Why GLP-1 Medications Cause Extreme Fatigue (And How to Fix Low Energy)
Are you exhausted on Ozempic, Wegovy, Mounjaro, or Zepbound? You’re not imagining it. Fatigue is listed in the prescribing information, and it’s one of the most common complaints I see in the comments.
In this video, I’m Dr. Mohammed Paika, an internal medicine and obesity medicine physician. I break down the 4 biological reasons GLP-1 medications can leave you drained: unintentional undereating, quiet dehydration, the drug’s effects on the brain, and nutrient deficiencies like low iron and B12. Then I walk you through 4 practical fixes: a calorie and protein floor, a hydration plan that goes beyond plain water, the blood work to ask your doctor for, and how to talk to your prescriber about dose and titration. I finish with the warning signs that mean you need to be seen right away.

Something Strange Is Happening to People on Ozempic (Doctor Explains)
People on Ozempic and Wegovy are reporting a side effect that isn't on the label: they're losing interest in alcohol. Is it real, or just nausea? As an internal medicine and obesity medicine physician, I work this up like an unexplained finding in a hospital patient. We'll look at the patient reports and real-world data, consider every possible explanation, read the newest trials (including the 2026 Lancet trial), and finish with a practical plan.
In this video you'll learn: Why GLP-1 drugs may turn down cravings, and what "food noise" really is What the Lancet, JAMA Psychiatry and earlier trials actually found Why the first human trial failed, and how it shaped the one that worked What we still don't know before this becomes a standard treatment 5 safety rules if you drink while on a GLP-1

How Long Retatrutide Stays in Your System (Half-Life Explained)
How long does retatrutide actually stay in your system? Retatrutide, the triple-hormone drug people call "GLP-3" or "reta," has a half-life of about 6 days. That one number explains when side effects hit and why the dose has to climb slowly. It also explains why a short pause before surgery may not be enough, and why the drug lingers for about a month after your last shot.
I'm Dr. Mohammed Paika, board-certified in internal medicine and obesity medicine. In this video, I walk you through the 4 clocks retatrutide sets in your body and what the phase 3 trials really show, including up to 30.3% weight loss. You'll learn about the side effects people aren't talking about, like changes in skin sensation and heart rate. I also explain why the "research use only" vials sold online are a serious risk. Retatrutide is not FDA-approved. The regulated routes are a clinical trial, Lilly's limited early access program (through your doctor) or waiting for approval.

Not Losing Weight on GLP 1 (A Doctor Explains Why)
Not losing weight on a GLP-1? The problem usually isn't you, and it usually isn't the medication either. It's the variables nobody checked.
I'm Dr. Mohammed Paika, a physician board-certified in Internal Medicine and Obesity Medicine. I've spent about 15 years as a hospitalist, and I'm also a certified personal trainer and nutrition coach. In this video, I walk you through 8 evidence-based reasons the scale can stall on semaglutide (Ozempic, Wegovy) or tirzepatide (Mounjaro, Zepbound), plus a bonus. They range from diabetes and insulin resistance to medications that quietly cause weight gain, eating too little, muscle loss, sleep apnea and dose problems. At the end, I share my 4-question test to tell whether your GLP-1 is working, stalling or needs to be changed.

How Long Tirzepatide Stays in Your Body (Half-Life Explained) | Mounjaro/Zepbound Guide
How long does tirzepatide (Mounjaro, Zepbound) actually stay in your system? It's not 5 days. In this video, I explain tirzepatide's half-life using 4 clocks it sets in your body. Those clocks explain when side effects hit and what to do if you miss a dose. They also explain why a 1-week pause before surgery may not be enough, why the birth control rule is 4 weeks, and how long the drug takes to clear after your last shot.
Then we get to the clock that matters most: what happens after you stop. You'll see what the SURMOUNT-4 trial found about weight regain and why muscle loss is the real risk. I'll also share how I regained 45 pounds when my own structure disappeared, and the 2 paths that protect your results.

