GLP-1’s or Bust? Will all VSG patients need GLP-1 meds?19 Sep 202400:15:43

In this episode, we cover a wide range of topics related to diabetes, weight loss, and bariatric surgery. We kick things off by talking about the underlying cause of diabetes. Most people believe that diabetes is caused by an over consumption of carbohydrates. But that’s incorrect. In fact, studies show that the consumption of animal fat is what causes your insulin receptors to become less sensitive.

Next, we answer the question, “How long should you be on the Metabolic Reset Diet?” It’s a question we get asked daily and, as usual, the answer isn’t universal. Rather, it’s about determining what things you need to change in order to make the Metabolic Reset Diet work for you long-term. We offer practical advice on how to do this.

Next, we tackle weight regain after bariatric surgery, especially for those who’ve had a sleeve gastrectomy (VSG). Almost half of patients deal with weight regain, and it’s not just about eating too much or slacking on exercise. The conversation dives into why this happens, focusing on hormonal changes and the body's natural resistance to weight loss. We also talk about how combining surgery with medications like GLP-1s can help manage weight over the long term.

Finally, we share some tips on how to minimize hair loss after bariatric surgery. It’s a common concern for most patients. We cover practical ways to reduce hair thinning, including preventative measures like proper nutrition, key supplements, and medication to build reserves that assist in keeping as much of your hair as possible after surgery.

Motivation, Laughter and Support with Laura Coppedge 12 Sep 202400:21:19

In this episode, Laura, a previous guest on the podcast, returns to share more about her successful peer support group for those who have undergone weight loss surgery. The conversation revolves around the value of community and how it provides a sense of connection and understanding for people on their weight loss journeys. Laura's group, lovingly nicknamed "Laura's Losers," meets regularly to offer emotional support, share personal stories, and discuss tips for staying on track.

Laura emphasizes the importance of creating a welcoming, living-room-style atmosphere where participants can openly discuss their struggles, especially around topics like post-op stalls and feeling isolated. She shares her own experiences, including her recent vacation where she was able to indulge mindfully while still staying true to her health goals. This helps alleviate some of the common fears that new patients have about life after surgery.

The conversation touches on the emotional toll of weight loss, particularly the societal pressure and feelings of guilt that many people face. Laura, along with the hosts, stress the importance of self-compassion and avoiding the trap of perfectionism. They remind listeners that there is no "finish line" when it comes to health, and that setbacks are a normal part of the journey.

The episode ends with a discussion of future plans for the group, including in-person meetups and continued support, underscoring the ongoing importance of community in long-term weight loss success.

Prep & Prosper: Meal Prep Secrets with Kristen Willard06 Sep 202400:22:44

In this discussion about bariatric meal prep and bariatric meal plans, Kristen Willard shares valuable tips for both patients who have undergone bariatric surgery and patients who are using a non-surgical approach to weight loss. The main focus is simplicity and practicality in creating meal plans. Kristen suggests starting meal prep with the meal you find most difficult, like breakfast, rather than feeling overwhelmed by prepping every meal at once. This gradual approach helps build confidence.

When it comes to bariatric meal prep, Kristen advocates for a flexible, ingredient-focused method. For example, cooking chicken in a slow cooker with salsa can serve as multiple meals, like in a lettuce wrap or a salad. This ensures variety without the need for extensive prep time. Additionally, Kristen emphasizes the importance of flavor to keep meals interesting by changing spices, sauces or dressings, preventing boredom from repetitive meals.

She also discusses strategies for getting kids to eat healthy. Kristen highlights the value of involving children in meal planning and meal prep, making it a family effort. By giving them choices between healthy options, kids feel more empowered and are more likely to enjoy the meals. She encourages parents to lead by example, showing that healthy eating can be enjoyable for everyone in the household.

Finally, Kristen recommends useful kitchen gadgets like an Instant Pot and a vegetable chopper to save time in prepping ingredients, making the process more manageable. Kristen's philosophy is that meal prep doesn’t have to be complicated; focusing on simple steps can lead to sustainable habits.

In Episode 12 of the Pound of Cure Weight Loss podcast, Dr. Matthew Weiner and dietitian Zoe Schroeder tackle essential topics for anyone on a weight loss journey, from understanding “dietitian vs. nutritionist” to exploring weight loss medications like GLP-1s and discussing revisions in weight loss surgery. This episode offers guidance and practical advice on balancing these tools for sustainable weight loss.

Celebrating Independence and Patient Advocacy

Dr. Weiner and Zoe open with a celebration of their practice’s one-year anniversary. As an independent practice, they emphasize how this autonomy enables them to focus on patient-centered care, unencumbered by larger healthcare networks. Dr. Weiner highlights the commitment to advocating for patients, particularly in navigating difficult insurance processes.

“If you hate your health insurance company, then you’ve got an ally in us,” Dr. Weiner says.

Dietitian vs. Nutritionist: Know the Difference

Zoe clarifies the crucial differences between dietitians and nutritionists, especially relevant for bariatric patients seeking dietary guidance. She explains that a “dietitian” must meet strict educational and licensing requirements, while “nutritionist” is an unregulated title. This distinction matters for patients who need specialized, evidence-based care.

Dr. Weiner advises, “Think of a dietitian as the medical professional. The education and licensing make a difference.”

Weight Loss Medications: GLP-1s and Insurance Barriers

The episode dives into the history and impact of GLP-1 weight loss medications, specifically Wegovy (semaglutide) and Zepbound (tirzepatide), which help lower the body’s setpoint and reduce food noise. Dr. Weiner details the barriers many face in accessing these medications, especially due to insurance restrictions. He recommends steps to improve approval chances, such as knowing your policy and gathering supporting documentation.

“Treat this process like a legal case,” Dr. Weiner advises. “Knowing your policy and meeting all requirements can improve your odds.”

Weight Loss Surgery Revisions: Stacey’s Story

Stacey’s story of gastric sleeve revision to bypass illustrates the importance of patient-centered care. Initially, Stacey struggled with GERD and dysphagia, making eating difficult. Dr. Weiner’s surgical revision alleviated her symptoms, allowing her to enjoy a balanced diet and achieve further weight loss. Stacey’s experience demonstrates how revisions can significantly improve comfort and quality of life for certain patients.

Q&A: Berberine, Calorie Needs on GLP-1s, and Robotic Surgery

In the listener Q&A, Dr. Weiner and Zoe tackle popular questions:

  • Berberine vs. GLP-1s: Berberine offers minor benefits, but it is not a substitute for GLP-1 medications. Zoe stresses that a nutritious diet remains the priority.
  • Caloric Intake on GLP-1s: Zoe recommends that GLP-1 users focus on adequate protein intake to protect muscle mass, aiming for 60-80 grams daily.
  • Robotic vs. Laparoscopic Surgery: Dr. Weiner explains that while robotic surgery offers certain advantages, surgical outcomes depend more on the surgeon’s experience and complication rates.

Episode Takeaways

This episode provides actionable advice for managing weight loss with the right resources. Dr. Weiner and Zoe emphasize the importance of understanding treatment options, navigating insurance, and working with qualified professionals to achieve long-term success.

A Dose of Dilemma: Which Antidepressants Cause the Most Weight Gain?15 Aug 202400:26:23

Can psychiatric medications really lead to dramatic weight gain? Does your socioeconomic status really determine if you’ll end up obese? And is there anything you can do about weight loss plateaus? Join us for an eye-opening episode of the Pound of Cure Weight Loss Podcast.

In today’s episode, we touch on a few key topics related to weight gain and weight loss challenges. First off, we dive into the role of medications that can actually cause weight gain. Things like antidepressants, mood stabilizers, and even birth control can seriously impact your body’s ability to maintain or lose weight. It’s not uncommon for people to be doing everything right with their diet and exercise, but these medications still push their weight up.

We also talk about frustrating weight loss plateaus. Everyone hits them at some point, and the key is not to freak out. Reverting back to only drinking protein shakes, is not going to solve the problem. Small changes—like adjusting your veggie intake or swapping out processed foods for whole foods—can make a big difference. And remember, plateaus are totally normal, so don’t feel like you're failing.

