 In this deeply human episode of PainSpeak, I’m joined by Fiona Symington, who shares her lived experience of chronic fatigue and persistent pain symptoms — and, importantly, what recovery felt like from the inside and what pain reprocessing therapy (PRT) meant to her. 

This is not a conversation about quick fixes, techniques, or “getting it right.” Instead, Fiona gently walks us through the emotional and nervous-system journey of living in a body that no longer felt safe, predictable, or trustworthy. We explore the early signs that something was wrong, the impact of not being believed, and how fear, hypervigilance, and constant symptom-monitoring can quietly become part of the suffering itself. 

Fiona reflects on the slow, often non-linear shifts that marked her path toward healing — moments of understanding, softening, and rebuilding trust rather than pushing or fixing. Together, we talk about identity, grief, and the unexpected challenges of recovery, including how unfamiliar safety can feel after long-term illness. 

This episode is especially for anyone who feels exhausted by trying, overwhelmed by advice, or quietly worried they’re “behind” in their healing. Fiona’s story offers reassurance rather than instruction, and hope without pressure — a reminder that recovery does not begin with effort or perfection, but often with understanding, compassion, and a nervous system learning that it is no longer under threat. This conversation is shared as one person’s experience, not medical advice, and listeners are encouraged to listen gently, at their own pace. I am really gratefu to Fiona for her time and deeply impressed that she is now training to be a clinical psychologist in order to do the research and help people like her.

Welcome to the second part of this 2 part episode where I speak to Dr Sabina Brennan. a clinical psychologist, neuroscientist, fellow podcaster and best selling author of the book on how to beat Brain Fog. 

As we know, Brain fog and congitive dysfunction is one of the most common sysmptoms noted in Longcovid. Treatments are still emerging and I thought it would be a fascinating and hugely useful to hear from a researcher and scientist and someone with lived exeperience herself on how to overcome brain Fog. Dr. Brennan discusses the reasons for brain fog, the present causes and the treatment option available and why we must pay attention.

In this episode, we delive deeper into how her 4 week program can really help with brain fog reduction in a careful evidence based manner. There are no quick fixes here however Sabina shows that done properly, there can be lasting recovery from something as complex as brain fog.

It has been a hugely gratifying  and humbling experience to get the chance to set up the Longcovid Service for Berkshire and work with a great team from primary and community care. It has been 7 months now and we are still learning everyday about the condition of Longcovid and how it seems to affeect multiple systems. Right now we continue to  offer them support, a listening ear, appropriate testing and treatments where available in a  trauma informed manner.

In this episode, I speak to Dr Sabina Brennan. a clinical psychologist, neuroscientist, fellow podcaster and best selling author of the book on how to beat Brain Fog.

Welcome to a Pain Speak Special focussing on a new book on pain "The Painful Truth" by Oxford based junior doctor and academic scholar Dr Monty Lyman released on 15th July. This becomes the 7th book on pain released in the UK in just the 6 months of this year! We're currently experiencing a Renaissance in pain science and it is great to add Monty's book to this fantastic list of  resources for our patients and general public and for anyone interested in knowing more about pain science.

In recent years our understanding of pain has altered so radically it's fair to say that everything we thought we knew about pain is wrong. I made the case in my book as well.  As Dr Monty Lyman reveals, we misunderstand pain - with harmful consequences. This is Dr Monty Lymans second book. His first one " the Remarkable life of the skin" was shortlisted for the Royal Society Science Book Prize". He is a remarkable story teller and in this book , The Painful Truth, he explores and presents cutting-edge research, encompassing phantom aches to persistent pain, and interviews survivors of torture as well as those who have never felt pain.

It shows the myriad ways in which pain acts as a protector sometimes unnecessarily. He not only provides hope for reducing and managing pain, but takes us to a deeper understanding of what it means to be human. In the end he presents the hopefuly side of taking this knowledge and using it for improving our health and I am glad that i got a chance to be featured as well!

