| 133: From Burnout to Balance: Understanding Cortisol, Oxytocin & Women’s Hormones | 18 oct. 2026 | 00:57:05 | |
Dr. Anna Cabeca joins me to talk about why so many midlife women are told their labs are normal while they feel exhausted, foggy, and out of step with their own bodies. We talk about what shifts in perimenopause, how falling progesterone and insulin resistance can show up as everyday symptoms, and why Dr. Cabeca treats hormone imbalance as a metabolic question first.
We also look at how cortisol and oxytocin work together, what burnout can look like physiologically, and simple ways to support oxytocin. From there we get into keto and urine pH, what the research says about hormone therapy, and how the vaginal and urinary microbiome may relate to bladder symptoms.
Topics We Cover in This Episode:
Why "normal" labs don't always mean you feel normal
What happens to progesterone and estrogen between ages 35 and 50
Why hormone symptoms may start as metabolic ones
How cortisol and oxytocin interact, and what burnout can look like
Free, everyday ways to support oxytocin
Why keto doesn't work the same way for every woman
What the research says about hormone therapy, and why the environment matters
How the vaginal and urinary microbiome may connect to bladder symptoms
If you've been feeling tired, foggy, or dismissed when your labs come back normal, tune in to this conversation with Dr. Anna Cabeca. Subscribe so you don't miss an episode, and share it with a friend who needs to hear it.

| 132: The Chemical Load We Carry: Toxins, EMFs & Hormonal Health | 11 oct. 2026 | 00:58:23 | |
What if I told you right now, this very moment, your body is carrying a chemical load that you have never been tested for, never been warned about, and never asked for? In this episode, I’m joined by Nick Pineault, author of The Non-Tinfoil Guide to EMF and creator of the documentary Body Burden: The Toxins We Carry, to talk about what “body burden” actually means and how the chemicals we encounter through our food, water, air, plastics, household products and technology may intersect with hormones, metabolism and overall health.
We talk about how environmental medicine approaches toxic exposure, when testing may be useful, what Nick learned from discovering his own elevated lead, mercury and mycotoxin levels, and why reducing exposure may matter just as much as focusing on detoxification. We also get practical about plastics, water filtration, sleep, EMFs, sweating and elimination—and discuss why looking at the cumulative burden, rather than searching for one single toxin to blame, may be the more useful way to think about environmental health.
Topics We Cover in This Episode:
What “body burden” actually means—and what can accumulate over time
Why testing for specific toxins can be more complicated than it sounds
What Nick discovered when he tested his own body burden
Why reducing exposure comes before trying to “detox”
How plastics and endocrine-disrupting chemicals may intersect with metabolism
The connection Nick sees between EMFs, sleep and other environmental stressors
Simple changes that can reduce everyday exposure
Why environmental medicine takes a systems-based approach
If you’ve ever wondered whether the environment around you could be affecting your hormones, metabolism, sleep or energy, this conversation gives you a lot to think about without turning it into another list of things to fear.

| 131: Cell Care Over Self-Care: A New Way to Think About Your Health | 04 oct. 2026 | 01:00:03 | |
Dr. Monisha Bhanote joins me to talk about cellular health and resilience, and why fatigue, brain fog, digestive issues, and other everyday symptoms may be signals from our cells rather than things we should simply accept. We talk about her approach to cell care, cellular consciousness, and why health practices need to be personalized rather than copied from the latest trend.
We also explore why Dr. Bhanote considers travel a stress test for the body, how digestion, immunity, focus, energy, and recovery are connected, and what Ayurveda can teach us about listening to those signals. We also talk about muscle, mitochondria, and creating health rituals that work with your body instead of against it.
Topics We Cover in This Episode:
What cellular resilience actually means
How to recognize the signals your body is sending
Why travel can stress multiple systems at once
The five pathways connected to resilience
How Ayurveda can inform personalized health choices
Why copying someone else's wellness routine may backfire
How muscle supports cellular health
A simple exercise for becoming more aware of your body
If you’ve been feeling tired, foggy, or like your body just isn’t responding the way it used to, tune in to this conversation with Dr. Monisha Bhanote.

| 130: Why Your Hormone Protocol Isn’t Working: Mold, Lyme & Chronic Inflammation | 27 sept. 2026 | 01:01:06 | |
What happens when you’re doing all the “right” things for your hormones and health, but you still don’t feel better? In this episode, I sit down with Dr. Neil Nathan to explore why underlying factors like mold, Lyme disease, inflammation, nervous system dysregulation, and detoxification challenges may complicate treatment.
We also talk about why some patients react unexpectedly to hormones or supplements, how Dr. Nathan approaches complex chronic illness, and why looking beyond the obvious diagnosis can sometimes change the conversation.
Topics We Cover in This Episode:
Why hormone protocols don’t always work as expected
How mold and Lyme may affect the immune system
Why “detox” isn’t a one-size-fits-all approach
What persistent or unusual symptoms may be telling you
How inflammation can present differently from person to person
What cytokine testing can reveal
Why environmental exposure matters
When it may be time to look deeper
If you’ve experienced persistent symptoms, unexpected reactions to treatment, or the frustration of doing everything “right” and still not feeling better, this is a conversation you’ll want to hear.

