Nobody Designed This is a 52-week exploration of why organisations behave as they do—and how we might build them differently. It argues that organisations are rarely designed as coherent systems; they evolve through accumulated decisions, inherited structures, shifting priorities, compromise and local adaptation. Across four linked themes: The Science of Organisations, The Future Executive, Designing Public Institutions and The Adaptive Organisation; the series offers concepts, models and language to help readers recognise patterns, challenge assumptions and intervene more intelligently.

Chapter 11: Institutional Capability - Why talented people are not enough to make an organisation capable

Season 1 · Episode 11

Wednesday, October 21, 2026 • Duration 21:28

In Chapter 11 of Nobody Designed This, we explore Institutional Capability and why talented people are not enough to make an organisation capable. This chapter distinguishes individual competence, organisational capacity and enduring institutional capability. It argues that capability is not a possession but a renewable conversion system: the institutional ability to join knowledge, people, routines, technology, authority and learning so that resources become reliable outcomes across time, turnover, stress and adaptation. It introduces the Institutional Capability Stack and four tests of capability: repeatability, turnover, stress and learning.

This chapter explores the concept of institutional capability, distinguishing it from simple resource availability or individual talent. It argues that true capability is a renewable conversion system that connects knowledge, people, and technology to produce reliable results over time. Through the framework of the Institutional Capability Stack, the author illustrates how performance is often found in the "joins" between organisational layers rather than the layers themselves. To verify this strength, the text proposes four essential tests: repeatability, turnover, stress, and learning. Ultimately, the source warns against capability debt, where short-term successes are achieved by exhausting the very systems required for future performance. Sustaining an organisation therefore requires disciplined stewardship of its collective routines rather than a reliance on isolated heroic efforts.

Chapter 10: Organisational Friction - how effort disappears into the cracks

Season 1 · Episode 10

Wednesday, October 14, 2026 • Duration 24:59

In Chapter 10 of Nobody Designed This, we explore Organisational Friction.

This chapter explains why organisations full of capable and committed people can consume extraordinary effort without producing an equivalent effect. It defines organisational friction as the additional work, delay and human burden created when purpose must cross organisational boundaries, rules, systems, and relationships. It distinguishes protective friction from avoidable obstruction and introduces the Organisational Friction Map and Friction Profile for revealing where effort and time are absorbed, displaced, and normalised.

This chapter explores the concept of organisational friction, which represents the hidden costs and human burdens incurred when work crosses misaligned boundaries, systems, and relationships. It distinguishes between wasteful obstruction, which forces employees to perform unnecessary navigation work, and protective friction, which provides essential safety and quality checks. By introducing the Organisational Friction Map, the author categorises the system sources that drain efficiency and create heavy cognitive and emotional loads for staff. Rather than demanding more effort from individuals, the text argues for intentional design that makes productive tasks easier while ensuring safeguards remain purposeful and proportionate. Ultimately, the source provides a diagnostic framework for leaders to identify where invisible labour compensates for design flaws, enabling them to repair the organisational "joins" that impede progress.

Chapter 9: Why Large Organisations Drift - how reasonable local decisions create an unselected organisational direction

Season 1 · Episode 9

Wednesday, October 7, 2026 • Duration 20:41

In Chapter 9 of Nobody Designed This, we explore Why Large Organisations Drift and how reasonable local decisions create an unselected organisational direction. This chapter explains how large organisations can move a long way from their intended direction without making a single decision to do so. It distinguishes drift from useful adaptation, failed implementation and emergent strategy, and introduces the Organisational Drift Triangle: a time-based diagnostic for comparing declared purpose, lived operating practice and real resource commitments.

This chapter explores the concept of organisational drift, a process where large entities unintentionally stray from their stated missions through the accumulation of small, local decisions. The author introduces the Organisational Drift Triangle as a diagnostic tool to examine the tensions between declared purpose, actual work practices, and resource allocation. By analysing high-profile failures at Boeing and within the NHS, the source illustrates how routine adaptations and shifting priorities can compromise safety and strategic integrity. While some divergence can represent healthy learning, drift occurs when these shifts remain unrecognised or unselected by leadership. Ultimately, the text argues that systemic coherence requires constant monitoring of how daily behaviours and funding choices align with an organisation's official goals.

