Welcome! and Thank you for listening.  Today I have a plant based strong man on the podcast.   Nathan Majnaric, SCCC, CSCS.  Most plant based athletes lean toward the endurance world so it is great to have the opportunity to speak with a powerlifter, strength coach, and a blue belt in jujitsu that is not only fueled by plants, but has improved on plants.  It was not a compromise but an asset to fuel plant based while continuing to advance in his chosen sports but also from a basic health standpoint.

Most lifestyle diseases are a result of impaired metabolism.  Our muscles are a major user of energy, and the organelle that is the power plant of the cell is the mitochondria.  As we lose muscle mass, we lose mitochondria.  This leads to impaired energy utilization as well as the risks of falls and brain perfusion of nutrients.  Bottom line, we all need to train to maintain our muscle mass or regain muscle mass if lost through inactivity.  It can be the difference in living independently or the ability to participate actively in life. 

We will discuss approaches to strength training as well as fueling for recovery.  How much and how often or is anything better than nothing are often topics of concern. 

Given Nathan’s diverse training experience, we feel comfortable of offering online training programs specifically designed for an individual’s goal.  We will also begin to offer limited personal instruction for those with specific needs and the desire to have a someone that can interact with them to achieve a specific goal.

A New England Journal of Medicine Study looked at how much energy people thought they consumed, and how much energy they actually burned.  They compared two groups of overweight individuals based on reported energy consumption.  Group one had difficulty loosing weight despite a reported 1200 kcal or less consumption of food and vigorous exercise.  There were some surprising findings and some consistent findings that I think you will find interesting. 

Nutrition is one of the biggest contributors to a healthy life span yet we ignore it's significance and celebrate eating poorly.  We are made to believe that overindulgence is necessary for true happiness.  We look at food restrictions as a form of punishment.

We age and our cells age.  About every three months our red blood cells turn over for example.  What happens to the old cells? How do we eliminate them?  Our immune system plays a vital role in eliminating old cells or senescent cells.  Unfortunately, our immune system also slows down so we start to accumulate these "zombie" senescent cells.  This month Nature magazine discusses new drugs and repurposed drugs to eliminate these cells.  However, these medications also have side effects and substantial costs.  Since we all age, should everyone start these medications or do we wait until there is a problem like diabetes, Alzheimers, or vascular disease?  Another alternative might be to decrease the amount of zombie cells in the first place or protect our immune system so it functions longer.  

You guest it.  Nutrition and exercise play a key role as an alternative to decrease zombie cell accumulation.

I was fortunate to go to Blue Ridge and Blairsville Georgia this past weekend to toe the line at the Cruel Jewel 50 mile ultra race.  The ultra running community are my people.  Tenacious, positive, and tremendous athletes all humble at the same time.  It did not goes as I would have wished for, but it did go for how I was trained.  I got to see where I was at a fitness level and experience some of the most beautiful and peaceful forests.  That area crosses the Appalachian trail.   People on the trail give other hikers trail names.  I had a trail angel this weekend.  I gave him the name of Russ Angel because of time he gave to rescue me and others after we pulled the white flag.  I hope you enjoy my trail tails.  It was a great weekend despite coming up short.

Ultra running and life are so much alike.  You need a plan and you need to work the plan.  When it does not work, you need to adjust the plan.  The bottom line is forward progress.  Maybe not that day, but the next day it is time to get back on the journey whether it is trail or health.  If you would like some help with your health journey, visit our website to fine out how you can join our practice.

In the Journal of Nature on May 2, 2024 there was an article discussing the most basic molecular changes with endurance training that affects a multitude of processes that have the potential to impact the health of the immune system, and organs such as the heart, kidneys, colon, brain and liver.  There were positive changes at the genetic level with endurance training of just 8 weeks. What are you waiting for? That was not the case with omega 3 supplements or a fat pad around the heart.

Risk factors for Lifestyle diseases are increased BMI, hypertension, hyperlipidemia, smoking, sedentary lifestyle and metabolic dysfunction ranging from glucose intolerance to diabetes.  Traditionally, younger people do not get lifestyle diseases because they may either have no risk factors or only have one.  More and more younger people now have multiple risk factors in the presence of glucose intolerance.  This places them at increased risk for the early development of lifestyle diseases.  