Finally, we touch on how socioeconomic status plays a major role in weight gain. People from lower-income backgrounds often have a harder time losing weight, partly because of limited access to healthier foods. Living in areas where fresh produce is hard to come by (hello, food deserts!) makes sticking to a healthy lifestyle that much tougher.

So, it’s a mix of factors outside your control like medications, hitting plateaus, and even where you live that can affect your weight loss journey.

Mounjaro, Second Time’s the Charm07 Feb 202400:45:15

In Episode 11 of the Pound of Cure Weight Loss podcast, Dr. Weiner and co-host Zoe cover the essentials of weight loss medications, with a focus on Mounjaro, insurance coverage for weight loss meds, and the benefits of combining GLP-1 medications with bariatric surgery. This episode dives into practical advice for navigating insurance barriers and understanding how a multi-modal approach can help achieve sustainable weight loss results.

Why Are Weight Loss Meds Covered by Insurance So Limited?

Dr. Weiner opens with a discussion on the hurdles in getting weight loss meds covered by insurance. Many insurers only cover GLP-1 medications like Mounjaro for diabetes treatment, not obesity, due to the misconception that weight loss is simply a “lifestyle choice.” He explains that Mounjaro is effective because it controls food noise, helping patients manage weight long-term by working with the body’s natural hunger-regulating hormones. Dr. Weiner encourages listeners to advocate for their right to medical support in their weight loss journey, noting that GLP-1 medications are a crucial part of this approach.

The Power of Combining Mounjaro with Bariatric Surgery for Lasting Weight Loss

Dr. Weiner highlights the potential of combining Mounjaro with bariatric surgery to create lasting weight loss outcomes. Unlike traditional dieting, Mounjaro aids patients by lowering hunger and reducing cravings, making it easier to maintain healthy habits post-op. Adding GLP-1 medications like Mounjaro to a bariatric surgery regimen helps prevent weight regain and supports a stable, lower weight set point, which is critical for long-term success.

Why Focusing on the Set Point is Better than Counting Calories for Weight Loss

Dr. Weiner discusses the metabolic thermostat concept, explaining why simply cutting calories isn’t enough for durable weight loss. Instead, lowering the body’s weight set point is essential. GLP-1 medications like Mounjaro reduce food noise, helping patients naturally control cravings and manage their weight without the intense hunger that often follows calorie restriction. This supports a balanced approach to weight management, helping patients maintain a healthy relationship with food.

Navigating Food Noise and Hunger with Accountability and Support

Support systems and accountability play key roles in successful weight management. At Pound of Cure, the support groups and virtual sessions offer structure and motivation for patients, especially those managing post-op food noise. Zoe advises that a diet rich in whole foods, such as vegetables, lean proteins, and fruits, helps patients feel fuller longer, making it easier to reduce cravings. This focus on nutrient-dense foods is essential to the Pound of Cure philosophy of long-term success.

Exploring New Weight Loss Tech

In a lighter segment, Dr. Weiner examines a new weight loss technology: a vibrating capsule that suppresses appetite by stimulating the stomach. Although intriguing, he cautions that this may cause nausea rather than true appetite control. Dr. Weiner recommends sticking with effective treatments like Mounjaro and bariatric surgery for now.

Audience Q&A

Dr. Weiner and Zoe address questions on managing hunger versus heartburn post-op and how to approach “cheat meals.” They recommend reframing indulgences as “treats” rather than “cheats” to foster a positive relationship with food. This sustainable mindset aligns with the Pound of Cure’s approach to weight management.

Conclusion

Dr. Weiner and Zoe close by highlighting the importance of combining surgical, nutritional, and pharmacological support to lower the body’s metabolic set point. With comprehensive support, patients can achieve long-term weight loss, effectively manage food noise, and lead a healthier lifestyle.

Why Your Meds Cost So Much31 Jan 202400:41:36

Welcome to another insightful episode of the Pound of Cure Weight Loss podcast! In this episode, Dr. Matthew Weiner and Zoe Schroeder tackle some pressing issues for patients, including the rising cost of GLP-1 weight loss medications, and how to manage weight regain after surgery. Whether you’ve undergone bariatric surgery or are using weight loss medications, this episode delivers science-backed insights on how to navigate the challenges of post-op life and optimize your diet for long-term success.

Why Are Medications So Expensive? A Deep Dive into Drug Pricing

The episode kicks off with a crucial conversation about rising drug prices. Many listeners have raised concerns about why medications like Ozempic (Semaglutide) and Mounjaro (Tirzepatide) are so expensive. In January 2024, more than 700 medications will face price hikes, and unfortunately, GLP-1 weight loss medications are on the list. Dr. Weiner highlights the complexities of drug pricing in the U.S., from Medicare's inability to negotiate drug prices to the overwhelming variety of insurance policies that influence costs.

Dr. Weiner also touches on a pivotal moment in 2003 when the Medicare Prescription Drug Improvement and Modernization Act (MMA) was passed. This law, still in effect, bars Medicare from negotiating drug prices directly with pharmaceutical companies, which has left patients paying inflated prices for essential medications. It’s a frustrating scenario for many, but Dr. Weiner encourages listeners to stay informed and advocate for change by supporting legislation that could help lower drug prices.

The Role of GLP-1 Weight Loss Medications in Obesity Treatment

As the conversation shifts, Dr. Weiner and Zoe dive deeper into the role of GLP-1 weight loss medications like Wegovy (Semaglutide) and Zepbound (Tirzepatide) in treating obesity. These medications have revolutionized how doctors approach weight management by helping to lower the body’s set point.

The podcast emphasizes that GLP-1 medications aren’t standalone solutions. Instead, they work best when combined with nutrition and lifestyle changes. While GLP-1 drugs help to suppress food noise and ease the challenge of constant hunger, they are most effective when integrated into a comprehensive weight loss plan, like the Metabolic Reset Diet.

If you're wondering whether you’ll be on GLP-1 medications forever, the answer depends on your personal situation. For many, these drugs will be part of their long-term weight maintenance strategy, particularly for those with chronic obesity.

Managing Weight Regain After Surgery: Addressing Patient Concerns

Another key topic covered in this episode is managing weight regain after bariatric surgery. Weight regain is a common concern among patients who have undergone procedures like gastric sleeve or gastric bypass surgery. Dr. Weiner and Zoe recognize that it’s a frustrating issue, but it’s not uncommon. The hosts share real patient stories, including one about a woman who experienced weight regain after a sleeve gastrectomy. She was worried that her surgery was failing her, but Dr. Weiner reassured her that weight fluctuations are normal, especially in the months following surgery.

If you’re experiencing weight regain, the first step is to inspect your diet. Are you focusing on produce? Often, small slips in nutritional habits can lead to unwanted pounds creeping back. Dr. Weiner suggests focusing on what you can add to your diet rather than what you should restrict. This strategy can help patients avoid feeling deprived, which often leads to unsustainable eating patterns.

It’s not uncommon for patients to panic during weight stalls or minor regains. Zoe points out that this is where the support groups and nutritional counseling of

A Better Version of Me24 Jan 202400:41:18

In Episode 9 of the Pound of Cure Weight Loss podcast, Dr. Matthew Weiner and Zoe Schroeder dive into some of the most exciting developments in the field of obesity treatment. They discuss the latest innovations in GLP-1 medications, the effectiveness of weight loss surgery, and the many emerging obesity treatment options. The episode also features a touching patient success story and practical tips on mindful eating. Let’s break down the highlights of this episode and what it means for those struggling with obesity.

The Expanding Landscape of GLP-1 Medications

The episode begins with a news segment discussing a key development in the world of GLP-1 medications: Pfizer has stopped a study on a drug called Danuglipron, an oral GLP-1 agonist similar to Semaglutide (found in Ozempic and Wegovy) and Tirzepatide (found in Mounjaro and Zepbound). The trial faced a significant dropout rate—around 50%—due to severe nausea, which isn't uncommon with oral GLP-1 medications.

GLP-1 medications have been transformative in the field of obesity treatment. These drugs are designed to mimic the body’s natural hormones that control blood sugar and appetite, making it easier for individuals to lose weight and keep it off. In addition to Semaglutide and Tirzepatide, newer drugs like the upcoming Retatrutide (a triple agonist) show promise for even greater weight loss.