This is the untold story of pain - our most elusive feeling and I am glad that I got a chance to be interviewed for the book and he has agreed to come on to this podcast. Hope you enjoy it.

Longcovid - Chat with Darren Brown Co-Founder of LongCovid Physio17 May 202100:51:02

It has been a hugely gratifying  and humbling experience to get the chance to set up the Longcovid Service for Berkshire and work with a great team from primary and community care. We have learnt a lot and are continuing to learn as we help our patients suffering from Longcovid and offer them support, a listening ear, appropriate testing and treatments where available in a  trauma informed manner.

In this episode, I speak to Co-Founder of Longcovid.Physio . - a global peer support and advocacy group for Physiotherapists (Physical Therapists) and support workers living with Long COVID.

He is a clinical and academic Physiotherapist specialising in the field of HIV, disability and rehabilitation and leads the HIV rehabilitation service at Chelsea and Westminster Hospital NHS Foundation Trust. Darren contracted COVID-19 in March 2020 and continues to live with Long COVID. An advocate for Long COVID research, he has become a familiar person enabling meaningful engagement of people living with Long COVID in all responses to COVID-19.  Darren has an impressive day job at London already and he has helped amass an incredible set of resources for patients and healthcare professionals through this website.

Longcovid - What can be done about loss of taste and smell?12 May 202100:36:46

It has been a hugely gratifying  and humbling experience to get the chance to set up the Longcovid Service for Berkshire and work with a great team from primary and community care. We have learnt a lot and are continuing to learn as we help our patients suffering from Longcovid and offer them support, a listening ear, appropriate testing and treatments where available in a  trauma informed manner.

In this episode, I speak to associate Professor Jane Parker based at the University of Reading. Trustee and Advisor to the charity Abscent. A chemist by training, she is based at the Flavour Centre and has had huge experience of work around olfaction and food smells. With previous experience of looking after people with smell disorders in other viral infections and brain injury, she is the right person to talk to us about taste and smell and the strange changes that happen in some people after COVID 19.

She talks to us in great detail about anosmia (complete loss of smell) and parosmia (altered smells) and how it can be managed and treated.

It has been a hugely gratifying  and humbling experience to get the chance to set up the Longcovid Service for Berkshire and work with a great team from primary and community care. In this episode, I speak to my consultant colleague Dr Blandina Blackburn, Occupational Health Consultant. Dr.Blackburn has been working with staff at the Royal Berkshire for many years and is a hugely respected national figure in Occupational Health and represented her profession at the highest levels.

There is a lot of confusion prevailing right now as to how return to work can be established when we are still learning about the long terms effects of Post Covid so I am glad that Blandina can inform us about the complexities and suggest a way forward for now. Rulings are likely to change and we might have her coming back once we know more about the condition and laws change  and courts make further announcements.

Longcovid - What are the effects on the brain?10 May 202100:25:29

It has been a hugely gratifying  and humbling experience to get the chance to set up the Longcovid Service for Berkshire and work with a great team from primary and community care. We have learnt a lot and are continuing to learn as we help our patients suffering from Longcovid and offer them support, a listening ear, appropriate testing and treatments where available in a  trauma informed manner.

In this episode, I speak to my consultant colleague Dr Abrar Hussain, Consultant Psychiatrist at the Berkshire health Foundation Trust and the lead for the Medically Unexplained Services locally and also supporting people with Functional Neurologic Disorders.

We had collaborated often to help many patients with overlapping physical and mental health issues. In this episode, he talks about the influence of viral infections and associated neuropsychiatric issues and highlights the multiple contributory effects on a persons physical and mental health. He also talks about the role of COVID 19 and treatments in ICU and associated PTSD and the impact of anxiety and hospitalisation and isolation.

Longcovid - What is the role of the occupational therapist?09 May 202100:27:02

It has been a hugely gratifying  and humbling experience to get the chance to set up the Longcovid Service for Berkshire and work with a great team from primary and community care. We have learnt a lot and are continuing to learn as we help our patients suffering from Longcovid and offer them support, a listening ear, appropriate testing and treatments where available in a  trauma informed manner.

