States and shares great information about how dynamic our interbody joints, our discs, are effectively adapting to movement and load.

He explains how discs are structured and work with the interbody joint getting its nutrition from loading and compression.

With his extensive expertise and experience, David provides an in-depth and comprehensive explanation of both the construction and function of these interbody joints and shares why the truth is that discs don’t slip.

KEY TAKEAWAYS

  • The disc is the joint between 2 vertebrae. Where one vertebra joins another there are complicated structures.
  • It’s the bulging of the material in the disc that causes people to think its slipping.
  • The outer aspect of the disc conveys information about motion, position and space.
  • In the centre of the disc, there are no nerves because it’s all about compression but there are pain structures on the outer layers of the disc structure.
  • Injury can be caused by doing something too quickly or doing something for too long.
  • Treatment should be based on the patient and their individual history.
  • After an injury in rehab, you need to make sure you don’t back to something for too long, too strong or too quickly.
  • Rehab should follow the process of unloading,  temporary rest then progressive loading to return people to where they were before.
  • Sitting, standing, bending are all normal and taking yourself away from your normal is more likely to result in harm to your discs.
  • Discs are developing and dynamic as we grow and if you do something long and often and the disc will adapt.

BEST MOMENTS

‘Symptoms are a warning system to tell us to change what we are doing’

‘These are just joints and they need to be returned to graded function’

‘A disc gets its nutrition from movement and loading’

Ice or Heat Which One Should I Use?22 Oct 202000:17:11

Rob and Dave discuss one of the questions they are asked the most ‘I’ve got back pain should I use ice or heat?

There’s lots of conflicting information out there but the answer is it doesn’t matter, neither will cause you any harm it’s all about working out which one is best for you and will provide the relief you are looking for from your back pain.

Rob and Dave explain how often you should apply relief treatment in this reassuring episode that explains exactly how heat and ice work and why it’s all about working out which one is the one that works for you.

KEY TAKEAWAYS

  • Whatever makes you feel better is the treatment to continue to use and if it stops providing relief then try the other.
  • You should be trying either ice or heat to test it, moving about and continuing to use the relief treatment that is best for you.
  • If someone is suffering with severe pain and cannot move the recommendation could be for ice first.
  • Ice can help to numb pain and relieve sciatica if you apply it directly to the area.
  • The relief treatment can be repeated for 10-15 minutes up to every hour whether its heat or ice.
  • Topical heat from products can work but they will not have as much impact as ice and heat.
  • Reusable ice and heat packs can be helpful when you are at work or away from your home.

BEST MOMENTS

‘Try ice and if it doesn’t help then try heat’

‘Each to their own it’s about whatever works for them’

‘You can interchange between ice and heat if it works for you’

Many people  (10% of the population) suffer from chronic widespread pain and in this episode, Rob and Dave share the best takeaways and nuggets of advice from previous interviews that they themselves use regularly.

This a great opportunity to hear the best takeaways from a range of great professionals who have been guests on the podcast and shared great practical advice.

KEY TAKEAWAYS

  • When there is pain in the back this is inflamed tissue that is reacting and changing in the same way as other tissues in your body.
  • Even when the source of the irritation has been removed the tissue will continue to be inflamed and will take some time to settle down and reset.
  • Work on stability and mobility to create the strongest back possible.
  • You need to strengthen and work on movement in a controlled environment.
  • It’s part of being human having to pick things up and you can practice a movement to develop strength.
  • We rest, de-load then build movement back up again.
  • Movement is key and a positive mindset will make a difference.
  • Muscles need movement and bones and discs need loads.
  • If something doesn’t work it just means you need a different approach and a different professional.
  • Patients who are proactive in their recovery always recover better than those who are not.

Avoiding Training Injuries & Myth Busting With The Endurance Physio08 Oct 202001:08:36

The Endurance Physio, Mike James joins Dave and Rob to look at how to avoid injury, train successfully for the events you want to take part in and share some myth-busting in this fascinating episode.

Mike explains how anyone can train for and take part in endurance events if they understand how to be consistent and what their starting point is.

He discusses the most common types of injuries and why it’s vital to put in the preparation and perfect each step of training consistently before reaching for the marginal gains along with great advice on exercises that will make the difference

KEY TAKEAWAYS

  • You can be either a robust or fragile athlete. Those who are fragile athletes just need to be more consistent.
  • People pick up the most injuries because they attempt to nail marginal gains before they have perfected the first 95%
  • In endurance athletes 75% -85% of injuries will be lower limb, soft tissue non-traumatic and overuse, the other 25% will be traumatic, upper body associated with falls.
  • With tissue it’s not the load, it’s the load you are not prepared for. People sometimes pick up injuries through doing something different.
  • With endurance sport, any treatment will be about preparing for load. Sometimes you need to stop but it is usually about modifying the movement.
  • Having a wide range of activities will benefit backpain.
  • Every patient I see can go on and self-manage.
  • Changing too quickly can cause overload for endurance athletes.
  • Most problems with endurance athletes come from being too keen, to eager and naïve.
  • Coaches can make a difference if you get a good coach with the right program for you.
  • Cardio is king but the secret is the timing of training across the year.
  • It’s about creating stability, robustness and different load through the tissues
  • You can only benefit from the training you are resting from and you can improve recovery by making sure the training is of the right intensity.

BEST MOMENTS

‘Most people get injured through simple errors and lack of consistency’

‘People try to accelerate through their apprenticeships’

‘Endurance athletes have poor beliefs about it all being about the amount of time and miles they do in training’

What Should I Do When my Back Spasms?01 Oct 202000:24:21

If you have regular back spasms what should you do?

This is frequently asked in clinic and in this episode, Rob and Dave explain what you should do and why initiating small movements that can be adapted through increased load and repetition are the key to improve your range of movement.

Pain or hurt doesn’t equal harm and it’s important to listen to your body and remember that the spine and body are designed to move so having a full range of movement and maintaining it is key for the human body. This episode includes some great advice on movements you can make that will make all the difference.