| 129: Why Is Testosterone Still Off-Limits for Women? | 20 sept. 2026 | 00:49:16 | |
Testosterone in women is finally getting more attention, but what does that actually mean for women and their care? In this episode, I sit down with Dr. Terri DeNeui to discuss the FDA’s focus on testosterone, why there still isn’t an FDA-approved testosterone treatment specifically for women, and the questions surrounding safety, dosing, and access.
We also talk about symptoms of testosterone deficiency, why “normal” lab ranges don’t always tell the full story, and the larger conversation around medical freedom and individualized care.
Topics We Cover in This Episode:
Why testosterone matters for women
What the FDA is currently considering
Why testosterone remains a controlled substance
When symptoms can appear
Why “normal” labs may not tell the whole story
How dosing and delivery methods matter
Where medical freedom fits into the conversation
Tune in to hear Dr. Terri DeNeui’s perspective on where women’s testosterone care stands today, what still needs to change, and what patients should be asking.

| 128: Viral Hormone Trends: What’s Actually Worth Your Time? | 13 sept. 2026 | 00:43:06 | |
Social media is full of hormone hacks promising better energy, easier weight loss, balanced hormones, and fewer symptoms. But how much of it is actually backed by science? In this episode, I’m breaking down seven viral trends, including cortisol cocktails, fiber maxing, pink salt, metabolism morning routines, and “hormone-balancing” foods, and separating reasonable health habits from overhyped claims.
I also talk about the way social media approaches PCOS and perimenopause, and why oversimplifying complex hormone conditions can do more harm than good. If you’ve ever wondered whether the latest hormone trend is worth trying, this conversation will help you look at these claims with a little more context and a lot less TikTok influence.
Topics We Cover in This Episode:
Why cortisol cocktails aren't what they claim to be
The problem with suddenly “fiber maxing”
What pink salt can actually do for you
Whether a metabolism morning routine works
The truth behind “hormone-balancing” foods
Why PCOS weight-loss advice needs more nuance
When a viral trend may be masking a bigger issue
Tune in, subscribe to Hormones, Metabolism and You, and share this episode with someone who could use a little more context the next time a hormone hack hits their feed.

We unpack why birth control is so often used to manage symptoms, what it may be masking, and how hormones like estrogen, progesterone, testosterone, insulin, and cortisol all fit into the bigger picture. We also talk about the different birth control options women may consider as they get older and why the right approach can change throughout different stages of life.
From painful and irregular periods to PCOS, weight struggles, low libido, anxiety, fatigue, and the transition into perimenopause, this mash-up brings together the questions women are asking and the answers they deserve to hear.
Topics We Cover in This Episode:
Why birth control may be masking more than you realize
The hormone connection behind PCOS and weight struggles
What your period could be trying to tell you
Why your 30s and 40s can bring unexpected changes

| 126: Health Care is So Broken - 3 Typical Patient Cases | 30 août 2026 | 00:48:42 | |
The healthcare system is not designed to keep you well, and I am done pretending that is okay. In this episode, I am breaking from the usual format to talk honestly about why I believe healthcare in the United States has become so reactive instead of preventative, and how that approach is leaving people sick, frustrated, and without answers.
I share my perspective as both a patient and a provider, including the ways insurance companies, healthcare systems, limited appointment times, and a diagnosis-by-diagnosis approach can keep providers from looking at the whole person. I also explain why symptoms and lab results cannot always be viewed in isolation, because our bodies do not operate in silos.
Then, I walk through three patient scenarios that I see repeatedly in my practice: a 19-year-old struggling with PCOS symptoms and insulin resistance, a woman in her 30s dealing with fatigue, weight gain, sleep issues and brain fog who was told her labs were normal, and a woman in her 40s experiencing perimenopausal symptoms who was told she was too young for hormones. These stories reveal just how much can be missed when we only look at whether a number falls inside a laboratory's "normal" range instead of asking whether it actually makes sense for the person sitting in front of us. Most importantly, I want you to leave this episode knowing that you are not crazy, your symptoms matter, and you can become your own best advocate when the system isn't listening.
Topics We Cover in This Episode:
Why "normal" lab results may not tell the whole story
The connection between symptoms that seem completely unrelated
What three very different patients had in common
Why the approach to preventative care needs to change
The piece of the puzzle that can be missed when diseases are treated separately
Why some women are told they are fine when they know something isn't right
What it really means to become your own best health advocate
Why waiting until something is completely broken may be costing you years