The excitement around GLP-1 medications isn’t just about the potential for weight loss. These drugs also offer patients options, including oral medications or weekly injections. As Dr. Weiner explains, some patients may prefer the convenience of a once-daily pill over weekly injections, while others find injections less disruptive to their daily routine. This flexibility in treatment methods allows patients to choose the approach that best suits their lifestyle and personal preferences.

Emerging Obesity Treatment Options: What’s Coming Next?

Dr. Weiner also offers a preview of the future of obesity treatment options, emphasizing that we’re on the verge of a significant transformation in the next three to five years. New medications like Retatrutide which works as a triple agonist (targeting GLP-1, GIP, and Glucagon), could push weight loss outcomes even further than what’s currently achievable with Semaglutide and Tirzepatide.

There are also many exciting developments coming from pharmaceutical companies worldwide. Dr. Weiner highlights that Novo Nordisk (makers of Wegovy and Ozempic) and Eli Lilly (makers of Mounjaro and Zepbound) are leading the charge, but they are not the only players. Companies like Pfizer, AltImmune, and Inivit Biologics are developing a range of new treatments that include both injectable and oral medications.

The introduction of more options means patients will have access to a wider range of tools tailored to their specific needs. This surge in new medications is expected to drive down prices, making these effective obesity treatments more affordable and accessible to a broader population. Dr. Weiner is optimistic that in the coming years, the variety of medications will give patients more choices, more control over their treatment, and ultimately lead to better outcomes.

Weight Loss Surgery: A Proven Tool in the Fight Against Obesity

While GLP-1 medications are making waves, weight loss surgery remains one of the most effective and long-lasting treatments for obesity. During the episode, Dr. Weiner discusses the value of bariatric surgery and how it has helped countless patients achieve sustainable weight loss.

One of the standout moments of this episode is Terri’s inspiring patient story. Terri, a bariatric surgery patient, shares her journey of losing over 110 pounds in just 14 months. She explains how, after years of struggling with her weight and health, surgery gave her the tool she needed to reclaim her life. Terri had gastric bypass surgery, which Dr

Paging Dr. Lilly17 Jan 202400:50:40

In this episode of the Pound of Cure Weight Loss podcast, Dr. Matthew Weiner and Zoe Schroeder, RD, dive into key issues surrounding weight loss, including the use of GLP-1 medications, the choice between obesity medications and bariatric surgery, and the ethical concerns surrounding pharmaceutical companies. Just to name a few!

GLP-1 Medications for Weight Loss: A Powerful Tool

Dr. Weiner opens the episode by discussing the rise of GLP-1 medications for weight loss like Zepbound and Wegovy. These medications, originally designed for diabetes, have become game-changers in obesity treatment by lowering the metabolic setpoint. Dr. Weiner and Zoe highlight the importance of understanding how each drug works, as some patients may respond better than others or require higher doses. While these medications are effective, they’re not a one-size-fits-all solution.

Ethical Concerns: Eli Lilly’s Partnership with Telehealth Companies

One of the episode’s hot topics is the ethical concern surrounding Eli Lilly’s partnership with telehealth companies. Dr. Weiner points out how these partnerships could lead to biased prescribing practices, favoring certain medications over others, or even bariatric surgery. This raises questions about whether patients are truly receiving the best care for their needs.

Obesity Medications vs Bariatric Surgery: Finding the Right Fit

Choosing between obesity medications vs bariatric surgery is a personal decision, and Dr. Weiner emphasizes that it’s important to weigh all factors. Medications like Zepbound and Wegovy require long-term use to maintain weight loss, while surgery offers a more permanent option but with risks. Deciding which path to take depends on your weight loss goals, tolerance for medications, and insurance coverage.

Hiatal Hernia Treatment Options: Surgery and Alternatives

Dr. Weiner also addresses hiatal hernia treatment options. For those experiencing severe symptoms, like acid reflux or food getting stuck, surgery might be necessary. However, many cases can be managed with lifestyle changes or medications. For patients who have had a sleeve gastrectomy and still experience reflux, Dr. Weiner suggests considering a gastric bypass as a potential solution.

Movement Sprinkles: Simple Ways to Stay Active

Zoe introduces the concept of "movement sprinkles," a practical way to stay active when you don’t have time for structured workouts. Small, intentional activities like quick walks or doing chores can add up throughout the day, keeping you moving and supporting your health.

Final Thoughts

Dr. Weiner and Zoe wrap up the episode by encouraging listeners to tailor their weight loss journeys based on their unique needs. Whether through medications, surgery, or a combination, understanding your options and making informed decisions is key to long-term success.

Tune in to Episode 8 for more insights on GLP-1 medications for weight loss, obesity medications vs bariatric surgery, and hiatal hernia treatment options, along with tips on how to stay active with movement sprinkles.

In Episode 7 of the Pound of Cure Weight Loss podcast, Dr. Matthew Weiner and co-host Zoe Schroeder, RD, dive into the latest developments in weight loss treatments, including GLP-1 medications for weight loss, the differences between gastric bypass vs. sleeve gastrectomy, and how to manage the side effects of GLP-1 weight loss drugs. This episode is packed with science-based guidance and actionable tips for your weight loss journey.

GLP-1 Medications for Weight Loss: Benefits Beyond the Scale

Dr. Weiner discusses GLP-1 medications like Wegovy (Semaglutide) and Zepbound (Tirzepatide), which are showing great success for weight loss. Beyond helping patients lose weight, these medications reduce the risk of cardiovascular diseases, such as heart attacks and strokes, by up to 25%. Additionally, research highlights the role these medications play in lowering cancer risks linked to obesity. GLP-1 medications not only suppress appetite but may have other positive effects on metabolism.

Side Effects of GLP-1 Weight Loss Drugs: What You Need to Know

While GLP-1 medications are effective, they do have side effects. Dr. Weiner and Zoe discuss nausea, vomiting, and gastrointestinal discomfort, common with medications like Wegovy and Zepbound. These side effects are due to the drug’s mechanism of slowing gastric emptying. Dr. Weiner advises starting with a low dose and eating small, well-balanced meals to minimize symptoms. More serious side effects like gallbladder issues and pancreatitis are rare but important to monitor.

Gastric Bypass vs. Sleeve Gastrectomy: Choosing the Right Bariatric Surgery

Dr. Weiner explains the key differences between gastric bypass vs. sleeve gastrectomy to help patients make informed decisions. He shares a patient story of a woman who initially tried GLP-1 medications but switched to gastric bypass due to severe side effects. Gastric bypass often results in more significant weight loss and is ideal for patients with severe acid reflux or those looking to avoid long-term medications, while sleeve gastrectomy is simpler but comes with a higher risk of weight regain and GERD.

Patient Success Story: From GLP-1 Medications to Bariatric Surgery

A patient in her 30’s transitioned from GLP-1 medications to a gastric bypass due to overwhelming side effects. Post-op, she achieved significant weight loss and resolved many issues caused by the medications. This story illustrates that some patients may prefer surgery when medication-related side effects impact their quality of life.

Nutrition Tip: High-Protein Lentil Pasta

Zoe shares a simple nutrition tip: adding lentil pasta to your diet. Lentil and chickpea pastas are high in protein and fiber, making them great options for bariatric and GLP-1 patients who need to eat smaller portions but still require nutrient-dense foods to support healing and weight loss.

Finally, Dr. Weiner and Zoe answer six questions from listeners including, whether or not Paxil causes weight gain, weight loss stalls on a Vegan diet, and supplement suggestions.

Conclusion: Finding the Right Weight Loss Solution for You

Episode 7 offers a comprehensive look at options like GLP-1 medications for weight loss and gastric bypass vs. sleeve gastrectomy. Whether managing side effects of GLP-1 weight loss drugs or considering surgery, Dr. Weiner and Zoe provide practical advice to help you make informed decisions on your weight loss journey.