KEY TAKEAWAYS

  • Someone who is in a back spasm with 10/10 pain and not able to walk should try to make small movements and increase them.
  • It’s about initiating a movement that uses something else, such as the wall or the bed to take your weight and increasing the movement repetition by repetition.
  • Just because you experience discomfort it doesn’t mean you should stop.
  • Approach the point of pain, back off and you will find you will be able to achieve more with each movement.
  • Little rotations and movements will make a difference.
  • With gentle movement and motion, it’s not about a set number of repetitions, it’s about moving little and often
  • If you undertake a movement that makes your body feel like it’s about to spasm you need to undertake a similar movement that puts the spine in a similar position without the load and potential threat.
  • Pain or hurt doesn’t equal harm.
  • The spine and body are meant to move so having the full range of movement is very important.
  • Maintaining an active range of motion is vital.

BEST MOMENTS

‘Listen to your body it will tell you when it’s too much’

‘The first repetitions will be sore but they will become easier’

‘Gently, gently along with little and often’

When Should I Push Through Pain?!25 Sep 202000:19:25

When you experience pain during any activity you will question whether you should push through the pain or stop and in this episode, Rob and Dave share great advice on how to return to activities or exercise safely.

People’s pain threshold is subjective and movement is an important part of the journey to recovery but it’s about starting back in the right way and understanding when you are experiencing increased pain. This episode is a great opportunity to learn more about when and how you can get back to the activities you love.

KEY TAKEAWAYS

  • Depending on your pain threshold you should never continue to exercise if the movement is causing increased pain.
  • You have to be aware of the 24hour pain response.
  • If you can still function and the pain is manageable then exercising could continue.
  • Any indications of nerve pain will mean a reduction in activity.
  • Everything is relative to your normal level of activity.
  • If you are returning to sport or exercise it’s about building back up to previous levels.
  • If you can’t run then do hill walking or have a plan to alternate between running and walking to begin the process and then build on this.
  • Begin with a decrease in the load on the body either through time, physical weight or number of repetitions.
  • Building resilience will increase your threshold.
  • Rest is relative, when you are told to rest it’s about taking a rest from the aggravating activity, you should still be maintaining movement.
  • Early movement is very important in the journey to recovery.

BEST MOMENTS

‘You have to return to any activity with common sense’

‘Activities such as gardening are weighted lower force and this is undertaken over a longer time so take regular breaks’

‘If you practice you can increase your tolerance and decrease the potential for problems in the future’

How can you get a decent night’s sleep when you have back pain or sciatica?

Dave and Rob provide some great tips and hacks to help anyone with back pain get through the night.

Any pain, including back pain and sciatica, can seem much worse at night when there is nothing else to focus on but there are things you can do and the easily followed advice in this episode will help you to achieve better sleep and good quality sleep always means better recovery.

KEY TAKEAWAYS

  • If you are a side sleeper placing a pillow between your legs stops rotation of the spine.
  • If you are a back sleeper place a pillow under your knees creating a bend and altering the angle of the spine.
  • A long pregnancy pillow can help you to elevate and change your leg angle often relieving sciatica pain.
  • Using a mindfulness and meditation app, like Headspace can help you to take your mind off the pain.
  • If there is an activity you do that alleviates pain, complete it just before going to bed.
  • Sometimes moving around in the night and then returning to bed can help you sleep.
  • Don’t forget about pain killers – you can take them when you wake at night and there are painkillers available to take at night.

Assessment, Pain Pattern Recognition and Specific Intervention with Prof. Stuart McGill11 Sep 202001:15:18

Rob and Dave are with Prof. Stuart McGill a world-leading expert in spine and back pain.

He takes a different approach to other clinicians and through assessment and identifications of patterns is able to test very specifically to identify the problem.

People often think that back pain is something you have to live with but in this episode, Prof. McGill discusses why a detailed assessment and understanding lifestyle and the pain patterns is key in being able to make a precise diagnosis and create hacks that make the difference.

This is a fascinating and detailed insight into how Professor McGill works and why it’s so successful, with great content and value helping anyone to understand more about their backpain and the positive ways forward.

KEY TAKEAWAYS

  • The primary cause of back pain is a stress concentration that exceeds the tolerance of that particular area.
  • People create stress concentration by the repetitions of activities that they complete.
  • If you have no load and no stress your body doesn’t thrive, you need an optimal amount of loading and stress but you cannot cross the tipping point
  • We create a very precise understanding of the problem and then look to build hacks to get around the issue.
  • We stop using the problem area to desensitise it then work to build it up to meet the required demand.
  • The spine is a flexible rod but when we carry load on it the demand changes and its necessary to use the support of the three-dimensional muscle girdle.
  • There are different layers of stability and there is always an appropriate stability for what you are doing.
  • The key is to coach the client to achieve sub-clinical symptoms.
  • Back pain changes as the tissues evolve and adapt.

BEST MOMENTS

‘Biology takes time for adaptation’

‘We use pattern recognition to converge on exactly what to test’

‘We investigate the person’s life to see if their body can meet the demands of their life’

The Best Life Hacks for Back Pain03 Sep 202000:38:24

In this episode, Rob and Dave are with Tom White, Chiropractor and they are sharing

the best hacks that can be used on a regular basis for back pain.

These tips and tricks can be incorporated into your daily routine to help with the daily activities and routines that everyone has to complete.

These life hacks for back pain will help you to manage the pain and are the recommendations that Rob, Dave and Thom share most often with their own patients.

KEY TAKEAWAYS

  • Using a bin bag on your car seat allows you to pivot on the car seat, means there is less friction on your lower back.
  • Using a 90-degree angle when getting up from the bed will help alleviate back pain.
  • Getting in early movement and steps will reduce stiffness and early morning back pain.
  • When you are playing golf check what’s in your bag before you begin playing and only take what you actually need.
  • If you are a golfer doing a reverse swing as part of your warm-up can help with stiffness.
  • Change the position you sneeze in by arching your back.
  • Use the wall to lean on when cleaning your teeth so that your hand reaching forward to the wall takes some of the strain.
  • Bring your foot up towards your knee to help you put socks and shoes on.
  • When standing in a queue adopt a split stance with your feet, one foot slightly in front of the other.
  • Everyone is different and their back pain will be different so you need to do what is right for you.

BEST MOMENTS

‘Getting in and out of cars is a big deal if you suffer from back pain’

‘Listening to your body is very important’

‘Try and change the way you do the activity’

Go Inside the Treatment Room – Weight & Back Pain – Correlation Doesn’t Equal Causation20 Aug 202000:05:23

Rob and Dave are  inside the treatment room in this episode discussing the most frequently asked questions about back pain and sharing great answers and advice.

