Optimizing Your Emotional and Mental Health During Menopause - #7610 Apr 202401:08:59

Two of the most common symptoms during the menopause journey are increased levels of anxiety and depression. The decline of women’s sex hormones creates a plethora of adverse effects, such as weight gain, insomnia or fragmented sleep, hair loss, loose skin, loss of lean muscle mass, brain fog, night sweats, loss of libido, vaginal dryness, and more. These devastating changes are driving women to feel frustrated, confused, and depressed about their current state of health, including how they look and feel.

Building your healthcare team during this time is essential for overall mind-body health. This should include a NAMS-certified OBGYN, an OBGYN who is well-versed in HRT, a General Physician, or a Functional Medicine MD, a health coach to help you navigate lifestyle behavior changes, a fitness expert to help you stay strong, and, for some women, a therapist or psychiatrist to support your mental and emotional well-being so you can continue to thrive in life.

Psychiatrists prescribe psychopharmacology to their patients and most leave it at that. What if there was a psychiatrist who also prescribed healthy lifestyle behavior modifications alongside medication? My next guest practices something called cosmetic psychiatry and it’s fascinating.

Dr. Andreea Seicean, MD, PhD, MPH, is the Clinical Director of Like MySelf Wellness. She is an ABPN Board-Certified Adolescent and Adult Psychiatrist, clinical investigator, scientist, and Professor. She completed her training at Case Western Reserve University, the Cleveland Clinic, and the University of Illinois at Chicago (UIC). Dr Seicean is an Adjunct Professor at the University of Illinois at Chicago where she continues to be active in research and teaches psychopharmacology (medication management) to psychiatry residents. She has extensive experience reviewing and managing medications and treating depression, anxiety, OCD, bipolar, PTSD/trauma, ADHD, and addiction. She also specializes in cosmetic psychiatry, focusing on improving performance, quality of life, and self-esteem. This can include stress eating and weight loss, school and work performance, insomnia, irritability, stress, and life transitions.

Heart Disease Risk and Midlife Women - #7527 Feb 202400:50:13

According to the CDC, over 60 million women (44%) in the United States are living with some form of heart disease. This is from the American Heart Assoc: more than one in three women is living with some form of cardiovascular disease. Heart disease and stroke cause 1 in 3 deaths among women each year – more than all cancers combined. According to the British Heart Association: after menopause your cholesterol levels can go up, increasing your risk of heart attack and stroke. Your body cannot control sugar levels (glucose) as well – this can increase your risk of diabetes, high blood pressure, and weight gain, which we know as metabolic syndrome.

Women have historically been studied less regarding heart disease risk, although that is changing with some very impressive women cardiologists. As women move through the menopause transition, it has been shown that as estrogen levels decline, a woman’s CVD risk may increase. Estrogen is heart protective and more is being done to educate and screen women to see if they are good candidates for MHT. But, still, so many women remain confused, and overwhelmed, by the barrage of information on the internet, the misinformation, or lack of information, provided by their doctors, and their current fear-based understanding of taking MHT and getting breast cancer. 

In this episode, we discuss standard lipid panels versus comprehensive lipid panels, including ApoB, Lp(a), the Cholesterol Balance Test, particle size and numbers, and what are optimal lab results. We also take a deep dive into my recent advanced lipid labs so you can see what it looks like and learn how to read the results. We talk about cholesterol, dietary and synthesized, as well as, saturated fats and their effects on lipids. Watch on my YouTube Channel for the most beneficial experience. Dr. Michael H. Davidson is board-certified in internal medicine, cardiology, and clinical lipidology. He’s a Professor of Medicine, Director of the Lipid Clinic at The University of Chicago Pritzker School of Medicine, and serves as the Chief Executive Officer of New Amsterdam Pharma. Dr. Davidson earned his medical degree from The Ohio State University College of Medicine and completed his residency and fellowship in cardiology at Rush University Medical Center in Chicago. An active researcher, Dr. Davidson’s clinical research background encompasses pharmaceutical and nutritional clinical trials. His extensive research on statins, novel lipid-lowering drugs, and nonpharmacologic risk factor reduction have established him as a world opinion leader.

 He's a prolific author and lecturer on lipid disorders, nutrition, and atherosclerosis, and has coordinated more than 1,000 clinical trials in areas of preventive cardiology, published more than 250 articles for leading medical journals, and has written 3 books on Lipidology. He has been named one of the The Best Doctors

in America for the past 15 years and was named Father of the Year by the American Diabetes Association in 2010.

Midlife Women and Autoimmune Diseases - #7108 Feb 202400:56:26

Women have a fourfold increase in risk of autoimmune disorders than men and 80% of the people who have autoimmune issues are women. Some autoimmune disorders are triggered during the perimenopausal through postmenopausal time frame (30-60 years of age). While more research is needed, it is proposed that sex hormones, the microbiome, environmental factors, and the X chromosome may be very much connected to autoimmune disorders in women.

