We're going beyond the basics of boundaries and into the complexities and nuance to make sure that you are using boundaries in a way that is EMPOWERING. Believe it or not, boundaries can be used in a way that actually disempowers us and makes our lives smaller and smaller.


In this episode, Kristeen & Doctor Becc will talk about the most empowered way to use boundaries with both a short-term and long-term game plan in mind. Here, boundaries can be used as a temporary measure to buy us time to address the underlying issues that are causing us to need them in the first place. That's how we use boundaries to make our lives BIGGER - not smaller.

Here's some cliff notes:

ā¤ļø Boundaries can be implemented to protect & maintain our standards and safety.

ā¤ļø Setting boundaries is about taking control of your own experience - not about controlling, manipulating or sending a message to other people.

ā¤ļø We may want to use boundaries as a temporary measure when we find ourselves feeling affected by another and we are not able to attend to our thoughts or nervous system response in the moment.

ā¤ļø Ultimately, we want to aim to be so secure in ourselves that boundaries become unnecessary - others don't affect us.

ā¤ļø As humans - relational beings - we want to acknowledge that this is an *ideal* to work to rather than something that you 'should' have mastered in every scenario.

ā¤ļø Non-judgmentally expecting that being unintentionally affected by others will always be a part of our experience, this is where we can use boundaries to take great care of ourselves.

In this episode of Am I Doing It Wrong, we dive into a common self-care misstep: thinking that JUST ANY time away from work will be rejuvenating.

Nope.

Just because you're not at work or working doesn't mean you're putting deposits in your energy bank.


In particularly in this episode, we dive into how we've fallen into the trap of mistaking *socialising* for a method or rest and rejuvenation. Maybe you can hear this in your thoughts too as you say, "I've had the weekend off and spent time hanging out and having fun with friends. So I should be refreshed for work on Monday".

In fact, having MORE social time can actually create the need for MORE rejuvenation time. It's all about making sure you can listen to what your body & mind is asking for in the moment.

1ļøāƒ£ We want you to stop thinking something is wrong with you if you DON'T feel refreshed after a weekend socialising. For may of us, we have to recognise this as something that we want to invest energy in but that we don't *always* get energy back from.

2ļøāƒ£ We want you to be so clear about HOW YOU REJUVENATE so that when you want do, you can hit the right buttons to get that result.

On one hand, we can all nod our heads at the idea that perfectionism is unrealistic, unfair to expect of ourselves and even detrimental to try to aim for...


... But what about when you're a doctors? What about when your performace effects other people in their most vulnerable states?

  3. Set an Intention:Ā Decide how you WANT to feel. Maybe you crave calmness, focus, or confidence.
  4. Find the Thought:Ā Identify thoughts that can evoke those desired feelings.

Here are some practical tips Kristeen uses to stay positive:

  • The Joy List:Ā Create a list of activities that bring you immediate joy (dancing, singing, listening to a favorite song) and do one when you need a quick mood shift.
  • Clear Your Head:Ā Meditate, take a walk, or do anything that gives you mental space.
  • Set the Intention:Ā Declare how you want to feel and who you want to be in the present moment. This helps you embody your best self.

How Self-Sabotage Manifests

  • Study Avoidance:Ā Procrastination cleaning, feeling incapable when studying, etc., are ways to avoid the discomfort of studying and potential negative thoughts about oneself.

Counteracting Self-Sabotage

  • Self-Awareness is Key:Ā Monitor your mind and recognize your emotional states. How do your thoughts make you feel as you approach the task you're trying to avoid?
  • Choose Your Emotional State:Ā Recognize that suboptimal emotional states can interfere with learning and performance. You can choose to study from a calmer, more focused perspective.
  • Compassion vs. Condemnation:Ā Your brain's self-critical thoughts may stem from a desire to protect you from potential failure and a blow to your identity. Understanding this allows for a more productive negotiation with your mind.

Negative feedback is not the only place that our minds can spin out on us. For many of us, we have a painful reaction to receiving no feedback at all. In that space, our mind fills in the gaps between people's (in)action and their implications.

Ā 

Specifically, the situation we're talking about is when we're consciously or subconsciously expecting recognition or positive feedback ad we don't get it. We make a simple calculation: if positive feedback means I'm on track, I'm not failing, I'm good enough.Ā 

Ā 

… Well then, a lack of positive feedback means I'm not on trackĀ 

… I'm a failureĀ 

… I'm not enoughĀ 

Ā 

You personally may not recognise such blatant statements filling this space in your mind, but nonetheless, this episode is here to encourage you to notice where you have unmet expectations that a reaping havoc on your happiness and confidence.