‘Does carrying extra weight result in back pain?’

Hear about causation and correlation and why it’s not true that additional weight causes back pain but there is correlation and the changes made to positively impact on weight will also impact on back pain.

KEY TAKEAWAYS

  • Correlation is two things happening at the same time.
  • Causation is where one thing will directly affect another.
  • Back pain is not caused by weight but back pain and weight gain often go hand in hand because of a lack of activity.
  • Back pain can be the result of a lack of activity.
  • Weight gain can be the result of a lack of activity.
  • Back pain and weight gain have a correlation to each other.
  • To reduce weight an individual can undertake movement activity and this will also positively impact on back pain.
  • The two are linked by a common factor but they are not directly caused by one another.

BEST MOMENTS

‘Correlation is when it’s likely that two things will happen at the same time’

‘Being swallowed up by the sofa watching Netflix reruns will certainly aggravate your back but getting back to the gym will decrease back pain and your weight’

Strength Coach for Arsenal - Dominic May06 Aug 202000:51:01

In this episode, Rob and Dave are talking about back pain and strength with Dominic May, a strength and conditioning coach who has worked with sportspeople around the world and is currently best known as the Arsenal Academy’s strength and conditioning coach.

Dominic explains exactly why it’s vital to have balance in the body and how strength underpins every movement, enabling the individual to effectively hold a core position and complete all other actions and movements.

Dominic provides a fascinating insight into his work with athletes and shares his wealth of knowledge about how you can improve your strength and condition at home.

KEY TAKEAWAYS

  • Strength underpins all athletic movement and athletic skills it allows you to hold a core position and then pivot, move and complete an action such as kicking
  • The body works as a connected system what happens in the back impacts elsewhere.
  • If there is an issue in a particular joint or muscle the body will compensate elsewhere.
  • If a bone has too much load it can result in a stress injury so load management is crucial.
  • We break down an athletes movement pattern into dicentric and concentric qualities.
  • What is the force they can absorb and tolerate and what is the force they can produce?
  • If you have a role that involves you working in an asymmetric way it’s important for long term health to undertake exercise to bring balance across the body.
  • Core training should include;

-Moderate force endurance training designed to stabilise the spine

-High force static training

-High force transference training

  • It’s all about multi-plane exercise – front, back and sides and anyone can use these principles to improve their strength.

BEST MOMENTS

‘Players are getting stronger and fitter every year’

We typically want asymmetric symmetry to be less than 10%

‘It’s vital to have balance in the body’

The Best Exercise for Back Pain Feat. Associate Professor Dr Daniel Belavy30 Jul 202000:42:04

Note: In the episode Daniel is introduced as 'Assistant Professor', this is incorrect. His official title is 'Associate Professor Dr Daniel Belavy'. We aplologise for this innacuracy.

Dave and Rob are with Associate professor Daniel Belavy, one of the authors of a new paper recently published in the British Journal of Sports Medicine which addresses the ultimate question ‘What exercise should I be doing for my lower back pain?’

They discuss how network metanalysis was used to compare different types of exercise and the parameters that mean this research can be effectively used to understand which type of exercise should be undertaken by those with chronic back pain.

KEY TAKEAWAYS

  • We used a methodology analysis called network metanalysis to compare different types of exercise.
  • This research focused entirely on bringing together studies that were about exercise as a treatment for lower back pain.
  • The mode of exercise varied from Pilates, stabilisation, to stretching and extension work.
  • Active exercise therapies are likely to be the most beneficial for back pain
  • Helping people to reduce their fear of movement is a big part of the journey to recovery.
  • Building confidence is part of the process in the treatment of back pain.
  • The analysis focused entirely on comparing active exercise treatment data in relation to chronic non-specific lower back pain.
  • The results of the study indicate that modes of exercise such as Pilates, resistance, aerobic, and stabilisation are the best types of exercise to undertake for lower back pain.

BEST MOMENTS

‘There are no exercises that are harmful for the body’

‘Active exercise therapies are likely to be the most beneficial for back pain’

What Is Sciatica?23 Jul 202001:30:02

Tom Jesson is a physio who has a particular interest in sciatica and in this episode, he explains exactly how your nervous system works and what sciatica is.

He chats to Rob and Dave about perceptions and what you can do if you have recent onset sciatica along with the possible ways forward for those who experience chronic sciatica. Tom shares detailed but very accessible information about sciatica and what to do in this episode packed with valuable content.

KEY TAKEAWAYS

  • The Sciatic nerve is the large nerve that runs down the back of the leg and the term sciatica is used to describe any pain connected with this nerve.
  • All the nerves in your body are connected in a network and are very strong but also very sensitive.
  • Once a part of your nervous system is sensitive then a range of different things can cause pain.
  • The pain that people experience can be very different.
  • Radicular pain is a term used to describe nerve root pain.
  • Radiculopathy refers to a problem with the nerve root that stops it working properly
  • A nerve tells you what is going on in the world but if it is irritated or damaged it may not send the signal.
  • It also sends signals down from your brain to activate functions in the body.
  • Loss of function can be caused by damage to the nerve.
  • If someone has recent onset sciatica then it’s important to be optimistic because the body will heal if you look after your general health.
  • Movement is good for the body and nourishes through increased blood flow to all areas.

Back Pain Myths - Sam Spinelli16 Jul 202000:59:03

Sam Spinelli is a hugely experienced strength and conditioning therapist based in Canada and in this episode, Rob and Sam explore some of the commonly held myths about back pain and its causes as they discuss posture, position, flexion and stretching.

Sam shares some great practical tips about movement and exercises that will have a positive impact along with detailed information about the research and data behind the work that he undertakes with his clients.

KEY TAKEAWAYS

  • Posture is multifactorial there are many reasons why you may be in a particular position or postures.
  • Different positions and postures regulate the stress on different tissues.
  • The times where stress exceeds the capacity of the tissues is the time when an individual experiences pain.
  • Changing posture can be about relieving stress on tissues and as humans, our bodies thrive on changing posture frequently.
  • In the workplace, there is no particular posture that is more likely to cause back pain.
  • When you lift the effects are determined by your familiarity with the position and posture and how often you are in that posture.
  • Your spine adapts to what you are doing.
  • You have to bend and flex as part of daily movement so preparing to do it can be beneficial.
  • Slow repeated movements can improve flexion.
  • There are options that provide better results than static stretching.
  • If you are looking to achieve a meaningful lengthening of muscle resistance training is the best option.

