A 20-Year Battle with Toxic Multinodular Goiter (Save My Thyroid Audit)

Toxic multinodular goiter is a type of hyperthyroidism in which a person has a goiter in the presence of multiple thyroid nodules. In today’s Save My Thyroid audit, I’m chatting with Nanette, who has been dealing with toxic multinodular goiter for over 20 years and I’ll be sharing practical advice to help her improve her health.
Nanette’s thyroid journey began at age 16 when she had her first thyroid nodule, which was then surgically removed. Later, at age 30, she found herself in the same scenario, and she had a second surgery. Following that, she’s been on and off of different thyroid medications and was eventually diagnosed with toxic multinodular goiter.
While this isn’t the same as the comprehensive consultations I do with patients, you’ll get a lot of value from the conversation. Nanette and I discuss long-term medication use, her treatment with propylthiouracil (PTU), dietary changes, the importance of stress management, why we need to address underlying causes like insulin resistance and inflammation, my approach to testing and lifestyle modifications, and more.

Air Hunger and Hyperthyroidism

Air hunger, also known as dyspnea, refers to shortness of breath or the feeling of not getting enough oxygen. While a thyroid hormone imbalance can be a factor in some cases, there are many other potential causes of dyspnea, including asthma, COPD, congestive heart failure, interstitial lung disease, pneumonia, and mental disorders.
For those with thyroid conditions, air hunger is more commonly associated with hyperthyroidism and Graves’ disease than hypothyroidism, though it may not always be the cause. Today, I’m explaining the connection between air hunger and hyperthyroidism, how it is diagnosed, the importance of addressing thyroid hormone imbalances, related conditions to consider, and the need for a comprehensive treatment approach to manage air hunger effectively.

My Graves' Disease Story

In late 2007, I found myself a little overweight and set out on a mission to lose weight through dieting and detoxing. The plan seemed to be working, and by 2008, I had lost a lot of weight. One day, while walking around a retail store, I decided to check my blood pressure on one of their automated machines. While my blood pressure was normal, my heart rate was elevated. This was my first clue that something was wrong.

Today, I’m back with another Q&A episode, answering multiple questions submitted by the Save My Thyroid community. While I can’t get to everyone’s questions every time, I love creating these for you, and I’ll do similar episodes every couple of months. Q1: What’s the connection between insulin and thyroid disease?
Q2: My son has complete hair loss, his blood pressure has been elevated, I feel something is off with his kidneys, and he has Hashimoto's. He’s ten years old and was diagnosed at three years old with Hashimoto's. He also has Down syndrome. He is dairy, grain, and soy-free and only eats organic foods. I’ve reduced all the toxins I can think of and taken supplements, but something is still off.
Q3: I took the vaccine, and I developed hyperthyroidism and Graves’. I’m on methimazole, one pill, five milligrams a day; my antibodies are 458 and coming down. How do I lower my antibodies now that my T3 and T4 are normal and I want to get off the methimazole?
Q4: Can Graves’ cause anhedonia despite steady thyroid hormone levels?
Q5: Can low-dose naltrexone (LDN) at 0.05 to 4.5mg help with hypothyroidism, Hashimoto's autoimmune disease?
Q6: I would like to discuss why methimazole suppresses TSH and why some people have antibodies for both Hashimoto’s and Graves’.
Q7: I've done a 24-hour iodine loading test, which showed 60%, which is below the 90% level, so I’m deficient in iodine. How long should I wait before testing again?

Healing your thyroid when it comes to conditions like Graves' disease and Hashimoto's thyroiditis starts with finding and removing the autoimmune thyroid triggers that are impacting you.
According to the triad of autoimmunity, three components are necessary for autoimmunity to develop: genetic predisposition, environmental triggers, and a leaky gut. For most people, a combination of these components and multiple triggers are at play.
Today I’m exploring the four main categories of triggers as well as testing that you may want to consider. I discuss how food, stress, infections, and environmental toxins can impact the thyroid, when I recommend testing, why testing is not always worth the cost, my experience with identifying my triggers when I dealt with Graves’ disease, and more. Which viruses can trigger thyroid autoimmunity?