The Economics of Obesity27 Dec 202300:35:45

In Episode 5 of the Pound of Cure Weight Loss Podcast, Dr. Matthew Weiner and Zoe Schroeder discuss the latest developments in weight loss treatments, including GLP-1 medications for weight loss, managing acid reflux after VSG, and practical self-pay options for weight loss drugs. Whether you’ve had bariatric surgery or are considering it, or using a GLP-1 medication for weight loss, this episode provides actionable insights for your weight loss journey.

The Power of GLP-1 Medications for Weight Loss

One of the most exciting advancements in obesity treatment is the introduction of GLP-1 medications for weight loss. Medications like Zepbound (Tirzepatide) and Wegovy (Semaglutide) help patients feel fuller longer, reducing food noise and calorie intake without deprivation. Zepbound, now FDA-approved for weight loss, has demonstrated effectiveness similar to bariatric surgery, with diabetics losing 21% and non-diabetics losing up to 26% of their body weight.

These medications are ideal for those struggling with obesity, managing diabetes, or seeking additional weight loss post-op.

Exploring Self-Pay Options for Weight Loss Drugs

Accessing GLP-1 medications can be expensive, with Zepbound costing around $550 per month, even with a coupon. Many patients face inconsistent insurance coverage, making it necessary to explore self-pay options for weight loss drugs.

Dr. Weiner suggests several ways to reduce costs:

  • Coupon Programs: Many companies offer programs that lower the cost of medications including the use of a coupon or income-driven price reduction.
  • Stretching Medications: Some patients, under professional guidance, use smaller doses or inject every 10-14 days to extend the medication's duration.
  • Canadian Pharmacies: In some cases, buying medication from Canada can be more affordable, but it's crucial to consult with a healthcare provider first.

Dr. Weiner also recommends the Pound of Cure Platinum Program, which uses creative dosing strategies to reduce costs even further, often below the price of compounded medications.

The Role of GLP-1 Medications in Post-Op Weight Maintenance

GLP-1 medications like Zepbound are also effective in preventing weight regain for post-op patients. These medications suppress hunger and slow digestion, mirroring the effects of bariatric surgery. Contrary to misconceptions, GLP-1 medications work well alongside surgery, helping patients maintain and even enhance their weight loss results.

Dealing with Acid Reflux After VSG

Acid reflux after VSG (Vertical Sleeve Gastrectomy) is a common issue that can significantly impact quality of life. VSG reduces stomach capacity, which may increase pressure on the esophagus, leading to reflux. Managing reflux post-op is essential to prevent complications like Barrett’s esophagus.

Dr. Weiner recommends several strategies:

  • Medications: Proton pump inhibitors (PPIs) such as Prilosec can help reduce stomach acid.
  • Dietary Adjustments: Avoid acidic, spicy, and fatty foods, as well as items like tea and mint that relax the stomach. Eating smaller, more frequent meals can also help.
  • Surgical Revision: In severe cases, revising the sleeve to a Roux-en-Y gastric bypass may effectively reduce reflux.

Conclusion: Combining Tools for Success

Episode 5 of the Pound of Cure Weight Loss Podcast is an insightful conversation that covers crucial aspects of weight loss treatment, from the latest GLP-1 medications to acid reflux after VSG. Dr. Weiner and Zoe provide a well-rounded perspective, emphasizing that the combination of tools—whether it’s surgery, medication, or dietary changes—offers the best chance for long-term success.

Meat Snack! 20 Dec 202300:28:48

In Episode 4 of the Pound of Cure podcast, Dr. Matthew Weiner, a bariatric surgeon and weight loss specialist, and Zoe Schroeder, a registered dietitian, dive into key topics around sustainable weight loss. They explore sleeve gastrectomy vs. gastric bypass, patient success stories of overcoming weight regain after bariatric surgery, and the growing use of GLP-1 medications for weight loss. The hosts also provide practical nutrition tips and answer listener questions related to post-op challenges, focusing on the intersection of surgery, medication, and nutrition.

Bariatric Surgery vs. GLP-1 Medications for Weight Loss

Dr. Weiner and Zoe start by discussing an article from The Atlantic that examines the benefits of sleeve gastrectomy vs. gastric bypass alongside the rising popularity of GLP-1 medications like Wegovy and Zepbound. Dr. Weiner highlights that both bariatric surgery and GLP-1 medications cause similar neurohormonal changes that regulate appetite and metabolism, aiding in weight loss. While surgery has a significant upfront cost, it often proves more affordable in the long term compared to the high monthly expenses of medications like Ozempic. He emphasizes that combining surgery with GLP-1 medications and a structured nutrition plan provides the most sustainable weight loss, especially for those who experience weight regain after bariatric surgery.

Overcoming Weight Regain After Bariatric Surgery

Dr. Weiner shares a patient success story involving a woman who initially lost 125 pounds after a sleeve gastrectomy but experienced weight regain, reaching 310 pounds. With the help of Dr. Weiner’s nutrition program and GLP-1 medications for weight loss, she returned to 250 pounds. Dr. Weiner underscores that weight loss is not linear, and setbacks such as weight regain are common but can be overcome with the right tools and medical support.

Nutrition Tip: Pre-Planning Grocery Delivery

Zoe offers a practical nutrition tip for busy individuals and vacations—pre-plan your meals and order groceries for delivery while you are still traveling. This strategy helps ensure you return home to healthy food, reducing the temptation of takeout and setting you up for a successful week of balanced eating. Both Dr. Weiner and Zoe stress the importance of maintaining a nutritious diet, whether you’ve had bariatric surgery or are managing your weight with GLP-1 medications.

Weight Regain, Cottage Cheese, and More

Dr. Weiner addresses a listener’s concern about weight regain after a sleeve gastrectomy. Explaining that while the initial weight loss is due to the hormonal shifts the surgery causes as well as the stomach size reduction, the lack of intestinal changes makes weight loss less durable. He suggests GLP-1 medications like Wegovy or Zepbound to support long-term weight management. Another listener asks about the benefits of cottage cheese, which Zoe describes as a high-protein, versatile food that can aid recovery after bariatric surgery.

The episode also covers the SADI procedure as an option for weight loss, iron supplementation after surgery, and the potential dangers of rapid, unintentional weight loss. The hosts remind listeners to monitor their health closely and seek medical advice if they notice unexpected changes.

Conclusion: Sustainable Weight Loss

Dr. Weiner and Zoe reaffirm their commitment to helping listeners achieve sustainable weight loss through a comprehensive approach that combines surgery, medication, nutrition, and personalized support. They encourage listeners to see their weight loss journey as ongoing, with setbacks serving as opportunities to learn and improve.

The First Rule of the Pound of Cure Program13 Dec 202300:38:21

In this episode of the Pound of Cure Weight Loss podcast, bariatric surgeon Dr. Matthew Weiner and registered dietitian Zoe Schroeder dive into key topics in weight loss, focusing on the role of GLP-1 medications, bariatric surgery, and addressing complications such as acid reflux after sleeve gastrectomy. They share expert insights to help patients navigate their weight loss journey, whether through surgery, medication, or tailored nutritional strategies.

The Rising Power of GLP-1 Medications

Dr. Weiner kicks off the discussion by exploring how GLP-1 medications like Wegovy have revolutionized weight loss. Initially developed for diabetes, these medications now help patients regulate appetite and manage weight effectively. However, Dr. Weiner notes that the cost of these medications can be a major barrier, with prices reaching $1,500 per month for some patients. He also warns against unregulated compounded versions of these drugs, which may offer cheaper alternatives but come with significant risks, such as contamination and ineffectiveness.

To get the most out of GLP-1 medications, Zoe emphasizes the importance of pairing them with proper nutrition. “These medications are powerful, but they work best when combined with a diet rich in fiber-dense foods like vegetables, fruits, and legumes,” she explains. This strategy helps maximize fullness and keeps hunger in check.

Bariatric Surgery and Acid Reflux

One of the most common complications after bariatric surgery is acid reflux, particularly after sleeve gastrectomy. Dr. Weiner points out that around 20-30% of patients develop some level of acid reflux or GERD (Gastroesophageal Reflux Disease) after the procedure. While medications like proton pump inhibitors can help manage mild cases, more severe instances may require a sleeve gastrectomy revision to a gastric bypass.