| 125: How to Dementia-Proof Your Brain: Dr. David Perlmutter on Hormones, Gut Health, and Inflammation | 23 août 2026 | 01:06:21 | |
What if protecting your brain starts decades before memory problems ever appear? In this conversation, I sit down with neurologist and bestselling author Dr. David Perlmutter to talk about what current research says about dementia prevention, why metabolic health may play a much larger role than many people realize, and why waiting until cognitive symptoms develop may be waiting too long. We get into the connection between blood sugar, insulin resistance, inflammation, gut health, sleep, exercise, and brain function, along with why so many of these risk factors begin showing up in our 30s and 40s rather than later in life.
We also spend time on topics that generate a lot of questions in my practice, including hormone therapy, women's brain health, alcohol, stimulants, and the current state of dementia treatments. Dr. Perlmutter shares his perspective on why prevention deserves more attention, how lifestyle interventions fit into the conversation alongside medical care, and what practical markers people can monitor to better understand their long-term brain health.
Topics We Cover in This Episode:
Why brain health may be shaped long before memory loss ever begins
How metabolic health and inflammation may influence cognitive function
The relationship between gut health, sleep, and long-term brain resilience
Why women face unique considerations when it comes to dementia risk
How Dr. Perlmutter views bioidentical hormone therapy in the context of brain health
What current evidence suggests about alcohol, stimulants, and cognitive wellness
Which everyday health markers may provide insight into future brain health
Why prevention may be more impactful than waiting for symptoms to appear
If you're interested in protecting your brain health for the long term and understanding how everyday lifestyle choices fit into that picture, I think you'll love this episode.

Heart disease is still the leading cause of death, yet many people are left wondering whether they're focusing on the right risk factors.
In this episode, I sit down with board-certified cardiac surgeon Dr. Philip Ovadia to discuss how his perspective shifted from treating advanced heart disease in the operating room to helping people prevent it in the first place.
We talk through common questions about cholesterol, LDL, statins, coronary calcium scores, and why he believes heart health requires looking beyond a single lab value to better understand metabolic health, inflammation, and insulin resistance.
Topics We Cover in This Episode:
What LDL cholesterol can and cannot tell you about heart disease risk
How insulin resistance and inflammation fit into the bigger picture
When a statin may make sense and when other factors deserve attention first
The current conversation around bioidentical hormones and cardiovascular health
Who should consider a coronary artery calcium scan and what the results can tell you
Why metabolic health may be one of the most important pieces of heart disease prevention
Practical questions to ask when evaluating your own cardiovascular risk

| 123: Hashimoto’s, Thyroid Labs, and Exercise: Are You Helping Your Body or Stressing It More? | 09 août 2026 | 01:05:22 | |
If you have Hashimoto’s and feel like you’re doing everything “right” but somehow feeling worse, this episode is for you. I’m answering your most frequently asked questions about Hashimoto’s, exercise, thyroid labs, thyroid antibodies, medication adjustments, estrogen, and why symptoms don’t always match what your lab work says. Throughout our conversation, I explain why looking at one lab value or one symptom in isolation often leads to frustration, and why understanding the bigger picture is essential when managing thyroid disease.
I also share ways to think about exercise, recovery, inflammation, medication tolerance, and the relationship between your thyroid, gut, hormones, metabolism, and stress response.
Topics We Cover in This Episode:
Why exercising harder isn't always the best strategy for Hashimoto's
How to recognize when your workouts are exceeding your body's recovery capacity
Why thyroid antibodies can fluctuate without necessarily signaling that your condition is worsening
How to interpret thyroid labs alongside symptoms instead of viewing them separately
The connection between stress, gut health, blood sugar, hormones, and thyroid function
Why some people feel overstimulated on thyroid medication even when their labs suggest they need more

What does it really mean when a man starts feeling constantly exhausted, loses motivation, struggles with brain fog, or notices changes in his physical and sexual health?
In this episode, I sit down with Dr. Stephen Sanders, founder of Whole Men's Health, to talk about why these symptoms deserve more attention and why men's health is about far more than testosterone alone. The symptoms many men experience long before they seek medical help
How stress, sleep, gut health, and hormones influence one another

