James Hilovsky explains how physicians can use franchises to diversify income or transition out of clinical practice. After 25 years in the restaurant industry, from Del Taco to Pieology, he shifted to franchise consulting to help others avoid the mistakes he had witnessed others make.
He covers why franchises provide systems, support, and franchisee networks that make them safer than starting from scratch, plus specific industries that appeal to healthcare professionals: IV drip therapy, senior care, medical billing, and home-based options. He works with 600 franchises across all industries, helps with budget planning and territory selection, and offers services at no cost to clients. Typical timeframes range from 60 days for home-based franchises to a year for larger builds.

After 32 years in internal medicine practice, Dr. Anne Hirsch made a purposeful transition from clinical care to physician coaching, discovering new ways to help healthcare professionals thrive. Rather than leaving medicine due to burnout, she chose to step away at the peak of her career to pursue a different kind of healing, helping physicians overcome the struggles that threaten their professional satisfaction and personal well-being.
Dr. Hirsch brings unique insights from her dual role as a practicing internist and chair of her hospital's peer review committee, where she witnessed firsthand how communication breakdowns and behavioral issues can derail medical careers. Now certified as a physician coach, she specializes in helping doctors navigate burnout, perfectionism, imposter syndrome, and the communication challenges that often lead to peer review situations.
Dr. Hirsch shares practical strategies for achieving work-life harmony, the critical importance of communication skills in preventing patient safety issues, and how physicians can rediscover meaning in their careers. She also discusses her additional role as medical director for "Healthy Practices," showing how retired physicians can leverage their expertise in new ways while maintaining professional fulfillment.

What's New in My World and Is Coaching Worth the Expense?
Épisode 327
mardi 21 novembre 2023 • Durée 30:08
In this week's show, John provides a short update on what's going on with NewScript, and the
Nonclinical Career Academy. And he spends a minute or two sharing updates on two of his
most popular guests.
He spends the rest of the show sharing his answer to a question offered by one of his listeners:
Is coaching/consulting by a career expert to overcome very specific challenges worth the cost of the coaching?
As John explains during the episode, it depends on severral factors, including the
complexity of the problem, the specificity of the problem, and the skills and reputation of the
coach.
While there is sometimes no objective answer to a question like this, John describes how he
would approach the question. And he offers advice on how to optimize your chance of getting
the most from the relationship. He also explains why coaching and consulting appear to be
expensive, yet offer abundant value to the coachee.

How to Prepare to Sell Your Growing Healthcare Business
Épisode 326
mardi 14 novembre 2023 • Durée 27:57
In this week's show, John brings back a popular guest. She appeared twice in the past to describe the founding and management of her MedSpa business.
Recently, Dr. Lisa Jenks was approached by several private equity firms about purchasing her MedSpa. At first, she didn't consider a sale.
But as time went on, she thought about her future retirement, and the idea began to appeal to her. She started by talking with some of the firms to gather information and better understand her options. Later, she selected a firm to work with and began discussions in earnest.
After agreeing on a sale price, they began working out the details of the sale. Six months later, the necessary contracts were signed and Lisa passed ownership to the new owner.
During our conversation, Lisa explains:
- why she decided to sell her business,
- the factors that made the offer attractive to her,
- what she did to prepare for the sale, and
- how long she would remain as an employee and consultant for the new owner.

Top Insider Tips on Branding and Social Media
Épisode 325
mardi 7 novembre 2023 • Durée 38:01
In today's show, John presents his interview with Dr. Dana Corriel from Doctors on Social Media which he recorded a few years ago. The information she provides regarding branding and social media is as true today as it was when we recorded the session.
These principles will be helpful in at least two ways:
- First, they apply to promote any current or future business you may be involved with as an owner or an advisor.
- Second, the principles also apply to promoting yourself as a candidate to a clinical practice, corporation, or nonprofit, whether as an employee or a board member.

What Is the Best Way to Generate Income and Create Wealth?
Épisode 324
mardi 31 octobre 2023 • Durée 27:46
In this week's show, John answers this question: what is the best way to generate income and create wealth? It's an important question. Clinicians sometimes limit their options when addressing their dissatisfaction with the stress and burnout of working in the current healthcare system.
The most obvious choice is to find employment in a nonclinical position that is less stressful. It's a valid option that usually solves the burnout problem with a career that pays well and improves work-life balance.
But an often more life-changing answer is to pause and answer this question: how can I generate income and create generational wealth as part of my transition away from day-to-day patient care?
One of the answers John provides is to become a business owner. However, successful business ownership can be a high-stakes endeavor that takes more time than a simple career pivot and requires sizable cash investment. So, John explains how to reduce both the risk and the timetable for becoming a business owner in today's presentation.

Why Hospital Leadership Jobs Are Both Challenging and Rewarding
Épisode 323
mardi 24 octobre 2023 • Durée 30:48
In April of this year, NewScript presented its 2023 Nonclinical Career Summit. The Summit consisted of 12 LIVE lectures and Q&A over 3 evenings. The speakers were fantastic, spending 50 minutes addressing topics from preparing for career transition, to specific nontraditional careers.
My presentation covered the topic of Hospital Leadership Jobs. Today's podcast is an excerpt of the first 25 minutes of that presentation. It covers the following topics:
- my journey from full-time family physician to hospital CMO,
- why hospital leadership is a great career,
- which areas to start as medical director,
- the salary to expect and how to find salaries of others working in nonprofit hospitals,
- answer the question, does your specialty matter?, and,
- the five areas in which to develop new skills.
The episode closes with an invitation to purchase access to all twelve of the lectures presented at the 2023 Nonclinical Career Summit by going to nonclinicalcareersummit.com

Best First Resources to Use Before You Start Your Nonclinical Career Journey
Épisode 322
mardi 17 octobre 2023 • Durée 27:33
For the past 2 months, John has been posting 30-minute responses to questions for NewScript. If you're not a NewScript member, you may recall that Dr. Tom Davis and John created NewScript 2 years ago as a place for clinicians who are disillusioned and disappointed by the current healthcare working conditions to get together to support one another, learn from each other, and access all kinds of resources to help them.
Inside the NewScript membership, we provide recorded audio and video lessons, live events, and access to a dozen Mentors with expertise in nonclinical and nontraditional careers.
John started holding Q&A sessions 2 months ago. The sessions are recorded and posted for NewScript members to view at their convenience.
And he thought that the material in a recent session would be helpful to many of you, especially if you're a new listener. He hopes you enjoy it

How to Market Your Surgical and Nonsurgical Careers Like a Pro
Épisode 321
mardi 10 octobre 2023 • Durée 42:32
John invited Dr. David Geier back to the podcast for this week's interview. His first appearance was in August of 2018 in Episode 49. He was an early adopter of using social media and podcasting to help promote his practice and speaking engagements.
Dr. Geier is a local media personality with thousands of television appearances. As a podcaster, he has posted over 400 podcast episodes. And he has written a book called That's Gotta Hurt: The Injuries That Changed Sports Forever, which we discussed during his first appearance.
David is a triple board-certified orthopedist who has shifted his approach to sports injuries and chronic joint problems over the past few years. During our interview we talked about his experiences with burnout, his YouTube channel, how he uses speaking engagements to promote himself, his new medical practice, and how he uses artificial intelligence (AI) to facilitate content creation.
