#101 COVID19 Part 2: Self Care26 Mar 202000:18:33

Resources for all areas of lifestyle from food, movement, sleep, stress management, mental health, wellbeing are available on my website http://thegpshow.com/.

I hope this is of use to you.

#74 Deprescribing guides with Dr Peter Tenni Pharmacist14 Apr 201900:54:29

Dr Peter Tenni is widely recognised as one of Australia's leading clinical pharmacy practitioners and trainers with over 40 years’ experience. He is a clinical senior lecturer at the University of Tasmania, a Director of Medscope and Director of Consultant Pharmacy Services.  He is the lead author of the deprescribing guides from the PHN Primary Health Tasmania which we are going to discuss today and will be updated on the website after May 9th.  These cover a range of principles and specific medications from opioids, benzo, PPIs, statins, aspirin, antihypertensives, antihyperglycaemics and many more.

I am very grateful for these because there is no shortage of guidelines on how to prescribe more medication but very few on how to get people safely and beneficially off medication in an evidence-based manner.

We discuss the general need and principles of deprescribing and then focus on two high-risk medications; opioids and benzodiazepines.  You can visit the website to view the current guidelines directly and these are soon to be updated.

Today we discuss his latest book "The Truth About Food: why pandas eat bamboo and people get bamboozled" which can be found on Amazon and kindle at  https://www.amazon.com/dp/B07H8N32NX?ref_=pe_3052080_276849420

He has no conflicts of interest, except all profits from the book sales go to the not-for-profit True Health Initiative.

There are few areas that get as heated as nutrition and food. In this podcast we try to build bridges and consider that there is a lot more in common to different healthy eating philosophies than there is different.  We discuss the importance of a whole-food way of life, the bias and fallacies that can and has influenced this discussion, the approaches to nutrition statistics, the industries/media/egos/ideologies that may influence the food system, the environmental impact of food, reconnecting with real whole food and much more.

A listener asked me to do an episode on "what do we do if patients refuse their blood pressure and cholesterol medication?".  We live in an interesting time where medicine is rapidly evolving for lots of reasons and often discussions around lifestyle, nutrition and supplements are met with ideology and emotional reactivity.  But we need to have a mature, calm and open dialogue about these things. We need to accept we all (myself included!) need to continue learning and the only way to do that is to be humble and curious yet sensible and look at the evidence critically.  You may agree or you may disagree with some of these points, but appreciate it for what it is.

“Follow the person who seeks the truth; run from the person who says he has found it.” – Václav Havel

I must admit though I failed to raise the Mediterranean diet here and it should be noted (as we discussed in the diabetes episode) that this is a great whole foods way of life that is easy to stick to and enjoyable, and good at reducing CVD risk.  Though what we are really discussing in this episode are diet approaches designed to reverse existing coronary artery disease.

Some resources mentioned:

- Nice chart incorporating Calcium scores and traditional CVD risk http://www.onlinejacc.org/content/72/4/434

- Risk calculator with various factors http://chd.bestsciencemedicine.com/calc2.html

Conflicts of Interest: Dr Kaplan is an ambassador for the Sydney Running Festival sponsored by Blackmores and Bioceuticals