In this episode, I speak to my fellow colleague Tarryn Ashworth, a specialist occupational physiotherapist. Tarryn has been working with COVID patients admitted in hospital from the first wave onwards. Normally working in an inpatient manner, this has been a new role in bringing her expertise to the outpatient management of Longcovid patients.  When I set up the Longcovid service, Tarryn has been invaluable in providing assessment and support for those patients struggling with fatigue and working alongside Harriet to set up personalised plans for our patients.

We have learnt a lot and are continuing to learn as we help our patients suffering from Longcovid and offer them support, a listening ear, appropriate testing and treatments where available in a  trauma informed manner.

In this episode, I speak to my fellow colleague Frankie Knight, a specialist respiratory physiotherapist. Frankie has been working with COVID patients admitted in hospital from the first wave onwards and has also done longstanding respiratory rehab for patients with long term lung conditions.  When I set up the Longcovid service, her experience alongside her colleague Laura Cornish, has been invaluable in providing assessment and support for those patients struggling with breathlessness and chest pain symptoms despite having normal tests and scans. Breathing pattern disroders are very common and she talks about simple measures which you can do for yourself in this episode.

What if chronic pain isn’t a sign of damage — but a learned alarm the nervous system can unlearn?

In this episode of PainSpeak, I’m joined by Caroline Davies, a specialist pain physiotherapist with over 25 years of experience working across the NHS, the US healthcare system, and now in independent practice in the UK. Caroline’s work sits at the cutting edge of modern pain care, blending deep physiotherapy expertise with contemporary neuroscience and emotion-focused therapies.

Together, we explore one of the most important — and often misunderstood — shifts happening in pain medicine today: the move from viewing chronic pain purely as a problem of tissue damage, to understanding it as a problem of persistent threat signalling in the brain and nervous system.

From Physiotherapy to Pain Reprocessing Therapy - This episode follows close on the heels of the Channel 4 documentary on 8th Jan featuring Dr. Rangan Chatterjee - The drug Free doctor as he interviewed a patient with chronic pain who had used Pain Reprocessing therapy to reduce her pain significantly. Of course this has given us the chance to understand in more detail what PRT is and isnt!!

About Caroline Davies

Caroline began her career in traditional musculoskeletal physiotherapy, working extensively with complex pain presentations including CRPS, pelvic pain, cancer-related pain, and trauma-associated pain. Over time, she noticed a familiar pattern many clinicians will recognise: people whose scans looked “reassuring,” whose tissues had healed, yet whose pain continued — sometimes worsening — despite doing everything they were told.

In this conversation, We talk candidly about the limits of conventional approaches, the frustration clinicians can feel when progress stalls, and the curiosity that eventually led her to train in Pain Reprocessing Therapy (PRT), Emotional Awareness and Expression Therapy (EAET), and more recently, Internal Family Systems-informed approaches.

As always on PainSpeak, the aim is not to tell you what to think — but to help you understand pain differently, so that change becomes possible.

Longcovid - What do patients feel and want from a service?20 Apr 202101:16:16

It has been a hugely gratifying  and humbling experience to get the chance to set up the Longcovid Service for Berkshire and work with a great team from primary and community care. We have learnt a lot and are continuing to learn as we help our patients suffering from Longcovid and offer them support, a listening ear, appropriate testing and treatments where available in a  trauma informed manner.

In the first episode of this series, I speak to 2 patients who are part of the Longcovid SOS group .

Each tell their story from their perspective and it is more  than important in a condition like Longcovid to know what the lived experience of these patients are and their symptoms  to be taken into account when designing a service that can meet their needs. Each persons symptoms are unique and thei experiences are different and I have learnt a lot from talking to my patients on how varied their presentations have been and how they have told their story.

In future episodes, I will be talking about our present upto date understanding around  Longcovid and newer studies and research and my team at Reading Berkshire.

