Ep 072 - PICU (and Entrepreneurship) with Dr. Arup Roy-Burman

This is another exciting conversation with a physician who is following a non-traditional career path!
Dr. Arup Roy-Burman
Dr. Roy-Burman is a pediatric intensivist (aka pediatric critical care specialist) and former Medical Director of the Pediatric Intensive Care Unit at UCSF Benioff Children’s Hospital. Dr. Roy-Burman is now the CEO of Elemeno Health, which he cofounded in 2016.
Dr. Roy-Burman completed his undergraduate degree at UC Berkley in 1989; completed his medical degree at UCSF in 1994; completed his residency in pediatrics at Stanford in 1997; and then returned to UCSF for a fellowship in pediatrics critical care (aka PICU fellowship), which he completed in 2000.
After his fellowship, Dr. Roy-Burman took his first attending job at the Children's Hospital of Oakland eventually crossing the Bay to fill the role of Medical Director of the PICU at UCSF Benioff Children’s Hospital in 2011, where he was also the Director of Transport, Access and Outreach. In this dual role of Medical Director of the PICU and Director of Transport, Access, and Outreach allowed Dr. Roy-Burman to interact with large swaths of the healthcare system from inpatient to outpatient and with all sub-specialities who consult in the PICU. With this experience of the inter-workings of the hospital system combined with his clinical understanding of patient care and provider pain points, he decided to co-found Elemeno Health, which received backing from famed accelerator and venture capital firm, Y Combinator. At a high-level, Elemeno Health is a SAAS (software as a service) company whose software aims to help push best-practices to front-line providers as well as capture feedback from these front-liners, thereby closing the "knowledge-practice gap."

Dr. Marty Makary
Dr. Makary is a New York Times bestselling author and Johns Hopkins general surgeon and Professor of Health Policy. His book The Price We Pay (book trailer below) takes on surprise medical bills and reveals how individuals and businesses can lower their health care costs. Described as “a must-read for every American” by Steve Forbes and a “deep dive into the real issues driving up the cost of health care” by Dr. Don Berwick, The Price We Pay is the “The Big Short” of American Medicine. Makary is a frequent medical guest on NBC and FOX News and a leading voice for physicians, writing for the Wall Street Journal and USA Today.
Dr. Makary serves as executive director of Improving Wisely, a national physician collaboration to reduce unnecessary medical care and lower health care costs. He is also the founder of Restoring Medicine, an advocacy effort to help people who can’t afford their medical bills. His current research focuses on the appropriateness of medical care, administrative waste, price and quality transparency, and the impact of health care costs on low-income populations.
Dr. Makary was the lead author of the original articles on the Surgical Checklist and later served in leadership with Atul Gawande on the World Health Organization Surgery Checklist project. Makary has published more than 250 scientific articles, including articles on payment reform, vulnerable populations, and opioid prescribing guidelines. He is also an advocate for treating medical conditions when possible with healthy foods and lifestyle medicine. Dr. Makary has been elected to the National Academy of Medicine and named one of America’s 20 most influential people in health care by Health Leaders magazine.
Lastly, his book Unaccountable was adapted for television into the hit medical series The Resident. His newest book, The Price We Pay, now available for order online and in stores on September 10, 2019, tells the stories of health care’s disruptive innovators and the new movement to restore medicine to its mission.

Dr. Miguel Escalón
Dr. Escalón (@dr_escalon) is the Program Director of the Physical Medicine & Rehabilitation Residency at Mount Sinai Ichan School of Medicine.
Dr. Escalón completed his undergraduate degree at The University of Chicago in 2005; completed his medical degree and a masters of public health at the University of Illinois at Chicago in 2009; completed his residency in Physical Medicine & Rehabilitation at Baylor College of Medicine in 2013; and finally a fellowship in Spinal Cord Injury Medicine at Mount Sinai in 2014.
In addition to Program Director, Dr. Escalón serves as the Director of Critical Care Rehabilitation. In this position, Dr. Escalón works closely with physical therapists, occupational therapists, speech therapists, and recreational therapists, as well as neuropsychologists, and other medical and surgical specialists in order to provide comprehensive care in the rehabilitation and mobilization of critically ill patients, especially those patients in the ICU.
Outside of the clinic, Dr. Escalón is involved with research into brain and spinal cord injury especially as it relates to pain, spasticity and the role of technology in recovery. In particular, he is involved in the use of robotic exoskeletons to facilitate walking after such injuries.

