I'm 41 and a carrier for a genetic disorder. Should I do another round of IVF now, or wait until after a healthy pregnancy with our one good embryo? (Ask the Egg Whisperer)19 Dec 202100:29:49
Recently, Elizabeth shared a question on AskTheEggWhisperer.com. She is 41, and she and her husband are carriers of a recessive disorder that results in severe physical and mental disability. Here's what she says, "I'm 41 AMH 0.95. We had a little girl naturally when I was. 40, but she didn't survive. We have a 25% chance of recurrence to screen for the disorder.
We did our first round of IVF and we got results back today out of seven eggs, five fertilized, three made it to blast. We have one normal that is not affected. There was another normal blast, but it was affected, and there was one with abnormal chromosomes. We're so happy to have a good one to use.
We might want two children though. I have two more rounds covered by insurance, but they can't be used for embryo banking. Should I wait until after a healthy pregnancy with our good one to do IVF again? Or should we do another round now?"

Recently, Tracy shared a question on AskTheEggWhisperer.com. She is 29, and her husband is 39.
Here's what she says, "I have hypothalamic amenorrhea. I've had two egg retrievals. The first egg retrieval, three fertilized after seven retrieved, one blast tested abnormally. The second retrieval, we added an Omnitrope and dexamethazone during stimulation. ICSI and calcium solution to the eggs. Five retrieved, one fertilize, one blast, and it was normal. I eat very well, I take supplements, I meditate, and I am doing acupuncture.  Any ideas on why have such poor fertilization? Do you think I have poor quality eggs, or is that the protocol that's hurting them? Should we be testing my husband's sperm further? What would you do for my next cycle?

Recently, Rita sent a question in to Ask the Egg Whisperer. She is 30 and lives in Boston. Here's what she has to say, "I recently found out I have silent endometriosis, and had surgery for it. I'm now experiencing worse and more aggressive endometriosis. I've switched fertility doctors to one who specializes in excision surgery and fertility, and his recommendations was to do one cycle of fast-track IVF and bank as many embryos as possible.
What is fast track IVF, opposed to regular IVF?
He did not put me on an "endo friendly" protocol. Do you get fewer eggs with an endo friendly protocol?
My dream is to be a mom of five. My AMH is 4, FSA is 7.
I've been taking CoQ10, 0 fish oil, vitamin D, NAC, and I just started melatonin.
Is there anything else you can recommend?"

Recently, Kelly sent a question in to Ask the Egg Whisperer. She is 31 and lives in California. Here's what she has to say, "I'm in the middle of an IVF cycle and I was deemed a poor responder. My AMH is 3.1. My follicle count was 27 at baseline. I only had nine follicles that were responding on day eight.
I was told is due to my BMI, but I feel like the protocol just wasn't aggressive enough.
I'm thinking of canceling and advocating for a new protocol. What would you suggest for me?
I want three to four children and our diagnosis is male factor infertility. My husband is 38, and has 2% morphology. I have one blocked tube and was told it's scar tissue, but it's not in hydrosalpinx. I love the show and I have learned so much."

I've had two failed frozen embryo transfers, reciprocal IVF with my wife's embryos. They were poor quality day three embryos. And my wife's latest retrieval yielded three PGT normal embryos. Would you recommend a medicated FET or more of a natural FET with no birth control pills, ovulation trigger shot, and progesterone supplementation only if necessary? Why? What levels do you like a progesterone estrogen to be on the day of the FET?
We are so excited and we feel like we actually have a shot with, with these gorgeous new embryos.

Recently, Jessica sent in a question to Ask The Egg Whisperer. Here's what she asked, "In terms of adding HGH to a protocol, does it add any benefit to do it during stims, or is it better to prime with it before your IVF cycle starts?
My doctor's having me take Aygestin five days after ovulation for 10 days, then we'll schedule my baseline. Can I try naturally this month? I hate wasting a month and possibly a good egg."

He was the first scientist to optimize and validate next generation sequencing for PGT-A and PGT-M and he continues to work on new emerging fields in IVF.

Recently, Amy sent a question in to Ask The Egg Whisperer. Here is her question, "We just finished our fifth IVF cycle and we finally have two PGS normal embryos. I am in shock. We're preparing for a December transfer. I had two transfers in the past. The first one resulted in my son in 2017. And unfortunately the second one resulted in a miscarriage at eight weeks.
My new fertility doctor wants to stick with the same protocol, since both of my two transfers were technically successful. I just turned 38 and I do have mild endometriosis. I had a surgery in 2013 to confirm that, so this makes me nervous. Should I continue with the same protocol? Is there anything else I should do before transfer?"