This is particularly important for those of us working as doctors or in the healthcare in general as a culture that hasn't got a lot of positive feedback (or feedback of any flavour) built in. This isn't something that is going to rapidly and dramatically change, so let's do what we can to best support ourselves to show up and have the experience we want.Ā

22: Doctors, your procrastination is EMOTIONAL06 Feb 202400:30:48

Okay guys, here's a hard truth.


All of your procrastination is ultimately due to you trying to avoid emotions. Your want to avoid the unpalatable or even downright repelling emotion of what every task or activity you planned to be doing may be conscious or, more commonly, subconscious.


When not conscious (aware) of our reasoning, our minds will default to avoiding discomfort. Whenever we're not conscious about our choices during our day, we are likely to subconsciously choose to avoid the discomfort.


When we appreciate that about our human minds, we can start to appreciate how important it is that we prepare ourselves with strategies for following through with things we associate with discomfort - even the SLIGHTEST tinge of discomfort.


In this episode, we zoom in on what things we typically find ourselves procrastinating on and dissect out what discomfort is driving that procrastination.


If you can see yourselves in these examples, you will have a first key piece of awareness that can allow you to break your procrastination Cognitive Programs in the future. Getting conscious of something that is currently subconscious to you is an enormous step towards being able to change it.

When we can see the problem clearly, we are better positioned to choose impactful solutions. In this episode, we introduce the concept of DEPOSITS and WITHDRAWALS on your energy. NOTE: thinking about energy in terms of deposits and withdrawals is not an original concept but it is widely spread and we are unaware of the original creator to give credit to.Ā Ā

In this episode, we first talk about what it really means when we say we want to rest or we need rest. For most of us, what most of us mean is that we want to feel less tired and we think that being idle and not exerting ourselves is the way to achieve that. Like us, many of you may have found the 'just rest' and 'just do less' advice… essentially useless. It seems that with this model we can never seem to rest enough.Ā

Luckily, there's a more sophisticated way to think about rest. In this episode, we detangle the result of feeling rested or more energise with being idle or creating ease in the body.Ā

Clinical work is pretty magical. There is so much to love and enjoy about it. Something that comes with such highs apparently fairly comes with some lows too.Ā

One of them seems to be that most of us will agree it's EXHASTING.Ā 

Ā 

You can seemingly come home after a day of simply circling the wards at a pace that would make weekend golfers look athletic… Or sitting in a comfortable chair, chatting away with your patient and tapping away at the computer… YET, come home and be magnetised to the couch.Ā 

Ā 

All you can think of is being still and doing nothing… Maybe you can tolerate a bit of TV or a book. Your body is heavy, your mind craves to be quiet. You WANT to spend time with your kids and have a laugh with your partner. You WANT to be the kind of person who goes straight on to a nice dinner… Or even just the gym.Ā 

Ā 

But you hear your body's exhausted desperate whisper… "I can't".Ā 

Ā 

Okay, that may be a little dramatic. But it feels not too far from reality. Many of you likely notice your non-clinical partners can come home with notably more pep in their step. So it's natural to ask… Is it YOU? Are YOU the problem here?

We've cherry-picked this topic today because clinical work seems to tire the most resilient and enthusiastic of us. In this episode, we talk about what makes clinical work special including what makes it ESPECIALLY tiring.Ā

TALL POPPY SYNDOME. Be excellent. But be quiet about it.

Australia has a strong preference for or even culture of humility. We don't like it when people think a lot of themselves … Even if they've got a lot of good stuff to think about. Being humble is the expectation. And bragging is perhaps the worst offence.Ā 

Ā 

We even have a name for the cultural quirk: THE TALL POPPY SYNDROME.

Ā 

The Tall Poppy Syndrome refers to our tendency to criticise successful people. Knowing this tendency, we form fears of loudly successful or standing out for any good reason because we have the fear that we will be criticised - we'll be cut down. It's not necessarily something we do consciously, but often something we have learned implicitly and we see play out in our decisions and behaviour.Ā Ā 

Ā 

For some of us, even just THINKING a good thought about yourself can evoke feelings of bragging. Let alone SPEAKING it.Ā Ā And 'bragging' is certainly a dangerous thing if it makes you stand out as a Tall Poppy.