BEST MOMENTS

‘Start crushing life’

‘A static bent posture does not show a consistent relationship with more back pain’

‘Often there is a lot more to peoples posture than just a worry about back pain’

What is Arthritis/Degenerative Disc Disease (DDD)09 Jul 202001:00:42

We chat to Jack March, a Specialist Rheumatology Physio about his role in practice as an Advanced Level Physio at Nuffield health, alongside that he is also Project lead of The Physio Matters Podcast and has lectured extensively on Rheumatology, arthritis and Inflammatory Back Pain.

In this episode, we chat about what Arthritis actually is and the ways in which exercise and lifestyle can help to optimise your body system.

It’s a huge topic and Jack shares some great advice about joint inflammation what any individual can do to optimise and access the right resources at the right time.

KEY TAKEAWAYS

  • The term arthritis means joint inflammation, Osteoarthritis is the most common form with the joint ageing over time.
  • Anything that increases the resting level of inflammation slightly to a higher level will then accelerate the age-related changes to your joints.
  • Everyone’s joints deteriorate but it is individual in terms of when it happens and whether there is pain and osteoarthritis.
  • Osteoarthritis isn’t about developing a disease it’s about altering the speed of the normal ageing experience.
  • You have to take people on their individuality and if you have pain the treatment is the same as the treatment to reduce the risk of it happening.
  • Your genetic component is linked to your predisposition and pain threshold response.
  • If the issue isn’t keeping you awake at night and it isn’t reducing the distance you can walk to a great degree that is restrictive then a joint replacement is not the right solution.
  • You need to optimise your body system and exercise is anti-inflammatory in its own right.
  • The biggest change comes in changing the body system through exercise, dietary and lifestyle choices.
  • Exercise has many beneficial effects including; the anti-inflammatory component, improvement of the joint and weight loss.
  • The short term fixes don’t work whilst the longer-term fixes do work you just need to be consistent and persevere.
  • If your symptoms are mild then seek early advice from a healthcare professional who can help you on the right path moving forwards

BEST MOMENTS

‘Some people can have a small change to the joint and lots of pain whilst others could have a large change to the joint and very little pain’

‘There are 5 types of different arthritis but hundreds of different conditions that can cause it’

‘It’s always about treating the person’

Spinal Surgery Success Stories02 Jul 202000:31:05

In this episode, Jack Barton, who works in construction and is also a CrossFit Coach shares his experience of surgery and recovery. After his back injury impacted on the movements he could make and he took the decision to have Microdiscectomy surgery to resolve the issue.

He discusses the process leading up to the decision and the surgery along with what it was like in the 6 week initial recovery period and beyond.

This is a good opportunity to hear from someone who has been through a surgery, undertaken the rehabilitation and returned to an exercise regime successfully.

KEY TAKEAWAYS

  • CrossFit is functional based training and includes weights, gymnastics and metabolic conditioning so the movements are varied
  • I tried physio and went through 2 rounds of injections which had little impact and were uncomfortable before deciding on surgery.
  • It was challenging to accept the need to have surgery and know that as an active person I would have to go through a 6-week recovery period with a restricted exercise regime.
  • The course of physio was based on stretches and as a fit person, I was able to successfully move through the recovery sequence fairly quickly.
  • My biggest fear when I started to exercise again was that there would be a relapse.
  • I took things slowly and benefitted because I was healthy and had a high level of fitness, I also understood my body and knew how to move.
  • I’m 85% better than I was and if I could adhere daily to the stretching and other exercises I would be even better.
  • Good body awareness makes a huge difference along with understanding how to move and stretch your body.
  • If you are considering surgery you need to understand that the better you go into surgery the better you will come out of it.

BEST MOMENTS

‘I knew the difference between gym aches and pains and pain or an ache that is an indicator of something to worry about’

‘You don’t have to stop doing exercise you just have to vary what you are doing’

‘You have to go into surgery as strong as you can’

Crossfit and Back Pain25 Jun 202001:10:45

In this episode, Rob Crowley Chiropractor and CrossFit Competitor talks about the value of CrossFit as both a fitness methodology and a competitive sport.

He shares what it means to an individual who trains in CrossFit in addition to their working week and how professional CrossFit elite athletes work towards their goals and how exposure to movement and load is the key to building resilience, strength and endurance.

Rob reflects and shares great advice from all angles - as a chiropractor, physiologist, CrossFit competitor and CrossFit coach. Cross fit can be hard to define and this is a great opportunity to understand more about how and why CrossFit continues to grow in popularity year on year.

KEY TAKEAWAYS

  • Load, movement and variety of exercise is very important to decrease back pain and keep people healthy.
  • CrossFit is designed to be constantly varying functional activity performed at high intensity across times and domains.
  • CrossFit has moved from the domain of athletes to being within the reach of the average person and this brings unique challenges for those individuals in terms of mobility and what they can achieve early on.
  • The ability to coach complex and varying movements is about scaling for the differing abilities and experience.
  • You need to understand the phases and stages of each lift to successfully build the necessary skills.
  • There is a difference between CrossFit as a fitness methodology and CrossFit as a competition sport.
  • In CrossFit, there is a paradox with movements where your lower back needs to be very stable such as weight lifting but there are also gymnastic movements that require the lower back to be very mobile and they might be in the same workout.
  • The same treatments work with athletes and the average person but often the average person doesn’t feel they need the level of care an athlete has.
  • Bodies aren’t fragile there isn’t a bad movement it’s having the strength, endurance and skill to perform the movement.

BEST MOMENTS

‘I’ve never avoided loading my body and over the years this is what has made me resilient’

‘We need to not make people afraid of lifting things it’s what we are designed to do’

‘A CrossFit workout will always feel about the same, the amount you feel f#cked afterwards will change as you get fitter and recover quicker’

Building the Golf Athlete - Shane Lawlor Sports Chiropractor18 Jun 202000:58:17

In this episode Shane Lawlor one of Europe’s leading golf performance therapists discusses the range of issues he sees repeatedly in his specialist area, golf and the solutions that he is able to offer.