My guest for this episode is Professor Denis Martin, Professor of Rehabilitation. He is Director of the Centre for Rehabilitation, Lifestyle medicine and Human performance at Teesside university. With academic roots at the University of Ulster and Napier University, he forged an impressive career spanning roles at Sheffield Hallam University and Queen Margaret  University, Edinburgh. During his time in Scotland, he also served as a chair of the pain association, Scotland and vice chair of the Scottish Parliament cross party group. He is renowned for his research in pain, disability and rehabilitation, and has over £25 million pounds secured in funding and an extensive publication record. Prof. Martin has led significant projects, exploring the use of extended reality and digital technologies and rehabilitation, and he currently serves as the vice chair of the XR for rehab network and leads a research theme on physical, mental and social health in the Applied Research collaboration (ARC) NENC area of NE England.

The focus of this episode is the Citizens' Jury Report that Denis has led on at the North East Nprth Cumbria. This is going to be officially released on 13th Nov 2025 at NewCastle.

Cormac Ryan is Professor of Clinical Rehabilitation at Teesside University and a community pain champion for the Flippin’ Pain campaign. He was awarded his PhD from the Glasgow Caledonian University in 2008 where he looked at the relationship between physical activity and persistent lower back pain. Cormac has published over 100 peer-reviewed journal articles and obtained over £1m in research funding. He enjoys fishing and long walks away from his children.

Flippin Pain is doing a free webinar Pain education for the nation on 15th October. It is part of EU pain awareness day and will feature Cormac along with a number of other speakers who are either patients with lived experience of pain or pain champions in their area.

Additionally, a substantial number of diagnostic laparoscopies yield negative results, and adhesions, while common findings, are not always the root cause of pain. In many instances, the underlying cause of CPP remains unclear so what is paramount is a validation of the patient symptoms and a  multidisciplinary holistic approach. This involves a combination of medical treatments, therapies, and lifestyle modifications. Pain management programmes can be beneficial, providing guidance, coaching, and support from a pelvic pain specialist. 

Trauma-Informed Care

A significant proportion of women with CPP have experienced adverse childhood experiences (ACEs), which can have long-term health consequences, including chronic pain. Trauma-informed care is an important aspect of CPP management. This approach involves understanding the impact of trauma on patients, validating their experiences, and providing sensitive and supportive care.

Overcoming Pain - Diving deep into CRPS15 Oct 202400:10:27

Dr. Ravindran's presentation, "Complex regional pain syndrome: Trauma informed CRPS Care An integrated approach," focuses on trauma-informed care for CRPS patients. This was presented at the UK annual conference organised by CRPS UK. In todays epsiode, I  explore innovative social prescribing approaches with Sandra Mitchell-Phillips, CEO of Ways to Wellness, focusing on neighborhood health, reducing health inequalities, and supporting preoperative care to improve outcomes and reduce NHS costs. Today’s guest is someone who needs no introduction to the world of Pain Care. He has been one of my biggest influences in how I teach pain Science education so a big welcome to Professor Lorimer Moseley, Bradley Distinguished Professor at Adelaide University in Australia. He has authored over 430 scientific papers, written several bestselling pain books, and his educational work reaches millions of people each year through healthcare systems such as the NHS and the US Veterans Affairs.

He is also the founder of Pain Revolution, an initiative aimed at transforming pain understanding in communities, and in 2020 he was appointed an Officer of the Order of Australia for his contribution to humanity through pain science, communication and education.

In this insightful interview, Professor Lorimer Mosley discusses the evolution of pain education, the importance of understanding pain as a protective system, and innovative approaches like pain science education, community engagement, and digital tools to transform pain management.

My guest today is Dr Helen Makins, a Consultant in Pain Medicine at Gloucestershire Hospitals NHS Foundation Trust, UK and someone whose career has taken her across clinical practice, education, commissioning, global health and now national leadership.