Ā 

This actually presents a challenge for people in medicine. Many of us have learned these sentiments on our way to medicine:Ā 

Ā 

  1. Meritocracy: if you just work hard, you will be rewardedĀ 
  2. Tall Poppy: if you stand out as successful, you will be criticised

Ā 

Together: work hard and be quiet about it.Ā 

Don't think positively about yourself.

… And, eventually, if you just work hard enough, you'll be rewarded or anointed.Ā

Recipe for prosperity and a happy life? Not quite.Ā

It is extremely inconvenient to be tired right now. Relatable?


We're talking about that kind of tiredness that isn't neutralised by a sleep-in, where you don't get re-booted from a workout or revived when you just work out you were missing some Vitamin Social Life.Ā 

Ā 

We're talking about the kind of tiredness that won't allow itself to be ignored. The kind of tired where it seems that your body shuts down all non-essential systems to the point that you are able to be still and blank.

Ā 

Tiredness is not a problem in itself. When paired with being committed to your commitments, however, it does create significant challenges.

Ā 

On one hand, it's great that you're committed and that you have the discipline to show up regardless of how you feel.

Ā 

On the other hand, to disregard how you feel is a dangerous line to walk or anything but a short-term emergency.Ā 

Ā 

Being able to recognise the signals that our bodies send to us is critical for long-term success. Our human bodies have needs that must be met to facilitate health and performance. And how we feel is the mechanism by which our body can communicate this to get our attention.Ā 

Ā 

Doctors have, unfortunately, been encouraged to practice emotional suppression making it easier to miss or dismiss the messages. This may appear adaptive in the short term but emotional suppression is, however, a blunt tool by which I mean, we cannot suppress selectively just one feeling but instead, it's all or nothing. When we choose to suppress one emotion, we take the others down with it.Ā 

Ā 

And we lose important information about our life direction and our health in the process.Ā 

Ā 

In this episode, we are talking through our current real-time experience of managing this tiredness without avoidance, overriding, or denial. And we're meeting our limitations and revealing areas for further work here. We are walking the talk from Episode 1 of Am I Doing it Wrong the podcast!

Episode 1 brought forward the question of whether 'balance' was achievable or even the goal for doctors. Instead of balance, we have landed on contrast - allowing alternating times of being over-subscribed and then more free and light.Ā

Doctors aren't generally used to being BAD at things. But Becc has recognised something that we're pretty bad at across the board.Ā

Self-validation

Ā 

She's very passionate about bringing this skill to doctors for the benefit of both our professional and personal lives.Ā

Why? Because it's extremely productive.Ā 


And because invalidating ourselves can be extremely destructive.Ā 

Ā 

What is self-validation? What does it mean to validate yourself? The simplest way to think about self-validation is that it's the opposite of self-denial, dismissal and resisting our own experience.Ā 


The objection to self-validating that Becc has found to be ubiquitous in her high-achieving, successful clients is that looking by giving more attention to the 'negative' aspects of our experience, we'll only magnify them. That they'll then be more likely to play out in our behaviours or take up more space in our minds.

Paradoxically, it's *usually* the opposite.Ā 

Ā 

Think… Can you can recall many experiences where resisting and denying yourself has cost an enormous amount of attention and energy? That's attention and energy that can't go to your productive outputs.Ā 

Ā

"Wouldn't it be nice if.."

With her mind dismissively responding

"You should just be easy"

"You should just accept"

"Don't be too much"

"Don't inconvenience them"

"They'll feel like they have to say yes"

"They wouldn't say no even if they wanted to"

"Don't risk putting pressure on them"

So, before she'd even considered what she wanted, she'd counted herself out.Ā 

Ā 

Here's the thing about Santa.Ā 

Ā 

If you don't write the letter, how can he fulfil your wish?


And, if you don't believe in Santa, you won't ask for what you want. And, so, you won't get it.Ā 

Ā 

If you don't believe, you won't receive.Ā 


Or, more accurately, if you don't believe, YOU WON'T ASK, and so you won't receive.Ā 

Ā 

When we allow these responses that arise in our minds to make what we want seem unreasonable or unrealistic, we don't act like it's possible - we don't ask for it.Ā 

Ā 

We need to allow ourselves to believe in order to get in the arena.Ā 

Ā 

And, it's time to get in the arena.Ā 

Ā

There are many reasons any of us may not ask a question when it would otherwise be in our integrity to do so.Ā

It reveals our knowledge gapsĀ 

We think we "should know this by now"