With extensive expertise and experience in the treatment, screening and rehabilitation of golf injuries Shane has travelled the world with professional golfers and has worked with athletes at all levels.

He shares the methods and approaches that make a difference in physical performance and discusses what it really takes to build an athlete.

KEY TAKEAWAYS

  • The pros get back problems because they hit too many balls at speed and the amateurs get problems because they don’t rotate in the right way.
  • In the treatment room, a selection of movement screens are used to work out what the problem is.
  • We know now even if you have  back pain you need to be moving around. It’s about putting evidence and experience together.
  • Back problems for amateur golfers are usually focused around hip mobility.
  • Golf is a close chain movement where your feet are planted and then your hips are moving over them.
  • The rotation should come from the hips but if this doesn’t happen then the strain is on the back.
  • The type of disc and facets in the back can determine whether someone is suitable for a particular sport.
  • When you are building a golfer, you are building an athlete. Early specialisation of sports can be a problem because you overload on a particular pattern very early on.
  • If you can get the core working effectively then this lessens the strain on the back.
  • Extra mobility doesn’t mean you are more stable it’s all about control. You can’t add length without adding strength and you are mobilised by strengthening it’s important to stay strong.
  • It’s a very diverse sport and no 2 swings are the same so you should never try and copy someone else’s swing. What makes someone a pro is their ability to get around their own limitations.

BEST MOMENTS

‘If you build your mental game you can make it your reality’

‘If you can’t hit a ball 300 yards or more then you are at a significant disadvantage so young players are always chasing speed as this means more distance’

There is no best coach it’s the coach who best suits the person’

Chronic Pain with Dr. Derek Griffin11 Jun 202001:05:28

Derek Griffin specialist musculoskeletal physiotherapist with expertise in chronic pain disorders is talking about chronic pain and how pain is understood on both an individual and societal level.

There are many factors that influence an individual’s experience of pain and Derek explains the importance of listening to the patient and bringing understanding to the narrative.

Everyone will seek to make sense of their pain and understanding the many factors that interact to not only influence the pain but determine the response can help in the management and treatment of individuals.

We are subject to many societal beliefs about back pain from early on in our lives but the narrative can be changed if we are prepared to listen.

KEY TAKEAWAYS

  • Chronic pain is an umbrella term for a number of conditions so it’s important to recognise each individual and their story.
  • Pain is a societal problem and people have beliefs about pain whether they are suffering it or not. Its widely believed that pain equals damage to tissues resulting in pain but there is not a 1 to 1 relationship between pain and damage.
  • There are many factors that influence an individual’s experience of pain
  • You can have damage and not have pain and also have pain without any obvious injury.
  • There is a huge interaction between our beliefs, behaviours, how we cope with pain and how the nervous and immune systems respond which influence pain and its persistency
  • If you can link key concepts to the patient’s story you can help them to reflect.
  • Even when you can’t change someone’s life there are ways to help them improve their coping mechanisms.
  • There has to be a blame-free narrative and there are many things are involved in the experience of pain.
  • It’s always about listening to the individual and their story.
  • There are lots of reasons around the societal beliefs about back pain and how it persists but it about shifting the beliefs at a population level to make change

BEST MOMENTS

‘No activity is too little’

‘It’s human nature to try and attribute something to a particular event because it brings control’

‘It’s about changing the narrative of pain’

Back Pain and Cycling - Is it normal? How Can I Prevent it?29 May 202000:49:53

Discover today how your back pain when cycling can be eliminated with a simple bike fit. Today we are joined by Ricky Davis, chiropractor and bike fitter who explains the critical importance bike fitting can have on your comfort and performance on the bike whether you're a lifelong cyclist or just starting out. Ricky shares some key exercises for improving your strength in and out the saddle and why getting that fine balance is extremely important.

KEY TAKEAWAYS

  • There’s a 50/50 split for acute trauma injuries on a bike e.g. falling off and fracturing something, and then the chronic overuse and pain.
  • Disc overload or disk anti-pressure can often cause the sciatic nerve troubles close to that area and they can refer down and into the legs. Not only is there mechanical pressure in the saddle but also the lower back, making many people fear cycling.
  • There are specific stretches and techniques for cycling and mobility. Certain yoga poses such as ‘Pigeon Pose’ whereby you stretch through your hips. As well as strengthening exercises like ‘Dead Bugs’ which are an amazing core exercise whilst in a peddling movement, coupled with one-legged glute exercises.
  • The majority of the issues related to lower bike pain in connection with bike fitting is that somebody can be too low in terms of the front end of the bike meaning they’re putting excess stress onto the lower part of the back or they are too high on a road bike you have to have a balance between the two.
  • If you are first getting into cycling, getting a bike fitted correctly to you will change your view on cycling. If you go cycling without this, and find it painful it will put you off. Pain is the biggest limiter of success on a bike as you are not able to do what you love to do.
  • You can increase performance with a good bike fit. The reason you will be faster and perform is that if your muscles are in the right ranges then all your power from your glutes will be going into the pedal. If everything is working as it should, we can improve on things.

BEST MOMENTS

“People think that’s just a normal feeling, that is just cycling but it is not, you don’t need to be feeling this stuff.”

“These are going to be invaluable to strengthen you off of the bike as well.”

“Discomfort will be the thing that limits you the most.”

Back Pain in Professional Sports - A Chiropractors Opinion21 May 202001:00:59

Nick is a Sport Chiropractor who has worked in the premiership for the last 3 years alongside a multidisciplinary team of physios, doctors and coaches.

We discuss how common injuries including back pain is in top-level athletes and how its managed in the elite sporting environment. As well as the challenges healthcare professionals face in dealing with the best of the best.

KEY TAKEAWAYS

  • 49% of the population has suffered from back pain and individuals often identify sport as the cause.
  • This can be because they don’t do anything, then do something intense increasing dramatically the load on the body.
  • In football, both intrinsic load and external load are measured to give both indicators of potential injury and peaks in performance.
  • As a sports chiropractor, you are looking for the characteristics of having flexibility, strength, stability and control in the right areas of the spine, then it can withstand a good amount of load.
  • The type of back pain suffered by most people is mechanical back pain and this affects both athletes and weekend warrior sportspeople.