“A revision to gastric bypass often resolves acid reflux by changing the digestive pathway,” Dr. Weiner explains. He adds that this revision can also benefit patients who haven’t lost as much weight as expected after their sleeve surgery. “Many of my patients feel significantly better after a sleeve gastrectomy revision, with their reflux symptoms gone and their weight loss back on track,” he says.

Nutritional Support After a Sleeve Gastrectomy Revision

For patients undergoing or considering a sleeve gastrectomy revision, Zoe stresses the importance of adjusting their diet to suit their new anatomy. “Post-revision, the focus should be on small, frequent meals that are high in protein and low in fat,” she advises. Patients also need to avoid foods that can irritate the stomach, like spicy foods, caffeine, and alcohol, to support healing and ensure long-term success.

Enjoying Iced Coffee Without the Guilt

In a lighter segment, Zoe addresses a listener’s question about how to enjoy iced coffee without compromising health goals. She advises against artificial sweeteners, which can disrupt gut health and weight loss. Instead, she suggests using natural flavors like vanilla extract, cinnamon, and nutmeg. The episode offers actionable advice to help listeners make informed choices and achieve lasting success.

How to Find Weight Loss Success when You Can't Find Wegovy06 Dec 202300:30:43

In this episode of The Pound of Cure Weight Loss podcast, Dr. Matthew Weiner and Zoe Schroeder, RD discuss the key components of achieving sustainable weight loss through a combination of GLP-1 medications, weight loss surgery, and effective nutritional strategies. Dr. Weiner, a leading bariatric surgeon and non-surgical weight loss specialist, and Zoe, a registered dietitian, share their holistic approach to weight loss and weight management from their Tucson, Arizona practice.

At the heart of their philosophy is an emphasis on sustainable weight loss strategies that go beyond quick fixes. Whether it’s through weight loss surgery like sleeve gastrectomy and gastric bypass, or the use of GLP-1 medications such as Zepbound and Wegovy, or a nutrient-dense diet like the Metabolic Reset Diet, Dr. Weiner and Zoe stress the importance of nutrition and long-term support.

Patient Success Story: Weight Loss Without Wegovy

The episode features a powerful success story of Joey, a patient who achieved significant weight loss without surgery or GLP-1 medications.

Joey’s diet was terrible. He came to the practice hoping to get on GLP-1 medications. But, due to the shortages, Dr. Weiner recommended Joey start on the Metabolic Reset Diet - a sustainable weight loss strategy focused on a whole foods like fruits, vegetables, nuts, seeds, beans, and lean protein. Within a couple weeks Joey was dropping weight easily. In less than a year he had reached his goal weight without the use of GLP-1 medications or surgery.

His journey underscores how patients can lower their weight set point and maintain their results through lifestyle changes alone.

The Role of GLP-1 Medications in Sustainable Weight Loss

We also delve into the role of GLP-1 medications in promoting sustainable weight loss. With Belgium's recent ban on Ozempic for weight loss, Dr. Weiner and Zoe discuss the bias against treating obesity as a medical condition. It can leave patients feeling helpless and alone, blaming themselves for a disease that is largely outside of their control.

We also explore alternatives to Ozempic, Wegovy, Mounjaro and Zepbound with the use of compounded GLP-1 medications like Semaglutide and Tirzepatide. We weigh the pros and cons of compounded medications and discuss the importance of combining these medications with the right nutrition to maximize success.

Slow Weight Loss After Surgery and Managing GLP-1 Side Effects

Listeners are reassured that slow weight loss and weight loss stalls after weight loss surgery is normal and part of the body's natural process. Dr. Weiner highlights how consistency in your nutrition is crucial in overcoming plateaus. Zoe provides practical tips on managing side effects of GLP-1 medications, such as nausea and headaches, by focusing on clean eating and whole foods.

Optimizing Your Diet for Sustainable Weight Loss

The episode concludes with advice on building sustainable weight loss strategies through nutrition. Zoe emphasizes the importance of choosing plant-based proteins and whole foods over highly processed options, while Dr. Weiner reinforces the power of a mostly plant-based diet in long-term success, whether or not you're using GLP-1 medications or undergoing weight loss surgery.

Through their engaging discussions, Dr. Weiner and Zoe offer listeners a comprehensive guide to achieving lasting weight loss by combining GLP-1 medications, weight loss surgery, nutrition, and sustainable weight loss strategies. Their holistic approach is grounded in practical, science-backed advice to help individuals navigate their unique weight loss journeys.

Wegovy: I'm Baaaaack!08 Aug 202400:45:57

In this episode, we unpack some fascinating insights from an NBC News article that highlights why most people stop using Wegovy after two years. Dr. Weiner shares his thoughts, explaining that a lack of access due to shortages, high costs, and insurance coverage issues are significant reasons why people discontinue these medications. The Pharmacy Benefit Managers (PBMs) who released the data might also have an agenda since they are not keen on covering expensive weight loss medications. Dr. Weiner believes that many people would continue using Wegovy if they had consistent access without these barriers.

We also hear Martin's inspiring weight loss journey. Initially struggling with his weight from a young age, he tried every fad diet before opting for weight loss surgery. Martin shares his experience with the lap band surgery in 2009, which didn’t work out as planned. He eventually transitioned to a gastric bypass after his wake-up call, realizing that he wanted to lead a healthier life for his family. Through a combination of gastric bypass, GLP-1 medications like Mounjaro, and the 10-to-1 protein rule, Martin achieved a significant weight loss. His story underlines the importance of finding a personal "why" and using it as motivation to make lasting lifestyle changes.

Dr. Weiner discusses the heritability of weight loss response to gastric bypass surgery, emphasizing that genetic factors often play a significant role in the success of such procedures. He cites a study showing that the genetic relationship between first-degree relatives had a more substantial impact on weight loss success after surgery than merely living together. This highlights the potential genetic influence on how individuals respond to both surgery and weight loss medications.

We also dive into practical advice on reaching protein goals post-surgery. For those struggling with limited food volume, we recommend strategies like fortifying meals with protein powders and choosing nutrient-dense options like Greek yogurt or smoothies to meet protein targets without consuming large quantities of food. Achieving these protein goals can help speed up weight loss and break through plateaus.

Overall, this episode covers the complexities of weight loss, from understanding the barriers to medication adherence to navigating genetic influences and personalizing nutrition strategies.

Clickbait! Why do we only want to hear the bad things about the new GLP-1 medications?27 Nov 202300:29:16

In the world of weight loss, there’s no shortage of buzzwords, fad diets, and conflicting advice. But the reality is that sustainable weight loss isn’t about quick fixes or extreme measures. Instead, it requires a comprehensive understanding of how the body regulates weight and metabolism. Dr. Matthew Weiner, a weight loss surgeon and specialist, emphasizes the importance of educating yourself on all aspects of weight loss—from surgery to nutrition and the role of medications like GLP-1 agonists (e.g., Wegovy and Zepbound). Let’s dive into the science of weight loss and how understanding the bigger picture can lead to long-term success.

The Role of Education in Weight Loss

Education is one of the most vital tools for any weight loss journey. As Dr. Weiner points out, "If you get the patient as educated as possible, you're going to be so much more successful and have such a better experience." Many people are bombarded with misinformation from social media and the news, making it harder to sift through what actually works. This is why we advocate for a scientific, fact-based approach to weight loss.

Debunking Common Myths: Calories in vs. Calories Out

One of the most persistent myths in weight loss is the idea that "calories in vs. calories out" is the key to losing weight. This model suggests that if you simply eat less and move more, you’ll lose weight. However, this concept oversimplifies a complex system. The body’s metabolic rate, appetite, and energy expenditure are all interconnected. When you eat fewer calories, your metabolic rate slows down as your body tries to maintain its current weight—a concept known as the “set point theory.”

The body is designed to resist weight loss, much like it resists holding your breath for an extended period of time. The same principle applies to extreme calorie restriction; eventually, your body’s natural survival mechanisms will take over, making long-term restriction unsustainable.

The Importance of Context in Weight Loss

Not all calories are created equal, and context matters significantly in weight loss. The body responds differently to nutrient-dense foods compared to processed, calorie-dense options. Surgery and medications like GLP-1 agonists can help regulate appetite and metabolic hormones, but they must be paired with proper nutrition for effective results.