Most people think GLP-1 medications like Ozempic, Wegovy, Mounjaro and Zepbound are just for weight loss. The research tells a much bigger story. I'm Dr. Mohammed Paika, board-certified in internal medicine and obesity medicine, with more than 15 years as a hospitalist. In this video I count down 12 evidence-graded health benefits of GLP-1 medications beyond the scale, plus a bonus from the newest cancer research.
You'll learn what the major trials show about your heart, liver, kidneys, blood pressure, sleep apnea, inflammation, alcohol cravings, joint pain and blood sugar. I also cover what the latest Alzheimer's trials actually found. Stay for number 1: a major analysis of the SELECT trial found that about two-thirds of the heart protection wasn't explained by weight loss.

Why do two people on the same Mounjaro dose get completely different results? In this video, I break down a study of more than 3,000 people with type 2 diabetes from the SURPASS 1–4 trials. It looked at who is most likely to lose 15% or more of their body weight on tirzepatide (Mounjaro / Zepbound), and why.
You'll learn the 7 factors that predicted the biggest weight loss. Several of them are things you can change, sometimes before your first injection. I also cover what newer research says about Ozempic and Wegovy, why real-world results are lower than trial results, and the 6-month check-in that tells you whether you're on track.

#GLP1 #Ozempic #Zepbound #Wegovy #weightloss
Losing weight on Wegovy, Ozempic, Zepbound, or Mounjaro? What you eat while you're on it matters more than most people realize. In this video, board-certified internal medicine and obesity medicine physician Dr. Mohammed Paika breaks down the 3 nutrition gaps that show up again and again in GLP-1 patients — protein, fiber, and fluids — and the 10 foods that address them, backed by real research on muscle preservation, gut health, and the GLP-1 pathway itself.
You'll learn why eating less doesn't automatically mean eating well, what the research says about muscle loss on semaglutide and tirzepatide, and exactly which foods to prioritize so you protect your muscle, support your gut, and get more out of your medication — not just a smaller number on the scale.

When Should You Increase or Decrease Your GLP-1 Dose? (Doctor Explains)
Should you increase your GLP-1 dose — or actually decrease it? Before you touch that pen, run through the 7 checks I use with my own patients (and on myself) to decide whether it's time to go up, hold steady, or come down. This isn't about chasing the highest dose on the label — it's about finding the dose you can tolerate and sustain for the long run.
I'm Dr. Mohammed Paika — internal medicine physician, board-certified in obesity medicine, and a GLP-1 user myself. In this video I walk through dose duration, your real weight trend, hunger and food noise, tolerability, plateaus vs. real stalls, the rest of your system (protein, sleep, training), and your actual goals — with real hospitalist cases on what happens when dosing goes wrong.

GLP-1 Medications May Be Changing Your Inflammation—Here’s What We Know
GLP-1 drugs like semaglutide and tirzepatide don't just lower weight and blood sugar — the research increasingly shows they change inflammation too. In this video, Dr. Mohammed Paika (board-certified internal medicine + obesity medicine, hospitalist) breaks down what CRP actually measures, why chronic inflammation happens, how much of the anti-inflammatory effect is really just weight loss versus a separate biological pathway, and why tirzepatide's second receptor (GIP) may make it different from semaglutide.

How To Lose Weight On Tirzepatide (Doctor Who Prescribes It)
If you're on tirzepatide or semaglutide and the scale isn't moving the way you expected, this video is for you. I'm Dr. Mohammed Paika, board-certified in internal medicine and obesity medicine — I've been dealing with these medications in the hospital since they first came out, and I've managed my own weight through diet and training alone for over a decade. In this video I break down what tirzepatide and semaglutide actually are, why they were invented, how they work in your body, the exact dosing timeline, why the shot is only once a week, what you have to do around the medication for it to actually work, and what happens once you hit your goal weight. Most important is what to do besides taking the medication which is what my GLP-1 Masterclass is designed to teach you.

Peptides didn't start with a wellness influencer — they started with a dying 14-year-old boy in 1922 and a shot of insulin. Today that same category includes everything from FDA-approved medications to unregulated research chemicals sold online with zero oversight.
In this video I break down over 20 peptides — from BPC-157 and TB-500 to semaglutide, tirzepatide, retatrutide, and the newest FDA regulatory update on compounding access — organized into the three levels of biology they actually work on: cellular energy, tissue repair, and systemic control (your hormones, metabolism, and nervous system).