Studies claiming that cholesterol is not bad often look towards a young population with no other risk factors.  They may not manifest disease as quickly and it is assumed that they never will.  Some may not but we just don't know.  Often, we hang onto studies that support our nutritional choices even though we may not have anything in common with the study population.  Risk factors are there and cumulative.  Work on eliminating them and you might just avoid a lifestyle disease.

What are your health goals?  Do you have a bucket list?  My grandmother used to say that if wishes were horses, beggars would ride.  I think there is a lot of confusion between wishing we were healthy and having a plan to do something about it.  We wish there was a magic pill or supplement that would make us young, healthy, energetic, and happy.  It would be even better in the form of a gummy that tasted like candy.  Sorry, that is not an option. 

I think it is important to see or imagine yourself as you would like to be, and then initiate the step to complete that journey.  It is not wishing.  It is action.

I just turned 62, and I am walking and running better than I did the last half of 61.  I am working the plan to achieve my bucket list.  And as I empty the list, I am filling it up.

Who are you?  What story do you use to answer that question?  Perhaps your story fits who you are but not who you wish to be.  Today's podcast is about changing the story to become more healthy.  Our story can help us accept our circumstances, but we often forget we are the authors of our future selves.  

Changing who you are is about changing your story.  Did a health crisis change your story?  Health care providers are at a very high risk of planting seeds to change people's story.  Perhaps you were active then had an accident.  The doctor said you have bad knees.  You rewrite your story to fit your bad knees.  You cannot run, walk, or do sports.  You gain weight because of your bad knees.  You no longer enjoy travel because of your knees.  Your story totally changes because of what your doctor said about your knees.

Jami Dulaney MD Plant Based Wellness Podcast Episode 467: Early Detection versus Prevention15 Mar 202400:32:15

Telomeres are the end caps on DNA that protect it from unraveling.  They can be measure in white blood cells to determine their length.  The shorter they become, the more likely for the DNA to start to develop errors.  This leads to failure of the bodies organs and functions i.e. aging.  It has recently been linked to the risk of developing heart failure.  

There is another blood test that looks for DNA fragments in the plasma of the blood that is associated with colon cancer.  Detecting advanced cancers is pretty good.  It is not so good at detecting early treatable cancers.  It is just a blood test.  However, it is not perfect so what will you do with the results.  

If a breast cancer tumor expresses estrogen receptors then inhibiting estrogen should decrease the cancer growth.  How much is enough? 

All of the above sound like reasonable tests and solutions.  But do they prolong survival ?  Very few studies actually look at overall survival resulting from an intervention.  They look at disease free survival, or tumor regression, or ability to diagnosis a cancer.  It seems as though a diagnosis could lead to more rapid treatment or more aggressive treatment would lead to a better outcome.  But what are the downsides of a therapy or early diagnosis that might ultimately lead to side effects that shorten lifespan or decrease quality of lifespan.  

Doing something and being aggressive are seen as positive interventions.  Why don't we view lifestyle changes in the same way?

Burro racers will tell you that a burro needs to feel safe in order to try something new.  He will not be lured into a bad situation as he sees it.  Horses are much more about pushing to the limits and need a variety of coaching to reach their potential.  

When we discuss lifestyle changes with people, we need to know how they react to various forms of coaching.  

Some people join a bootcamp that have not exercised in years only to get beat up in the first few sessions by a drill master that is trying to whip everyone in shape.  They get injured and never go back, and reinforce why they are not cut out for the change. Some people are over the top and need a coach to actually slow them down before they hurt themselves.  Finally, there are those that continue steady forward progress completing step after step with a continued eye on the goal.  Knowing which type you are is important when you decide to embark on lifestyle changes.

Are we at the mercy of our genetics?  Should we just accept our fate as being genetically flawed is some way or another.  There is talk about plasticity of the brain or forming new circuits.  Body builders change their shape through specific workouts.  We can become runners after starting a couch to 5 K program.  So why do we think we cannot change our genetic fate when it comes to cancer or cardiovascular disease. 

