Often we find ourselves trying to emulate others, and not tapping into our own creativity or our 'inner power'.  We are creative beings (humans). At times, we lack self belief, and it is run over by self doubt.

Mary Rensel, MD is an Assistant Professor of Medicine at the Cleveland Clinic Lerner College of Medicine and Director of Pediatric Multiple Sclerosis and Wellness at the Mellen Center of the Cleveland Clinic. She has graduated from the Medical College of Ohio and completed her Neurology and Neuro-immunology fellowship training at the Cleveland Clinic. She is boarded in Neurology and Integrative Medicine and is a fellow of the American Academy of Neurology. Dr. Rensel’s work has focused on Adult and Pediatric Multiple Sclerosis, Brain health and Integrative Medicine. Her work in academic medicine often intersects with wellness, advocacy, innovation, coaching and strategic initiatives. She is a “Best Doctor” of Cleveland since 2010 per the Cleveland Magazine.  She is the owner of Brain Fresh, a neuroscience based professional development  support system.

What comes to mind when discussing hustle culture?
Do you think it is true that people are rewarded for pushing through and collecting accolades, financial security?

It is no secret these questions make me think of 9 to 5 from the 1980s, but then again, that was my decade of birth.

In this episode, I spoke with Jennie Blumenthal, who 20 years as an executive in Corporate America counseling Fortune 500 companies on growth strategy and digital transformation and left during the pandemic pivot.   She now coaches executive women through their own process of transformation.
Having experienced burnout, she stepped away from the corporate life, which she wrote about.  Her book,  Corporate Rehab: Ditch the Hustle Culture and Thrive Again, covers these topics, as well as her own transformation as it unfolded. She narrates her steps.

It is no secret that documentation has become burdensome in medicine.
Physicians did not go to extended schooling and training to write notes that appease insurance and the legal system.
Notes are/were meant to document an encounter that happened between physician and patient.
The EHR has become the 'bane of my existence' while working primary care and even during the days I used to do inpatient medicine.  Using a computer to check a box and look at the screen instead of looking at the patient has diluted the physician-patient relationship.

What if we could leverage AI [artificial intelligence] to help alleviate that burden while appeasing the paper gods and insurance companies?

I had the pleasure of having a delightful conversation with Matt Dallmann,the developer of SmartNoteMD, has spent the last 20 years in private healthcare management, consulting and medical billing.

He shares his inspiration for developing SmartNoteMD as well as his passion for contributing to the world of medicine.  Collaboration with physicians and AI developers can be a possible solution to help prevent burnout/moral injury.
The documentation is not going away any time soon.  But, with brilliant minds like Matt's, it is possible to find solutions.

Dr. Diane Shannon, MD, MPH is a trailblazer with regard to physician burnout. When she saw that the system did not allow [due to many pitfalls] her to deliver patient care like she had envisioned, she changed her narrative.  She is an author, speaker, and professional coach with a plethora of experience.

Burnout can feel isolating.  As physicians, we don’t discuss how we feel may be because it shows vulnerability, which we [as physicians] are taught not to show.  We are trained not to ask for help or it is a sign of weakness, failure.


 Dr. Shannon had the courage to tell her story, and by doing so, it led her to all the great work she has done!

The work box, play box, family box, and social box.  Over all of it is the spiritual box, as it influences how we as humans interact with all around us--including ourselves.

In this episode, I will cover my spiritual journey and recent travel.  I find that I had to balance by spiritual box in order to balance my other boxes.

It is integral to the human essence and soul.
We have lost the understanding and awareness of our physiology. We don't recognize distress from flight or fight. We end up in burnout. We start to shut down. We run on autopilot. Humans are social creatures. We need our tribe and ways to connect...to replenish our energy or cup. When one box (work, play, family, spiritual) becomes unbalanced or overwhelmingly consuming it can tilt one to burnout.

Physician’s are trained to put patients’ needs above their own at all costs. Sick? Take medicine and power through. Feeling depressed and lethargic? Suck it up and think about the patient’s problems instead of your own. This learned act of putting your own needs to the side time after time causes suffering on both a physical and emotional level.


On the show today to speak about her journey to find balance being a medical professional and healthy human is Dr. Luyi Kathy Zhang, a hospice & palliative care physician, mindset & life coach and author. She also hosts The Purpose Filter podcast, which helps living people apply dying lessons before it’s too late.


Like many physicians, Dr. Luyi Kathy Zhang’s identity was so wrapped up in medicine that she couldn’t see the other parts of herself. Her position in palliative care was always a difficult and emotional experience, but the COVID-19 pandemic tested her in ways she could have never imagined.


Her time working in New York during the height of the pandemic was marked by a loss of control, leading to panic attacks, excessive crying, and burnout. As she shares on this episode, she experienced suicidal thoughts and was forced to take steps towards healing.


To anyone listening to this interview, Dr. Luyi Kathy Zhang wants you to remember that you DO have control over your own life — even if it doesn’t feel that way. You are resilient and you can start living a life in line with your hopes and values!