GLP-1 Medications: A Game-Changer for Appetite Control

The introduction of GLP-1 medications, such as Wegovy and Zepbound, has been a breakthrough in obesity treatment. These drugs help regulate appetite and slow down gastric emptying, making you feel full longer. However, some of the media coverage around these medications has been misleading. For example, an article from Rolling Stone highlighted cases of gastroparesis (a condition where the stomach empties more slowly) in users of these drugs. Dr. Weiner clarifies that this slowing of the stomach is actually one of the medication’s intended effects—allowing patients to feel full on smaller amounts of food.

The key to success with these medications is adjusting the dosage and pairing them with a balanced, whole-food diet. Trying to eat large, high-fat meals, such as fast food, while on GLP-1 medications can exacerbate side effects like nausea. Patients who focus on lean proteins, vegetables, and whole foods, on the other hand, tend to see the best results.

Nutrition: What Should You Be Eating?

When it comes to diet, a whole-food-based approach is crucial. Processed foods, high in sugar and unhealthy fats, can disrupt the body’s natural hunger signals, making it harder to regulate appetite. In contrast, whole foods like vegetables, fruits, lean proteins, and beans help nourish the body and support weight loss efforts. Adding more nutrient-dense foods to your diet helps crowd out less healthy options, making it easier to meet your weight loss goals without

It Pays to be Thin01 Aug 202400:46:18

In this episode, we tackle some hot topics and share inspiring stories. We start with a big piece of news: the CEO of Novo Nordisk being brought before a Senate committee. This is huge because Novo Nordisk is a major player in the GLP-1 medication world, and there's a lot of scrutiny on how these drugs are priced and covered by insurance.

We also dive into the incredible story of Leslie, who needed a combination of nutrition, exercise, GLP-1 medications, and bariatric surgery to lose an astonishing 157 pounds. Her journey is a powerful reminder that weight loss isn't a one-size-fits-all solution and often requires a multifaceted approach.

On a different note, we discuss some eye-opening research showing that obese people tend to earn less money than their non-obese counterparts. This wage gap is especially pronounced as education levels increase, which is both shocking and troubling.

We also get into the nitty-gritty of insurance coverage for GLP-1 medications. It's frustrating to see how many barriers there are for patients trying to get these potentially life-changing drugs. We offer some tips on how to navigate these challenges and advocate for better coverage.

Overall, this episode is packed with important information and real-life stories that highlight the complexities of weight loss and the broader societal issues related to obesity.

The Great GLP-1 Compounding Dilemma26 Sep 202400:21:51

In Episode 41 of the Pound of Cure Weight Loss podcast, we explore the growing use of GLP-1 medications for weight loss, particularly focusing on compounded versions of Semaglutide and Tirzepatide. These medications, typically found in Ozempic, Wegovy, Zepbound, and Mounjaro, have become incredibly popular for their effectiveness in helping patients lose weight. With shortages of these factory-produced drugs, compounding pharmacies have emerged as a more accessible and affordable alternative for many. But what exactly does compounding mean, and is it safe?

We break down how compounding pharmacies create custom versions of medications, often offering a lower-cost option. While the appeal is strong, compounded meds come with potential risks. Quality and dosage can vary significantly, raising concerns about safety and effectiveness. We share patient stories and real-world examples of how compounded versions can differ from the original medications in potency and results, and we explain why patients should approach compounded GLP-1 meds with caution.

Beyond the legal and safety considerations, we also discuss practical tips for those taking GLP-1 medications for weight loss, such as staying hydrated. These meds not only curb hunger but also reduce the body’s natural thirst response, making it easy to become dehydrated. We offer simple strategies for maintaining hydration while on these medications, which is crucial for overall health and well-being.

Finally, we look ahead to the future of compounded medications. As shortages ease and patents become enforceable, compounded versions of GLP-1 drugs may become less common. We also touch on the possibility of generic versions hitting the market, making these life-changing medications more affordable and accessible. If you're considering compounded meds or just want to learn more about your options, this episode offers valuable insights into how to make the best choices for your weight loss journey.

Bariatric Secrets: What Really Influences Weight Loss after surgery?10 Oct 202400:18:56

In episode 43 of the Pound of Cure Weight Loss Podcast, Dr. Matthew Weiner and Zoe Schroeder, RD, explore key bariatric surgery weight loss factors, the best diet for weight loss, and how to manage malabsorption after bariatric surgery. The episode emphasizes that post-surgery success involves much more than effort alone.

Bariatric Surgery Weight Loss Factors

Dr. Weiner explains that multiple factors influence weight loss after surgery, including age, gender, and genetics. For example, younger patients lose more weight than older people do. And men, tend to lose more weight than women. Diabetes also complicates weight loss due to higher insulin levels. However, following the Metabolic Reset Diet can help reverse diabetes and promote more weight loss both before and after surgery. Factors like mobility issues and medications can also slow the process.

Best Diet for Weight Loss

The hosts highlight that the best diet for weight loss focuses on nutrient-dense foods like fruits, vegetables, beans, lean proteins, and nuts. Dr. Weiner stresses that it's not about eating less but rather, eating differently. Highly processed foods pack in “bad calories” that hinder your metabolism, while whole foods support weight loss and overall health. The goal is to shift your calorie intake toward nutrient-dense foods and away from processed junk.

Malabsorption After Bariatric Surgery

Dr. Weiner also addresses concerns about malabsorption after bariatric surgery. Particularly after procedures like a gastric bypass, where certain nutrients like iron, calcium, and vitamin B12 may not be absorbed efficiently. Lifelong supplements and regular blood work are crucial to manage these deficiencies.

Long-Term Success: More Than Just Surgery

While surgery can significantly lower your body’s setpoint, long-term success depends on lifestyle changes. Dr. Weiner and Zoe recommend combining surgery with nutrition and movement in order to produce the best, most durable weight loss.

When those aren’t enough, Dr. Weiner will also combine surgery with GLP-1 medications to reduce hunger and make it easier to maintain a healthy diet. These medications lower the setpoint and help manage "food noise," but they need to be taken consistently to prevent weight regain.

Conclusion

In this episode, Dr. Weiner and Zoe emphasize that bariatric surgery weight loss factors include things beyond effort, like age and the type of surgery chosen. The best diet for weight loss focuses on nutrient-dense foods, and malabsorption after bariatric surgery is manageable with proper care.

Emotional Wellness: Lora’s Guide to Bariatric Mental Health03 Oct 202400:29:03

On today’s episode we explore one of the most important yet often overlooked aspects of bariatric surgery—the emotional journey. While many focus on the physical transformation, it’s the emotional and mental changes that often determine long-term success. Bariatric surgery isn’t just about losing weight; it’s about reshaping your relationship with food and developing healthier ways to manage emotions.

The Emotional Impact of Bariatric Surgery

We’ll start by diving into the emotional impact of bariatric surgery. Many people use food as a way to cope with stress, sadness, or boredom. But after surgery, that emotional crutch isn’t available anymore. This shift can lead to feelings of frustration, anxiety, or even what’s known as "bariatric grief." In this episode, we discuss what it’s like to let go of old habits and how to find new ways to handle difficult emotions.

Emotional Eating After Bariatric Surgery

One of the biggest challenges many people face post-surgery is emotional eating. Although your stomach is smaller and physically can’t handle the same amount of food, the emotional triggers that lead to overeating don’t just disappear. Emotional eating can show up in new ways, like making bad food choices. We’ll talk about how to spot these behaviors early and provide practical tips, such as keeping a food journal and working with a therapist, to address emotional eating head-on.

Mental Health Support is Crucial

In this episode, we also cover why mental health support is so crucial after bariatric surgery. Many people go through a psychological evaluation before surgery, but the real emotional work often needs to happen afterward. We’ll explore how the adjustment to a new lifestyle can bring up anxiety, fear of weight regain, or feelings of frustration, and how important it is to develop healthy coping strategies. From practicing mindfulness and talking to a therapist to joining support groups, we discuss several ways to manage emotional ups and downs.