It is broadly known and accepted by many functional medical doctors, but not so much in the conventional healthcare model, that consuming gluten causes a leaky gut that leads to gut dysbiosis that can then lead to chronic inflammation and autoimmune disorders.

Pilates For Your Menopause Journey - #7025 Jan 202400:00:53

Did you know that midlife women are at risk of losing 1% of their lean muscle mass per year starting at age 30? Did you know that midlife women are also at risk for declining bone health, leading to fractures, loss of strength and balance, and, eventually, loss of independence? Exercising during midlife is crucial to a woman's longevity plan. Fragility fractures are fractures resulting from a fall from standing height or less. These fractures, which most commonly occur at the hip, spine, or wrist, are an indication that the body's bones have been weakened by an underlying illness, mostly due to osteoporosis. Lean muscle mass keeps our bones strong and healthy.

Exercise is a midlife woman’s greatest weapon to use against adverse aging consequences. There are many different forms of exercise from dancing to swimming, hiking to cross-country skiing, weight lifting to yoga, pilates and so much more. Ideally, you should find joy in movement, but sadly, many women have yet to find what works for them during midlife. I’m a huge proponent of moving the dial with weight-bearing exercises during midlife. While some women don’t love going to the gym, yoga, and pilates are both excellent ways of using your body weight to build lean muscle mass.

My guest today is someone very special to me. Not only is she a longtime friend of mine, but she is an expert pilates instructor, who even invented a form of pilates for younger children.

Lily Horowitz is a leading certified Pilates instructor with over 30 years of experience in the fitness industry. She was first exposed to Pilates 25 years ago as a student of the method. After personally realizing the benefits of the work, and seeing changes in her own body and physical ability, she was then trained and certified.

Why Midlife Women Need a DXA Scan - #6918 Jan 202401:14:50

If you’re a midlife woman, then your bone health should be at the top of your list to check. As we age and transition from peri-menopause, to menopause, to post-menopause, our bone health is at risk, for some more than others.

Here’s a stat from the Bone Health & Osteoporosis Foundation:

Of the estimated 10 million Americans with osteoporosis, about eight million or 80% are women. Approximately one in two women over age 50 will break a bone because of osteoporosis. A woman's risk of breaking a hip is equal to her combined risk of breast, uterine, and ovarian cancer.

Our healthcare system isn’t about preventive medicine but waiting until the fire is burning to come in for the rescue. Perimenopause can start for women in their 30’s, the average age of menopause is 51. There’s a possible 15-20 years that your bone health could be declining, in conjunction with declining and fluctuating hormones, let alone years of a sub-optimal lifestyle and diet choices.

One of the tests I recommend for all my midlife women clients to take is a DEXA scan, which is the gold standard for assessing your bone mineral density, and risk of osteoporosis. Unfortunately, most health insurance companies will not pay for this test as a preventive measure until you are 65 years old, 70 for men unless you have prior risk factors. You will want to be working with a doctor, practitioner, or health coach who is trained to properly read the results and help you navigate any bone health issues that you may have with lifestyle/diet, supplements, and possibly medications. You do not need a doctor’s prescription to go ahead and get a DXA scan, but if you want to try and have your health insurance cover it, then you do.

Dr. Kristi DeSapri is back for a second time to go over what a DXA scan is, how it works, how to read the report, and then what the next steps are to take if needed. We will use my very recent DXA as an example. Dr. Kristi DeSapri is a board-certified internist specializing in midlife women’s health. After fellowship training at the Cleveland Clinic, she has worked in private practice and academic medicine for over 13 years, including as the director of the Northwestern Women’s Bone Health program at the Center for Sexual Medicine and Menopause at Northwestern Medicine in Chicago. She is currently opening Bone and Body Women’s Health, a concierge midlife women’s health practice in the Chicago area, focused on consultation and management of perimenopause, menopause, osteoporosis, and sexual health.

Up to 70% of post-menopausal women experience hair loss, hair thinning, widening of the part, or a receding hairline. Red light therapy, also known as low-level laser therapy (LLLT), is an effective treatment for female hair loss that has gained attention due to its non-invasive nature and promising results. Menopause is not just about gaining weight, having hot flashes, and not sleeping well. It’s also about the decline of our cellular health, which contains these essential organelles called mitochondria. I’ve talked about mitochondria in passing on numerous podcasts, and dedicated a full episode on Urolithin A, a gut metabolite that induces mitophagy, or the cleaning up of our mitochondria, so be sure to check that one out.

Mitochondrial Health, Urolithin A and Women's Health - #6430 Oct 202300:56:28

Inside most of our cells, as well as plants, fungi, and animals, are the mitochondria, or power plants of cells. These little organelles make chemical energy, called ATP, or Adenosine Triphosphate, which is our body’s energy currency. About hair growth, for example, the cells that make up our hair follicles need a lot of energy to grow hair. About our skin cells, if we don’t have healthy mitochondria making enough ATP, we won’t have healthy, youthful-looking skin. This is true for our kidneys, liver, heart, brain, and muscle cells. They all need optimal amounts of healthy mitochondria to produce optimal amounts of ATP. You can see how this topic is a priority when thinking about your longevity plan.