The Most Common Mistake Patients Make on Phentermine
The #1 mistake I see patients make on phentermine isn't a dosing error — it's that they stop eating. In this video, Dr. Mohammed Paika (board-certified in Internal Medicine and Obesity Medicine, 20 years as a hospitalist) breaks down exactly why appetite suppression gets confused with starvation, the real physiology of what happens to your metabolism and muscle when you under-eat on phentermine, a patient case from the hospital floor you won't forget, and the protein-first framework to actually keep the weight off long-term.
You'll learn: – What phentermine actually does in your body (and why it's still the most-prescribed weight-loss medication in the U.S.) – The 3-stage physiology of "starvation mode" — and the real data behind it – A protein-first meal-timing framework you can start today – Why resistance training is non-negotiable for protecting your metabolism – Why phentermine isn't your only option — and how to know if it's the right one for you

Zepbound and Wegovy are the two most talked-about weight loss medications right now — but the headlines comparing them skip the context that actually matters. In this video, I break down how tirzepatide (Zepbound) and semaglutide (Wegovy) actually work in your body, compare the real clinical trial data — including the new SURMOUNT-5 head-to-head trial — and walk through the side effects, cost, and questions you need to bring to your physician before choosing between them.

If I Started Retatrutide Today, Here’s EXACTLY What I’d Do!
Retatrutide is producing weight loss results we used to only see after bariatric surgery — but the medication is only half the story. In this video, Dr. Mohammed Paika (board-certified in internal medicine and obesity medicine) breaks down the five decisions he'd make before ever taking a first dose: how to find the right dose instead of chasing the highest one, why your heart rate and mood deserve the same attention as the scale, how to protect your muscle while the weight comes off, how to build a supplement plan instead of guessing, and why the medication should never be allowed to do all the work on its own.
Important: as of this video, retatrutide is still investigational and not yet FDA approved. Eli Lilly has completed three positive Phase 3 trials and plans to file for approval in Q1 2027. Right now it's accessible only through clinical trials or a narrow compassionate-use program — this video is the plan to have ready before that changes.

Tips That Can Help Manage Your Ozempic Side Effects
Nausea, constipation, bloating, gas — if you're on semaglutide (Ozempic or Wegovy), gastrointestinal side effects are the most common thing you'll run into. In this video, I break down exactly why they happen and the specific, evidence-based strategies that actually reduce them: portion sizes, meal timing, which foods make it worse, hydration, injection timing, dose titration, and the red flags that mean it's time to call your doctor — not internet folklore, actual clinical data and two decades of hospital medicine.

Best Foods to Eat on GLP-1: Ozempic, Mounjaro, Zepbound & Wegovy Diet
Eating less on Ozempic, Wegovy, Mounjaro, or Zepbound doesn't automatically mean your diet is helping you lose fat. In this video, I break down the best foods to eat on a GLP-1 — protein, carbs, healthy fats, and fiber — so you preserve muscle, manage side effects, and get better results from your medication.
I'm Dr. Mohammed Paika, a board-certified internal medicine and obesity medicine physician. Everything in this video comes from the same nutrition science I use with real patients, including two cases straight from my hospital work.

Ozempic and Mounjaro Warning! What They’re Not Telling You!
Ozempic and Mounjaro can be powerful weight-loss tools — but they can also affect your body in ways many people aren't told before starting. In this video, I break down the real risks, the paradox in the trial data, and what to actually watch for.
In this video I walk through why people with type 2 diabetes tend to lose less weight on these medications than people without it, why a meaningful share of the weight lost on GLP-1s can be muscle rather than fat, what the data actually shows about gastroparesis and other GI complications, what happens metabolically when you stop the medication, how bariatric surgery compares, and exactly what to do to protect your muscle and your health if you're on one of these drugs.
Timestamps: 00:00 The risks nobody tells you about 01:12 Who I am, and why this topic keeps me up at night 03:04 The paradox no one is talking about 04:56 Why your brain needs data from your gut 07:01 The detail your prescriber probably never mentioned 09:26 The lesson every doctor should remember 11:21 A quick word before we go further 12:14 The muscle you didn't know you were losing 17:03 When the stomach stops working 20:16 What happens when you stop 21:03 Why bariatric surgery tells a different story 24:11 What to actually do about it 26:30 The bottom line 27:36 Where to go from here

Stop Eating These Foods on Ozempic & Mounjaro Before It’s Too Late
Some foods can make Ozempic or Mounjaro side effects much worse — even if you're taking the medication exactly as prescribed. In this video, Dr. Mohammed Paika (board-certified in Internal Medicine and Obesity Medicine) breaks down the 5 food categories most likely to cause nausea, reflux, and bloating on a GLP-1 medication — and exactly what to eat instead to support fat loss without fighting your own treatment.