| 120: From PCOS to PMOS; Why Hormones Are Not the Only Problem | 19 juil. 2026 | 00:33:00 | |
In this interview with Terri DeNeui, we explore why the conversation around PCOS is shifting toward a broader understanding of metabolic health with the emerging term PMOS.
Rather than viewing irregular cycles, infertility, and hormone imbalances as isolated ovarian problems, we discuss how insulin resistance, gut health, inflammation, stress, and lifestyle factors may be driving many of the symptoms women experience. I also share why this updated perspective could change the way clinicians evaluate and support patients across different stages of life—not just during their reproductive years.
We also spend time talking about practical treatment considerations, from comprehensive lab testing and lifestyle interventions to the appropriate role of GLP-1 medications.
Topics We Cover in This Episode:
Why some experts are moving from the term PCOS to PMOS—and what that shift represents
How insulin resistance and gut health may influence hormone balance
Why irregular menstrual cycles deserve a broader metabolic evaluation
The role inflammation and stress may play in hormone-related symptoms
Why body composition can be a more meaningful measure than weight alone
How to think about GLP-1 medications as one tool rather than a complete solution
Practical lifestyle habits that support long-term metabolic and hormonal health
Why treating symptoms without addressing underlying metabolic dysfunction may leave important questions unanswered
If you're interested in a more comprehensive way of thinking about hormones, metabolism, and women's health, I think you'll find this conversation both practical and thought-provoking.

| 119: Beyond Sick Care: Why Insurance Isn’t Coming To Save You | 12 juil. 2026 | 00:59:44 | |
For many people, the healthcare system becomes frustrating long before they receive a diagnosis. Symptoms are dismissed, lab work comes back "normal," and the next step is often another prescription rather than a conversation about why those symptoms developed in the first place.
In this episode, I'm joined by board-certified anesthesiologist and hormone optimization specialist Dr. Mike Fortunato to discuss his journey from conventional medicine into functional medicine, and why he believes many patients are looking for something today's healthcare model wasn't designed to provide.
Our conversation explores the realities of practicing medicine inside an insurance-driven system, the role hormones play in long-term health, and what Dr. Mike observed after years of caring for increasingly unhealthy surgical patients.
Topics We Cover in This Episode:
Why "normal" lab results don't always reflect optimal health
How Dr. Mike's own health challenges changed the way he practices medicine
What anesthesiology taught him about the declining health of today's patients
Why hormone optimization is about more than treating symptoms
How insurance influences the care many patients receive
The role lifestyle plays alongside hormone health
Why prevention often receives less attention than disease management
How patients can become stronger advocates for their own health
If you've been searching for a broader perspective on healthcare, hormones, and long-term wellness, I hope you'll join us for this conversation.

| 118: Stop Starving, Start Healing: The Real Root Cause of Weight Loss Resistance | 05 juil. 2026 | 00:54:36 | |
What if the reason you're struggling with weight, low energy, or hormone symptoms isn't just about eating less and exercising more?
In this episode, I sit down with functional medicine practitioner Dani Conway to talk about the bigger picture of metabolic health. We discuss why so many women are told their lab work is "normal" even when they still feel exhausted, inflamed, or stuck, and why looking beyond the scale often leads to better answers.
Dani shares how her own health journey shaped the way she now helps women address gut health, hormones, inflammation, and metabolism with a personalized, root-cause approach rather than a one-size-fits-all plan.
We explore the role digestion and detoxification play before focusing on hormones, why testing matters more than guessing, and how factors like stress, sleep, nervous system regulation, nutrition, exercise, and even peptide therapy fit into the conversation.
Topics We Cover in This Episode:
Why gut health and detoxification may need attention before addressing hormone balance
How to recognize signs that your body may not be processing nutrients or toxins efficiently
Why "normal" lab results don't always tell the whole story
What comprehensive testing can reveal beyond standard blood work
Why stress and nervous system regulation can influence weight loss, even with healthy habits
How to think about nutrition and exercise without relying on one-size-fits-all advice
If you're looking for a more personalized way to think about hormones, metabolism, and sustainable health, I think you'll find plenty to consider in this conversation.

We explore common symptoms like fatigue, brain fog, sleep disruption, anxiety, memory issues, and weight changes, along with some of the lesser-known experiences that often go unrecognized.

One of the most overlooked aspects of health may be influencing how you think, learn, focus, perform, and even respond to stress every single day. In this episode, I sit down with neuro-optometrist Dr. Bryce Appelbaum to explore the difference between eyesight and vision, and why seeing 20/20 doesn't necessarily mean your visual system is functioning optimally. We discuss how the eyes and brain work together, the role vision plays in learning, reading, athletic performance, spatial awareness, and everyday confidence, and why many functional vision problems often go unrecognized.
We also spend time talking about something that affects nearly all of us: screen time. Dr. Appelbaum explains what prolonged screen exposure is doing to our visual system, nervous system, sleep, stress response, and overall well-being.
Topics We Cover in This Episode:
How eyesight and vision are related, and why they're not the same thing
The subtle signs that may point to an underlying functional vision problem
Why screen fatigue may involve much more than tired eyes
What vision has to do with focus, learning, reading, and information processing
The connection between visual function, stress, and nervous system regulation
Why some people struggle with motion sickness, bridges, tunnels, or spatial awareness
How vision training differs from traditional eye care approaches
Simple daily habits that may help support long-term visual health
Whether you're a parent, athlete, professional, avid screen user, or simply interested in optimizing your health, this conversation offers a fascinating perspective on the connection between vision and whole-body wellness.

