Every year, the American Society of Plastic Surgeons compiles statistics on how many cosmetic surgery procedures are performed every year by its members.  On today's show, we talk about the most common procedures, and the ones that are gaining or losing popularity.  

There are dozens of different facelifts that are performed by different plastic surgeons.  Some lift only the skin, most are the two layered "SMAS" lifts, but in vogue are the "deep plane" lifts.  The question is whether the riskier deep plane lift is any better than the more popular SMAS lift.  On this show, we answer this question.  And we also discuss how lips are made bigger and more beautiful, without the "duck lips" look.  

Sometime around the 50th birthday, women will experience menopause.  It's a life changing event involving declining hormones and accelerated aging.  And along with menopause, many women experience psychological disturbances.  The Menopause Makeover is a procedure or combination of procedures designed to reverse some of the cosmetic changes the occur at menopause.  This includes drooping of the eyelids, face, and breasts, and accelerated aging of skin.  In this podcast, we discuss how plastic surgeons perform procedures to counter the cosmetic effects of menopause.  On this episode, we also discuss the TCA peel, designed to even out skin tone and reduce splotchiness.

The fractionated CO2 laser has become the standard for rejuvenation of sun damaged faces.  It can improve wrinkles, splotchy pigmentation, and aged skin.  New uses for the laser include dramatic reduction in earlobe wrinkles.  Combined with hyaluronic acid filler, earlobes can be completely restored to a youthful appearance.  Earrings can now be displayed without turning on their side.  We discuss this small, but significant advance in facial cosmetic surgery on this show.

Tattoos have never been more popular.  When you get one, you trust that the artist is "clean" and you won't get infections from the procedure.  And you trust that the injected pigments are safe.  Turns out that they are completely unregulated by the FDA and that people who get tattoos have a 27% increase in lymphomas!  And lasering those tattoos off makes things even worse.  Tune in to this show as we discuss this disturbing information.

I have had many mothers and daughters coming in together and having the same procedures.  From Botox to Fillers to breast lifts, so many 30 year olds have come in with their 60 year old mothers to have the same procedures.  We discuss the mother/daughter procedures on this show.

Most of plastic surgery is wonderful.  Procedures have been developed that help people look normal.  NORMAL.  That's the goal in both reconstructive and cosmetic procedures.  But every now and then a real wacky procedure comes around - like muscle stripping for big calves, or adding implants to make breasts larger during the same procedure where they are being made smaller (yes, I said that correctly).  Or placing the eyebrows half way up the forehead...or making lips that look like sausages.  On tonight's show, we tackle the strange and wacky and dubious, and plain stupid procedures in plastic surgery. I also talk about my appearance on the Oprah TV Show.   I have to say, this was one of the best shows I've done in my 16 years on the radio.

Tonight we discussed one of my favorite procedures - eyelid lifting surgery, called blepharoplasty surgery.  In this local anesthesia procedures, upper and lower lid skin is removed, along with a small amount of fat, to improve the appearance of the eyelids.  A great procedure that really rejuvenates the face.

We also had a special guest, Beverly Hills Dermatologist Dr. Vicki Rapaport, spoke about a new problem called "maskne" - acne under the mask that is used during the COVID19 epidemic. She also gave practical tips on how to use masks.

The decision to have cosmetic surgery is a big one.  Choose your plastic surgeon wisely, and then prepare for your consultation.  The consultation is an hour long "interview" - you interview your doctor and he/she interviews you.  You decide which procedure is best for you and whether that doctor is the right person to do the procedure.  On tonight's show, we discuss what should be discussed in a consultation.
And my special guest was Dr. Ira Jacobson, the chief of hepatology at NYU.  Dr. Jacobson performed a colonoscopy on ME and, although the procedure was a pain in the ..., all went well.  Tonight, Ira discussed how he was part of the team that cured hepatitis C.

COVID19 is a terrible scourge.  But it has allowed many people to have cosmetic surgery because so many people are telecommuting - performing their jobs from home.  And so many procedures now can be performed with just a day or two lost from work - as long as no one sees you.  And with masks, even procedures that cause bruising of the face can be hidden.  Those are some of the reasons cosmetic surgery is so popular now.  

We also discussed the chemical in hair dye and henna dye - it's not as safe as you would like.  