It’s important to know this information and understand how you can pivot your lifestyle in a direction that supports mitochondrial health, especially as you age. As women age, and go through menopause, their mitochondrial function is less optimal in both quantity and quality, and they become damaged, thus, they can’t take care of business.

Every day we hear more and more about the importance of gut health and how it relates to all chronic disease states, such as cancer, Alzheimer’s Disease, cardiovascular disease, and type 2 diabetes. Social determinants of health are nonmedical factors that influence health outcomes. Think of healthcare access and quality, economic stability, neighborhood, and built environments, such as parks, community fitness centers, and access to nutritious food. Many of us don’t have to think about these things because we live in well-established communities, have access to multiple grocery stores, and have opportunities to safely exercise at parks, hike on trails, and workout at community gyms. Social determinants of health affect not just our neighbors in the United States, but globally. Women's health has long been affected by social determinants of health. I’ve invited a very special guest to the podcast to help share his expertise and knowledge on prioritizing global, and national, efforts to make sure that all humankind has access to opportunities that lead to positive health outcomes.

Dr. Bernard Toney Jr. is a former White House Medical Officer, and global & public health professional, who has developed medical plans, infectious disease mitigation strategies, and public health preparedness at the highest level of government. During his service in the White House Medical Unit, he conducted 84 domestic and five overseas missions in support of the President and Vice President. He assessed hospital systems and witnessed the social determinants of health that lead to health disparities and inequalities in the United States and worldwide. He is passionate about clinical medicine, research, education, and working with internal and external stakeholders to advance initiatives to improve global health security. Since May 2022, Dr. Toney has been with the NIH full-time as an associate investigator.

There are many causes of hair loss and stress is a top contributor. Whether it’s caused by an acute stressor, such as chemotherapy or going through a divorce, or chronic stressors, such as dieting, not sleeping, or taking certain medications, stress is most likely involved in the hair loss equation.

Telogen effluvium is a type of hair loss that involves rapid shedding of hair over a short period, usually a few months after a stressful event. A new study found that hair loss affects over 52% of women over 50 years old, and 80% of women by the time they are 60 years old. During peri-menopause, menopause, and post-menopause, women's hormones are fluctuating, decline, and causing chronic hair shedding, thinning, and hair loss in many women. During women's midlife years, there are other hormonal changes going on causing weight gain, leading to yo-yo dieting, increased stress, disrupted sleep, and challenges with exercising. Menopause and Midlife Women's Heart Health - #5712 Aug 202301:07:56

Many midlife women that I work with, and are friends with, fear getting breast cancer more than heart disease. The focus on cardiovascular health is often not listed on my client’s health history form as an area of concern. Did you know that heart disease is the #1 cause of death for both women and men? Over 60M women in the U.S. live with some form of heart disease. While there are many unique risk factors for women developing heart disease, which we are going to cover in this episode, one of them is the menopausal transition. All women will experience menopause, and while some develop worse symptoms than others, the door to the 4 horsemen of chronic disease states opens up: Alzheimer's Disease, cardiovascular disease, cancer, and type 2 diabetes.

Navigating one’s risk of heart disease is almost impossible to do in the conventional medical healthcare model, which is set up to help you after you’ve had a cardiac event, not prevent it from happening. The amount of information on the topic, specifically as it relates to midlife women, is next to silent. Women are struggling to have conversations with their doctors about menopause, let alone heart health. In doing research to prepare for this episode, I was challenged to find articles and podcasts related to midlife women and CVD risk, treatments, and lifestyle interventions. I found plenty in the scientific data, but my clients, who are non-medical midlife women, don’t look there for the latest science. Most of my clients are also not having discussions about nutrition, sleep, stress management, and exercise with their doctors.

I am honored to have a very special guest today, who is going to help all of us midlife women understand what our CVD risks are, what we can do to prevent heart disease, what labs and tests to ask for from our doctors, and how menopause and menopause hormone therapy play a role in increasing or decreasing our risk.

Dr. Garima Sharma, MD received her Internal Medicine and Cardiology fellowship training at Temple University Hospital, Philadelphia. She was faculty at Johns Hopkins School of Medicine. and has served on the American College of Cardiology Fellow in Training Committee, Early Career Professional Committee, and the Women in Cardiology Leadership Council. She specializes in cardiovascular disease in Women and Preventive Cardiology and is the Director of Women’s CV Health and Cardio-Obstetrics, INOVA Health System. Lastly, she is the Governor of the Maryland Chapter of the American College of Cardiology.