We think it's not acceptable not to know the answerĀ 

We think we should learn it ourselves, on our own time

We don't want our colleagues to draw false assumptions about usĀ 

We don't want our colleagues to draw true assumptions about usĀ 

We assume people don't want to take time to answer questionsĀ 

We think asking the question is annoyingĀ 

We don't want others to feel frustrated with usĀ 

We don't want to make others uncomfortable if they don't know the answerĀ 

We would rather avoid any negative reaction

We decide it's not worth it if there's a possibility of a negative reactionĀ 

We are uncomfortable taking up space

We are uncomfortable asking for others' timeĀ 

We don't think we have 'earned' the questionĀ 

Ā 

And this is just a small sample of thoughts that can accidentally get in our way.Ā 

Ā 

There's another component too.Ā 

Ā 

Simply not being present and intentional.Ā 

Ā 

Being present and intentional are critical ingredients for creating change in our lives. This requires core skills of mindfulness and directing attention (focus). It also often requires some time spent in advance to decide what helpful rules you create for yourself in advance of opportunities.Ā 

Ā 

For example, you might decide in advance, like Becc, that you will err of the side of asking questions. In this case, the decision in the moment has been made simpler - I will ask my questions UNLESS there is an outstanding exception (say, time-critical situations).Ā 

Ā 

There's a lot to be said for setting intentions, especially when you consider biases of minds such as CONFIRMATION BIAS.Ā 


If we set an intention of asking questions - and taking opportunities to ask more questions, we will register more opportunities in my day. Opportunities that may have been missed if we were prioritising other things.Ā

12: When you DON'T do not harm21 Nov 202300:39:01

The first rule: do not harm. Okay but what about when you do? It sounds like the simple, obvious and easy minimum - to not harm our patients as we care for them. But we all have a library of experiences where our treatment decisions did do harm. And that's because medicine is not just hard from a sense of needing to build and accumulate knowledge and skills to care for patients. It's not just hard because we are working to integrate multiple data points and data sources from each individual patient and synthesise that into a congruent impression. It's not just hard because executing one treatment plan can exacerbate or create another issue or reveal another vulnerability in the patient. It's not just hard because of the limitations and demands of the healthcare system that mean that we cannot access ideal clinical care conditions or even conditions that we feed are the minimum standard. But it's also hard because we are constantly working with uncertainty. And the truth is, when you're working with such limitations, complexity and uncertainty, you don't get to simply do no harm. In a doctor's professional life, there will be many instances where we cause more harm than good in the noble pursuit of doing the best thing for our patients. That reality is extremely difficult for a lot of us to face. It's important that we find a clear balance between what is in our control and what is not in our control. Both situations are equally difficult. It can be extremely difficult to know that you have made a mistake, or that your skill level contributed to the harm done to a patient. Taking responsibility is a cornerstone of growth. For doctors, however, it's easy for us to not recognise the contextual influences of the 'mistake', for example … that we work as a team in healthcare, not as an individual … that we are in training … that our skills are never perfected even when we are experienced … that support, supervision and training provision should appropriately mitigate deficits and risk … that the healthcare system 'Swiss cheese' model should usually function to prevent any domino effects of a mistake. It's common for us to over-estimate what is in our control It's common for us to take over-responsibility … And make erroneous and hindering conclusions in retrospect about how we single-handedly could have prevented something or how we have really just proven we're not the 'right kind of person' to do medicine. This is over-responsibility. To be someone who is conscientious to provide great care for our patients and, with that, to do no harm wherever humanly possible, is an advantageous trait of a doctor. Let's not use that against ourselves. There's nothing useful or empowered about trying to control something that you can't. There's nothing that will make you feel more out of control than trying to control something that you can't. This is a MAJOR part of being a doctor that doesn't get enough attention. It needs to be done well to create good outcomes - for our patients and for ourselves as doctors and teammates.Ā

As the doctor coming on, you have a short period of time to extract all of this important information from your colleague. Information that isn't always documented or possible to find elsewhere. You're receiving a lot of important information at once. You're also often handed a lot of uncertainty.Ā 

Ā 

As the doctor giving the handover, you're synthesising and summarising to save your colleague's precious brain space. And you're not only communicating what you do know but, importantly, what you don't know.Ā 

Ā 

… That's a lot.

Ā 

And it's the perfect storm for these mindsets of perfectionism, imposter syndrome and people-pleasing to come along and make your life harder.Ā