And tonight, my special guest was Dr. Robert Grant, Chief of Plastic Surgery at both Columbia and Cornell.  Dr. Grant discussed the management of plastic surgery patients during this epidemic.  We also discussed breast reconstruction with smooth implants.  

You're wearing a mask anyway, so why not take advantage of it.  Procedures that normally would keep you in hiding can be performed now and you'll not lose any time.  Wrinkle filler, lasers, peels and dermabrasion, lip augmentations and rhinoplasties are all procedures that cause bruising or wounding and require at least a few days in hiding.  Not any more!  On tonight's show, we discussed the procedures that are well suited for this unusual (and hopefully temporary) era.

Tonight, we completed our discussion of the most common plastic surgery procedures.  Breast augmentations, liposuction, eyelid lifts, rhinoplasties, and others are discussed tonight.

We also discussed the new botulinum toxin which will be sold by the Revance company.  A new study showed that this new "Botox-like" drug works for 6, as opposed to 4 months.  

Each year, the American Society of Plastic Surgeons, the largest plastic surgery organization in the world, asks its members the number of procedures they did last year.  The list is out, and once again, breast augmentation leads the surgical procedures, but Botox dwarfs every surgical procedure with over 7 million injections last year.  Tune in to hear the full list...and more....

Most cosmetic surgical procedures are performed solely for cosmetic benefit.  But there are two procedures that also have medical benefit.  On tonight's show, we discussed breast reductions and tummy tucks.  Breast reductions relieve back and neck pain and decrease yeast infections. Some plastic surgeons do strange things.  Like the one who is doing "drive through Botox" in his own garage.  Makes no sense - from an infectious disease, epidemiological, medical, social, or economic standpoint.  So why would he do this?  Could it be to get publicity?  Is your car an appropriate place for a medical procedure?  Is the lighting optimal?  Is it clean?  

On tonight's show we discussed this ridiculous procedure, as well as why you should be wearing a mask during the coronavirus pandemic.  We also discussed my facelift and wrinkle filler techniques.

PRP - or platelet rich plasma is one of those mythological procedures in cosmetic surgery.  Sometimes called the Vampire facial, this procedure takes your own blood, removes cells, and reinjects it into your face or scalp. It's supposed to grow hair, but a recent study showed that dirt cheap minoxidil was actually better than this multithousand dollar procedure in growing hair.  Listen to tonight's show as I discuss this and other procedures in cosmetic surgery.

Ulthera uses high intensity sound energy to rejuvenate the skin.  It does that by literally burning the tissues under the skin.  This shrinks the tissue and generates new collagen.  It can raise the brow and jowls and tighten the skin of the neck.  We discuss who is the right candidate for Ulthera.  
We also discussed the nonsurgical rhinoplasty- what it can and can not do.  

When I perform cosmetic rhinoplasties (nasal reshaping surgery), I rarely touch the septum. Most patients have a slightly deviated septum (the cartilage between the nostrils) but unless the septum is obstructing the nose or tilting it significantly to the side, it should not be operated on.  That's because the septum is the structural support of the nose and it can collapse if it is operated on.  So many septums are operated on just to get the patient insurance coverage.  That's wrong at so many levels  - medically and socioeconomically.  

Tonight, we discuss the merits of operations on the nasal septum.

A lot of people have very large foreheads with high hairlines.  This condition has been long overlooked by plastic surgeons.  On this show, we discuss how I make large foreheads smaller.  

Liposuction is the most popular cosmetic procedure amongst the 25 to 65 year old population.  268,000 people had it last year.  The procedure is very safe and very effective - as long as you are healthy and your surgeon knows what he or she is doing...  Today, we talk all about this wonderful body altering procedure.  We also discussed celiac disease and the myths of gluten with my guest from RWJBarnabus Health.  Tune in.  

Botox, Xeomin, and Dysport are the three common botulinum drugs - they paralyze muscles and reduce wrinkles.  But on average they last only 4 months.  So my patients usually come in every 4 months for Botox.  But there's a new drug on the horizon.  This drug has great data that shows it lasts 6 months.   Justlast week, the FDA began its 10 month review process.  All indications are that this new one, as yet unnamed, will be sold before the end of this year.  Tune in to hear about the next generation Botox.