Hair Loss and Micronutrient Deficiencies - #5626 Jul 202300:54:59

Micronutrients are the nutrients our body needs in lesser amounts to grow and develop. These include vitamins and minerals, and our body cannot manufacture these, so we must obtain them thru diet and supplementation. There are water-soluble and fat-soluble vitamins, macrominerals, and trace minerals. One of the best ways to look inside your cell to see what your nutrient status is is by taking a micronutrient test. This test tells you if your cells are stable, borderline deficient, or functionally deficient in a wide range of nutrients. You then want to correct these intracellular micronutrient deficiencies so that your cells are optimally working. If you go back to biology class, you remember that the mitochondria inside your cells are the powerhouses of energy produced for your body. Without functioning mitochondria, you don’t have optimal energy being produced. Without optimal energy, you won’t have healthy hair growth. After all, you don’t need hair to survive. The only way to correct these deficiencies is through diet and supplementation.

Metabolic Flexibility and Midlife Women's Health - #5528 Jun 202300:00:47

Metabolic flexibility is the body’s ability to utilize different sources of fuel for energy - either glucose and fat, stored body fat or fuel from food. Most people are fueled by glucose because of their poor diet, especially the Standard American Diet. As we age, metabolism also slows down. Our mitochondria decrease in both quality and quantity, affecting our ability to produce energy within the cell. Signs that one is metabolically inflexible are weight gain, cravings between meals, and low energy.

One of the top complaints I hear from my midlife women clients is that they are gaining weight around their midsection for no reason. Well, we know there’s a reason, it just hasn’t been found yet. Women are feeling confused and frustrated as to why they are gaining weight when all other variables haven’t changed. They are still eating the same diet, and attending their usual exercise classes, yet, the weight is coming on. With weight gain, comes a slew of other health issues, such as fatigue, disrupted sleep, increased anxiety/depression/stress, joint pain, brain fog, and more. Underneath the hood of weight gain and menopause, women open the door to chronic disease states, such as an increased risk of Alzheimer’s, Cardiovascular disease, type 2 diabetes, and cancer. So, knowing what metabolic flexibility is and how it pertains to your overall health is quite important to your longevity plan.

Optimizing Midlife Human Performance - #5331 May 202300:55:50

Midlife is defined as being between the ages of 35-60 yrs old. This is when we start to see and feel changes, and not in the direction we want. Brain fog, forgetting where we left our keys, more trips to get Botox, belly fat, dreaded hair thinning, chronic fatigue, horrible sleep, and, sadly, low to no libido – the list goes on and on. As we age, the doors to chronic disease open up: heart disease, Alzheimer’s, type 2 diabetes, and cancer all become real possibilities for most of us. I’m all in against aging poorly. I’m all in when it comes to optimizing my overall health and wellness, especially while in menopause. The thing about being all in all of the time is that it gets easier. Your new routine becomes your new normal, and it’s enjoyable, delicious, and reaps sexy rewards, like having a libido, rocking a bathing suit, healthy hair growth, and youthful skin, and you can go places and do lots of activities. What does your version of “all in” mean? What are the things you should be aware of, doing, taking, and planning for as we age?

Osteoporosis And The Midlife Woman - #5222 May 202300:57:08

Today I am focusing on osteoporosis and midlife women. It’s called the “silent disease” because you can’t feel your bones getting weaker. The National Osteoporosis Foundation offers these facts:

-1 in 2 women will break a bone due to osteoporosis after the age of 50.

-A woman’s risk of fracture is equal to her combined risk of breast, uterine, and ovarian cancer.

-24% of hip fracture patients age 50 and over die in the year following the fracture.

-A bone density test is the best way to diagnose osteoporosis and determine a treatment plan.

-They recommend you get this test at age 65- which is when most health insurance companies will pay for it.

A PubMed publication stated that women are most likely to be diagnosed with osteoporosis 10-15 years post-menopause, which would make them about 65. At that point, treatment will be a band-aid. It certainly won’t be preventive.

Some of my midlife clients have discovered they have osteopenia while working with me because I encouraged them to get checked. They’re in their 40’s, they’re peri-menopausal. Could you imagine if they waited another 20 years to get checked?

My guest today is a bone health and menopause expert who’d going to break it all down for us today. It’s definitely a topic not talked about enough until it has to be, which is often too late.

Dr. Kristi Tough DeSapri is a board-certified internist specializing in midlife women’s health. After fellowship training at the Cleveland Clinic, she has worked in private practice and academic medicine for over 13 years, including being the director of the Northwestern Women’s Bone Health program at the Center for Sexual Medicine and Menopause at Northwestern Medicine in Chicago. She is currently opening Bone and Body Women’s Health, a concierge midlife women’s health practice in the Chicago area, focused on consultation and management of perimenopause, menopause, osteoporosis, and sexual health.