Dustin Steffey and Jaden Norvell sat down and shared their perspective on burnout.  Coming from humble beginnings, Dustin shares his story, and Jaden provides perspective.  Dustin obtained his Doctorate Degree in Organizational Leadership. He spent his time in his doctorate studying behaviors of people with an emphasis on the millennial generation.  Jaden is pursuing his undergraduate degree from Colorado State University.

Doctors are admired for dedicating their lives to jobs that are vital to the health of our society, which makes it easy to see them as superhumans. Even within the medical profession itself, doctors are taught in a stoic culture with militaristic training that results in lack of sleep, food, and self.


This education affects the health of doctors themselves, but also the doctor-patient relationship. Dealing with the emotions and pain of their patients is overwhelming, and doctors can easily experience burnout, causing mistakes, lessening patience, and breeding apathy.


On the show today to talk about these issues is Dr. Diana Londoño, the founder of Physician Coach Support, where doctors can get peer support over Zoom 24/7. She is also a certified life coach, and one of the few female Latinx urologists in the country.


Dr. Diana Londoño has experienced burnout twice herself, inspiring her to write and speak passionately about wellness and humanity in medicine on social media and on television. She says if her platform, Physician Coach Support, can pull just one doctor from the brink of burnout, it is all worth it.

Being a physician is not easy by any means.  We live in a fast paced world, that is just getting faster and faster. 
We are constantly bombarded with messages for work or play, and other sounds from the world around us.
As for physicians, we are like race car drivers...always going at our top speed! 
We need to learn to pump the breaks before we crash into the wall of burnout.
On this episode, I spoke with Rita Farruggia, who has experience in corporate America in marketing.  She is no stranger to a fast paced life and taking work home.  Yet, she made the transition to living a balanced life.  She is self taught, and took her passion for living a wellness lifestyle and created www.happybeingwell.com.
She goes over very simple ways to practice self care throughout the day, and how to be efficient [something we physicians love to do] while doing so.

It's no secret that becoming a medical doctor is considered one of the most revered and respected professions in the world. Then why are physician satisfaction rates so low and burnout rates so high?

This is a question our host Dr. Sapna Shah-Haque is determined to get to the bottom of and find solutions to change this “norm” across the profession. 

In today’s episode, we have invited Dr. Nii Darko, a trauma surgeon and podcast co-host alongside his wife Dr. Renee Darko called Docs Outside The Box.

After years working in a hospital setting, both Dr. Nii Darko and Dr. Renee Darko decided they wanted to take back their autonomy and decided to change the way they provide healthcare where they are the masters of their own domain and not under the thumb of the faulty system they once were a part of.

It is a road less traveled, but it has opened life up for both Dr. Nii Darko and Dr. Renee Darko, where they don’t feel the burn out and can essentially give better care to their patients because of it.

So this begs the question, if doctors took back their autonomy, would it increase satisfaction? Also, what does taking your autonomy look like and how can we achieve it?

In this episode, I sat down with a fellow classmate from medical school, Dr. Anna Esparham, MD.  She is commonly known as Dr. Anna, and is a triple board certified physician in pediatrics, integrative medicine, and acupuncture. 

Dr. Anna has always been adventurous in her daily practice of life, and in she dives into detail about life, career, and personal experience with burnout and chronic pain. 
To say the least, she has an impressive list of accomplishments, including her own spiritual journey that she has had over the last several years that has led her to find her authentic self, which has influenced her career changes/shifts.

-Medicine is time consuming.  Make time for yourself.
-When we become disconnected with our authentic selves, there are consequences.
-Dr. Anna beautifully said, "The more I lost sight of myself the more I lost connection with my spirit."  
-"I am like this combo of  play spirit and just trying to bring completeness and healing with whomever I become connected with and when I am limited in that capacity, I feel I can’t bring out my true gifts."  Don't limit yourself....you limit your potential!
-Trust yourself!  You know.  This is when the healing truly happens.

Boundaries...a Necessary "Evil"13 Sep 202200:06:41

We have boundaries, physical, mental, emotional, and so the same can carry over into medicine. It really is okay to say no. During my fourth year medical school rotation, I actually rotated in outpatient and addiction medicine. And one thing that really helped me was not only just sitting in the meeting and listening to what patients had to say, which also gave me an insight on, you know, what everybody does have struggles, including physicians, since we are human, but drawing boundaries and saying no is actually pertinent to self preservation.

-The word 'no' is not evil, nor are boundaries...they are perceived as such when the receiving party does not get what they want.
 
-You can be a team player and say 'no' for self preservation as it is part of self care

-Practice saying 'no' in the mirror...it just might become a habit!

I sat down with Dr. Varnado, MD, commonly known as Dr. V to her patients. She's an OB/GYN turned lifestyle medicine physician. After feeling burned out, losing her spark, asking for help, she changed her own narrative. She took her skills from bedside/OR to coaching, connecting with patients to get to the root of the problem as 80% of disease is caused by lifestyle, and her own podcast, Office Visits with Dr. V!

