Dr. Justin Bhullar is the founder of High Performance Practice. He started looking at joining the best practices in his fourth year of dental school. He ended up in Canada as an associate. He then purchased several rural practices that he managed remotely. After selling he tried other practice models including startups. Today he has six practices in Canada and manages them from Florida. He thinks that mistakes are where you learn the most. Partnering with a good business coach or dentist who is good with the business aspects.
Mistakes: Be clear on you why and whether it’s a good fit for you. Discover and learn from the mistakes of others. Not every dentist wants to be a business owner or spend the time to learn it.
What are the biggest issues practice owners have besides not investing the time? There are over 300 potential issues he has found in working with practices. Common ones deal with people and systems. He is very transparent and doesn’t want to waste people’s time. He also focuses on a few smaller problems to help get to the next step in your practice. It’s about more than collecting a check.
Trying to work on your practice between patients or after work means something suffers. You have to take time to work on the business and not just in the business. Their aim is to give you more time to grow your business while doing less chairside. While working five days may be necessary, you won’t know until you try it a different way. There is no magic pill, but trying new things and tracking results will show you what can work better. When your ego gets in the way, things were often made worse. Listening and learning from people who have found success in areas where you are wanting to learn is necessary. They can help you with compacting the time by heading off mistakes as well as encouraging you as a mentor.
His newest project Dental Business Mentor is a partnership with Dr. Paul Etchison. This focuses on looking at performance indicators. By making video modules of concepts he teaches, people are able to gain the knowledge to implement the changes they need in their practice. They offer a year of access for a low price to help more people. In his own practices, he is finding issues that come up to address to add to their offerings. This method can save you money and give access to your entire team. Another benefit is that you can submit questions and if it is something that is a shared concern by others, they will create a video. The modules also give you time to pause and reflect or pace yourself.

Dylan thinks a big reason so many practice owners run into this problem is that they simply don’t know where to find the right programs. He believes that, “there are a lot of options out there for you so long as you know where to look.”
For the past five years, Dylan has been doing just that—helping people (self-employed people, small business owners, families, etc.)—with finding affordable healthcare.
According to Dylan, there are only three avenues someone can go to get insurance. None of these “avenues” are any better than the other; everyone is unique and the best approach plans on your specific situation.
1) If you’re employed, you can go through your employer. That means you have a W-2 job, work for a Fortune 500 company, etc. and can utilize the benefits of your organization.
2) Self-employed people don't get these perks. Then use the Obamacare marketplace. These plans approve everyone and cover everyone without exclusions.
The problem with these programs is often charged based on income. If you're making decent money, you'll pay more for insurance than rent or a mortgage.
3) Another option for those who are self-employed is through the private market. According to Dylan, this is a bit of a “wild west” option where a plan might cover you for 3-6 months before kicking you out and telling you to look elsewhere—it’s hard to get good, long-term coverage.
Private programs give greater rates depending on medical history. They work similarly to life insurance, where you must prove you're low-risk.
If you’re someone who isn’t in need of high-risk procedures, this could be a good option for you.

There’s a plethora of financing services to choose from, but Compassionate Finance takes the cake.
Many companies claim to offer a generous amount of financing for patients, but this is often tricky marketing. They fail to discuss what percentage of the actual diagnosed treatment is approved.
For example, there may be as much as $10,000 worth of dental care diagnosed and presented, but the patient gets approved for a mere $500.
So, these companies can boast a high approval rate—as much as 87% — but when a patient cannot afford the rest, they decline the care.
“The average lender out there today is rejecting anywhere from 40-60% of applications,” Oliver says. “It really leaves the treatment coordinator, the doctor, and the practice in a very uncomfortable and awkward position.”
Compassionate Finance is all about getting as close to 100% approval as possible. This allows you to present every patient an offer before they leave your practice without needing to give you a large down payment or large pay-in-full sums.
This makes life much easier for patients who need help to afford care. But, Compassionate Finance also strives to benefit your practice.
With the software, you don’t have to give up 10-15% of your hard-earned fee, and you net even more thanks to Compassionate’s low default rates.
Compassionate is a great tool for practices using it as their primary in-house financing tool and even as a supplement to third party financing.
Many practices can be guilty of what Dr. Glenn calls “sticky note financing.” This is when you make a note of what individual patients owe for treatment, which can lead to an AR mess on your hands.
If you don’t keep track of AR, you may find yourself yelling at your team to call all these patients, which may not lead to money in the bank. It doesn’t have to be this way! There’s an easy-button, and it’s called Abella AR.
The average practice carries about $50,000 of ninety-plus day AR—that is, AR accumulated ninety days or more after treatment. That’s a significant sum you’ve rightfully earned getting left on the table.

