Most of plastic surgery is wonderful.  Procedures have been developed that help people look normal.  NORMAL.  That's the goal in both reconstructive and cosmetic procedures.  But every now and then a real wacky procedure comes around - like muscle stripping for big calves, or adding implants to make breasts larger during the same procedure where they are being made smaller (yes, I said that correctly).  Or placing the eyebrows half way up the forehead...or making lips that look like sausages.  On tonight's show, we tackle the strange and wacky and dubious, and plain stupid procedures in plastic surgery. COVID19 is a terrible scourge.  But it has allowed many people to have cosmetic surgery because so many people are telecommuting - performing their jobs from home.  And so many procedures now can be performed with just a day or two lost from work - as long as no one sees you.  And with masks, even procedures that cause bruising of the face can be hidden.  Those are some of the reasons cosmetic surgery is so popular now.  

We also discussed the chemical in hair dye and henna dye - it's not as safe as you would like.  

And tonight, my special guest was Dr. Robert Grant, Chief of Plastic Surgery at both Columbia and Cornell.  Dr. Grant discussed the management of plastic surgery patients during this epidemic.  We also discussed breast reconstruction with smooth implants.

You're wearing a mask anyway, so why not take advantage of it.  Procedures that normally would keep you in hiding can be performed now and you'll not lose any time.  Wrinkle filler, lasers, peels and dermabrasion, lip augmentations and rhinoplasties are all procedures that cause bruising or wounding and require at least a few days in hiding.  Not any more!  On tonight's show, we discussed the procedures that are well suited for this unusual (and hopefully temporary) era.

Some plastic surgeons do strange things.  Like the one who is doing "drive through Botox" in his own garage.  Makes no sense - from an infectious disease, epidemiological, medical, social, or economic standpoint.  So why would he do this?  Could it be to get publicity?  Is your car an appropriate place for a medical procedure?  Is the lighting optimal?  Is it clean?

Myths and truths of Platelet Rich Plasma - WOR show 6/20/202021 Jun 202000:56:11

PRP - or platelet rich plasma is one of those mythological procedures in cosmetic surgery.  Sometimes called the Vampire facial, this procedure takes your own blood, removes cells, and reinjects it into your face or scalp. It's supposed to grow hair, but a recent study showed that dirt cheap minoxidil was actually better than this multithousand dollar procedure in growing hair.  Listen to tonight's show as I discuss this and other procedures in cosmetic surgery.

Ulthera uses high intensity sound energy to rejuvenate the skin.  It does that by literally burning the tissues under the skin.  This shrinks the tissue and generates new collagen.  It can raise the brow and jowls and tighten the skin of the neck.  We discuss who is the right candidate for Ulthera.

Why I rarely operate on the nasal septum during rhinoplasty. WOR show 2/29/2001 Mar 202000:56:00

When I perform cosmetic rhinoplasties (nasal reshaping surgery), I rarely touch the septum. Most patients have a slightly deviated septum (the cartilage between the nostrils) but unless the septum is obstructing the nose or tilting it significantly to the side, it should not be operated on.  That's because the septum is the structural support of the nose and it can collapse if it is operated on.  So many septums are operated on just to get the patient insurance coverage.  That's wrong at so many levels  - medically and socioeconomically.

Botox, Xeomin, and Dysport are the three common botulinum drugs - they paralyze muscles and reduce wrinkles.  But on average they last only 4 months.  So my patients usually come in every 4 months for Botox.  But there's a new drug on the horizon.  This drug has great data that shows it lasts 6 months.   Justlast week, the FDA began its 10 month review process.  All indications are that this new one, as yet unnamed, will be sold before the end of this year.  Tune in to hear about the next generation Botox.
