Season seven starts with the part of my story that still catches in my throat, even after 14 years: my dad died, and within months my mom began to fall apart in ways that grief alone couldn’t explain. People around her told me I was overreacting, that it was depression, that it was mourning. I had to trust what I knew about my mother, push for testing, and brace for the avalanche of becoming responsible for her care, her safety, and the life she used to run with ease.

John from Georgia Focus asks the questions a lot of families quietly carry: how do you balance this, how do you keep going, and what does Alzheimer’s caregiving look like when someone can’t reliably tell you what they need? I talk through late stage realities like limited speech, incontinence, assisted walking, and why caregivers can’t wait for “I’m hungry” or “I need help.” We also get honest about the emotional whiplash, the isolation, and the strange moments that become funny later, because humor isn’t disrespectful, it’s survival.

J Smiles struggles with abnormal memory loss and brain fog. Her immediate fear is the early onset of Alzheimer’s  -- any caregiver's nightmare.

A Burglar is Breaking in on me & Zetty!22 Mar 202100:27:22

For once J is able to get Zetty in the bed before 2am, excited and sound asleep all hell breaks loose when the house alarm blares "intruder alert."

Caregiving is so nuanced. We can identify the major things swiftly -- medicine and food must be consumed; bills paid and so on.

What happens when non-harmful modifications appear? Those creature comforts - habits - preferences that you are accustomed to your LO choosing like clockwork. Perhaps it is always drinking coffee in the morning or wearing pajama pants instead of a nightgown... a favorite watch or red lipstick.

These subtleties can sting us more than we realize. J Smiles recounts supporting  a caregiver newbie through the process of recognizing "NEW" mom can  prefer things "OLD" mom would have found  despicable.

Who knew skipping sleep could add up so quickly? Not, J Smiles!

Pivoting on Pins and Needles - the life of caregiver Erika Watson09 Nov 202001:15:57

J Smiles and Erika have been thick as theives for over 20 yrs. Howard University and Delta Sigma Theta Sorority cemented their relationship. But as only kids who love to travel, who appreciate fine dining, who have never missed a proper brunch and who seamlessly merge Rosé with a Rick Ross beat... they take the listener on Erika's abrupt realization of her mom's cognitive decline as if recounting one of their jaunts in Washington, D.C.

Erika's dry wit mixes very well with J's storytelling. Even though Erika is typically private, J Smiles, eases her into revealing one detail, one day at a times. Alzheimer's disease is very mysterious. Caregivers are often guessing what their LO's (loved ones) are feeling, experiencing, etc. CG's live a life of magical charades.

The first time ALZ shows up in an ordinary daily event can turn a caregiver's heart on tilt. J Smiles takes time to share a handful of specific examples of just that. She was not prepared for the initial instance when Zetty did not recognize herself in a photograph.

There are many things that science can track but this is an area where a heart must rely on other humans. Shock and awe create pain and frustration for J Smiles with each first event. She never sees it coming. J hopes her stories can help others navigate the first time, unknowns.

Routine does not mean regular when it comes to doctors office visits for Zetty. Much to J Smiles dismay, these common appointments are quite eventful.

J Smiles pours out her heart in this episode.  Believing Zetty has a simple Urinary Tract Infection (UTI), she is forced to call 911.  It turns out to be much more serious. The ER presents one obstacle after another as J tries to guide the staff on best practices and techniques to get Zetty to acquiesce to the many tests. Routine blood and urine samples become stressful, silly displays of the massive void regarding Alzheimer's disease awareness within the ER staff. 

Fortified by love, J fights to reduce her mom's "process" pain and establish a care plan with as many protective parameters as possible. J Smiles uses precise analogies and examples to give the listener an audio-visual of the emergency room and series of mishaps experienced due to ill prepared medical personnel. Ever wondered how far you'd go to care for someone you love? How much dignity you'd sacrifice? I found my answer on the side of I-20 during Atlanta rush hour.

When my mother started talking to people who weren't there, walking into walls, and fidgeting constantly, I suspected a UTI—those sneaky infections that can make dementia symptoms appear dramatically worse. After three grueling weeks of attempting to collect a urine sample from my incontinent mother, we finally succeeded. With my precious cargo secured, I had exactly one hour to reach a lab 45 minutes away.

What followed was nothing short of a caregiver's nightmare. Halfway to the lab, my body betrayed me, forcing an emergency roadside stop that left me sweating, shaking, and utterly humiliated—but still clutching that precious urine sample. Have you ever witnessed someone persevere to no end to maintain a connection with someone they cared about? That's exactly what happened when my mother's DC circle refused to let her disappear after my dad's death.

When Betty began bombarding my mother's phone, email, and even her old-school answering machine with messages, I initially dismissed it as an unwelcome intrusion. We were deep in grief, struggling with the aftermath of my dad's passing, and I had no energy for reconnecting with old friends. Little did I know this persistent woman would create one of the most healing moments in our early dementia journey.

Betty and two other powerful Black female professionals had worked with my mother for decades, forming bonds I hadn't fully understood. "If you don't have nobody in your life willing to go that hard in the paint and keep worrying the sh*t out of people they think can get to you," I now realize, "then you need to check your crew." Their insistence on seeing my mother face-to-face led to a remarkable reunion at a DC hotel that changed everything.

When these women demanded time alone with my mother, I reluctantly stepped away, worried about leaving her without my supervision. When I came back, I heard something that stopped me in my tracks – my mother's genuine, unrestrained laughter echoing down the hallway. 

This experience fundamentally changed my approach to caregiving. Sometimes the best thing we can do is step aside and allow longstanding connections to work their magic.