● This week, Dr. Glenn Vo spoke with Kayvan Mott of The Doc Sites. Although Kayvan may be a new face, the Nifty Thrifty community may be more familiar with his co-founder, Ali.
● The Doc Sites is a website design and marketing company that works for—you guessed it—doctors. Many people ask Kayvan, “Why did you get started in dentistry?”
● Kayvan attended UCLA to pursue graphic design, while his business partners Ali and Parham bring experience in development & SEO and communications, respectively.
● The team served their first client around 2007; this client happened to be a dentist. “I don’t have a brand, I don’t have clients,” he told the three. “Just do whatever you website marketing guys do and get me going.”
● From there, it’s safe to say the soon-to-be Doc Sites crew delivered. Since his humble beginnings, this dentist went from owning one practice to 13 over the span of eight years. Although they couldn’t take credit for all his success, the team definitely got the ball rolling in his favor.
● This client recommended the team to his wife, an orthodontist, who then recommended them to her brother, another practice owner, and so on… “Before we knew it, 60-70% of our clients were dentists,” Kayan says. “It came organically.”

Then, fresh out of dental school, Glenn ran into some deep water with an upset client who was offering to sue him. Kyle was able to help him overcome that situation, and they’ve been pals ever since.
● Whether you’re a new dentist—like Glenn was—or you’ve been practicing for a long time, chances are that you’re bound to run into issues surrounding disability and malpractice insurance.
● Wallace Speciality Insurance Group has evolved to serve more than 6,000 clients, and most of the day-to-day is handled by Kyle’s team. That said, he’s maintained the role of “claim’s counseling” where he takes emails, calls, etc. from clients who are under a threat from a patient but that threat has not yet “escalated.”
● Typically, in order to get sued, you’ll need to really mess up in something that Kyle deems a “black-and-white” or “obvious” situation. Otherwise, being sued is often more of a menacing threat than a reality. That said, you still need to be careful in order to prevent lawsuits!
● At the end of the day, we’re all human. It’s important to not let your emotions take the helm, and Kyle is your guy when it comes to pragmatically navigating a situation where you may be in jeopardy of a lawsuit.

This week, Dr. Glenn Vo sat down with Kevin Tallman, CEO of Mango Voice. It’s not often that Glenn gets to speak with a company’s CEO on Facebook live, so he considers their meeting to be Mango’s State of the Union Address.

Dr. Glenn recently spoke with a fellow Vo, Dr. Chris Vo, known as the “crypto dentist.” Although the two aren’t related, Glenn jokes that there are so few “Vo’s” in the area they might as well be cousins.
Chris is one of the founders of “The Crypto Dentist,” a two-man team run in tandem with his pal, Mitch. At the Crypto Dentist.
Some people may assume crypto is a “scam.” Many invested have endured a “beatdown”—including Glenn and Chris.
However, Glenn notes that many people continue to invest in stocks despite their volatility, yet they don’t write the stock market off as “BS.” He knows it’s important to remember that “these things are cyclical in nature.”
At the Crypto Dentist, Chris aims not only to fill in gaps where knowledge is lacking but to combat the rampant misinformation circling the internet. There are many sources and people to avoid sharing ill-founded advice and even spreading scams. Chris values credibility first and foremost—“I’m a dentist, I have a reputation, and I’m trying to lead my colleagues.”
Although many people may be interested but afraid to invest in crypto, Chris says now may be the best time to follow through. He illustrates the example of a mall in which everything is 80% off—would you go home and come back when everything’s at retail price? “The time to buy is when everything’s on sale,” Chris says, “Which is right now.”
Although Chris has devoted himself to dentistry.” He grew up fascinated by technology; it is surprising to learn that Chris began mining Bitcoin in 2017.
Glenn recalls the “dot-com crash,” and he notes that although the crash occurred, “that doesn’t mean dot-coms weren’t worth anything.” In fact, “what it showed was that the cream rises to the top.” That is, the strongest competitors made it through the fallout.
As a dentist, Chris often sees his colleagues struggling to figure out how to invest. The Crypto Dentist’s mission is to provide “basic educational courses” for those who are just starting to test the waters. He believes it's an effective way for dentists to build a passive income.
That’s not to say investing comes without its risks. Chris believes the best way to help offset risk involves investing in one of the “blue chips” of the crypto world—Bitcoin and Ethereum.
Like all cryptocurrencies (and stocks, for that matter), Bitcoin “goes up, then down.” However, “on a long trajectory, it’s going up.” That’s why Chris thinks Bitcoin is the first asset dentists should invest in.
Chris emphasizes that before anyone buys, they should know precisely what they’re getting into. “I’ll tell you what’s not a winning strategy: buy high and sell low without conviction. That’s what happens a lot,” Chris says.
Your security and ability to succeed are top priorities. That’s why Chris is upfront with his clients about hard lessons he’s learned from his investment journey. For example, Glenn and Chris were impacted by the recent decline of the Celsius Network, a sort of “crypto bank.”
This isn’t a criticism of crypto’s potential, but Chris points out that crypto is an experiment “the same way the internet was an experiment… 15-20 years ago.

