Guest Dr. Edwin Kim
Dr. Kim is a renowned pediatric allergist and immunologist who has extensive experience in treating children with food allergies. In this episode, he shares his research on sublingual immunotherapy, a method of allergy treatment that is gaining popularity among allergists and parents of children with food allergies.

SLIT
Sublingual immunotherapy, also known as "SLIT," is a form of food allergy treatment in which diluted allergen is placed under the tongue daily. The concentration is increased until the patient reaches a maintenance dose. The maintenance dose is continued for years. The goal of SLIT is to desensitize the patient's immune system to specific allergens, reducing the allergic response and decreasing the risk of a severe allergic reaction when he allergen is accidentally ingested.

If you have heard of OIT or SLIT (oral immunotherapy and sublingual immunotherapy, respectively), then you've probably wondered how these therapies treat food allergies and, if they are so great, why isn't everyone doing them? Well, in part, it's due to potential risks, but there are also some other factors to consider.

Join Dr. Hoyt as she discusses all this with her colleague Dr. Sakina Bajowala. Dr. Bajowala is a board-certified allergist with specific expertise in the treatment of food allergies. She practices outside of Chicago and helps families navigate the world of food allergy and, in many cases, helps make her patients significantly less allergic to foods. Dr. Bajowala is a Fellow of the American Academy of Allergy, Asthma and Immunology and is incredibly well respected by her colleagues in the OIT space.

An Easy Approach to Early Food Allergen Introduction with Dr. Katie Marks Cogan08 Aug 202300:36:02

Did you know that many food allergies can be prevented? Early introduction of potentially allergenic foods, such as peanuts, can actually prevent the development of food allergies. Awesome, right?

But how exactly are you supposed to feed a baby peanuts? You aren't - peanuts themselves absolutely are a choking hazard, so you have to use a developmentally appropriate form of a peanut-containing food. 

That's where it gets tricky.

Dr. Katie Marks-Cogan, board-certified pediatric and adult allergist, had the same concerns, so she and her team invented Ready, Set, Food! It's a series of food powders that allows food proteins to be introduced to babies right through their bottles.
On this episode, Dr. Hoyt interviews Dr. Marks-Cogan, and these two allergists/moms discuss early allergen introduction and the Shark Tank-awarded and allergist-approved food product that is helping moms and dads get these foods into babies' diets early and often.

The world of food allergy treatment has been buzzing with headlines claiming that Xolair (omalizumab) is "superior" to oral immunotherapy for treating multiple food allergies. As a board-certified allergist who both prescribes Xolair and performs multi-food OIT, I'm here to cut through the hype and provide clarity on what these treatments actually offer.

Let me take you behind those headlines to examine the OUtMATCH study that sparked these claims and we will decide whether Xolair truly is superior to OIT.

What is OIT?

"OIT" stands for Oral ImmunoTherapy. It is a months to years long process in which the immune system is taught to tolerate an allergen or allergens.

Dr. Hugh Windom was an early-adopter of OIT and has desensitized MANY patients using safe, effective protocols. Dr. Windom is very well recognized and respected in the OIT allergist community (and he's super nice and easy to talk to!).

Accidental ingestions of food allergens happen all too often, so kids with food allergies need access to epinephrine. While them having their own epi is ideal, that just isn't always the reality.

Stock epinephrine is epinephrine prescribed to a school or other entity to be used to treat a child or adult having an anaphylactic emergency. What is "stock epi" ? It's epinephrine prescribed to an individual or, more commonly, to an entity to be used in case of an allergy emergency, such as when someone is having anaphylaxis but does not have their own epinephrine auto-injector.

In part two of this two-part series, Pam and I talk with the amazing Kristi Grim of FARE (Food Allergy Research and Education). Kristi has been with FARE for years and was the original lead and continued director of FARE's college programming as well as lead of youth and of professional education.

Kristi and her team at FARE have done tremendous work in helping families safely send their kiddos to college. Programs for families include:

Colleges are taking notice of the increase in food allergies and want more information. To meet this need, FARE has great resources for colleges!

FARE even gives awards to colleges to honor excellence in food allergy accommodations!

Schools develop food allergy-related policies to try to keep kids safe, but how many of these policies are evidence-based?

And how do policies impact the emotional well-being of kiddos with food allergy?

Join me as I talk with my colleague Ashley Lahoud of Kent State University's Pediatric Anxiety and Allergy Research Clinic. There, Ashley and her fellow researchers are studying just how school policies are affecting the mental health of children who have food allergy. By better understanding this issue, we will have more information by which to provide beneficial guidance to schools on developing policies regarding food allergy.

Oral immunotherapy (OIT) has revolutionized the way we manage food allergies in children. Join Dr. Alice Hoyt as she interviews Dr. Richard Wasserman, a true pioneer in this groundbreaking treatment.

Food allergy management certainly can be challenging, but we continue to have more and more evidence to guide us in making best possible decisions, especially regarding management in schools. I'm delighted to have Dr. Matthew Greenhawt on this episode. He's the Director of the Food Challenge and Research Unit at Children's Hospital Colorado. Dr. Greenhawt has authored over 200 journal articles, most of which pertain to food allergy. 

I've previously discussed Dr. Greenhawt's article "Managing Food Allergy in Schools During the COVID-19 Pandemic" on episode #12 when I talked with school nurses about food allergy management during COVID-19. You can find a link to the journal article here