Menopause and PCOS - #5030 Mar 202301:02:14

PCOS, or Polycystic Ovary Syndrome, is one of the most common causes of female infertility, affecting 6% to 12% (as many as 5 million) of US women of reproductive age. There’s no known cause of PCOS, but several mechanisms have been linked to this syndrome, such as hormonal imbalance, insulin resistance, and genetics.

Women with PCOS can develop serious health issues, especially if they are overweight. Here are some stats from the CDC:

-more than half of women with PCOS develop type 2 diabetes by age 40, just as a woman has most likely entered peri-menopause

-women with PCOS are at higher risk of CVD, and as a woman goes through menopause, their risk of CVD also increases

-women with PCOS generally have elevated LDL cholesterol and decreased HDL cholesterol—which also increases the risk of heart disease

-women with PCOS are more likely to develop plaque clogging their blood vessels which can lead to blood clots that in turn can cause a stroke

Most women that I work with who have PCOS are in their childbearing years. Today, I'm going to focus on PCOS and menopausal women – a topic not covered enough in my opinion. PCOS never goes away. Once properly diagnosed, it’s yours to keep your whole life. Menopause is challenging enough. Adding PCOS to the equation is even more challenging for women.

He's the first physician in the United States to be double board certified in Obstetrics and Gynecology by the American Board of Integrative Medicine. After a year of amazing hair growth, and very little hair shedding, I made one change that triggered a massive shedding, bringing me back to where I was over a year ago. With my oldest son's wedding in six months, i decided to try two newer modalities on the market and am sharing my experience with you.

When working with my clients on their hair growth journeys, as well as my own, I work with three buckets: lifestyle interventions, personalized supplement protocol, and a medical component with a well-versed doctor in hair loss. The average age of a woman in menopause is 51 but can range between 40-58 years old. We are going to spend a third of our lives in menopause so why not make it as vibrant as possible? There is no need to suffer during perimenopause, menopause or post-menopause when we have options to support us. Perimenopause and menopause may include a variety of symptoms such as hot flashes, brain fog, low libido, anxiety, hair loss, and weight gain. Menopause also opens the door to an increased risk of chronic diseases, such as cardiovascular disease, stroke, Alzheimer’s, osteoporosis, and metabolic syndrome.

I’ve said this before on previous podcasts, and I will say it again, at the end of the day you get to make the final decision on whether or not to take hormone replacement therapy, whether it’s HRT or BHRT. At the very least, educate yourself on the topic, the benefits, and the possible health consequences if you don’t do anything. Then, make your decision.

My guest today is Dr. Wen Shen, an associate professor of clinical gynecology and an associate professor of oncology at the oncology center in the Johns Hopkins Medicine Department of Gynecology and Obstetrics. She is also the clinical director of the Menopause Consultation service and Co-Director of the Women’s Wellness & Healthy Aging Program.

A board-certified gynecologist, Dr. Shen has concentrated on menopause medicine for more than 15 years. She specializes in treating peri-menopause and menopausal symptoms taking into consideration the woman’s overall health. She often works with her patients’ primary care provider to recommend non-hormonal therapies, alternative treatment options and menopause hormone therapy as appropriate for the individual patient.

In addition to clinical care, Dr. Shen focuses on educating her patients, other physicians and future physicians on women’s post reproductive health. She is engaged in clinical trials for new therapies for the treatment of menopause symptoms. She is actively developing curricula and educational aids for future physicians and health care providers on menopause medicine and the importance of healthy aging. 

Dr. Shen has published numerous articles and book chapters on menopause and perimenopause. She is a fellow of the American College of Obstetricians and Gynecologists and is on the Board of Trustees for the North American Menopause Society.

Dr. Shen earned her Master’s in Public Health and her Medical Degree from the Tulane University School of Medicine. She completed her residency in Gynecology and Obstetrics at The Johns Hopkins Hospital. During the 18 years Dr. Shen spent in general practice she became aware of the lack of physician education on post-reproductive health in women.

Since her 2005 return to Johns Hopkins, she has committed herself to raising awareness of menopausal and post-reproductive health in women to help them stay their healthiest beginning in peri-menopause and as they age.

Medical Weight loss with Semaglutide - #4505 Jan 202301:15:31

I work with a lot of men and women who want, and need, to lose weight. Some are struggling to lose 50+ pounds, while others can’t seem to shake the last 10 pounds off. As we age, our mitochondria diminish in quantity and quality, leading to a reduction in cellular function. Burning body fat requires optimal cellular function and energy. If you think back to biology class in high school, remember that the mitochondria reside inside our cells and produce ATP, our energy currency, that runs the mother ship- your body. Without optimal cellular functioning, our body struggles to perform. That’s the simple version. Other factors that influence our inability to efficiently burn body fat are poor diet, sedentary lifestyle, stress, poor sleep, toxins, and genetics. Some people are genetically wired to be fat, affecting appetite control, metabolism, food cravings, body-fat distribution, and more.