Implementing sleep dentistry in your practice takes more than just getting the clinical skills. It's also about knowing how to get paid through medical insurance—and that's the big difference between those who do a few cases every month, to the ones who do dozens.
For this week’s episode of the Nifty Thrifty Dentists podcast, Dr. Avi Weisfogel—Founder of IAOS—joined Dr. Glenn Vo via FB Live to talk about their upcoming courses and how you can effectively implement Sleep Dentistry in your practice.
IAOS is an organization that helps dentists build businesses to successfully diagnose and treat sleep apnea. Over 200 dentists across the country have successfully deployed the IAOS model to create stand-alone sleep businesses to run alongside or in the place of their dental practice.
Getting paid through medical insurance can be difficult to navigate for any dental practice. But when it comes to sleep apnea, insurance companies make it extra difficult for practice owners to make money.
Avi knows how to navigate the behind-the-scenes process to prevent your claims from getting rejected. It’s not easy, but the dividends you’ll reap and the impact you’ll make are worth it.
Sleep dentistry can be a good way to earn passive income as well—take it from a fellow dentist Avi helped to “live the dream.” He has a practice in Manhattan but lives in Ohio; he only comes in for two Mondays each month to see five to seven patients a day—everything else is done via telemed.
What’s Avi’s story?
Avi got into sleep dentistry right out of school in 1998, he saw his first sleep patient three days into his year-long residency.
1999 when Avi was more of an amateur—all his knowledge came from attending a weekend-long course that wasn’t too different from today's courses.
The patient soon returned and told Avi that he had changed her life. From there, Avi understood the power of sleep dentistry.
Eager to explore how he could make a difference in the world of sleep, he spent countless hours trying to implement OSA treatment within his practices using traditionally-taught methods. But he failed to make it a meaningful part of his practice.
When Avi had his practice and advertised for sleep apnea treatment, he got maybe one or two to come in. Even then, he’d have to send those patients to a physician.
Thus, early on, Avi learned that implementing sleep apnea treatment into a dental practice can be a headache unless you know how to control the whole process.
After over 10 years of trial and error, Avi acquired Healthy Heart Sleep, a Durable Medical Equipment (DME) company with no relationship to his dental practice. Through this company, he worked with doctors from across the US to help them establish diagnostic programs within their medical practices and direct the recommended treatment paths for each patient.
By 2013, Avi began to help other dentists by sharing this unique method with them so they could build similarly-designed businesses. As a result, IAOS was born in 2016.
Avi is currently offering a program for practice owners interested in sleep apnea. In this weekend course, you’d get an idea of what you’re getting into, learn about what’s possible, and find out whether it makes sense for you to put in the effort to pursue the full program.