This Training Method Is Getting Men Over 50 Jacked
Building muscle after 50 doesn't work the way it used to — and "just lift heavier" is exactly why most men stay stuck, sore, and stalled. In this video, I break down an evidence-based training framework grounded in NASM's OPT model and backed by ACSM/NSCA resistance-training guidelines for older adults — built to build real muscle while protecting your joints, tendons, and long-term mobility. I also cover why muscle matters even more if you're on a GLP-1 medication, and what the research actually shows about lean mass loss during weight loss.

10 Things You Should NEVER Mix with Ozempic or Mounjaro
Ozempic and Mounjaro are two of the most powerful tools we've ever had for weight loss and metabolic health — but what you combine them with matters. In this video, Dr. Mohammed Paika, MD (board-certified in Internal Medicine and Obesity Medicine, hospitalist for nearly 20 years) breaks down 10 medications, substances, and habits that can quietly work against your GLP-1 treatment: SGLT2 inhibitors, metformin, cannabis/THC, artificial sweeteners and sugar alcohols, compounded/gray-market GLP-1 products, skipping resistance training, SSRIs, caffeine, diuretics, and stacking multiple GLP-1 medications. You'll learn the actual physiology behind each risk, real warning signs to watch for, and what to do instead — so your treatment stays safer, easier to stick with, and more effective.

Peptides are everywhere online right now, but most videos explain one compound in isolation and skip the actual evidence. In this video, I break down every major peptide people are talking about — repair peptides, growth hormone secretagogues, longevity and immune peptides, hormonal peptides, and the GLP-1 family — with the real human trial data, FDA regulatory status, and honest caveats behind each one. No hype, no fear-mongering — just what the research actually supports.
I'm Dr. Mohammed Paika, a hospitalist physician. I see the downstream effects of unmanaged metabolic and hormonal health every day at the bedside, and this video is the same evidence-based breakdown I'd give a patient.

Where to Inject for MAXIMUM GLP-1 Weight Loss | What the Data Shows
Injecting your GLP-1 in the same spot every week and wondering if another site would work better? I break down the actual Eli Lilly pharmacokinetic study comparing abdomen, thigh, and upper arm injections for tirzepatide (Mounjaro/Zepbound) — what the data really shows about absorption, side effects, and appetite suppression, and where the study's small sample size means the "abdomen wins" headline deserves a second look. By the end, you'll know exactly how much injection site affects your results — and where to actually focus if you want long-term success on GLP-1 therapy. Is creatine actually worth it after 50 — or is it just hype? In this video, Dr. Mohammed Paika (board-certified in Internal Medicine and Obesity Medicine) breaks down what 680+ clinical trials say about creatine for muscle, brain, and heart health after 50 — plus the one thing almost nobody tells you if you're losing weight quickly, especially on a GLP-1 medication like semaglutide or tirzepatide.
You'll learn: the real history of creatine, how it works in your cells, why muscle loss (sarcopenia) accelerates after 40, the safety facts worth knowing (including the kidney-lab myth), why losing weight fast can mean losing muscle, and simple, no-nonsense dosing.

Retatrutide: The Biological Reset for Rebound Fat Loss
Is retatrutide the real deal, or just another overhyped GLP-1 headline? In this video, Dr. Mohammed Paika — board-certified in Internal Medicine and Obesity Medicine — breaks down exactly how retatrutide's triple-hormone mechanism (GLP-1 + GIP + glucagon) works, what the Phase 2 trial data actually shows, and where the online hype outruns the evidence.
You'll learn why weight loss and fat loss aren't the same thing, why muscle loss is the most overlooked risk on any GLP-1 medication (and exactly how to prevent it), what's really happening in your brain on these drugs, and the honest side-effect and safety picture — including the boxed warning most videos skip.
Retatrutide is still investigational and has not been FDA-approved. This video is for education only and is not medical advice — always talk to your own physician before starting or changing any treatment.