Dr. Raj Dasgupta
Dr. Dasgupta (www.beyondthepearls.net; rajdasgupta.com) is a sleep medicine attending at as well as an assistant program director of internal medicine at the University of Southern California Keck School of Medicine.
Dr. Dasgupta completed his undergraduate degree at the University of California, Irvine in 1997; completed his medical degree at the Medical University of Silesia in Poland in 2003; completed an Internal Medicine residency at Michigan State University in 2006; completed a fellowship in Pulmonary/Critical Care at St. Luke’s & Roosevelt Hospital of Columbia University in 2010; then completed a Sleep Medicine Fellowship at Henry Ford Hospital in Detroit, Michigan in 2011.
In case you didn’t catch it, Dr. Dasgupta is quadruple board certified in Internal Medicine, Pulmonology, Critical Care, and Sleep Medicine, and so when not practicing sleep medicine, he practices pulmonary / critical care. One of his passions, however, is resoundingly for medical education, and he has received the faculty teaching award for the last 3 years in a row at Keck. In addition, Dr. Dasgupta has taught USMLE Step 1, 2, and 3 as well as Internal Medicine Board Review prep classes around the world for the past 14 years, and he just published the 1st book in a series he authored titled “Medicine Morning Report: Beyond the Pearls” printed by Elsevier. Of note, Dr. Dasgupta has appeared on various media platforms and television shows such as the “The Doctors”, “The Wellness Hour”, “ESPN” ,“Larry King Now” and “You Can Do Better”. Dr. Ciro Ramos-Estebanez
Dr. Ramos-Estebanez (crecns.com) is a Professor in the Neurocritical Care & Stroke divisions at Case Western Reserve University School of Medicine.
Dr. Ramos-Estebanez has completed extensive clinical training including an Internal Medicine residency at Albert Einstein, a Neurology residency at Harvard, a Neurocritical Care fellowship at the University of Miami, and finally a Ph.D. in neuroscience at the University of Cantabria in Spain. Additionally, he will complete his MBA at Case Western’s Weatherhead School of Business in 2018.
Dr. Ramos-Estebanez is a physician-scientist who combines a clinical career with various research interests. He has published extensively in both the basic & clinical neurosciences. He is currently developing a new metabolic MRI sequence in a stroke rodent model in collaboration with Case Western’s Biomedical Engineering Dpt., as well as conducting clinical research involving neuro-ophthalmological outcomes in collaboration with his Ophthalmology & Neurosurgery colleagues. Lastly, Dr. Ramos-Estebanez is leading a multinational project on lumbar puncture in the presence of mass effect.

Dr. Michael Abrams
Dr. Abrams is a pediatric ophthalmologist currently on sabbatical to work as CEO of FreeLinc Technologies (www.freelinc.com) in Sherborn, Massachusetts.
Dr. Abrams completed his undergraduate degree at Haverford College; his medical degree at a combined program at Dartmouth and Brown Medical Schools; an ophthalmology residency at State University of New York at Buffalo; and then a fellowship in pediatric ophthalmology at Children’s Hospital of Detroit.
Over his career, Dr. Abrams has served as a General Medical Officer and Battalion Surgeon in the U.S. Army, spent 16 years in private practice as a pediatric ophthalmologist, and regularly published his research in peer review journals during this time. He is a volunteer faculty member for ORBIS international, and has a specific interest in Connected Health and in establishing distributive educational hierarchies in evolving medical systems. He has served on numerous boards, including the Children’s Eye Foundation (www.childrenseyefoundation.org), and the Utah Medical Insurance Association (www.umia.com), where he helped to guide a $150 million merger and acquisition. In 2013, Dr. Abrams took a sabbatical from clinical medicine to take the top job at FreeLinc, a research and development company focused on the adoption of Near Field Magnetic Induction (NFMI) as a new wireless communication standard to be used with their proprietary wireless body area network technology that hopes to revolutionize Connected Health.