 As Dr. V said, “I started to lose my spark.”  She was tired, but of course she was tired from delivering babies….but we also know the following:

  • It is hard [medicine], we’re used to medicine being hard
  • Once we empower ourselves to do something, we’ll be better.
  • Advocate for yourself.
  • The government and admin will not change things.
  • We are literally dealing with life and death. Take care of yourself!
  • Your spirit knows!
  • If you don’t like where you are, change your narrative. Your family will reflect the difference.
  • We don’t have to have the life we were told we have to have. It can look like what you want it to look like.
  • You are worthy of the life you deserve it.
  • Ask for help!

Giving Yourself a Voice in the Workplace Discussing Medicine and Burnout with Dr. Warren Kane, MD31 Aug 202200:33:37

In this episode, I sat down and talked with Dr. Warren Kane, MD, a practicing psychiatrist.  He shares his experience with burnout, and was able to use his tribe [family/wife] as an external meter as to what he was doing/feeling.  He checks in with himself on a regular basis to see how he is doing, as he does not want to go back to being burned out.  

He shares his optimism and pearls of wisdom, including for physicians to remember that we as individuals have a voice!

Having values that are similar to your organization is going to go far.

If you feel your voice is being heard and that speaks to your values/ the way you want to practice medicine you’re going to feel less burnout, you’re going to feel a sense of community.

Get help. Burnout is a risk factor for anxiety and depression…and once you hit that, it may impact your clinical skills, may impact you to need medicine.

He is the founder and CEO of Kane Psychiatry PLLC and specializes in Professional Burnout, Anxiety and Depression among High Achieving Professionals & Athletes. Over the last few months, I have sat down and talked to amazing physicians, counselors, and other individuals who have been vulnerable and shared a side that may be they have not shared with many.  Burnout has been the topic, but also spirituality, grieving, how burnout can affect relationships with our loved ones, including ourselves.

I wanted to reflect on what I have learned via interviewing these amazing human beings!
Each person has a creative side. We are more than just physicians.
We are creative beings.
Some have written a book, started a podcast, started a coaching business.  And quite frankly, that is still within the realm of medicine but in a nontraditional sense.

-Know your worth.
-Practice self care.
-Boundaries are important,
-It is okay to say no.
-Be your authentic self.
-Get help.  You are worth it.

How do physicians grieve?  
In a healthcare system and profession where we encounter a barrage of patients not all with good outcomes, information, loss of self, identity, and many other things, how do we as physicians grieve?  Medicine teaches us to NOT show our emotions, and many physicians keep grinding away neglecting self care, and also where we are on an emotional scale.
Losing a patient is not easy at all.  It stays with us.
Chronic diseases with poor outcomes--it stays with us and we feel for our patients.

In this episode, I sat down with Anne-Marie Zanzal, who has a history of working as a chaplain in healthcare/hospice, and a certified grief counselor to discuss grief. 
She also brings to the forefront her experience of grief when she came out as a lesbian later in life.  She currently coaches women who are coming out later in life as well [individual or group coaching].  There is so much to grief, and it does not just happen with death but also other points in life.

She also shares how she became involved with real estate investing in order to create options (financially, lifestyle, and connecting with people in a way medicine does not cultivate today).  Dr. Froe founded Money with Mission, and she is doing great things in her community as well as helping other physicians do the same.

On Season 2 Episode 2 of The Worthy Physician Podcast, I sat down with Dr. Mitul Mehta MD.  He is a board-certified ophthalmologist with fellowship training in retinal diseases.  He provides a great viewpoint on burnout and life as a physician.  As he points out, it is important to work to live, not live to work.  I appreciate his time and insight as he has always been a mentor to me.  Not only is he a great physician and person, but what you hear on this podcast is what you get—sincerity and great advice.

I sat down and graciously talked with Dr. Shalev, an Emergency Room Physician of 16 years who felt that every shift was taking a year off her life.  She was able to find a way to reclaim her life and be fully present with her family.  She started exploring the world of real estate investing and finally found a way to make recurring income.  Now her mission is to share this knowledge with fellow physicians.  On this upcoming episode of The Worthy Physician Podcast, she shares her story of burnout, how she pivoted, and where she is now, available to help other physicians.

Happy New Year [2022] from The Worthy Physician!

Going forward, burnout is real.  Self care is not selfish.  In each episode, hopefully the listener has learned something.  Mindfulness is needed to be practiced.  It can be short and simple.  Label and honor your emotions.  Happiness is found internally not via material things…it is okay to talk about mental health, and motivation can come from the simplest things.  In 2022, how are you going to fill your bucket of energy?

Golden handcuffs: a phrase that came about in the late 1970s referring to financial compensation/allurement to keep individuals of a highly compensated [amount not defined] profession within the company/profession in spite of being unhappy.  This is a phrase that has been used to describe medicine.  Yet, is do we as physicians experience golden handcuffs or a golden opportunity? 

As physicians we meet several people.  Do we have the opportunities [golden] that can make a difference? How do we spot them?  One’s perspective is influenced by multiple things, all individual.
