Episode Description: In this enlightening episode of The Affirming Minds Podcast, Shaina Siber, LCSW is joined by Dr. Kiki Fehling, a licensed clinical psychologist and expert in Dialectical Behavior Therapy (DBT). Together, they dive deep into the topic of shame, its impact on mental health, and how therapists can address it effectively in clinical practice. Dr. Fehling shares valuable insights into how shame differs from guilt, common ways it manifests in clients, and why addressing shame can be transformative in therapy, particularly for marginalized communities such as LGBTQ+ individuals.

---

Key Topics Discussed:

  • Shame vs. Guilt: Dr. Fehling explains the key differences between shame ("I am bad") and guilt ("I did something bad") and how these emotions affect clients differently. She discusses why guilt can sometimes be helpful, while shame often leads to isolation and hiding.
  • The Role of DBT in Addressing Shame: Dr. Fehling shares how Dialectical Behavior Therapy (DBT) skills, especially opposite action, can be used to help clients reduce feelings of shame. She walks listeners through how opposite action works, particularly in reducing shame-driven behaviors.
  • Shame in Marginalized Communities: LGBTQ+ individuals are often at higher risk for experiencing shame due to societal stigma and discrimination. Dr. Fehling explores how this impacts their mental health and shares strategies to address these issues in therapy.
  • Exposure Therapy and Shame: Dr. Fehling discusses how therapists can use exposure therapy techniques to address shame, creating a safe therapeutic environment that encourages clients to gradually face the things they feel ashamed about.
  • Self-Compassion as a Tool for Shame: The importance of cultivating self-compassion as an antidote to shame is highlighted, with Dr. Fehling outlining how mindfulness, common humanity, and self-kindness can help clients heal.
  • Practical Therapeutic Techniques: Throughout the episode, practical DBT skills like self-validationself-soothing, and opposite action are discussed as powerful tools to combat shame and help clients build resilience.

---

Key Takeaways:

  • Understanding Emotions as Biological Signals: Emotions, including shame, are biological signals that provide information and urges. Guilt, for example, urges repair, while shame urges hiding and isolation.
  • Creating a Shame-Free Therapeutic Space: Building trust and safety in the therapeutic relationship is essential when addressing shame. This often requires therapists to provide a non-judgmental, validating environment and help clients navigate difficult emotions.
  • Using DBT Skills for Shame: Opposite action is a cornerstone technique for reducing shame. By acting in ways that counter shame-driven urges, clients can reduce the emotional intensity over time.
  • Shame in Social and Cultural Contexts: In some cultures, shame can serve an adaptive function, but in many cases, it becomes problematic and hinders personal growth. Therapists must recognize the nuanced ways shame affects clients, particularly those from marginalized communities.

Key Takeaways:

  • Rethinking How We Talk About Racism with Youth
    • Many young people lack the language to express racism-related stress, making early conversations critical
    • Using “cultural stress” instead of “racism” can broaden the conversation and make it more inclusive
    • Clinicians should normalize race-related discussions as part of intake & ongoing therapy
  • The Beat Diagram: A Mindfulness Tool for Coping
    • Body sensations
    • Emotions
    • Action urges
    • Thoughts

How it works: Young people learn to pause, observe, and name their internal experiences before reacting, creating space for empowered coping rather than emotional reactivity.

  • The Empowered Coping Process
    • A 3-step framework for helping youth navigate racism-related stress:
      • Clarify the impact – What is happening? What emotions are coming up?
      • Identify coping resources – What skills, support, or actions are available?
      • Implement strategies – What is the best way to respond in alignment with values?
    • Why Building Cultural Pride is Essential
      • Early messaging about racial identity should center pride and self-love
      • Encouraging young people to explore books, media, and cultural experiences strengthens their sense of self
Cultural mindful moments—noticing when racial identity is felt in different spaces—can help kids gain awareness and confidence

Episode Summary: In this affirming and insightful episode, host Shaina Siber sits down with Kara Cruz, LMFT — a therapist, clinical supervisor, and women's mental health educator — to unpack the often-invisible forces that contribute to burnout: emotional labor and the mental load. Together, they explore how these burdens show up across caregiving roles, relationships, and professional life, particularly for those who are cisgender, femme-presenting, or socially perceived as women.

Kara and Shaina also explore the societal conditioning that encourages self-abandonment, people-pleasing, and perfectionism in caregiving roles, and how these patterns often show up in the therapy room — both for our clients and for us as clinicians. Listeners will gain tools to better identify and validate these experiences, along with practical interventions rooted in Internal Family Systems (IFS), values clarification, and boundary work.

This episode offers both validation and direction — a must-listen for therapists, caregivers, or anyone who’s ever felt overwhelmed by the invisible labor they carry.

---

Topics Covered:

  • The difference between emotional labor and mental load
  • Gendered and cultural expectations around caregiving
  • How perfectionism, people-pleasing, and self-abandonment contribute to burnout
  • Working with high-functioning, high-achieving clients who carry invisible emotional burdens
  • Boundary-setting as a clinical and personal skill

  • Intersectional considerations: class, gender identity, and relationship structure
  • Naming guilt and shame without pathologizing it

Maria is also an educator, course creator, and content producer, known for her down-to-earth, no bullshit style on TikTok and YouTube, where she breaks down complex mental health topics for clinicians and the public alike. When she’s not building trauma-informed systems or supervising clinical teams, Maria can be found chasing her two toddlers, tending her herbal tea garden, or crafting raw, radical content on her podcast Brain Trash that calls out injustice and reimagines what psychiatric care should be.

Episode Summary: Cults have long been a source of fascination, with high-profile cases like Jonestown, Waco, and NXIVM dominating headlines and documentaries. But beyond the sensational stories, cults and high-control groups are far more prevalent — and often much harder to identify — than most people realize.

In this episode of The Affirming Minds Podcast, host Shaina Siber is joined by Dr. Alisha Powell, PhD, LCSW, a psychotherapist specializing in racial trauma and religiously abusive organizations/cults. Together, they explore the psychological, emotional, and behavioral impacts of cult involvement, the clinical challenges survivors face in leaving, and trauma-informed therapeutic approaches that support healing, reintegration, and rebuilding identity.

Whether you’re a therapist looking to expand your toolkit or someone interested in understanding the lived experience of cult recovery, this conversation offers insights, compassion, and practical strategies.

---

Topics Covered:

  • The psychological, emotional, and behavioral dynamics of cult involvement
  • Why the term “cult” is sociological and colloquial — and why groups rarely identify themselves this way
  • The unique vulnerabilities that make people susceptible to cult recruitment
  • Long-term impacts on identity development, grief, and trust
  • Two major clinical challenges in cult recovery: trauma/attachment wounds and reintegration into society
  • Trauma-informed, evidence-based therapeutic approaches — including narrative therapy — that support survivors in reclaiming autonomy and rebuilding trust

---

About Our Guest: Dr. Alisha Powell, PhD, LCSW, is a psychotherapist specializing in mental health and couples therapy, with a focus on racial trauma and religiously abusive organizations/cults.