| 115: PCOS Has a New Name: Why It’s More Than an Ovarian Disorder | 14 juin 2026 | 00:42:55 | |
In this episode, I explain why PCOS is now better understood as Polyendocrine Metabolic Syndrome (PMOS).
While the traditional name focuses on the ovaries, the reality is that this condition often extends far beyond reproductive health. I discuss why many women with PCOS don't actually have ovarian cysts, how the condition can affect everything from insulin and metabolism to cortisol, inflammation, gut health, and hormone balance, and why this updated framework may lead to more accurate diagnoses and more personalized care.
I also walk through the different patterns I commonly see in clinical practice, including insulin-resistant, lean, inflammatory, adrenal, post-pill, postpartum, and mixed presentations. We explore why women are so often overlooked when they don't fit the typical PCOS profile, the lab markers that can provide a more complete picture of what's happening beneath the surface, and why understanding the root drivers of symptoms matters. Whether you're dealing with irregular cycles, fertility challenges, acne, weight changes, hair loss, fatigue, or simply feel like something isn't adding up, this episode offers a broader way to think about a condition that has long been oversimplified.
Topics We Cover in This Episode:
Why many experts believe the term PMOS better reflects the reality of this condition
The key reasons PCOS has historically been misunderstood and underdiagnosed
How insulin resistance can influence hormone symptoms long before blood sugar becomes abnormal
Why women who are thin can still experience significant metabolic dysfunction
The different patterns that can contribute to PMOS symptoms and why they matter
How stress, cortisol, inflammation, and gut health may influence hormone balance
Which lab markers can help uncover what is driving symptoms beneath the surface
Why individualized treatment strategies often produce better outcomes than a one-size-fits-all approach

You’ve been told everything is “normal”… but you know something isn’t right. In this episode, I sit down with Dr. Efrat LaMandre to unpack one of the most frustrating and isolating experiences in healthcare today: medical gaslighting. We dive into what it actually is, why it happens more often than we think, and the deeper systemic patterns inside medicine that lead providers to dismiss symptoms when labs don’t tell the full story. This is not about blaming doctors, it is about understanding the gap between patient experience and clinical training so we can start to bridge it.
Together, we explore the hidden dynamics at play in the exam room, from the limitations of evidence-based medicine to the influence of pharmaceutical-driven research and the rigid frameworks many providers are trained within. We also get into the powerful mindset shifts that can completely change how you navigate your own health journey, including how to advocate for yourself without burning bridges and why “normal” results don’t always mean optimal health. If you’ve ever felt dismissed, confused, or like you’re falling through the cracks of the system, this conversation will open your eyes and give you a new lens to move forward with clarity and confidence.
Topics We Cover in This Episode:
Why “normal labs” can still leave you feeling anything but normal
The surprising truth behind what medical gaslighting actually is
What’s really happening in a provider’s mind during your appointment
The hidden limitations of evidence-based medicine no one talks about
How patient expectations can unintentionally shape the outcome of a visit
The difference between ruling things out and truly getting answers
A powerful analogy that will completely reframe your healthcare experience
The subtle ways you can advocate for yourself more effectively
If you’re ready to start approaching your body and your care with a deeper level of understanding and empowerment, make sure you subscribe so you don’t miss what’s coming next. Your symptoms are real, your experience matters, and this is just the beginning of learning how to navigate it all differently.