  • All the changes are very normal and pain doesn’t equal harm it’s an alarm signal.

All about Fibromyalgia!21 May 202001:23:13

Dr. Depak Ravindran is a Consultant Pain and MSK Specialist at Circle Rehabilitation. He is also lead consultant in pain medicine at the Royal Berkshire Hospital in Reading.

One of his key areas of interest is Fibromyalgia. A Chronic pain condition which not only causes a significant amount of back pain but also makes the pain from other conditions much worse!

We talk today about the widespread problem that is Fibromyalgia, how common it is and its relationship to other pain conditions. Deepak is a world-renowned expert on this topic, so some takeaways for sufferers as well as healthcare professionals.

KEY TAKEAWAYS

  • Fibromyalgia is a chronic widespread pain condition characterised by sleep deprivation, memory loss, extreme fatigue and widespread pain throughout the body.
  • Listening is an important part of a consultation and helps to build a detailed picture of people’s lives.
  • Fibromyalgia affects around 3 million people in the UK
  • Younger people are now being diagnosed with fibromyalgia and this has an impact on both society and the economy.
  • It’s a dysfunction of the nervous system and the question is what are the changes for young people that are leading to this dysfunction?
  • The immune system and the nervous system are tied tightly together and they can both be profoundly affected by stress
  • If you look  closely at what you eat you can potentially make a huge difference to fibromyalgia
  • Pain is not a signal something is wrong it’s an alarm system and then the brain decides how dangerous it is ‘it’s a danger signal’ the brain makes a decision based on prior experience
  • Some patients will have both fibromyalgia and back pain and it is likely they will have an amplification as the body is wired to be more sensitive.
  • When you have a sensitive system it’s important to reinforce the message that it alright and therapies can make a difference

BEST MOMENTS

‘Hurt does not equal harm, this is an important message to understand’

‘Any treatment approach must be tailored to the individual and we  need to find multiple ways to calm the nerves’

‘Once a diagnosis is made moderating your lifestyle and completing activities routinely that help your body are key’

Do I need Surgery for my Back pain? A Spinal Surgeons Perspective21 May 202000:57:42

Rath Sundaram is a Consultant Orthopaedic, Trauma and Spinal Surgeon at Gloucestershire Hospitals Foundation Trust and is the spinal lead for Gloucestershire Hospitals. As well as working in the NHS he has a large Private patient base.

The elective work he provides is mainly for sciatica, spinal stenosis, spondylolisthesis, scoliosis, cervical radiculopathy and myelopathy. All which are significant causes of back pain, and disability.

We go into detail about the different types of Spinal Surgeries for back pain, when they should be done, benefits and limitations of each, as well as tips on when it's appropriate to see a consultant spinal surgeon and much more.

So, if you have back pain and are worried about having surgery or if you are just exploring your options then this episode is for you!

KEY TAKEAWAYS

  • The majority of diagnosis are made from the individual’s history and with an examination, a scan would be done to confirm this.
  • Only 5% of individuals will require surgery because the majority who have spinal pain symptoms will heal over time.
  • There is considerable research around the types of people who do well and are the right people to operate on.
  • No two patients are the same it’s about relative anatomy and the way individuals are built.
  • We treat the person, not the scan.
  • The evidence shows that physical therapy can help things to settle down over time. If this doesn’t reduce pain then surgery can be considered.
  • The more common operation is a lumbar discectomy procedure.
  • Fusion surgery is done when there is instability and movement in the spine. The most common cause of this is spondylolisthesis
  • With surgery people are usually home the next day. Your body will be tired and good therapy will help get you back to your normal state as quickly as possible.
  • Decisions about treatment are based on the level of symptoms and the quality of life. It is about whether someone can live their everyday life.

BEST MOMENTS

‘The spine is a dynamic structure’

‘When you are in severe pain for a period of time it can be difficult to accept that it will get better in time’

‘It is reasonable to consider a scan after 6weeks if the treatment isn’t making a difference ’

Axial Spondyloarthritis or AxSpA is an inflammatory arthritis where back pain is the main symptom. AxSpA is an umbrella term that includes AS or Ankylosing Spondylitis as well as Non-radiographic axial spondyloarthritis. Both can be crippling!

KEY TAKEAWAYS

  • Axial Spondyloarthritis is an umbrella term for a number of conditions where the ligaments attached to the bone become inflamed.
  • In AxSpA the immune system loses the ability to tell those cells that connect the ligaments to the bone as being a part of you.
  • It is an alteration of the immune system so for most people its managed through inducing remission.
  • Next steps will usually be an anti-inflammatory medication and for many, this will be sufficient to control the symptoms
  • With AxSpA there are very specific pattern it is a younger person’s condition with onset usually being under 45 although it may not be diagnosed for a number of years.
  • Classic symptoms include spinal stiffness for a period usually in excess of an hour and pain and stiffness that wakes people in the night, only alleviated by loosening through movement.
  • If you are diagnosed early and treated early the best outcomes are achieved.
  • When medication and pain management are under control, key lifestyle factors are avoiding smoking, keeping active and healthy eating.
  • It’s about regular exercise, whatever that is for the individual, you should be aiming to lift your heart rate for 150mins a week.

The McKenzie Method for Treating Back Pain21 May 202000:31:43

John Thompson is a physiotherapist and lead instructor for the McKenzie institutes in Europe.

This episode is dedicated to the McKenzie method for treating and solving lower back pain.

The McKenzie Method® of Mechanical Diagnosis and Therapy® (MDT) - MDT is a philosophy of active patient involvement and education which is trusted and used by practitioners and patients all over the world for back, neck and extremity problems. This approach continues to be one of the most researched physical therapy-based methods available.