During menopause, many women start gaining weight. One year it may be just 5 lbs. But, because their hormone levels are dropping, women struggle to shed those pounds. The next year, it may be another 5-7 pounds, until each year it continues to accumulate. Twenty to forty pounds later, post-menopausal women are overweight, fatigued, stressed, not sleeping, and their lipid profiles and metabolic biomarkers change for the worse. They may become insulin resistant as a result. This chronic condition will lead to pre-diabetes, type 2 diabetes, cardiovascular disease, dementia, bone density loss, and more.

Then there’s the obesity epidemic. You can view the stats on the CDC.gov website, but here’s a snippet:

From 2017-2020 nearly 42% of the US population was obese. I’m sure it’s higher now, especially due to the pandemic.

Staggering numbers.

The first line of defense for weight loss is always lifestyle interventions and personalized supplementation. But, what if you are doing everything right and you still cannot shed those pounds? The next line of defense may be medical weight loss interventions. And that’s what we are here to learn about today. If you watch tv, you’ve seen commercials for Wegovy, Trulicity, or Ozempic. These are safe, effective, and proven medical weight loss interventions that are used for type 2 diabetics but are now being used more and more for weight loss. They are called GLP-1 receptor agonists and my guest today is going to put this all in layman's terms for you to understand what they are and how they work.

Dr. David Yablonsky has been practicing medicine for over 20 years and is a board-certified internist. Today my guest is Dr. Vered Stearns, Director, of Women’s Malignancies Disease Group, Medical Director, Under Armour Breast Health Innovation Center, Professor of Oncology, Breast Cancer Research Chair in Oncology, Assistant Director for Faculty Affairs, Sidney Kimmel Comprehensive Cancer Center, Johns Hopkins School of Medicine. Dr. Stearn shares her expertise on lifestyle interventions that reduce your risk of developing breast cancer.

Dr. Stearns’ clinical research has focused on improving current therapies by individualizing strategies for the treatment and prevention of breast cancer. She was among the first to evaluate the role of specific cancer-related genetic mutations in breast cancer survivors who had been treated with tamoxifen to prevent the reoccurrence of the disease. She also has led similar research on the association between genetic mutations and outcomes for women treated with aromatase inhibitors, a class of drugs used to treat breast cancer in post-menopausal women.

Among her many honors, Dr. Stearns was selected by Forbes in 2017 as one of the 27 top breast cancer oncologists in the country. In 2022, Dr. Stearns received the Kimmel Cancer Center Mentoring Award in recognition of her dedication to trainees and faculty members. Dr. Stearns joins Weill-Cornell Medicine as Director of Translational Breast Cancer in the Department of Medicine and Associate Director for Clinical Affairs at the Meyer Cancer Center.

In her new role, she looks forward to developing programs and mentoring team members across cancer types at sites in Manhattan, Brooklyn, and Queens.

Optimized Midlife Exercising - #4116 Nov 202201:06:05

How would you like to be ripped in your 50’s and 60’s? Why not? Who says it’s too late? I’m 54 years old, in menopause, and look and feel better now than I did when I was in college. How is that even possible? A lot of work, daily dedication to my vision, and figuring out my optimal lifestyle, which includes nutrition, supplements, bioidentical hormone therapy, restorative sleep, stress management, and, yes, lifting heavy weights multiple times per week. It’s a full-time job staying in shape, but the rewards are incredible. My clothes fit me better, I look better naked, I have a solid libido, and I feel great. But, it’s not just vanity I’m going after, it’s also about what’s going on under the hood. Lean muscle mass is our longevity organ and brings a lot to the table.

Lean muscle mass increases your BMR (Basal Metabolic rate), meaning that you burn more calories all day, it helps you burn stored fat every day, it reduces your risk for injury or falling, it improves your bone density (and during menopause that should be a huge concern for you ladies), it improves your blood glucose control and insulin sensitivity, it increases your mobility, your strength, and supports heart and brain health.

One of the biggest challenges my clients face is getting to a gym, or creating a home-gym, where they can lift heavy and build lean muscle mass. They make time to walk 20 minutes a day, they ride their bikes around the neighborhood, or they gravitate towards a treadmill or step machine. I’m all for moving, and I’m all for finding that form of exercise that you love, because then you’ll enjoy doing it. The science speaks and weight training, or resistance training, is the best form of exercise you can engage in for overall health, hands down.

I’m not a certified fitness trainer, but as a health coach, I work with my clients to figure out what is blocking them from participating in this form of exercise. Do they have an old injury that they need to tend to? Do they feel tired and unmotivated? Do they struggle with finding the time to carve out muscle-building workouts?

Whatever the reason is, there’s a way to get back into it without fear of injury, without fear of exhaustion, and without fear of messing up their daily schedule.

What's Fungi Got To Do With Gut Health? - #4002 Nov 202201:07:09

Health begins in the gut. If you’re a gardener, then you know that healthy soil is crucial to growing healthy plants. Think of your microbiome as the garden from which your overall health grows.