| 102: Your Mouth May Be Driving Inflammation - The Hidden Dental Link to Hormones Metabolism and Brain Fog with Dr. Michelle Jorgensen | 15 mars 2026 | 00:58:35 | |
>>Join our FREE monthly Gut Health Webinar, so you can begin your Healing Journey. https://modernendocrinewellness.com/join-gut-health<<
What if one of the most overlooked drivers of inflammation, fatigue, brain fog, and hormone imbalance is something most people rarely think about… their mouth?
In this episode of Hormones, Metabolism, and You, Dr. Cassie Smith sits down with Dr. Michelle Jorgensen, DDS, a biologic dentist and founder of Living Well with Dr. Michelle, to explore the powerful connection between oral health and whole-body wellness.
After experiencing severe mercury toxicity from years of drilling out silver fillings as a traditional dentist, Dr. Jorgensen was forced to rethink everything she had been taught about dentistry and health. That journey led her to study functional medicine, nutrition, and biologic dentistry to understand how dental issues can influence the entire body.
Together, Dr. Cassie and Dr. Michelle unpack some of the biggest questions patients have about dental health, including mercury fillings, root canals, fluoride, bad breath, and the surprising ways dental infections may contribute to systemic disease.
If you have ever wondered whether hidden dental issues could be affecting your energy, hormones, or metabolism, this episode may help you connect some important dots.
Key Takeaways
• Your mouth is part of your overall health system, and dental issues can influence inflammation, metabolism, and immune function.
• Traditional silver fillings contain about 50% mercury, which can release small amounts of mercury vapor over time.
• If mercury fillings are removed, it is important that dentists follow safe removal protocols to avoid increased exposure during the process.
• Root canals remove the nerve and blood supply from a tooth, leaving dead tissue behind that may harbor bacteria and create ongoing immune stress.
• Some biologic dentists recommend removing infected root canal teeth and replacing them with ceramic implants rather than leaving the tooth in place.
• Ceramic (zirconia) dental implants are often preferred in biologic dentistry because they are inert and do not conduct electricity like titanium.
• Fluoride strengthens enamel but may interfere with thyroid function because it competes with iodine in the body.
• Many cavities are not caused by poor brushing but by mineral deficiencies and poor nutrient absorption, often linked to gut health.
• Low stomach acid or chronic use of acid-blocking medications may reduce mineral absorption and increase the risk of tooth decay.
• Hydroxyapatite toothpaste can help remineralize teeth by replacing lost minerals without fluoride.
• Gum disease and oral microbiome imbalances are common causes of bad breath.
• Simple practices like tongue scraping, oil pulling, and supporting mineral intake can improve oral and overall health.

What if the reason you still feel tired after optimizing hormones is not your hormones at all?
In this episode of Hormones, Metabolism, and You, Dr. Cassie Smith sits down with Dr. Terri DeNeui in Destin, Florida during an advanced hormone provider refresher to talk about the most common issue clinicians see after “optimizing hormones”: patients still feel tired, foggy, inflamed, or stuck. They break down why “normal” labs are not the same as optimal, why Free T3 matters, and how deficiencies in vitamin D, B12, and ferritin can block thyroid hormone function and keep symptoms alive. You will also hear how gut inflammation, H. pylori, and chronic acid blockers can sabotage hormone metabolism, why many “side effects” blamed on hormones are actually root-cause problems, and how to spot misinformation about estrogen and testosterone, especially for women.
About Dr. Terri DeNeui
Dr. Terri DeNeui, DNP, APRN, ACNP-BC, is a board-certified nurse practitioner, nationally recognized speaker, author, and the founder of EvexiPEL® and Evexia Medical. She holds advanced training in hormone replacement, preventive wellness, and functional medicine, and is known for her root-cause approach to optimizing hormones and whole-body health. Through her clinical work and provider education, Dr. DeNeui has helped transform the lives of thousands of patients and trained clinicians across the country. She is also the author of Hormone Havoc, a practical guide that clears up myths and misconceptions about hormones in women and men.
Key Topics Covered
Why patients can still feel “off” even when hormones look optimized
The thyroid lab most clinicians miss: Free T3
Why “normal” lab ranges often reflect a sick population, not optimal health
Vitamin D, B12, and ferritin: the hidden drivers behind fatigue, brain fog, and poor thyroid response
Ferritin and thyroid hormone: why low ferritin can block hormone function at the cellular level
Gut root causes behind low iron: heavy bleeding, hormone metabolism issues, H. pylori, and inflammation
Why chronic PPI use can worsen nutrient absorption and hormone conversion
Misconceptions about estrogen and cancer, and why metabolism and inflammation matter
Misconceptions about testosterone in women, plus why dosing and proper oversight change outcomes
Why hormone care works best when you treat the body as a system of systems

In this special 100th episode of Hormones, Metabolism, and You, Dr. Cassie Smith celebrates the milestone with a best-of compilation featuring some of the most downloaded and impactful conversations from the show. Each segment reflects a powerful truth listeners continue to come back to: when you feel off, there is usually a biological reason, and real answers start with understanding the root cause.
You will hear highlights from key conversations, including:
Tamar Samuels: Debunking common nutrition myths and social media trends, including restrictive “one-food” weight loss approaches and the confusion around anti-diet culture. This segment emphasizes personalized nutrition, metabolic health, and the importance of sustainable habits that support both body composition and long-term health.
Terri DeNeui: A powerful discussion on hormone therapy in your 30s, 40s, and 50s, including the truth about estrogen and cancer risk. This segment breaks down the long-term misunderstanding from the Women’s Health Initiative and explains why timing, personalization, and proper hormone support are critical for brain, heart, bone, and overall longevity.
Dr. Bruce Hoffman: A deep dive into chronic fatigue, mitochondrial dysfunction, and the concept of the cell danger response. You will learn why energy production is central to healing and how factors like toxic load, circadian rhythm, sunlight exposure, hydration, and lifestyle habits directly influence mitochondrial health and recovery.
Dr. Cassie Smith (Solo): A practical breakdown of thyroid optimization, including ideal functional ranges for TSH, Free T4, Free T3, Reverse T3, and thyroid antibodies. This segment explains why symptoms matter more than standard lab ranges and how inflammation, gut health, stress, and cellular function affect thyroid hormone activity.
Dr. Tyna Moore: An insightful conversation on GLP-1 microdosing and peptide therapy beyond weight loss. This segment explores how low-dose GLP-1s can support brain health, inflammation, metabolic function, and longevity, while also highlighting the importance of gut health, hormone balance, and foundational lifestyle habits before using advanced therapies.
Together, these listener-favorite moments reinforce the core message of the show: you are not broken, your symptoms are not random, and lasting change happens when you address the biology behind the symptoms.
Key Takeaways
This special 100th episode highlights a core theme across listener favorites: when symptoms don’t match normal labs, the issue is often deeper biology, not willpower. Sustainable health requires personalized nutrition, strong lifestyle foundations, and attention to real markers, not trends or extremes. Hormone therapy fears are often based on outdated information, thyroid care goes beyond TSH alone, and tools like GLP-1s work best as supportive therapies, not shortcuts. The consistent message throughout is safety-first, root-cause care with the right intervention at the right time.