Community is Key

A key theme of today’s discussion is the importance of community. Having a strong support system can make all the difference in your post-surgery journey. Whether it’s friends, family, or a support group, feeling understood and supported plays a huge role in staying on track emotionally. We’ll share how connecting with others who are going through similar experiences can help you feel less alone and offer practical advice on building your own support network.

Emotional Ramifications of Weight Regain

Lastly, we’ll touch on the reality of weight regain and why it’s important not to view it as a failure. Weight can fluctuate, but it’s part of the process. By focusing on your emotional well-being and continuing to build healthy habits, you’ll be better equipped to handle any bumps in the road without turning back to old, unhealthy patterns.

So, whether you’re thinking about bariatric surgery, in the early stages of recovery, or years post-op, this episode offers valuable insights into managing your emotional health and navigating the mental challenges that come with such a life-changing procedure. Tune in for a conversation that will help you embrace your new, healthier life while taking care of your emotional well-being along the way!

Gut Instincts: Can an Endoscopy Fix Weight Regain After Stopping GLP-1 meds?17 Oct 202400:20:05

Episode 44 of the Pound of Cure Weight Loss podcast offers a deep dive into cutting-edge treatments and strategies for long-term weight management, focusing on GLP-1 medications for weight loss, the emerging technology of duodenal mucosa ablation, and the issue of weight regain after GLP-1 treatment cessation. Hosted by Dr. Matthew Weiner, bariatric surgeon, and Zoe Schroeder, registered dietitian, this episode explores innovative methods to combat weight regain while delivering practical nutritional advice for sustained success.

The episode begins with a light conversation about nutrition, as Zoe highlights how bananas are often unfairly demonized. Dr. Weiner suggests a simple hydration solution—water and a banana—as a healthier alternative to processed electrolyte supplements, setting the tone for the podcast’s emphasis on whole, unprocessed foods.

Weight Regain After GLP-1 Medications

The focus then shifts to the weight regain after GLP-1 medications—a challenge faced by many patients after discontinuing these treatments. Dr. Weiner introduces Revita, a new device that uses duodenal mucosa ablation to help maintain weight loss following the cessation of GLP-1 drugs. Revita recently received FDA breakthrough device designation for its ability to treat patients who experience weight regain after stopping GLP-1 medications for weight loss. Dr. Weiner discusses how this endoscopic device works by ablating (burning) the mucosa in the duodenum, a section of the small intestine, potentially mimicking the effects of gastric bypass surgery. This approach aims to create a bypass-like effect without traditional surgery, helping to address the critical issue of weight regain after GLP-1 medication cessation.

Duodenal Mucosa Ablation - Skepticism and Safety Concerns

While Dr. Weiner expresses some skepticism about the theory behind the duodenal mucosa ablation, he acknowledges the potential of the Revita device, particularly given the high economic burden and ongoing shortages of GLP-1 medications for weight loss. He notes that while the data is promising, safety concerns—such as the risk of perforation and chronic inflammation—must be addressed before widespread adoption of this technology.

In addition to discussing duodenal mucosa ablation and the challenges of weight regain after GLP-1, Dr. Weiner and Zoe offer practical nutritional guidance. They emphasize the importance of long-term dietary changes, including incorporating more whole foods, fruits, and vegetables, especially for patients on GLP-1 medications for weight loss or recovering from weight loss surgery. The podcast underscores that successful weight management requires sustainable habits, from meal planning to making healthier choices amidst busy schedules.

As they wrap up the episode, Dr. Weiner and Zoe reflect on the future of obesity treatment, acknowledging that a combination of GLP-1 medications for weight loss, non-surgical techniques like GLP-1 medications and the duodenal mucosa ablation, and proper nutrition will continue to evolve. While these treatments are not magic solutions, they hold significant promise in tackling the persistent challenge of weight regain after GLP-1 cessation.

Episode 44 provides a valuable perspective on the latest developments in weight management, particularly for those using GLP-1 medications for weight loss or considering alternatives like duodenal mucosa ablation. Dr. Weiner and Zoe's thoughtful discussion offers essential insights for patients navigating the complexities of weight loss surgery, medication, and nutritional strategies for long-term success.

A Cut Above - Surgery Outperforms GLP-1 Meds for Durable Weight Loss02 Jan 202500:28:09

Welcome to episode 58 of the Pound of Cure Weight Loss Podcast, where Dr. Matthew Weiner and Zoe Schroeder dive into cutting-edge obesity treatments like GLP-1 medications (like Ozempic and Mounjaro) and their role alongside bariatric surgery and nutrition. This episode delivers insights into new research, nutrition strategies, and the future of affordable Semaglutide production.

GLP-1 Medications vs. Bariatric Surgery: A Data-Driven Comparison

A recent study presented at the American Society for Metabolic and Bariatric Surgery examined 40,000 participants’ weight loss outcomes across three approaches: lifestyle changes, GLP-1 medications, and bariatric surgery.

  • Lifestyle changes led to 7% weight loss but most of the weight returned within four years.
  • GLP-1 medications like Wegovy achieved 15-22% weight loss, though over 50% was regained upon stopping treatment.
  • Bariatric surgery offered the most durable results, with gastric bypass patients maintaining 25% total weight loss after 10 years.

Dr. Weiner emphasized that combining these tools often yields the best results, rather than choosing one in isolation. “When used strategically, they complement each other to lower your metabolic setpoint effectively,” he explained.

Addition vs. Restriction: A New Approach to Dieting

Most diets focus on restriction, which fails over time. Instead, Zoe advocates for an addition mindset, emphasizing nutrient-dense foods like vegetables, lean proteins, and healthy fats.

“When you focus on what you can add rather than what you must cut out, it naturally crowds out less healthy choices and fosters a sustainable, positive mindset,” she explained.

Semaglutide’s Global Impact and the Rise of Generics

Dr. Weiner highlighted a groundbreaking development in the global production of Semaglutide. Due to a favorable ruling in India, generic versions could be available by 2026, potentially lowering costs significantly.

“This could be a game-changer for millions who currently can’t access Semaglutide,” Dr. Weiner noted, though challenges like cold-chain transport and international legality remain.

Nutrition Strategies for GLP-1 Success

For those using GLP-1 medications, Zoe shares essential dietary tips to optimize results and minimize side effects:

  • Prioritize vegetables, fruits, lean proteins, nuts, seeds, and legumes.
  • Avoid greasy and processed foods that can worsen nausea or digestive issues.

“This unprocessed, nutrient-dense diet mirrors the advice we give post-bariatric surgery patients,” Dr. Weiner added.

Key Takeaways

1.      GLP-1 Medications: Transformative but most effective when paired with proper nutrition and, in some cases, surgery.

2.      Restriction Diets Are Outdated: Adopt an addition mindset to foster long-term, sustainable success.

3.      Bariatric Surgery’s Durability: Surgery offers unmatched long-term weight loss results, especially when integrated with other tools.

4.      Affordable Semaglutide Is Coming: Generic production in India could reshape global access by 2026.

Final Thoughts

Episode 58 reveals the synergistic power of combining GLP-1 medications, bariatric surgery, and the right nutrition strategies. If you’re ready to take control of your metabolic setpoint, this episode provides the tools and insights you need.

Novo Nordisk’s Weekend at Bernie’s07 Nov 202400:47:48

In Episode 49 of the Pound of Cure Weight Loss Podcast, Dr. Weiner and Zoe examine the steep costs of obesity medications like Wegovy, the influence of pharmacy benefit managers (PBMs), and the role of big pharmaceutical companies in shaping healthcare. Highlighting recent Senate hearings led by Bernie Sanders with Novo Nordisk’s CEO Lars Fruergaard Jørgensen, this episode sheds light on how the current system prioritizes profit, often at the expense of patients’ access to care.

Wegovy Cost: Breaking Down High Prices in the U.S.

Wegovy, a GLP-1 medication, costs around $1,349 per month in the U.S., while in other countries like the U.K., prices are as low as $92. Dr. Weiner explains that this discrepancy illustrates a broken healthcare model, where the U.S. market fuels corporate profit but leaves millions unable to afford essential treatments. With 72% of Novo Nordisk’s revenue coming from U.S. sales, the company’s pricing strategy relies on American consumers, who are left paying significantly more for the same medications.