This week, Dr. Glenn Vo was super excited to have the Founder and CEO of OnDiem, Joe Fogg, on the podcast. Glenn has been trying to get Joe to speak on the Nifty Thrifty Podcast for some time now, but Joe might just be the busiest man in dentistry.
OnDiem is a staffing agency that has modernized the temp agency. Instead of technology leveraging humans, OnDiem is all about humans leveraging technology to help their customers—customers like Glenn.
OnDiem's name comes from the services they provide. When you think of “on-demand,” you might think of television services. However, OnDiem aims to give an “on-demand” service for dental practices that are looking to hire a hygienist.
About two or three months ago, Glenn had a staffing issue—there was a lack of hygienists in his dental practice. While OnDiem didn’t have much of a presence in Texas at that point, Joe was able to lend Glenn a hand.
Those same hygienists that Joe helped Glenn to find turned out to stick around, too—they’re still working for Glenn at his dental practice.
Joe was able to help Glenn thanks to how hard he and his team have been working behind the scenes. They’ve been building a significant population to cover the entire country, launching a massive national rollout with so many initiatives.
Joe believes that humans have opportunities in “per diem” and “carpe diem.” Over 50% of hygienists are part-time, meaning they have extra days off and more opportunities to earn money. They have an opportunity to go earn some extra money and get more exposure to other dental practices, learn more, etc.
Joe and his team at OnDiem have learned that most private dental practices don’t have HR or a payroll—they outsource that to their CPA. When it comes to working with other temp agencies, it can be a headache and a hassle to go through all the paperwork.

Dental Start Up to Dental Acquisition with Dr. Kari Mann and Dr. Glenn Vo

Dr. Glenn Vo was excited to sit with a good friend for this week’s episode of the Nifty Thrifty Podcast. Dr. Kari Mann has a lot of stories to tell and advice to give as someone who has evolved from owning a start-up to completing the acquisition of another practice.
If you tune into the video version of this week’s episode, you’ll quickly notice that Kari has been doing pretty well for herself. She tuned in from Cape Coral, Florida, where she sat in her lavish home.
After graduating from dental school, before Kari moved to Florida
Kari always knew she wanted to own a practice; Kari’s father helped with the business. He would visit practices throughout Seattle—probably about 40 or 50 in all—always video chatting with Kari as he scoped out each location.
Unfortunately, Kari and her father struggled to find a practice that was either realistically priced or willing to sell to a recent graduate. Kari was also surprised to experience sexism when she came across a guy willing to sell to a new graduate but not to a “young female.”
Eventually, Kari found a practice that was considered a “Pseudo Startup.” The practice had been owned by a dentist for 40 years who—instead of selling it and getting out of it at its peak—had let the practice just drop off.
In other words, his dental practice was struggling to stay afloat. There wasn’t much cash flow thanks to a very small patient base.
Thus, Kari was left with the “bones of an office” with outdated and unreliable equipment. She had very little to go off of, building a brand new dental practice within a failing one.
Kari and her dad made do with 1,000 square feet to host four ops, a three-person front desk, a patient lobby, a staff room, a staff bath, a doctor’s office, a sterile lab, and a laundry area.
No matter, Kari and her father got to work. Her father utilized his construction background to help Kari set herself up for success.
Kari owned this Seattle practice for four and a half years. It was on track to do $1.6 million when she sold it. She worked four days a week—seeing patients about 30 hours a week—taking two-hour lunches every day so she could work on the administrative stuff.
Kari’s Seattle staffs were two full-time hygienists and two full-time restorative chairs. And while there still was room for growth, Kari decided to walk away.
Kari had had enough with the weather in Seattle. She was having a hard time during the winters especially, not to mention the living costs.
The lack of daylight was a big factor for Kari, as the sun wouldn’t come up until 9 am before dark again at 3:30 pm. She wasn’t keen on going to work in the pitch black and coming home in the pitch black every day.
If Kari could say something to anybody looking to buy a practice, it’s this: set whatever prospectus a broker gives you on a practice aside and get an accountant—especially if you’re not a numbers person.
Next, go through the numbers. Get to know your P&L, tax returns, monthly costs, cash flow analysis, malpractice, tax burden, etc.
If the numbers don’t pass the “stress test,” it might not be worth taking practice on. If you’re not the type of person to go in and flip that practice, then it’s not a good fit.
For Kari, being in a group practice or owning a thousand practices isn’t something she’d be interested in. One of the big reasons why Kari was all for owning her practice was because it meant she could do as much of the clinical dentistry as possible—she loves the process of sitting in the chair and treating patients.