KEY TAKEAWAYS

  • The system is a complete system of assessment diagnosis and treatment.
  • It aims to treat anyone for spinal pain or extremity pain that can be improved through movement.
  • It aims to move the patient to the end of their movement range and monitor how their symptoms behave.
  • The McKenzie system aims to identify recognised patterns. The 3 patterns seen the most are derangement, dysfunction and postural.
  • Postural – pain when in a particular position for an extended amount of time – this is about education to change the movement that is causing the pain.
  • Dysfunction – tight or shortened tissue always related to back or neck with the pain only happening when the tissue is stretched. Stretching out therapy encourages the tissue to loosen.
  • Derangement – variable in presentation with a loss in the range of movement that is not consistent. You can achieve a rapid change through exercise.
  • The diagnosis and classification is based on the symptoms shown by the patients.
  • The McKenzie method uses therapist techniques during both assessment and treatment and for those patients who can’t move themselves it is an essential component.

BEST MOMENTS

‘I felt I should be able to do more for my patients’

‘If you are a patient who has had episodic pain and finds pain in completing certain tasks and activities then you should consider the McKenzie method’

When Should I see a Healthcare Professional - The Region Beta Paradox25 Nov 202200:06:27

In this episode hear about the region Beta paradox  -the phenomenon that people can sometimes recover more quickly from a major incident and intensive pain than from mild pain or a less distressing experience

Rob also discusses the importance of explaining to your healthcare  professional why you have made an appointment and what you want to get back to doing to ensure the treatment plan is successful

KEY TAKEAWAYS

  • People often mistakenly think it will take longer to recover from a major incident or intensive pain than when they have a mild pain or mild distressing experience
  • We often only take action when the pain reaches a particular threshold
  • When something isn’t too bad we may choose to ignore it
  • When it’s awful we will do something about it taking action that makes it better faster
  • As a patient it’s important to tell your practitioner why you have booked in now and what you want to get back to doing
  • Explaining ‘why’ helps to inform the  direction of the treatment plan
  • It's more than just making the pain better its getting you back to the activities you want to do

BEST MOMENTS

‘You may get stuck in region Beta where the less worse thing lasts longer than the worse thing’

‘If we don’t know what you want to get back to, we might not fully prepare you for the demands of it’

The Worst Advice For Someone With Back Pain04 Nov 202200:10:59

This week Rob is talking about the worst advice he sees given for back pain and is explaining why you definitely shouldn’t be following it

He looks at abdominal bracing, stopping sports and activities and why movement is exactly what we should be doing supported by a skilled clinician to make sure the movement is what works for you, listen in and hear more

KEY TAKEAWAYS

  • You actually only need to use around 2% of your abdominal muscles for everyday activity so there is no need for extra bracing
  • People are under the assumption that their core needs to be active all the time
  • Move from the spine, that’s what it’s meant for
  • Pain is multifactorial and there is rarely a single cause
  • A skilled clinician can work out what’s the best exercise for someone at a particular time
  • The exercise is the one that’s best for you and one you enjoy, so you will do it

BEST MOMENTS

‘Since the nineties, the core has been blamed for just about everything’

‘We are meant to move our spines’

‘Pain is a warning sign from the body it doesn’t mean damage’

Your CrossFit Questions Answered and Why there Will Always be Wonky Squats28 Oct 202200:16:35

In today’s episode, Dave is talking gym, CrossFit and getting back into exercise in response to questions from across the Back Pain Podcast community and socials

Dave takes a look at the role of deconstructing and building towards a particular movement and why even if you have a wonky squat its ok  because everyone is different

KEY TAKEAWAYS

  • Full body movements are done in a group setting at high intensity
  • Often cornerstone exercises are done and  act as a marker for the level of fitness
  • All exercises are positive if done well. It’s not the exercise that’s wrong it’s the narrative around it
  • Humans are not all the same We are all different so we should act like it. We won't all conform to the same baseline
  • Deconstruct the exercise into its components and work at it as long as you are building and progressing you can be ok with what you are achieving
  • It's better to be fighting towards the next level even if you don't ever get there
  • Human movement has a bell curve and you may be an outlier

BEST MOMENTS

‘People who do CrossFit bloody love what they do’

‘Never stop aiming for the perfect movement but be ok with it if it never comes’

‘Some people just do have a bit of a wonky squat’

All About the SacroIliac Joint with Wilfried Simon AKA the Spine Physio30 Jun 202300:34:32

The sacroiliac joint is an area that remains contentious despite all of the research that’s taken place and in this episode, we are chatting to Wilfried Simon, an osteopath and physio from Belgium about the misconceptions, if it can be a cause of pain and what to do if it is

KEY TAKEAWAYS

  • It’s a connection point but there isn’t much movement, only 3 degrees and its deep within the spine
  • If you want the patient to be able to treat themselves and achieve autonomy they have to understand what’s wrong
  • When you have manipulation it’s about what it creates overall including how the person thinks about it
  • Identify the movement that is painful and work with the patient to make the movement less sensitive through changing the axis of the movement and building the confidence of the patient to complete movements
  • Trust the future and the process, not very day is the same, not every day is pain free but that’s what make the patient stronger and you stronger

BEST MOMENTS

‘It’s important to treat the person not the problem’

‘There is aways a combination of what’s happening mechanically to the joint and how the patient perceives and feels about what’s happening’

All About ACL Tears With Andrew Goodall18 Aug 202200:45:44

In this episode physio, Andrew Goodall goes through ACL tears from how you are likely to injure your anterior cruciate ligament through the likely treatments and the phases of recovery and rehabilitation

As one of the major ligaments inside the knee, it plays an important role in stabilising the knee joint and helps to control the movement of the lower leg but is a common sports injury and Andrew shares great informative content that will help anyone to know what to expect from recovery and how to make sure their treatment is the right one for them

KEY TAKEAWAYS

  • The mechanism, force and speed at which ASL happens means it’s likely there will also be other damage
  • Calming the knee down means that it’s possible to get a good quality image and reduce the pain
  • Getting the quadricep thigh muscles working efficiently as quickly as possible will bring  stability
  • Initially, it’s about doing less but maintaining strength and function
  • For most surgery is part of the process and in the UK 80% of people have surgery
  • Taking a section of the hamstring tendon and using it as an ASL graft is the most common approach in the UK
  • It’s bespoke surgery for your needs, your situation and taking into account the factors that have predisposed you to being injured
  • Rehab is likely to be around a year with anything less being exceptional progress
  • Phase one is calming down, phase two is the building of load and phase three will be moving closer to training
  • The final phase is preventative activities and a return to training

BEST MOMENTS

‘In some sports movements, the rotation through the leg translates as an inwards movement of the knee’