What if peptides and GLP-1s are not the problem, but the internet hype is making it impossible to know what is actually safe, effective, and worth your money?
In this episode of Hormones, Metabolism, and You, Dr. Cassie Smith sits down with Greg Jones, NMD to cut through the confusion around peptides, GLP-1s, microdosing, stacking, and the next wave of weight loss medications. Dr. Jones explains why lifestyle is the foundation (there is no magic shot), outlines red flags for online prescribing and questionable sourcing, and compares newer multi-agonists like retatrutide with semaglutide and tirzepatide. You will also learn how to manage common issues like nausea, reflux, constipation, fatigue, and plateaus, and why safety-first, individualized care with proper monitoring matters most.
Key Takeaways
Healing and body composition start with the foundation: nutrition, hydration, sleep, and resistance training
Peptides fall into different categories: some affect feedback loops and require more monitoring, others are used more directly by the body
GLP-1 dosing should be individualized; aggressive titration increases side effects and long-term risks
Retatrutide is a triple agonist (GLP-1, GIP, glucagon) and may increase energy expenditure, but appetite and food noise can vary by person
Side effects often improve when diet, alcohol intake, and hydration are addressed
Weight loss should be gradual to help preserve lean mass and reduce skin laxity issues
Exit strategies should be personalized: taper dose, extend dosing intervals, and keep the habits in place
When the shots go away, the habits need to stay
Episode Highlights
Why the foundation matters: “trash in, trash out” and why sleep plus strength training drive long-term results
Red flag checklist for online prescribing: no labs, no check-ins, “not for human use” labeling, and three-month scripts with no monitoring
What to monitor with GLP-1s and related therapies, including why baseline labs matter
Retatrutide vs tirzepatide vs semaglutide: what is different, what is promising, and what is still evolving
Real-world GLP-1 troubleshooting: reflux, constipation, fatigue, nausea, and why microdosing can change tolerance
Why plateaus happen and how to adjust safely without overcorrecting
Who is at highest risk for muscle loss during fat loss and how to protect lean mass in midlife
Growth-hormone-related peptides explained: CJC-1295, ipamorelin, tesamorelin, and what realistic goals look like
Why “stacks” can be risky, when combinations may be acceptable, and why dosing flexibility matters
Lightning round: the biggest plateau driver, perimenopause myths, and the single best habit for preserving muscle

What if perimenopause is not your body breaking down, but your body revealing exactly what it needs to function better?
In this episode of Hormones, Metabolism, and You, Dr. Cassie Smith sits down with Bria Gadd, known as The Period Whisperer, to reframe perimenopause as a powerful turning point rather than a decline. They discuss why symptoms like fatigue, sleep disruption, anxiety, stubborn weight gain, cravings, and loss of motivation often reflect an energy deficit and accumulated “health debt,” not a lack of discipline. You will learn how years of stress, overtraining, underfueling, and pushing through exhaustion can create survival mode physiology, why traditional diet and fitness strategies often stop working in midlife, and how small shifts in recovery, nourishment, gut health, and thyroid support can help restore your baseline so your body can feel safe enough to regulate again.
Key Takeaways
Perimenopause symptoms often reflect accumulated stress and energy depletion rather than sudden hormonal failure
The “Perimenopause Gift” is your body signaling that current habits are no longer sustainable
Health debt builds over time when stress, under-recovery, and under-fueling exceed your body’s capacity
When the body is in survival mode, it may hold weight, increase cravings, and slow metabolism
More intensity in workouts or stricter dieting often increases stress hormones and worsens symptoms
Recovery, mobility, walking, and nervous system support often need to come before high-intensity training
Gut health plays an important role in hormone metabolism, nutrient absorption, and inflammation balance
Thyroid symptoms are often overlooked, and a full thyroid panel may be necessary when symptoms persist
Food noise and sugar cravings can signal energy deficiency, not lack of willpower
Sustainable progress comes from rebuilding energy and resilience rather than pushing harder
Episode Highlights
Why many women feel like their body “changed the rules overnight” during perimenopause
The energy supply vs. energy demand model and how it affects hormones, metabolism, and mood
How years of high stress, perfectionism, and overperformance contribute to midlife hormone struggles
Why strength training should be supported with adequate fuel, recovery, and nervous system balance
The connection between gut health and proper estrogen clearance
Early signs of thyroid dysfunction that are often dismissed as “normal aging”
Simple daily habits that help rebuild baseline energy, including hydration, walking after meals, and intentional rest
Why joy, presence, and reducing pressure can improve physiology, not just mindset
Bria’s practical framework for getting out of health debt and restoring long-term metabolic resilience