The microbiome is the community of microorganisms, including fungi (the mycobiome), bacteria and viruses, that exist in our digestive tract, skin, mouth, and vagina. These microorganisms change in response to a host of environmental factors, such as stress, exercise, diet, medication, toxins, and other exposures.

I would say that over 90% of the people that I work with experience some level of GI distress. Initially, they may come to me for weight loss, hair loss, low energy, or a diagnosis of an autoimmune disease, but, low and behold, it always circles back to their gut health. When our gut is inflamed, due to chronic stress, autoimmune diseases, poor diet, food sensitivities, and taking medications, especially antibiotics, we experience negative health consequences. Health consequences could look like skin disorders, such as acne or psoriasis, chronic fatigue, migraines, brain fog, weight gain, mood disorders, and acid reflux. Some negative health conditions not talked about as much when it comes to gut health are erectile dysfunction, hair loss, and asthma. Think about it, if our gut is inflamed, and we are eating an optimal diet full of bioavailable nutrients, our gut cannot properly digest, break down, and absorb the nutrients. If we don’t absorb and utilize nutrients, we can’t make ATP, our energy currency, inside our cells. How is our body going to perform all of the biochemical functions necessary to sustain optimal life if we don’t have enough energy? Our body’s job is to keep us in a state of homeostasis and when it’s not, certain decisions have to be made. We don’t need hair to survive, and growing hair requires nutrients and energy.

 A healthy gut is imperative for good health and most people have no idea how to make that happen for themselves. Our microbiome directs how we look, feel and think.

A Holistic Approach to Managing ADHD - #3914 Oct 202200:59:21

ADHD looks different in everyone, usually coexisting with other conditions, such as depression, anxiety, OCD, disruptive behavior disorders, learning disabilities, sleep disorders, and substance abuse. There’s a wide spectrum of symptoms.

Here are some current stats:

It’s estimated that from 2016-2019, kids ages 3-17, 6 million were diagnosed with ADHD.

50% of those also had a behavioral disorder

30% had anxiety

17% had depression

77% of these kids received some type of treatment

ADHD treatments range from pharmacologic interventions to more holistic approaches, and some combine the two.

Most people’s experiences, such as mine when my kids were young, were meeting with your pediatrician or psychiatrist and being sent home with a script for a stimulant, told when to have your child take it, and to check back in 30 days. If that med didn’t work, there were a host of others to try. If that approach didn’t work, then you were sent home with multiple scripts to work together. Meanwhile, your child was zombie-like, not eating or sleeping, not hanging with their friends anymore, BUT they also weren’t bouncing off the wall.

What else can we do for our ADHD kids who will become teens, young adults, and older adults someday? I’m not against medication, for some, it’s very helpful. But, we must ask how diet, nutrition, supplements, sleep hygiene, exercise, and stress management all affect these symptoms.

Breast Cancer, Hormone Therapy & Lifestyle - #3806 Oct 202201:04:05

The aftermath of the Women's Health Initiative (1990s), left millions of women confused and scared about their risk of developing breast cancer if they used hormone therapy. The science was later debunked due to the study's flaws, but the dark cloud of fear still reigns. Can we undo the damage?

October is Breast Cancer Awareness month and I am celebrating my mother, a breast cancer survivor, and all the women (and men) who’ve battled this terrible disease.

In 1991, the National Heart, Lung, and Blood Institute began the largest clinical trial in the US to better understand how these diseases (breast cancer, heart disease, stroke, blood clots), affected post-menopausal women. More than 160,000 women, ages 50-79 participated in this 15-year-long study. In 2002, results found that post-menopausal women taking estrogen and progestin HRT had an increased risk for all of the diseases I listed before. As a result of these findings, millions of women stopped taking hormones, many doctors stopped prescribing these hormones, and the study was halted prematurely. Years later, it was discovered that the study wasn’t all that kosher. There were flaws.

By the time the flaws were discovered, the damage had already been done. Women, who would have been protected against these diseases, were now suffering from menopausal symptoms, a staggering increase in heart disease (the #1 killer for women post-menopause), hip fractures (in one study I read, there were an estimated 43,000 hip fractures reported each year after the study stopped), and those women are now much too old to begin thinking about HRT, like my mother.

Millions of women do not take hormones for fear of developing breast cancer. Their fear stems from either being misinformed or misguided by their doctor or they haven’t had the opportunity to talk to their doctor about their options. Meanwhile, they experience night sweats, brain fog, loss of lean muscle mass, increased body fat, low to no libido, hair loss, and don’t sleep. At the end of the day, we all get to make our own choices, but let’s just make that choice based on education, knowledge, and the current medical science.