Chapter 8: Complexity Is Created One Exception at a Time

Season 1 · Episode 8

Wednesday, September 30, 2026 • Duration 24:30

In Chapter 8 of Nobody Designed This, we explore how Complexity is Created One Exception as a Time. Thischapter explains how individually defensible exceptions, accommodations and inherited variants combine into costs that no single decision owns. It introduces the Exception Accumulation Curve and the Exception Test, showing howcoupling shapes cost and how legitimate variety can be preserved without unmanaged complexity.

This chapter explores how organisational complexity arises from the accumulation of individually justified exceptions and deviations from standard designs. The author introduces the Exception Accumulation Curve to demonstrate how separate variations eventually interact, driving up systemic costs for coordination, data reconciliation, and future changes. While some variety is necessary for legal or operational reasons, unmanaged adaptations often create a long tail of hidden dependencies that no single person owns. To combat this, the text proposes an Exception Test and a modular architectural approach to ensure that legitimate differences are purposefully governed rather than left to accumulate as technical or administrative debt. Ultimately, the chapter argues that proactive portfolio governance is essential to prevent an organisation from drifting away from its intended design through thousands of minor, uncoordinated choices.

Chapter 7: Every Organisation Has Two Operating Models

Season 1 · Episode 7

Wednesday, September 23, 2026 • Duration 27:14

In Chapter 7 of Nobody Designed This, we explore why Every Organisation Has Two Operating Models. This chapter explains why charts, policies and governance documents never fully describe how work gets done. It introduces the Dual Operating Model and the Operating Model Delta: a practical diagnostic for determining whether the relationship between authorised design and lived practice is creating capability, compensation, contestation, exclusion or critical dependence.

This episode introduces a Dual Operating Model framework to explain the discrepancy between an organisation's authorised design and its actual practices. While the declared model establishes formal authority and accountability, the lived model encompasses the informal relationships, social learning, and practical adjustments necessary to complete real-world tasks. The author identifies the Operating Model Delta as the space between these two layers, providing a diagnostic tool to evaluate if deviations create essential capability or hidden fragility. By examining patterns like compensation, contestation, and exclusion, leaders can determine whether informal workarounds should be legitimized or redesigned. Ultimately, the source argues that organizational success depends on balancing official structures with the nuanced, relational expertise that truly drives performance.

Chapter 6: Decision Latency

Season 1 · Episode 6

Wednesday, September 16, 2026 • Duration 21:17

In Chapter 6 of Nobody Designed This, we explore Decision Latency. This chapter explains why organisations can be full of urgent activity yet remain slow to respond. It introduces the Decision Latency Curve and a practical Decision Latency Trace, showing how elapsed time, distortion, rework, queueing and accountability gaps accumulate between first credible signal and observable effect. Its central argument is that organisational speed is not produced by demanding greaterurgency from individuals. It is produced by designing authority, information flow, governance, boundaries and execution capacity so that decisions can become effects.

This episode explores decision latency, defining it as the total time elapsed from the first credible signal of a problem to the observable effect of a solution. Rather than blaming individuals for a lack of urgency, the author argues that delays are usually the result of poor organisational design, such as fragmented authority, communication silos, and rigid governance cycles. Using the Decision Latency Curve andreal-world examples like the Manchester Arena bombing, the source illustrates how time is often lost during recognition and execution rather than just the formal approval stage. The text emphasises that structural fixes, including clearer delegation and better psychological safety, are necessary to reduce the risks associated with waiting. Ultimately, the goal is to create a differentiated system where the speed of a response is appropriately matched to the complexity and reversibility of the choice at hand.

Chapter 5: The Conservation of Bureaucracy

Season 1 · Episode 3

Wednesday, September 9, 2026 • Duration 22:51

In Chapter 5 of Nobody Designed This, we explore The Conservation of Bureaucracy. This chapter explains why bureaucracy persists even when everyone agrees there is too much of it. It introduces the Bureaucracy Conservation Cycle and distinguishes protective, performative, and inherited bureaucracy. Its central argument is that bureaucracy is rarely destroyed by reform alone; more often, it changes form, moves location, becomes embedded in systems, or reappears as another demand for assurance.