Most allopathic physicians focus on testing and following disease progression.  I  even heard  from a patient that was told by a cardiologist that he will know when he needs him.  Why wait until you already have disease?  The only preventative therapy is medication in the world of sick care.   Take as much as you can tolerate before side effects become intolerable.  

One such example is the release of new medications that become part of protocols that everyone with a diagnosis receives.  No individual evaluation.  One size fits all.  A pharmaceutical representative educates providers with the study results.  There is only time for the conclusion.  There is no discussion of the actual patients studied or of the patients that were rejected from the study because of intolerances.  Just prescribe the new drug and eat a free lunch.

However, you can greatly influence your longevity through nutrition and exercise.  Start the journey and continue to grow and build your comfort levels.  Track your wins or progress.  Note how you feel after exercise or eating well.  And if you would like some guidance or support, check out our website to see how you can become a member of our practice.

But how much time, and what is the quality of that time?  My grandparents died when I was fairly young and I wished I had more time with them.  For the last several years of their lives, they were not that mobile and limited in their activities.  It does not have to be that way.  But you cannot rely on luck.  How do you get to be the cool grandparent that does not hold anyone back and encourages living life to the fullest.

You had better be careful eating that plant based diet.  You could be deficient in all kinds of things.  You could even be anemic?  Are you tired?  Perhaps you suffer from iron deficiency.  These statements are all possible; if you eat a vegan diet of Oreos and vegan deli slices. 

Iron is vital to a whole host of reactions in and out of the body.  Have you ever seen rust?  It is vital to make red blood cells, repair DNA and metabolize nutrients.  It is also very hard to get rid of especially if you are a male.  So before your health care provider starts to worry you, listen to this podcast and find out if your are at risk.  

Why is there a national donut day and not a fruit day?  Why are people afraid of fruit?  Is eating fruit flavored things the same as eating fruit?  That seems to be just fine in mainstream information.  No one has blamed donuts for causing cancer.  In fact they have a holiday.  Why not fruit?  

Doctor: you are gaining weight.  Patient: It is the fruit.  The answer is in the podcast.

I raced the Long Haul 100 Mile Trail Race this past weekend.  There were many surprises and challenges over the 31 hours and 5 minutes that it took me to cross the finish line.  In fact, there was a time when I did not think I would see or even care to see the finish line.  

Ultrarunning is any running event over 26.2 miles.  This one was 100 miles.  Six times through a 16.7 mile loop at Colt Creek State Park in Florida.  It was describes as grassy trails and hard pack jeep roads.  Mother nature added water and mud.... Lots of it. 

I want to thank my crew: Michael Hubbard, Lynda and Lew Albert, and Timaree Hagenburger.  Without them, the story would be much different.  They helped me and cared for me selflessly in cold, windy, conditions all day, all night, and into the next afternoon.  The best part is that we are all ready to do it again.  Just not tomorrow.  Thanks to Andy Matthews, the race director for his tireless efforts to put on a race in extreme conditions.  I hope he gets the mud out of his jeep wheels someday.  

I hope you enjoy my story of the Long Haul 100.  

Life is better when we do it together, and even better if we do it outside.  Visit a state park with your friends and family and you will see it is the best $3 you will ever spend.

Early onset dementia, meaning occurring before 70 years of age, is associated with several risk factors that can be addressed now and prevented.  There are 15 risk factors that stand out among many.  It may seem obvious that a history of diabetes, previous stroke, heart disease or depression would be among them.  But how about hand grip, Vitamin D and social isolation?  I will discuss these and the others .

People often say this or that hospital is ranked number one and is the best.  Why? I present a study today on sodium and blood pressure.  The take away was that nearly everyone can lower their bp by reducing the sodium intake.  The best way to do this is make food yourself.  I will give you a few examples in today's podcast for celebrating a little healthier.

Welcome! and Thank you for listening.  Finally we have a randomized controlled study on a plant based diet versus an omnivore diet.  It was only 2 months, but it gives a lot of information to contemplate.  