Big Pharma’s Influence on Policy and Profits

This episode delves into Big Pharma’s control over drug pricing, particularly the stark contrast between their research and development costs and their profits. For example, while Novo Nordisk spent $21 billion on R&D, they spent twice as much—$44 billion—on stock buybacks and dividends. Dr. Weiner highlights how political influence plays a crucial role, with both major parties receiving substantial donations from pharmaceutical companies, leading to stalled efforts in drug price regulation and patient access.

Pharmacy Benefit Managers (PBMs): Hidden Gatekeepers Driving Up Costs

PBMs, the intermediaries between insurers and pharmacies, are supposed to negotiate better prices, but Dr. Weiner and Zoe point out that they often drive costs higher through hidden fees. In many cases, PBMs have financial incentives to push higher-priced drugs, creating a “pay-to-play” system that limits patient access. For example, PBMs profit by taking a percentage of negotiated drug price savings, meaning higher drug list prices generate greater profits for them. Dr. Weiner underscores how PBMs complicate and inflate pricing, making it even harder for patients to get GLP-1 medications through insurance.

Do We Need Health Insurance Companies and PBMs?

Dr. Weiner questions the necessity of insurance companies and PBMs in the healthcare system, arguing that they don’t provide direct patient care yet extract billions from the system. He suggests that healthcare can function without these financial middlemen, allowing for a more transparent, patient-centered system where costs are lower and patients have better access to needed treatments.

The Morality of Obesity Treatment Costs

The episode closes with Dr. Weiner and Zoe discussing the stigma surrounding obesity and the moral implications of restricting access to life-changing medications like Wegovy. They emphasize that obesity is a complex, chronic disease requiring comprehensive treatment, not simply a “lifestyle” issue. Dr. Weiner advocates for recognizing obesity treatment as essential, which could lead to a shift in how these medications are accessed and covered.

Final Takeaways

While the healthcare landscape remains challenging, Dr. Weiner believes patient advocacy, legislative changes, and innovation can improve access to these life-saving treatments. This episode offers insights into navigating the current system while emphasizing the need for systemic change to prioritize patient health over profits.

Is this the end of GLP-1 Compounded meds? Part 205 Nov 202400:39:09

In Episode 48 of the Pound of Cure Weight Loss podcast, Dr. Matthew Weiner and Zoe Schroeder explore the future of compounded GLP-1 medications, the dangers of the health halo effect, and debunk common diet myths that often lead to confusion for those trying to lose weight.

Compounded GLP-1 Medications: A Controversial Solution on the Brink of Extinction

Dr. Weiner and Zoe kick off the discussion by addressing the fate of compounded GLP-1 medications. Medications like Zepbound and Wegovy, FDA-approved for obesity, have faced shortages, leading to the rise of compounded alternatives. These non-FDA-regulated versions, sourced from China and mixed in US compounding pharmacies, have been a lifeline for patients who can't access the brand-name medications.

However, Dr. Weiner highlights that these compounded versions may soon become unavailable due to regulatory changes. Compounded GLP-1 medications can only be produced during shortages of the original drugs and with supply stabilizing, their production may soon be banned. Patients currently relying on compounded alternatives will need to explore other options, such as the Pound of Cure Weight Loss Platinum Program.

Diet Myths: Dispelling Common Misconceptions

Dr. Weiner and Zoe also tackle pervasive diet myths, particularly the belief that weight loss is simply a matter of calorie counting. This outdated “calories in, calories out” philosophy ignores the body’s hormonal responses to food. They explain that hormones like GLP-1, ghrelin and leptin play crucial roles in regulating hunger and metabolism, which is why focusing purely on calorie restriction often leads to failure.

Another myth they address is the idea that fat is the enemy. While processed and animal fats should be avoided, healthy fats from sources like avocados, nuts, and seeds are vital for long-term weight management. Additionally, Zoe clarifies that not all carbs are bad — distinguishing between refined carbs and whole-food sources like vegetables and legumes is key for sustained energy and weight loss.

The Health Halo Effect: How Marketing Tricks Consumers

The health halo effect is another topic of discussion, focusing on how food marketing can deceive consumers into making poor health choices. Labels such as “organic,” “vegan,” “low-carb” or “gluten-free” often create the illusion of health, even when the products are filled with sugar or unhealthy ingredients.

Dr. Weiner and Zoe stress the importance of avoiding these traps by focusing on real, unprocessed foods like vegetables, lean proteins, fruits, nuts, beans and seeds. This approach helps ensure that you’re making choices that genuinely support your health and weight loss efforts, rather than falling for misleading marketing.

Conclusion

In this episode, Dr. Weiner and Zoe offer valuable insights into the future of compounded GLP-1 medications, clarify persistent diet myths, and explain how the health halo effect misleads consumers. As the regulatory landscape changes and food marketing continues to evolve, staying informed and focusing on a balanced, whole-food diet is crucial to achieving long-term success in your weight loss journey.

Is this the End of Compounding?31 Oct 202400:29:24

In Episode 47 of the Pound of Cure Weight Loss Podcast, Dr. Matthew Weiner and Zoe Schroeder, RD dive into weight loss topics like compounded GLP-1 medications, ultra-processed foods and their link to heart disease, and processed meat alternatives.

Ultra-processed Foods: Are Some Worse Than Others?

A recent study published in The Lancet found that ultra-processed foods make up a shocking 73% of the U.S. food supply, with significant consequences for heart health.

Dr. Weiner and Zoe discuss how this study highlighted an 11% increased risk of heart disease for those who consumed the most ultra-processed foods. The study particularly focused on the worst offenders: processed meats and sugar-sweetened beverages. These foods were shown to have the strongest links to heart disease.

One fascinating finding from the study is that not all ultra-processed foods have the same impact. When processed meats and sugary drinks are removed from the equation, the difference in heart disease risk between people who eat a lot of ultra-processed foods and those who don’t becomes much smaller. This reinforces the idea that not all ultra-processed foods are equal when it comes to heart disease risk. Avoiding processed meats and sugary drinks can significantly lower your risk, while focusing on healthier food choices can improve your long-term health.

Nutrition and Heart Health: How to Eat Smarter

When it comes to improving heart health, Dr. Weiner and Zoe offer practical advice on what foods to choose instead of processed meats. Zoe emphasizes the importance of focusing on whole, unprocessed foods, such as:

  • Fruits and vegetables: These are packed with vitamins, minerals, and antioxidants that help support heart health.
  • Lean animal proteins: Opt for fish, chicken, and lean cuts of red meat as alternatives to fatty cuts of red meat and processed meats. These options provide high-quality protein without the unhealthy fats found in processed meats.
  • Nuts and seeds: Rich in healthy fats, fiber, and protein, these are great plant-based alternatives to meat.
  • Legumes: Beans, lentils, and chickpeas are nutrient-dense and provide a satisfying source of protein and fiber.

The key is to focus on nutrient-dense, whole foods rather than simply trying to restrict calories or follow fad diets. The Pound of Cure philosophy encourages a shift in mindset toward eating for nutrition and long-term health and weight loss rather than fast weight loss which will, ultimately, come back.

Zoe also mentions the importance of minimizing food noise—the constant mental distraction and cravings for unhealthy foods—by incorporating satisfying, high-quality meals into your daily routine. By making smarter food choices, you can naturally reduce your cravings for processed foods and improve both your heart health and overall well-being.

Replacing processed meats with nutrient-dense foods like fruits, vegetables, lean proteins, nuts, seeds, and legumes is a smart strategy for improving heart health and managing weight.

The Role of GLP-1 Medications in Weight Loss

The episode concludes with an in-depth discussion on the role of GLP-1 medications in weight loss, particularly focusing on the recent shortages and their implications. These medications, like Wegovy and Zepbound, have been highly sought after due to their ability to lower the body’s set point. However, earlier shortages led many patients to seek compounded versions of these drugs.

Dr. Weiner explains that compounded GLP-1 medications are custom-made by compounding pharmacies, which mix the active ingredients (Semaglutide or Tirzepatide) with saline, B12 or other additives.