What if chronic illness protocols stall not because you are doing something wrong, but because your body does not feel safe enough to heal?
In this episode of Hormones, Metabolism, and You, Dr. Cassie Smith sits down with Dr. Jabin Moore to unpack why so many chronic illness protocols stall even when you are doing “everything right.” They explain how the brain prioritizes survival over repair when the body does not feel safe, and how stress, trauma, mold, Lyme, infections, and gut dysfunction can run the same threat pathways in the body. You will hear the unforgettable “$100 energy bill” metaphor, learn why detox can backfire when the nervous system is dysregulated, and get practical ways to create real safety in your environment and your biology so healing can finally stick.
Key Takeaways
Healing does not happen when your nervous system is stuck in fight or flight
Safety is biological, not just emotional, and it shapes detox, digestion, hormones, and immune function
The “$100 energy bill” explains why chronic illness creates daily energy debt and no healing margin
Toxins, infections, and trauma can activate overlapping inflammatory and survival pathways
You often need a two-pronged approach: calm the brain while clearing the body
More protocols are not always better; too much “good” can overload an already depleted system
Start with the basics: safe air, safe water, safe food, then regulate the nervous system
Simple nervous system resets can work, even five minutes of stillness and nature exposure
Protein matters because low protein can worsen stress physiology and recovery capacity
A sustainable plan protects your energy instead of burning it all on performance or perfection
Episode Highlights
Why the brain can block detox, immune regulation, digestion, and cellular repair when it senses threat
Mold, Lyme, PTSD, chronic stress, and gut issues: why they can “speak the same language” in the body
The detox myth: you cannot force clearing without first shifting out of threat mode
How perfectionism and people-pleasing can drain energy and slow recovery
The difference between living in a safe environment and actually feeling safe in it
What nervous system work can look like in real life, including limbic retraining tools
Dr. Moore’s practical framework: safe environment, safe feeling, then advanced protocols

What if brain fog, bloating, exhaustion, and hormonal chaos are not separate problems, but signals of one shared root issue? In this episode of Hormones, Metabolism, and You, Dr. Cassie Smith is joined by returning guest Dr. Shivani Gupta, Ayurvedic practitioner, turmeric researcher, and author of The Inflammation Code.
Together, they explore how chronic inflammation quietly disrupts gut health, hormone metabolism, sleep, mood, and energy, especially during perimenopause and menopause. Dr. Gupta explains how ancient Ayurvedic principles align with modern functional medicine, offering practical, non-diet strategies to support digestion, estrogen metabolism, circadian rhythms, and nervous system balance.
This conversation focuses on working with the body rather than against it, using daily rhythms, simple rituals, and gut-forward practices to support long-term resilience at every stage of life.

What if your hormone symptoms aren’t a hormone problem at all, but warning signs of deeper toxic, gut, and immune stress your body has been carrying for years?
In this powerful episode of Hormones Metabolism in You, Dr. Cassie Smith sits down with Dr. Kelly K. McCann, MD, to unpack why hormone symptoms like fatigue, weight gain, brain fog, mood changes, and stubborn metabolic dysfunction are often not hormone problems at all.
Instead, Dr. McCann explains how these symptoms are signals pointing to deeper root causes such as environmental toxin exposure, mold, gut dysfunction, immune dysregulation, and chronic inflammation. Together, they challenge the conventional approach of treating hormones in isolation and highlight why normal lab results do not always mean the body is functioning optimally.
This conversation explores how everyday exposures from water contamination and plastics to non-stick cookware, pesticides, and personal care products can silently disrupt thyroid function, metabolism, and hormone signaling. The episode also dives into the critical gut hormone immune connection, mold related illness, nutrient depletion, mitochondrial dysfunction, and the concept of the cell danger response.