Although most viruses don’t directly trigger autoimmune thyroid conditions such as Graves’ disease or Hashimoto’s, certain types of viral infections can play a role in developing these conditions. This can happen via a number of different mechanisms, including molecular mimicry and direct bystander activation.
In my practice, I’ve seen numerous patients where different viral infections, were a potential trigger in the development of their autoimmune thyroid conditions. Today, I’m sharing some of the more common viruses associated with thyroid autoimmunity and what you can do to support your thyroid and immune system.

Thyroid conditions in children are often missed since getting a diagnosis is much less straightforward than for adults. Unlike adults, where screening follows a much more standard pattern, children don’t typically undergo annual testing, especially not for thyroid hormone levels.
They also often have symptoms long before their labs start to show abnormal results, and the symptoms they’re experiencing can easily be dismissed as side effects of growth and development.

Can I eat oats, rice, and other grains?

According to the Triad of Autoimmunity, three components are necessary for autoimmunity to develop: a genetic predisposition, environmental triggers, and a leaky gut. This is why grains such as oats and rice are excluded from an autoimmune paleo and a standard paleo diet.
While you don't need to eat grains to get the nutrients that you need, they can be quite tasty. Unfortunately, grains have compounds that can be harsh on the gut, including phytates and lectins, which can make it difficult to heal a leaky gut or can contribute to its development. Today, I’m discussing how grains affect the body and my recommendations for grain consumption while you are on your thyroid healing journey.

A goiter is an enlargement of the thyroid gland commonly caused by issues such as hypothyroidism, hyperthyroidism, insulin resistance, problems with estrogen metabolism, and iodine deficiency.
While it is possible to shrink a goiter, there are a number of factors to consider. Even though my goal is to save as many thyroid glands as possible, there is a time and place for thyroid surgery. Today, I’m sharing the factors that influence whether or not you can shrink a large goiter and when to consider thyroid surgery or natural treatment alternatives.

Can Candida Trigger Thyroid Autoimmunity?

While yeast is a normal part of your gut flora, issues can arise when its population gets too high. I commonly see Candida overgrowth in my patients with Graves’ disease and Hashimoto’s, with the most well-known species of yeast in the gut being Candida albicans.
Research shows that Candida can contribute to an increase in intestinal permeability, also known as leaky gut, which is one part of the triad of autoimmunity. Today, I’m exploring whether Candida can trigger thyroid autoimmunity and how to test for and manage Candida overgrowth.

Many people focus on diet and exercise when they want to lose weight. However, especially when it comes to stubborn weight, focusing on our health and addressing underlying imbalances gets way better results than eating less and exercising more.

