This episode explores opioids through a behavioral health lens, with attention to pain, prescription opioid use, opioid misuse and opioid use disorder, diversion, overdose risk, and recovery. The discussion distinguishes tolerance and physical dependence from addiction, reviews signs of problematic opioid use, and examines how stigma, bias, prescribing practices, and inequities in pain treatment can shape clinical care.

The course also addresses the changing opioid landscape, including illicitly manufactured fentanyl, counterfeit pills, polysubstance use, naloxone, fentanyl test strips, and other harm-reduction strategies. Treatment approaches for opioid use disorder are reviewed, including methadone, buprenorphine, extended-release naltrexone, contingency management, motivational interviewing, CBT, ACT, DBT, peer support, and recovery resources. Throughout, the focus remains on what behavioral health clinicians need to recognize, assess, discuss, and respond to within their scope of practice.

In this continuing education podcourse, we move beyond identifying risk to explore how clinicians can formulate suicide risk, directly address suicidal thoughts and behaviors, develop collaborative safety plans, reduce access to lethal means, and determine appropriate levels of care.

We discuss suicide-specific approaches including CAMS, CBT for Suicide Prevention (CBT-SP), and DBT, as well as the role of psychiatric treatment and interdisciplinary coordination. The course also explores chronic suicidality, medication considerations, hospitalization and alternatives to hospitalization, post-crisis care, and more complex presentations involving violence risk, postpartum psychiatric emergencies, financial crises, and emerging technology.

Suicide is one of the most complex and consequential issues mental health professionals encounter. In this CE-eligible episode of the Affirming Minds Podcast, Shaina Siber, LCSW, explores what we know about why suicidal thoughts develop, what increases the risk that thoughts become actions, and why understanding suicide requires looking beyond diagnosis alone.

We explore psychological pain, hopelessness, entrapment, perceived burdensomeness, thwarted belongingness, trauma, mental illness, chronic pain, social conditions, minority stress, and access to lethal means. We also examine how human cognition and our capacity to imagine the future may contribute to suicidal suffering.

The episode considers major theories in suicidology, disparities in suicide risk, the distinction between suicidal ideation and suicidal behavior, and the complicated role of psychiatric hospitalization. Throughout the conversation, the focus remains on understanding the individual person: What has become unbearable? What feels impossible to escape? And what does the person believe suicide would solve?

What happens when survival mode becomes an identity?

In this episode, Shaina sits down with Licensed Associate Marriage and Family Therapist Caleb Chestnutt to explore the mental health impact of chronic stress, trauma exposure, hypervigilance, and emotional suppression within military and law enforcement culture.

Drawing from both his clinical work and his lived experience serving in military law enforcement roles in the U.S. Air Force, Caleb offers a grounded and deeply nuanced conversation about the culture of endurance that exists within many first responder spaces — and the ways it can quietly shape relationships, emotional regulation, identity, and help-seeking over time.

This is a thoughtful and deeply human conversation about trauma, endurance, identity, and the emotional cost of operating in systems that reward strength while often stigmatizing vulnerability.

Are today’s teens truly more anxious — or are we simply seeing anxiety more clearly?

In this episode, we unpack the narrative of “the anxious generation” and explore what current research actually shows about adolescent anxiety. We discuss adolescent brain development and expected shifts in mood, sleep, and emotional intensity, and how to distinguish developmentally expectable distress from clinically impairing anxiety.

We also explore how contextual factors — COVID-19 disruption, social media and parasocial relationships, and expanded access to telehealth — shape our understanding of youth anxiety reporting without assuming pathology.

Finally, we delve into a critical relational factor: how caregiver anxiety and over-accommodation can unintentionally reinforce avoidance — and what differentiation looks like in practice when supporting teens.

Joining me is Sophia Vale Guarino, LCSW, author of Calming Teenage Anxiety — a practical guide for caregivers who want to support teens with clarity and steadiness.

Family estrangement is often portrayed as impulsive, trendy, or selfish. In reality, navigating low contact or no contact with a parent is typically a painful decision — one that is rarely made lightly and almost never without history.

As public debate around estrangement has intensified — including criticism directed at therapists — this episode asks an essential question: What is our clinical role in this conversation?

Host Shaina Siber, LCSW offers a trauma-informed, ethically grounded framework for clinicians working with adult clients who are considering distance from caregivers. She argues that therapy should not treat reconciliation as morally superior — nor estrangement as inherently empowered. Instead, clinicians are called to remain outcome-neutral and harm-responsive.

This episode provides:

  • A structured assessment model for evaluating impact, boundary history, accountability capacity, and safety
  • Clarification of what “boundaries” actually mean — and how they differ from requests or impact statements
  • Guidance for navigating trauma history, identity-based rejection, and developmental instability
  • A clear stance on avoiding coercion while naming harm when present

Estrangement decisions are complex, relational, and often grief-filled. This conversation offers therapists a grounded framework for helping clients move through these waters with clarity, agency, and care.

Dialectical Behavior Therapy (DBT) is one of the most empirically supported treatments in mental health, with strong evidence for reducing suicide risk, self-injury, and other high-risk behaviors. Yet access to full-fidelity DBT remains limited, raising important questions about dissemination, accessibility, and real-world use.

In this episode, Shaina Siber is joined by Dr. Shireen L. Rizvi and Dr. Jesse Finkelstein for a thoughtful conversation about DBT dissemination—how evidence-based treatments evolve, travel beyond specialty settings, and become usable tools for real life without losing their integrity.

The discussion centers on Real Skills for Real Life: A DBT Guide to Navigating Stress, Emotions, and Relationships, an accessible, visually engaging guide that presents 26 core DBT skills alongside decision trees to support flexible, context-responsive skill use.

At a moment of expanding awareness and increasing constraint, clinicians are more attuned than ever to the potential for harm in the therapy room—yet many feel uncertain about how to respond when identity, power, and politics arise in session.

In this episode, we explore cultural humility as a clinical stance, not a buzzword:

  • What it means to practice with ongoing self-reflection, accountability, and repair
  • How to navigate backlash, polarization, and institutional limits
  • How to adapt evidence-based treatments through a culturally humble lens
  • What it looks like to redistribute power, mitigate harm, and stay present when the work feels risky

Our guest is Dr. Lauren Wadsworth, clinical psychologist, consultant, and co-founder of Twin Stars Diversity Trainers. Dr. Wadsworth brings deep clinical experience and a justice-oriented lens to the nuanced terrain where identity and therapy intersect.

Whether you’re a seasoned clinician or in training, this conversation will challenge you to think differently about identity, power, harm, and ethics in everyday practice.

Why is behavior change so hard—even when we genuinely want to change?

In this episode of The Affirming Minds Podcast, we explore how survival wiring, unconscious patterns, and longevity science shape habit formation and behavior change. Drawing from behavioral science, psychodynamic psychology, and emerging healthspan research, this conversation examines why willpower alone rarely works—and what supports sustainable, values-aligned change instead.

Shaina is joined by Laura Elorza Isaza, Clinical Psychologist, to discuss autopilot living, discipline vs. motivation, shame-free approaches to habit change, and how understanding unconscious processes can help individuals design behaviors that support long-term mental and physical well-being.