What causes it thin uterine lining, and what thickness do you recommend for embryo transfer? (Ask The Egg Whisperer)27 Nov 202100:21:51
Recently, Kristin wrote in to Ask The Egg Whisperer. She's 39, and she's undergoing her first IVF cycle. Here's what she asked, "I am currently preparing my uterine lining for transfer. After two weeks of taking Estrace two pills, three times daily, my uterine lining only measured 4.8 millimeters. My doctor increased my estrogen by prescribing patches. (Two patches that are replaced every other day). After one week of using the patches and pills, my lining increased to 5.5 millimeters. My doctor wants me to continue on this protocol for another week and see what kind of progress we make.
What causes it then uterine lining. What thickness do you recommend for transfer? What can I do besides medication to increase my lining thickness?"

I am 44 and have type 2 diabetes. Should I consider IUI or IVF? (Ask The Egg Whisperer)20 Nov 202100:26:50

Recently, Kim sent in a question to Ask The Egg Whisperer. She is 44 and has type 2 diabetes. She wants to know if she should continue trying to get pregnant with her own eggs, or consider other options. 

Here's what she had to say, "I have had 2 normal pregnancies in the past, with the last one 10 years ago. I have a boy and a girl; my husband and I have tried to conceive for the past 4 years naturally without success.

We went to a fertility clinic and have done all test possible everything came back good besides that I was diagnosed with type 2 diabetes with 11.4 A1C and reduced it to 7 in 3 months, my AMH level was 0.25.

We have done an IUI cycle last month, and it was unsuccessful, and we are trying IUI for the second time with clomid and obedril shot as the last time.

Are we doing the right thing or are we wasting time?

I would really like to avoid IVF if possible, plus I would like to use my own eggs if possible. Thank you!"

IVF is a process that involves creating an embryo in a lab. To help explain the process of creating those embryos, I'm joined today by embryologist, Salu Ribeiro. We're are talking about what happens in the lab, after eggs have been retrieved, what makes a "golden egg," and how fertilization works. We'll explain some of the terms  you may hear when you're doing IVF: ICSI (which is Intracytoplasmic sperm injection) and PICSI (which is physiological ICSI, where sperm are first chosen for ICSI). And, we talk about how embryos grow, and what an embryologist looks for in eggs, sperm, and embryos. I love talking to Salu about embryos, and this conversation is a lot of fun!

PCOS stands for polycystic ovary syndrome. Many people interpret it to mean you have big cysts on your ovaries, but that's not true at all. What tests should I consider to determine if I have adenomyosis or endometriosis? (Ask The Egg Whisperer)07 Nov 202100:15:12
Cheryl sent in a great question to Ask The Egg Whisperer. She is 39. Here's what she asked,   "Thank you for your show, Dr. Aimee. I had two IVF cycles, but my egg maturation rate is very low (54%, 10%).    My AHM level is 0.67. My doctor did not find any signs of endometriosis, but some signs of adenomyosis. 
I saw there are four potential reasons for low maturation rates: 1) my age; 2) wrong IVF stimulation protocol; 3) endometriosis; 4) reproductive autoimmune failure. 
Are there any tests that I can do to figure this out?"

I'm already on a 2nd repeat course of antibiotics after 2nd biopsy still came back positive for CD 138 stromal cells. 

My MD is suggesting to go straight into a transfer after second round of antibiotics without an additional biopsy. I'm afraid I may have an antibiotic resistant form and am wondering if you ever perform endometrial scarring or D&C to treat it? 

Would you recommend to have a confirmation that the endometritis is gone before implanting? Also I am already on depot lupron 2nd month for endometriosis. Is it a concern that I will not get a period that would shed the bad cells somehow before a transfer? Would love your input on how you deal with resistant endometritis and implantation outcomes."