“There is no survival advantage to screening with mammograms.” - Dr. Jenn Simmons
For over 50 years, healthcare providers have told women how important it is to have a mammogram every year after a certain age. With mammograms positioned as the primary screening tool for breast cancer, these annual visits are dutifully maintained for years in the hope that they will act as an early warning system. Unfortunately, the data doesn’t support this theory.
Research shows that there was no survival advantage to screening with mammograms, nor does it result in more breasts saved. In addition, repeated radiation exposure causes thousands of breast cancers every year, as well as thousands of women treated for breast cancer that never needed to be treated. Luckily, we now have alternatives that are safer and less aggressive.
Today, I’m excited to chat with Dr. Jenn Simmons, who is on a mission to change the impact of breast cancer. Dr. Jenn started her professional career as Philadelphia's first fellowship-trained breast surgeon and spent 17 years as Philadelphia's top breast surgeon. Endometriosis is a structural immune disorder that affects around one in ten women. With this condition, tissue similar to the lining of the uterus grows anywhere in the body outside the uterus.
Each case of endometriosis can present differently, and the condition is often misdiagnosed or undiagnosed, with an average delay of diagnosis of around 12 years. Symptoms can start in pre-puberty, first showing up as gut or digestive issues. Later, we often see menstrual pain, bladder pain, and various comorbidities such as mast cell activation syndrome.
Today, I’m excited to be joined by Dr. Jessica Drummond to have a comprehensive chat about endometriosis, which can be a very complex condition to treat. Dr. Jessica is the Founder and CEO of The Integrative Women's Health Institute. While I can’t get to everyone’s questions every time, I love creating these for you, and I’ll do similar episodes every couple of months. Q3: Based on your experience and work with patients, roughly how long does it usually take to see significant changes in the antibodies while finding and removing triggers to address the cause of an individual's autoimmunity? Q4: Is it safe to use a multivitamin that contains copper if one has Graves’ disease and hyperthyroidism? Q5: Are there specific protocols that can help to restore my strength and vitality when dealing with Hashimoto’s? Q6: In 2019, my husband died, and shortly after, I was diagnosed with Graves’ disease and hyperthyroidism. When I was diagnosed, I had symptoms, but for the last two years, I've had weight gain, which is hard to lose, minimal palpitations, and no fatigue. I’m taking methimazole, and my T3 and T4 are within range, but my TSH is less than 0.01. Since I found your videos, I've become serious about finding the root cause of my issues. What do you think might be going on? Q7: Do thyroid nodules cause anxiety and nervousness? I've read that they don't cause any symptoms; is this true? Q8: What blood test do I need to get to see when I can start reducing my medication, carbimazole? Q9: Can you consume oats and oat milk, or does it need to be certified gluten-free oats? Q10: I have hyperthyroidism and cannot find much consistent data on it. I want to treat it with natural supplements and diet. Can you give me information on that? Q11: To date, my issue is benign nodules, no autoimmune, T3 and T4 within the normal range. Is there any way to shrink or improve this issue? Q12: What should I do regarding a diagnosis of subclinical Graves’ or hyperthyroidism and thyroid eye disease? Q13: Is it safe to take electrolytes? They have quite a lot of sodium.

Can PTU Cause Permanent Liver Damage?

Compared to other anti-thyroid medications such as methimazole, propylthiouracil (PTU) is not commonly prescribed by endocrinologists. It’s generally only prescribed in specific cases where the methimazole is contraindicated or not well tolerated.
It's not uncommon for PTU to cause elevated liver enzymes, and in rare cases, that can lead to liver damage. Thankfully, this is not something I’ve experienced in my practice, as it’s a rare side effect. Today, I’m sharing what the research shows about PTU's potential negative impact on the liver.

Nagina has a degree in Molecular and Cell Biology from UC Berkeley and has helped over 1,500 midlife women successfully lose weight and create a lifestyle change.

Thyroid conditions have also been shown to impact oral health, and in my experience working with people with thyroid and autoimmune thyroid conditions, I’ve seen an increase in dental cavities, especially among patients with hyperthyroidism.

Thyroid health has a complex relationship with blood pressure. Despite common misconceptions, blood pressure issues can arise in both those with hypothyroidism and hyperthyroidism. The good news is that by working on the underlying root causes of thyroid conditions and strengthening the foundation of your health, you will often also see improvements in blood pressure.
Today, I’m excited to be joined by Dr. Ellie Campbell to take a deep dive into all things blood pressure. Dr. Campbell is a native Chicagoan and a graduate of the University of Illinois, the Kirksville College of Osteopathic Medicine, and the Medical College of Georgia. Board-certified in Family Medicine for 30 years, she also holds certification from the American Board of Integrative Medicine. Dr. Campbell is a solo physician with specialty interests in interdisciplinary collaboration, Functional Medicine, reversing chronic disease, cardiovascular disease prevention, natural treatments for high blood pressure, and the many oral-systemic connections. Why do my thyroid hormones increase even though my TSI is decreasing?

Often, we’ll see an increase in thyroid hormones in those with Graves’ disease. This is due to thyroid stimulating immunoglobulins (TSI), a type of TSH receptor antibody that binds to the TSH receptors and causes the thyroid gland to release an excessive amount of thyroid hormone.
For some people on the healing journey, TSI and thyroid hormone levels decrease simultaneously. However, this isn’t always the case. Today, I’m sharing why you might find thyroid hormone levels increasing despite decreasing TSI.