What is a Health Coach? - #3714 Sep 202201:18:00

Health coaching dates as far back as the 1960s. Today, there are over 128,000 health coaches in the US certified by hundreds of programs. We can be found in hospital settings, doctors' offices, corporations, health insurance companies, spas, local and state government agencies, and working as independent business owners, like myself. Health coaches write cookbooks, diet & lifestyle books, their personal journey books, and host podcasts, like me again.

Health coaches come in all different flavors, most of us specialize in a health & wellness topics, such as autoimmune conditions, menopause, weight loss, trauma, chronic pain, sleep, stress management, ADHD, parents of autistic kids, Lyme disease, hair loss, and just feeling your best during middle age. The list goes on and on.

Some health coaches, like myself, go on the become a National Board Certified Health & Wellness Coach, which means that we’ve been trained at the highest level possible, conducted 50 hours of coaching, and studied for and took the Board Exam developed under the wings of The National Board of Medical Examiners.

So, what is a health coach anyways?

In this 3-part series, I speak with three very different health coaches about what we do, how we’re trained, what skill sets we use, how we differ from nutritionists and dieticians, how we work with our clients, and everything in between. In part 1, I spoke with Michael Krug. Michael is a National Board Certified Health and Wellness Coach (NBC-HWC) and certified personal trainer through the National Academy of Sports Medicine (NASM-CPT). He has been practicing since 2018 and works with busy professional men to help them build strength and lose weight while increasing their energy and confidence. Though he currently embodies health, Michael has seen both ends of the health spectrum. In 2012 at the age of 20, Michael weighed 275 pounds, his blood pressure was 160/10 (with medication), he had dropped out of college, and was dealing with extreme depression. Red Light Therapy to Optimize Your Health - #3630 Aug 202201:11:42

Photobiomodulation, light therapy, low-level laser therapy, and red light therapy all mean the same thing and act on the mitochondria within all of our cells to create more energy. Healthy cells that produce more ATP, our energy currency, mean healthier everything. I bought my first red light therapy device for my hair, which helped me stop my decades-long battle with hair loss due to hormonal imbalances and hypothyroidism. Vitiligo is a condition in which the skin loses its pigment cells (called melanocytes). This results in discolored patches in different areas of the body, including the skin, hair, and mucous membranes.

Kris met with many doctors and the advice ranged from staying out of the sun to using an umbrella to shield himself from the sunlight. By 9 years old the disease had spread to all regions of his body. His mother discovered a doctor in Germany who specialized in light medicine and prescribed Narrowband Ultraviolet light therapy. By 14 years old pigment had returned to normal, except for his hands and feet. Kris’s condition is now stable. His life experiences are what led him to believe that all people deserve natural options that are effective and safe to improve their overall health and wellness.

Today, I am focusing on men’s health, specifically testosterone. About 40-45% of men over 45 years old have low testosterone, or low “T”. Younger men can also experience low T, even when in their 20’s. Low testosterone levels are correlated with poor nutrition, age, sedentary lifestyle, poor sleep, alcohol or drug use, prescription drug use, exposure to toxins, stress, and genetics.

There are serious health risks associated with low T, such as an increase in cardiovascular disease, osteopenia, or osteoarthritis, a decrease in lean body mass, our longevity organ, chronic brain fog, and of course, sexual dysfunction. Low T leads to depression, more stress and a feeling of hopelessness and low confidence.

One of the first questions I ask my male clients is, “How’s your libido?” because it tells me a lot of information about his health and lifestyle. It’s a fantastic marker to look at for overall health in men. The good news is that there are many options and lifestyle changes available to optimize testosterone levels.

Turning Hair Loss into Hair Growth - #3410 Jul 202201:15:31

If you’re experiencing hair loss, this episode is for you. I have suffered for over 20 years with on-again, off-again, female pattern hair loss, or Androgenic Alopecia. This type of hair loss in men and women is generally caused by hormonal changes, aging, and genetics. Hormonal changes in women could be triggered by using birth control, going through menopause, and taking Hormone Replacement Therapy (HRT), or Bio-Identical Hormone Replacement Therapy (BHRT). In fact, most women experience hair loss during their menopausal years. The main difference between men and women with pattern hair loss, is just that, the pattern in which hair is lost. For women, it results in a wider part down the middle, thinning around the crown and temples. For men, hair loss is more focused on a receding hair line that moves towards the back until bald. Either way, it’s devastating. While some men rock a bald head and it’s more socially acceptable, women want full, lush, thick, and shiny hair all over their heads.

There are other forms of hair loss, including Telogen Effluvium, when significantly more hairs move from the growing phase to the shedding phase and can occur after a traumatic or stressful experience, such as going through a divorce, death of a loved one, massive weight loss from dieting, surgery, and childbirth. When my hair loss started over 27 years ago, it was during the decade I birthed 5 of my kids.

If you’re living with an autoimmune condition, you may also experience hair loss. If you’re malnourished, undereating, or lacking to consume enough protein, you may also experience hair loss. As you can see, there are many biological pathways that can lead to hair loss.