I'm so excited to have Dr. Bruce Lessey of ReceptivaDX join me on the podcast. Dr. Lessey has dedicated his research life to studying the lining of the uterus and more specifically, how to help embryos stick better. I have a double uterus, and two cervixes, and have been trying to get pregnant for seven years. Do you have any resources or guidance for me? (Ask The Egg Whisperer)31 Oct 202100:19:12
Recently, Jess sent a question in to Ask The Egg Whisperer. Here is what she had to say, "I'm 41, have been trying for 7 years to have a baby. My husband is 39 and has tested fine, no issues. We've went through two IVF rounds in 2018. My husband and I have tried basically every fertility treatment and have not yet had a baby. I got pregnant out of the blue in 2017 but it ended up being a blighted embryo. I have uterine didelphys (double uterus) with a complete septum and two cervixes.  Based on all the tests over the years, low ovarian reserve seems to be the main issue. I've never received a formal diagnosis. It's always been "unexplained". Do you have any resources or guidance you can please point me to?"

I'm 42, and have had 2 failed IVF cycles. What tests should I consider, and what should I do differently for this next cycle? (Ask The Egg Whisperer)30 Oct 202100:23:42
Recently, Isiuwa sent a question in to Ask The Egg Whisperer. Here is what she had to say, "I am a 42 year old woman who started IVF in 2019 that ended in a failure. I also did an HSG and was told that I have blocked fallopian tubes.  I recently experienced my second IVF failure. This second round, I decided to genetically test my embryos (2 euploid and 1 aneuploid/low mosaic).
Now that I have 2 failed cycles, I am seeking your advice on what you think I should do differently this time around. I am thinking of requesting the following tests to start with:
1. An ERA and Receptivadx tests.
2. Immunology tests such as thrombophilia and killer cells test.
3. A natural or modified natural cycle.

When is it Time to See a Fertility Expert?11 Jun 202500:28:40
If you're eager to get pregnant and are wondering if it's time to see a fertility specialist, you might be right. There are a number of factors to take into account when deciding whether it's time to see a fertility specialist. Of course there are a lot more details about each of these items in the podcast episode today, but here is a sneak peek of the 6 reason that may be telling you it's time to see a fertility expert:   1. If you're over 35: schedule an appointment if you've been trying for 6 months.  2. If you're over 37: consider seeing a fertility specialist (and not wait before seeking professional advice) to get your TUSHY checked.  3. If you have irregular cycles, make an appointment to see what may be causing that irregularity..  4. Painful periods. If you think your periods are more painful than average, or you are worried about the pain you're experiencing, you may suffer from endometriosis, which can make it more difficult to get pregnant. A fertility expert can help with a diagnosis and treatment.. 5. Heavy bleeding. If your periods seem especially heavy, this may be a sign that there is something blocking your ability to get pregnant. 6. Male factor or sperm issues. It takes an egg and a sperm to make a baby so if your male partner or sperm donor has a chronic medical problems, is drinking, smoking or partaking in drugs,  or is overweight, a fertility specialist can help determine if that is playing a role.

In this episode, Dr. Aimee is joined by Dr. John Norian. Recently, Rebecca from New Zealand sent a question to Ask The Egg Whisperer. Here's her question, "I'm 31 and have two children. My FSH is seven. We started trying to conceive in late in 2020, and got pregnant right away. Sadly, it ended in a six week spontaneous miscarriage. I got pregnant again after the miscarriage and this pregnancy resulted in also a miscarriage at 12 weeks of pregnancy. I'm RH negative, and had the RhoGAM after both losses. I'm booked for fertility appointment about a month. I've made lots of lifestyle changes, avoiding BP, and now eating a plant-based diet. What would you recommend as next steps for me as far as supplements and are there any other tests that you would recommend?"

How many vials of sperm do you need for IVF cycle? (Ask The Egg Whisperer)23 Oct 202100:20:36
In this episode, Dr. Aimee is joined by Dr. John Norian. Recently, Emma wrote in to Ask The Egg Whisperer a question from Wisconsin. Here's what she had to say, "I'm 33. My husband had treatment for a brain tumor four years ago, and his sperm did not come back. Luckily before his treatment, he froze 16 vials of unwashed sperm in the UK (England), where we're from. We had it shipped to Wisconsin, where we live now. We've tried IUI and have not been successful. We're now doing IVF with only five vials left. My question is how many vials do you need for one IVF?
How will we know if this sperm is sparkling without losing a vial for testing? 
My AMH is 1.6. My FSH is 7.3. We want two kids. I wish we had better information about IUI success and had not used so many of our frozen vials on IUI. Now we have only five vials left. How do we make the best chance of achieving our family dreams?"