‘I’m essentially trying to come out of the session with a diagnosis and a plan’

‘If you can rapidly reduce swelling and pain the person is more likely to be able to use the knee and keep muscle function’

‘Stability is dynamic and controlled by lots of systems, it’s the integrity of the ligament that is crucial’

The Calm Sh*t Down, Build Sh*t Up Approach to Managing Back Pain11 Aug 202200:11:39

Rob looks at the methodology of calming down and building up and explains how this approach to treatment works when you have non-specific back pain

He shares some of the different ways pain can be effectively calmed down and how the next step is to build things up both physically and mentally in the treatment journey

KEY TAKEAWAYS

  • Patients are looking for a cause but often there isn’t a specific reason
  • Pain is very real whenever you are experiencing it
  • Non-specific back pain means treatment targets are aimed at the whole back or whole body
  • Calming things done is about understanding the movement of activity that aggravates the pain
  • There are many ways to calm back pain down including heat, ice and understanding the role of non-physical stressors
  • Building your back up may include learning to cope with the pain in a better way and understanding the physical exercise that can be completed

BEST MOMENTS

‘You might want to have a scan to show something that you can blame the pain on’

‘You want to see a problem so that you know why you are suffering so much’

‘Reassurance is so important in recovery’

A Rarer Cause of Leg Pain - Vascular Claudication04 Aug 202200:10:30

There are lots of different things that patients think are causes of sciatica and it’s the role of the health care professional to explain when it is sciatica, what it is, what it is not and what you can do about it

In this episode, Rob discusses vascular claudication, a less common cause of leg pain but due to its risk factors, it's one that healthcare professionals stay alert to. He  shares the diagnosis of intermittent vascular claudication with a patient and explains the crucial indicators that led to the diagnosis along with the treatment

KEY TAKEAWAYS

  • Intermittent pain in both legs can have several causes
  • Intermittent peripheral arterial disease is to do with the blood supply. When you are walking muscles in the legs require more blood supply and can't get it
  • Patients often describe a cramping or burning sensation in the thighs
  • Vascular risk factors including smoking, weight, diabetes and age increase the likelihood of intermittent peripheral arterial disease
  • What he told us about the pain was crucial in making a clinical decision. The type and duration of the pain was an important indicator
  • High blood pressure raises the likelihood of it being vascular
  • A significant reduction in blood flow in the legs will be an indicator of a vascular issue
  • Patients with vascular claudication have a much higher risk factor for an acute vascular episode

BEST MOMENTS

‘As health professionals, we often provoke pain to understand the source’

‘Changes can take time but will make a difference’

‘It’s one we have to stay alert for as health care professionals’

  • How we explain back pain is a vital part of the process
  • The approach of different professionals should be similar and quality care should include pain education and activity advice
  • A package of care will focus on cognitive care and manual intervention and a good assessment should include a detailed history
  • Patients want is a good connection with their health care practitioner and a choice in the treatment to get better
  • The words we chose to use can impact recovery and have far-reaching implications
  • We want patients to leave the treatment room feeling positive not damaged broken and fragile
  • Understanding what the patient wants to  achieve is a vital element in a positive recovery journey

Managing Lower Back Pain and Sciatica01 Jul 202200:39:37

Rob and Dave use a real-life example to explore how they would treat and manage a  patient who comes to them having already experienced conflicting advice

They go through the steps that are covered in an initial appointment and discuss the points at which a scan is the right choice along with what sorts of manual therapy can be a good choice

Practitioners approach patients in different ways but you should always expect a very individualised treatment plan and this is a great opportunity to find out about what you should expect and why

KEY TAKEAWAYS

  • The first step should be taking a really detailed history and the patient's thoughts and beliefs are a vital part of the conversation
  • It provides insight and an understanding that helps with building a personalised plan
  • It’s the two-way conversation between the practitioner and the patient
  • With sciatica, it’s about calming down the most aggravated part first
  • Sometimes over testing  can make it more difficult to assess, move up to the pain, it’s not about aggravating it
  • Patients can be fearful and avoid particular movements but it can be used as part of the methodology of building toward graded exposure
  • The most important exercise is the one the patient does
  • You need to look at an individual patient's tolerance to pain and make  any exercise  personalised
  • A lot of changes seen on a scan are perfectly normal It's about focusing on the structural pathology
  • There are always the red flag questions that result in a scan being the right approach
  • Manual therapy provides  a window of opportunity to feel better

BEST MOMENTS

‘People who are fearful and worried about their pain tend to have  longer term  back pain’

‘If a patient believes something should be done it’s important to explore it’

‘If something is painful it doesn’t mean it's being damaged’

10 Years of Treating Lower Back Pain – How Our Practices Have Changed23 Jun 202200:34:52

Rob and Dave are both seeing patients every day in their clinics and in this episode they take a look at how things have changed over the last 10 years

They share the learning that has changed how they work with their patients and discuss the vital importance of movement and how evolving to work with patients as someone who is supporting that individual’s journey to recovery, not the hero clinician builds positive and productive relationships

Listen in to hear Rob and Dave reflect on how their practice has evolved and why it’s always important to stay on top of new developments if you want to be a great clinician

KEY TAKEAWAYS

  • It’s important to take a considered approach and look at emerging clinical research and trends. It's updating, evolving, and staying on top of it
  • Worry and anxiety about your posture is only going to potentially exacerbate your back pain
  • It’s not the specific exercises that make a difference it’s about keeping the movement going
  • We start with the basics of good quality movement and then fine tune
  • Some movement is important it doesn’t have to be a specific granular program
  • Conversations in the clinic have changed from ‘I’m the therapist here to save you’
  • The clinician is no longer the hero fixing you, the patient is now the hero with the clinic providing a helping hand
  • It’s not about waiting for someone else to do it for you, it’s about taking responsibility for yourself and your recovery supported by the clinician
  • You will have a much better long-term relationship if you are happy to give the praise away to the patient
  • Having difficult conversations is a skill that needs to be developed
  • Talking to patients about their fears is a crucial element in building a relationship and understanding the patient

BEST MOMENTS

‘We can’t be blinkered we need to be looking for the newest research and turning points in the science

‘When they are as knowledgeable as possible that’s when the magic happens’

‘I understand now the power of better movement and communication’