We are by history omnivores.  That means we digest meat, dairy, fruit, vegetables, whole grains, nuts and seeds.  Buts should we?  The problem with most studies is that they rely on food diaries.  As you well know, they are highly inaccurate.  People report what they think you want to hear or the supposed "right answer" a lot of the time, and this has even been shown in numerous psychological studies.  However, in this short study, the food was supplied.  They also had a twin to keep them honest and compete with. 

In as little as 2 months, the plant based twins saw a statistically significant improvement over the omnivore group with regard to lifestyle disease risk factors and weight. 

Two month or 8 weeks!.  They saw results.  The debate is over.  It works if you do it.  The if is the big question.  How do you sustain a healthy plant based diet?  It is not as hard as some people would like you to believe.  You don't need potato starch paper to put supplements on to swallow.  You don't need to eat dehydrated soup for every meal or count your blueberries.  You need a goal and simple meals.

Welcome! and Thank you for listening.  Today I have a guest, Dr. Matt Minard.  He is a physical therapist that specializes in running injury prevention and running mechanics.  There is a high incidence of runners that get injured.  Some would say that that is an excuse not to run.  However, the benefits of running markedly outweigh the risk of injury. In fact, runners have a less frequency of joint replacement than non-runners.  Running has been associated with a decreased risk of not only cardiovascular disease but cancer and cancer recurrence.  This is most likely from the anti-inflammatory benefits as well as the long-term reduction in sympathetic tone as seen in decrease blood pressure and heart rate.  The sense of well-being or perhaps runners high is associated with anti-depressant effects, and studies have shown that running can be a healthier alternative to medications. 

The problem with running is that we tend to do too much too quick whether that is volume or speed.  The most common injuries come from early stress to the small tendons, bones and ligaments that support our weight.  The biggest fallacy is permanent knee injury.  Most knee injuries are again associated with ligaments and tendons and not destruction of the joint. 

Most injuries arise from or are exacerbated by faulty mechanics.  A child runs perfect until we put clunky shoes on them.  Because we become more sedentary as we age, we are prone to more injuries when we suddenly take to running at a later age. 

Enter Dr Matt Minard.  He will share with us today some ques for more efficient and safer running with the goal of extending our running longevity for many years to come.   He has a podcast for those who like to listen, but I would suggest his Instagram clips and YouTube videos for excellent training tips and demonstrations.  He also does private analysis of running form and coaching if you become interested.   I hope you enjoy the podcast.

Thanksgiving food and overeating is not just hard on your waistline but can be very hard on your heart.  The sodium in a day's worth of overindulgent foods can very quickly make the blood pressure skyrocket, and the water retention can lead to heart failure.   A few quick tips can go a long way to keep you healthy. 

The holiday season can be very stressful.  Honesty is the best policy.  Being with your family and friends is of utmost importance.  The first step is to tell people how much they mean to you and how much you enjoy their company.  However,  you also need to ask for their support in your food choices.  Make it easy and bring a few dishes as possible.  Keep the food talk minimal and light.  It is not the time for a health lecture. 

If you get through this first feasting holiday, you will have the confidence to continue on your health journey.  Make every day count.  It is the people that make the holiday not the food.

After working in industry for many years he became fascinated with processed food ingredients.  He shared that every ingredient in food has a purpose.  Most are to provide shelf life, texture, and stability.  As time has passed more of these ingredients are derived from a chemical reaction versus plant source extraction.

Welcome! and Thank you for listening.  The Austin Rattler 50K trail run is in the books.  The race venue was Reveille Peak Ranch which is a 1300 acre ranch which hosts various endurance events and even hunting courses.  

Three 10 mile loops to completion on various terrains made it a very interesting event.  Ultra running is a series of trouble shooting events that include navigation, nutrition, pacing, and mental focus.  It is also a day to play outside.  I had a few new trouble shooting experiences to share. 

One thing we all need is to move, to play, and to be socially interactive. This race was what I needed to change my focus and absorb good energy from the other runners and great outdoors.