Helen is currently the National Clinical Lead for the Chronic Pain Programme at GIRFT – Getting It Right First Time, a major NHS initiative focused on improving clinical outcomes, reducing variation and redesigning services across the country.
But what makes Helen particularly interesting is the breadth of her work — from teaching pain education internationally with Mercy Ships, to developing innovative social prescribing programmes for young people with pain, to exploring the links between neurodiversity, childhood experiences and chronic pain.


She has also worked closely with the Faculty of Pain Medicine, championing better communication skills, clinician wellbeing and the integration of biopsychosocial approaches into training.


So today we’re going to explore:
• how someone ends up leading national pain transformation work
• what GIRFT might mean for the future of pain services
• why communication and de-medicalisation matter so much in pain consultations
• and how we might better support both patients and clinicians in this space.

 In March 2026, I attended the Live well with Pain Conference at Warwick organised by Emma Davies and Laura Hussey. It had some amazing speakers including Prof Roger Knaggs, the President of the British Pain Society. While I didnt get a chance to speak to him, I did speak to most of the other key note speakers and I thought what would they say to 3 questions I had about pain care and the hope that they have when they do the work. In this episode of Pain Speak, I interview Louise McAllan, a somatic experiencing practitioner, who shares her personal journey through chronic pain and the emotional connections that influenced her recovery. Louise discusses the challenges of navigating the healthcare system, the impact of diagnoses, and the importance of emotional awareness in healing. 

Louise McAllen is a Scottish somatic experiencing practitioner and mind-body coach. With over eight years of experience grappling with chronic symptoms, including functional neurological disorder (FND) and fibromyalgia, she has transitioned from patient to practitioner, helping others reclaim their lives from chronic pain. Her unique approach combines emotional awareness with physical well-being, making her a credible voice in the field of chronic pain management.

 She delivers group programmes, 1:1 work and residential retreats, and is pioneering Scotland’s first in-person Emotional Awareness and Expression Therapy (EAET) group in Edinburgh. Trained with SIRPA and Dr Howard Schubiner, she is committed to bringing evidence-based, emotion-focused approaches into mainstream healthcare - reducing stigma around mind-body conditions and sharing the message that meaningful improvement and recovery are possible when patients are given education, choice and appropriate support alongside conventional care.

 The conversation highlights the need for validation, information, and hope in the healing process, encouraging listeners to explore their emotions and reconnect with their bodies.

Transforming Pain Management: An Innovative Approach from the NHS

Sebastian is talking live at the Live well with Pain conference in Warwick in March 2026. 

 In this podcast recorded live in Brighton at the BSLM Annual conference in September 2025, i spoke to Sebastian Senociain, An integrated wellness practitioner with a variety of skills and knowledge helping people in primary care manage their pain. Innovative approaches to pain management are more crucial than ever now and we can see how Sebastian is redefining pain management within the NHS. His transformative work focuses on integrating holistic methods and fostering community, significantly improving the lives of those living with chronic pain.  

Sebastian’s journey began in Chile, where he trained as a medical doctor. His experience in primary care, filled with frustrations over the conventional disease-driven approach, led him to explore alternative methods of healing - including yoga, ayurveda, functional medicine and other approaches in physical and mental health.

At the NHS, Sebastian recognized a pressing need for a more integrative approach to chronic pain management. He noted that Cornwall had a higher than national average prescription rate for opioids, which sparked his initiative to shift the focus from medication to self-management. He piloted a program aimed at helping patients manage their pain through education and community support rather than solely relying on prescriptions.  The pain management program, started in 2023, was meticulously designed to address the diverse needs of patients. Sebastian emphasized the importance of validation and understanding patients' stories. Each 30-minute consultation allowed patients to express their experiences and concerns, forming a foundation for co-designing their care plans. This patient-centered approach not only improved their mental health but also aimed to reduce reliance on medication.  

He has presented his work now regionally and nationally and this was an option to see his poster and talk to him. The video can be seen on youtube.
