A Certified Occupational Therapy Assistant (COTA) plays a crucial role in the field of occupational therapy. Here are some of the key responsibilities and roles of a COTA: Implementing treatment plans: COTAs work closely with occupational therapists to carry out treatment plans designed for patients.

They assist in implementing therapeutic interventions and activities that aim to improve clients' functional abilities, enhance independence, and promote overall well-being. Providing client care: COTAs provide direct client care under the supervision of occupational therapists. They assist individuals with various tasks, such as therapeutic exercises, mobility training, and activities of daily living (ADL) like dressing, bathing, and eating.

They may also help clients with the use of assistive devices or adaptive equipment. Assessing client progress: COTAs observe and assess clients' responses to treatment interventions. They document and report the progress, changes, or challenges clients may face during therapy sessions. These observations are valuable in modifying and adapting treatment plans to better meet clients' needs. Collaborating with the occupational therapy team: COTAs work as part of a multidisciplinary team, collaborating with occupational therapists, physical therapists, speech-language pathologists, and other healthcare professionals.

They participate in team meetings, share insights, and contribute to the development and modification of treatment plans. Educating clients and families: COTAs provide education and guidance to clients and their families regarding therapeutic exercises, home modifications, and strategies to enhance functional abilities. They may teach clients techniques to manage their conditions, use adaptive equipment, and develop routines that support independence. Documentation and administrative tasks: COTAs maintain accurate and detailed documentation of client progress, treatment sessions, and any other pertinent information.

They may also assist with administrative tasks such as scheduling appointments, managing equipment inventory, and coordinating with insurance providers. Adhering to ethical and professional standards: COTAs follow ethical guidelines and professional standards in their practice. They prioritize client safety, privacy, and confidentiality. They engage in ongoing professional development to stay updated with the latest research, techniques, and advancements in occupational therapy. These roles and responsibilities may vary depending on the healthcare setting, client population, and the specific guidelines or regulations within a particular region

Treating Stiff Hands | How Less is More!

 I have received a common question this week regarding the treatment of stiff hands. Often, therapists become frustrated when they receive these types of questions because they feel like they are trying everything without any success. As a therapist myself, I have developed several treatment principles, and today I want to share one of them with you: "less is more."

When dealing with a stiff hand, it is important to understand that splints are not a cure-all solution, although they can be helpful in certain cases. Splints can be used in two ways: as a static progressive or dynamic tool to increase motion or as a means to block one joint in order to allow movement in another joint. However, splinting is just one aspect of a comprehensive treatment plan.

When evaluating and treating a stiff hand, it is crucial to identify the limitations and goals of the patient. Manual therapy skills are essential in treating a stiff hand effectively. Slowing down the movements during manual therapy and educating the patient about the nature of their joint can help manage expectations and alleviate frustration.

In terms of exercise selection, it is better to focus on a few key exercises that are performed correctly rather than overwhelming the patient with numerous exercises. Taking the time to explain the exercises and ensuring the patient understands how to perform them at home can lead to better results.

Finally, when providing a home exercise program, it is important to consider the patient's success and motivation. Instead of giving them a long list of exercises, focus on a few exercises that they can perform correctly and consistently.

In summary, when treating stiff hands, remember the principle of "less is more." Use splints as part of a comprehensive treatment plan, employ manual therapy skills effectively, select a few key exercises performed correctly, and prioritize the patient's success in their home exercise program.

How To Start Again After Failing The CHT Exam

Your desire to WANT to become a certified hand therapist is VITAL.

1. You have at least 3 years of experience as a licensed OT or PT (no assistants).

Answering a Question About a Deep Ulnar & Median Nerve Laceration Injury.

In this Podcast we talk about Nerves! I received a question about a 33 year old male with a deep laceration affecting his ulnar nerve, artery and median nerve as well.

Lacerations of the ulnar and median nerves can cause significant injury and functional impairment. The ulnar nerve and median nerve are two major nerves in the arm that provide sensation and motor control to the hand and fingers. When these nerves are lacerated, it can result in a variety of symptoms depending on the location and severity of the injury.

Symptoms of ulnar nerve laceration may include: Loss of sensation: Laceration of the ulnar nerve can lead to numbness or tingling in the little finger and half of the ring finger. Weakness or loss of muscle control: Damage to the ulnar nerve can cause weakness or paralysis of the muscles that control finger movements, resulting in difficulty with gripping objects or manipulating the hand. Claw hand deformity: Severe ulnar nerve injury can lead to a claw-like deformity, where the fingers are unable to fully straighten.

Evidence Based Hand and Upper Extremity Protocols (Second Edition) is a comprehensive manual with a total of 95 therapy treatment protocols, 23 of them new, for the most common surgical and nonsurgical diagnoses of the upper extremity. This manual is in a week-by-week format that is easy to use. Each protocol is based on the most current medical evidence and is uniformly outlined with indications, contraindications, recommended weekly therapy visits, and surgical outcome limitations.