This episode explores the conservation of bureaucracy, arguing that administrative burdens often persist or relocate rather than disappear because the underlying demands for assurance remain unchanged. While formal structures like rules and records are essential for fairness and consistency, they become problematic when they transition from protective functions to performative or inherited routines. The author introduces the Bureaucracy Conservation Cycle, illustrating how institutional anxiety leads to new controls, which in turn spark informal workarounds that further erode trust. Digitization and automation may inadvertently multiply these burdens by making it easier to capture data without evaluating its actual contribution to safety or efficiency. Ultimately, the source suggests that true reform requires examining the original purpose of a control and establishing clear criteria for its retirement. Effective leadership must therefore focus on maintaining purposeful structures that support responsiveness rather than merely accumulating evidence of activity.

Chapter 4: When Organisations Forget

Season 1 · Episode 4

Wednesday, September 2, 2026 • Duration 25:33

In Chapter 4 of Nobody Designed This, we explore Why Organisations forget.

This episode explores how institutions lose critical knowledge despite maintaining vast digital archives and formal reporting systems. The author defines organisational memory not as a collection of stored data, but as the active ability of past experiences to shape current decision-making. Through concepts like Memory Half-Life and the Organisational Memory Chain, the source diagnoses why vital understanding decays when the context, relationships, and routines behind information are disrupted. It emphasizes that tacit knowledge, the informal expertise held by people, is often more valuable than explicit records, which can become unintelligible once the original rationale is forgotten. Ultimately, the text argues that true learning requires a continuous cycle of capturing, contextualising, and reusing information to prevent "narrative debt." Efficient retrieval through modern technology is shown to be a poor substitute for an authentic institutional capability to remember and apply lessons.

Chapter 3: Transformation Debt - The Hidden Liability Every Organisation Carries

Season 1 · Episode 3

Wednesday, August 26, 2026 • Duration 19:55

In Chapter 3 of Nobody Designed This, we explore Transformation Debt: the hidden liabilities left behind by organisational change that consume time, attention, money and future capacity.

This chapter explores the concept of transformation debt, which refers to the lingering liabilities and unfinished tasks left behind after organisational changes. It explains how temporary compromises, such as redundant software or overlapping responsibilities, often become permanent fixtures that drain resources and hinder future progress. The author introduces a Balance Sheet framework to categorise these burdens into seven areas, including structural, technological, and cultural debt. By viewing these residues as financial interest, the source highlights how poorly managed history reduces an organization's capacity for innovation. Ultimately, the text advocates for a disciplined management approach that emphasizes deliberately retiring old systems rather than just adding new ones. This strategic shift ensures that inherited complexities do not eventually paralyse the institution’s ability to function.

Chapter 2: The Laws of Organisational Gravity

Season 1 · Episode 2

Saturday, August 22, 2026 • Duration 22:11

Chapter 2 builds on the idea that organisations are complex human systems by asking why they repeatedly move in predictable directions, even when leaders intend otherwise.

It introduces the metaphor of organisational gravity: structural forces that make some behaviours easier, safer and more likely than others. Like physical gravity, they do not make change impossible. But they shape the effort required, pull organisations towards familiar patterns and help explain why progress is difficult to sustain.

These patterns are often attributed to poor leadership, resistant staff or culture. Yet organisations can employ capable people, adopt ambitious strategies and commit to empowerment while still producing centralised decisions, filtered information, growing bureaucracy and short-term behaviour. The system continues to reward caution, escalation and visible compliance.

The chapter identifies six recurring pulls.

Decisions climb when authority is ambiguous but responsibility for failure is concentrated. Escalation becomes safer than exercising judgement locally, even when policy delegates authority.

Information reassures because bad news is costly to transmit. As information moves through management layers, uncertainty and uncomfortable evidence are softened, leaving leaders with a more reassuring picture than the frontline reality.

Exposure descends when authority and operational consequences become separated. Senior leaders may approve a policy, but staff and service users experience its burden and unintended effects.

Structures accumulate because adding a control, committee, report or approval is easier than removing one. Each addition may be rational alone, but together they create friction and make the organisation harder to navigate.

The present consumes the future when immediate pressures absorb the time, attention and capability needed for improvement. Transformation remains important but is repeatedly displaced by urgent demands.

when demonstrating compliance becomes more important than achieving the outcome the rules were intended to protect. Evidence of activity substitutes for evidence of value.