Today I have a guest interview with Greg Schnoor.  He is a brain cancer survivor and ultramarathon runner.  He was diagnosed inadvertently with brain cancer in his thirties while undergoing evaluation for recurring sinus infections.  His first reaction was how will my children remember me? He attributes his success on a persistent positive mental attitude and running.  Running long.  He took up running during his chemotherapy and has never quit. 

Running with Cancer is an inspiring book even if you don't run or don't have cancer.  The basic premise is savoring.  Savoring each moment in life and finding the positive in every situation.

What is a whole food? What is a clean food?  Those terms are thrown around with authority and ultimately blend into just food.  If you ask most people on a diet, they will throw out that they are eating 1200 calories a day.  That seems to be the magic number that has stuck in the diet world.  Apps are available that helps one to prove a point.  They are only as good as what is entered.  I have never had a person tell me that they consumed 3000 calories in a day, but I am sure it happens on a regular basis and more often than 1200 calories.  What about the weight of food?  Does a pound of chips have the same amount of energy as a pound of potatoes?  What about a pound of carrots.  

It turns out that vegetables, tubers, and fruit contain about the right amount of calories per pound for humans to consume and maintain a healthy weight.  It also turns out that we as a society do not burn near the calories as a typical hunter-gatherer society.  It is also true that men burn more calories per pound of body weight than women.

There are always exceptions and derivatives that push boundaries.  The diet police can really make life complicated to the point where a lot of people give up.  The cold hard reality is that there is no plant that will make you live forever.  There are some poisons that will definitely shorten your life.  And there is a whole lot of room in between.

Welcome! and Thank you for listening.  If you would like to get some nutrition or medical advice, just tell someone you are plant based.  They will tell you all the things that you will be deficient in and how keto has worked well for them.  Of course, they are currently on a break or having a cheat day, but it still works for them.  

It is pretty rare to have someone listen to your story and just say, good for you. I was fortunate enough to have my father give me a great piece of advice.  Make a decision and own it.  That advise is a little easier if the decision is something that is of shorter duration, but what if that decision puts you at a crossroads in life? When it comes to medical decisions, we try to rely on scientific data and statistics to get the best outcome.  However, it is very difficult to apply statistics of a group to one individual. 

You are unique.  Your life experience and values are unique.  All the AI in the world cannot make the correct choice for you.  Only you can decide, and that is something you can own.  You write your own story based on your health care decisions.  It is your story based on your perceptions of what is right for you.  The science is flawed and biased because it is conducted by humans that are learning along with their biases and interpretations.  The best outcome from a study may not be the one you get.  However, there is a lot you can do to have the outcome you desire.

Patients rights.  Informed consent.  Who really benefits from these concepts? If I inform you of the risks and benefits they are supposed to be to the best of my knowledge.  You would need to know the depth of my knowledge to fully understand that.  The other problem is when people are in destress or pain, they often only hear the first sentence or so.  Their minds are racing with their own questions and concerns.  Living often trumps some discomfort or chance of survivable injury.  I often hear that my doctor is the best.  He is one of the top in the world.  Back room news, there is no contest or ranking of the best.  Brick and mortar institutions carry little guarantee of the best.  They are often the most political and often owe quite a bit to their funding organizations.  Bottom line, stay healthy.  If you do become ill, do your research.  Bottom line, it is better to stay healthy.  

Staying healthy requires taking some responsibility.  If you want to decrease risk, improve your baseline health.  How?  Plant based nutrition and exercise.  The other key to success is being your own cheerleader.  No one has your ear better than your own inner voice.  You can do it.  One more step.  Anybody can walk 3 miles.  I get to see the sun rise while exercising.  It is all self talk.  Positive or negative.  You get to choose, and that choice affect your health greatly.  Enjoy the podcast.  Thanks for listening.

How about your movement goals?  Do you have a plan to achieve the goals or are you just floating through life hoping for a good outcome?  Most people are told that you cannot reverse lifestyle diseases.  They just happen and you must adjust to taking medications and being a "patient".  I do not believe that is the truth.  However, you must have a specific plan and monitor your progress as you go along.  