Dr. Ranjith Ramasamy
Dr. Ramasamy (www.ranjithramasamy.com) is the Director of Male Reproductive Medicine and Surgery as well as an Assistant Professor of Urology at the University of Miami.
Dr. Ramasamy completed his undergraduate degree at Rutgers University in 2003; completed his medical degree at Rutgers University Robert Wood Johnson Medical School in 2007; completed a urology residency at New York Presbyterian Hospital – Weill Cornell Medical College in 2013; then completed a fellowship, sponsored by the NIH, in Male Reproductive Medicine and Microsurgery at Baylor College of Medicine in 2015.
Dr. Ramasamy has been integral to the advancement of the field of male reproductive medicine and surgery. He has published over a 100 peer-reviewed articles and authored several book chapters on the topic; is on the editorial board of Urology and the Journal of Andrology, as well as a member of many other journals and societies; and has presented his work at over 25 national and international conferences from the United States to Indian, a remarkable feat considering that he only just completed his formal training two years ago.
Dr. Ramasamy is also the founder of a company called Uro-Logic that created an app called Urology Flashcards that has over 10,000 users and that is available for free in iTunes and on Google Play.
Of note, Dr. Ramasamy is a former television news anchor also having made appearances on the hit ABC series NY Med as well as ABC Nightly News to talk about foods with aphrodisiac qualities.

Dr. Michael Schacht
Dr. Schacht is the Associate Program Director of the Vascular and Interventional Radiology Residency & Fellowship at Indiana University.
Dr. Schacht completed his undergraduate degree at Northwestern University in 2005; completed a dual MD/MBA degree at Indiana University in 2010; completed a diagnostic radiology residency at the University of Louisville in 2015; and then completed a vascular and interventional radiology fellowship in 2016 at Indiana University, where he remains today as faculty.
Dr. Schacht is all about medical education. In particular, he loves spreading the word about interventional radiology. In fact, he recently submitted an article to Academic Radiology titled Improving Medical Student Exposure to Interventional Radiology.
In his free time, Dr. Schacht enjoys hiking, mountain biking, skiing, and traveling.

Dr. Jeremy Bordeaux
Dr. Bordeaux is an Associate Professor of Dermatology and Fellowship Director of Mohs Micrographic and Dermatologic Surgery at Case Western Reserve University School of Medicine and University Hospitals Cleveland Medical Center.
Dr. Bordeaux completed his undergraduate degree at North Carolina State University – Raleigh in 1998; completed his medical degree and an MPH at Duke University School of Medicine in 2002; completed a dermatology residency followed by a Mohs Micrographic Surgery and Cutaneous Oncology fellowship at the University of Massachusetts by 2007 after which he joined the faculty in Cleveland.
Dr. Bordeaux is a leader in the field of Mohs surgery, especially in the arena of advanced reconstructive techniques to hide the otherwise disfiguring consequences of removing skin cancer from the face. A testament to the patients he has helped and the contributions he has made to the field, including a handful of publications that have changed the practice of dermatologic surgery, Dr. Bordeaux has been voted Best Doctor’s in America 3 out of the last 4 years. On top of these accomplishments, Dr. Bordeaux has still managed to have an average of 1 medical student shadow him per week for the last 10 years.

Dr. Keith Murray
Dr. Murray is a Clinical Assistant Professor of Emergency Medicine and Medical Director of Tactical EMS at the University of Pittsburgh Medical Center
Dr. Murray complete this undergraduate degree at the University of Nevada in 1999; completed his medical degree at the University of Chicago Pritzker School of Medicine in 2007; completed an emergency medicine residency also at the University of Chicago in 2010; and then completed a wilderness medicine fellowship in 2011.
In addition to being medical director of tactical EMS, Dr. Murray speaks regularly about the field at grand rounds and in the communities surrounding Pittsburgh. His talks have titles such as “officer down analysis and the four phases of rescue”, “Tactical Hemorrhage Control”, and “IED Recognition and Bomb Blast Injuries”. Moreover, he is the lead instructor the Tactical EMS portion for the University’s EMS fellowship, as well as the Hand to Hand Combatives course for physicians and the ALICE Active Shoot Instructor Certification course.