This is where devices come in.  You have to actually use them instead of collect them.  A dusty scale does not hold you accountable or help monitor fluid balance.  Continuous glucose monitoring only helps if you use the information to adjust your intake.  You must use the data in real time.  Smart watches are great to track heart rate during exercise and at rest.  But you have to wear them and active them with exercise and then look at the data.  By doing this, you can get an idea of what your baseline numbers are and then track the changes over time.  

Consistency is key.  Intermittent attention to the plan results in yo yo results and no forward progress.

Community, mindfulness, endurance, grace, grounding, connection with others, and connection with nature are just a few words that come to mind.  I experienced them all on a volunteer level and the runners and crews experienced them exponentially.  

Dig deep, you can do more than you think you can are words of the race founder, Ken Chlouber.  Those are things to do when motivation deserts you.  And as he says, motivation will desert you when you need it most.  

Can't, try, and it is too difficult, are things that we hear too often in the regular world.  Comfort and easy will not be there for you when times get tough.  Yet, most of us prefer comfort and easy.  Hacks to make life more simple.  Prepared food, take out, and pleasure of the palate are what we turn to when motivation leaves us at the alter.

What will you do with the results of a test? How will you react? I was very happy to see normal bones and joints on a recent CT scan of my leg, but it did not show the source of my ankle pain.  Good news! Nothing bad.  I can return to training.  It could also be interpreted as I am still in pain and don’t know why.  I am taking the win and I am grateful.  It is good to exclude bad things even if you do not know exactly what the problem is.  The odds are in your favor for a good outcome.  Testing and procedures are a game of risk and reward.  Having a plan for the outcome before diving in is always a good idea. 

Walking an hour a day is good, but how fast do you need to go?  However, how fast your heart rate goes up is also important.

There are controversies around running ultra distances.  Is it healthy?  Can it even cause coronary artery disease?  Does it shorten life? I believe that is a reductionist way of thinking about it.  In a time where people are looking for longevity and life "hacks", the easy way out, and guaranteed outcomes, ultra running certainly is frowned upon.  

After all, who would drive from sea level to a remote area at 10,000 feet to run to 12,000 ft four different times for a distance of 49.2 miles (that is what my watch said).  It was hard and I got tired and dirty.  I was racing the clock to finish within cut off times.  My heart rate was elevated into the 130s for 14 plus hours.  I drained the glycogen from my body.  

I saw some of the most beautiful mountain ranges in the world.  The air was clean and smelled of pine.  The sky was bright blue and the mountain tops were covered in snow.  There were wildflowers growing and clear streams.  There were over 800 runners pushing their limits cheering each other on because it was about doing your best and wanting that for other Leadville family runners as well.  It was dealing with adversity mentally and physically.  Choosing strong over weak.  It was meditation.   It was celebrating a life worth living to the fullest. It was about the joy of being free from the mundane distractions of the world.   It was about being alive and back to nature.  It was regaining what humans were meant to do: move.  Thank you Leadville family for welcoming me with open arms. 

Did my coronary calcium score go up this weekend?  I know my love of life, other humans, and sense of wellbeing did.  That is all that matters.  I hope you enjoy listening to my story of the Silver Rush 50 mile trail race.

The Silver Rush 50 Mile Trail race is in the books.  Finish time 13:41:42 .   Third place in the 60-69 age group category.  It was definitely the hardest race I have done to date not only the distance but terrain and altitude.

Have you ever heard the saying, I'm not loosing weight but I am building muscle?  How much does a pound of muscle weight?  The same as a pound of feathers.  If your are working hard enough to build significant muscle, you will be burning energy and digging into your fat stores.  Glycogen is stored primarily in muscle and is accompanied by water.  The only true way to measure muscle content with a Dexa scan.  However, it is not covered by most insurances and exposes one to radiation.  A bioelectrical impedance scale is not a bad weight to estimate muscle.  There are errors if someone retains water secondary to kidney or heart disease but can be used to see changes in body composition.  This episode will look at a study comparing various body measurements with longevity.  Spoiler alert; I hope you are carrying lots of muscle.
