In this week’s episode, Dr. Nicole Rochester returns to the podcast. It’s been 5 years since she described how she was inspired to create her health navigation consulting business. Since then she has expanded her business and is now working with healthcare organizations to provide her services. She is also teaching other physicians to create their own consulting businesses.

Dr. Nicole Rochester obtained her undergraduate degree from Johns Hopkins University and her medical degree from the University of Maryland School of Medicine. She is a Clinical Assistant Professor of Pediatrics at the George Washington University School of Medicine.

During today’s interview, Nicole describes why she began working as a coach and consultant for patients with complex medical problems who needed help making decisions and navigating their healthcare journey. She also explains how she began providing these services on behalf of large health systems and hospitals, which is a much more sustainable model. She also shares her ideas for other healthcare coaching and consulting businesses to use a similar strategy.

In this week’s podcast episode, John addresses this question: Even if I’m burned out and frustrated, are there ways to stay in clinical medicine while achieving balance, satisfaction, and enjoyment? During this presentation, he discusses the following 5 options for remaining in clinical practice.

Take another stab at addressing the circumstances leading to dissatisfaction by aggressively renegotiating your employment contract. Review your contract and identify the major issues causing your frustration. Develop a strategy for renegotiating those issues with your attorney. These typically involve vacation time and covering for others’ vacations, work hours, salary, restrictive covenants, and inadequate pay for collaborating with midlevel providers.

If you’re looking for a new job, consider engaging an agent to represent you in contract negotiations with a new employer, often at no cost to you.

Start a new practice or join one that does not bill insurance companies. Direct Primary Care and Concierge practices, and narrow-focused practices often enable the physicians to work on cash-only, fee-for-service, or membership models, and to create a better lifestyle and income.

Almost any of us could start or transition to a narrow focused practice. John’s colleague was a full-time general pediatrician who transitioned to caring solely for patients with attention deficit disorder. He uses standard protocols for evaluation and management, and is an expert at billing for these services. He reduced his staff and overhead and his satisfaction and income have improved.

Finally, for those who want to achieve many of the above advantages, and others, without starting a practice, working as a micro-corporation may be a great choice. Incorporating and working as an independent contractor for a large group or hospital system provides more tax benefits, flexible investment opportunities, and off-loading the expenses for rent and staffing.

Today’s episode is Part 2 of my interview with Dr. Tod Stillson. Tod is a family physician who discovered that he could improve his autonomy and increase his income by incorporating and switching from employee to micro-corporation owner. In Episode 374 we learned that 10 years ago he discovered he could practice under a professional services agreement with the hospital system, so he incorporated and began working as a mini-corporation.

We pick up the interview with Tod, who explains how he shifted from employment to working using a professional services agreement. Then, he discusses a variety of benefits that you can access under this new model.

Dr. Laura Kaufman is a board-certified anesthesiologist who specializes in obstetrical anesthesia. She has always had an interest in patient safety and quality improvement, participating in and leading quality committees and teams.

When her employer became part of a new organization, she decided to begin her transition from full-time practice to part-time practice and consulting while completing her master’s degree in patient safety and healthcare quality at the Johns Hopkins Bloomberg School of Public Health.

How to Go from Surviving to Thriving in a Remote Healthcare Job13 Dec 202200:33:23

Working remotely has become all the rage, with its potential advantages of adding flexibility and autonomy to our lives, avoiding office politics, and practical savings in fuel and automobile maintenance costs.

But there are some real downsides that can occur. Today’s guest is Frieda Wiley, Pharm. D., who worked as a chemist before pursuing her pharmacy doctorate.

Like many of you, she was attracted to the idea of working remotely. She started doing that part-time, but eventually went full-time. And she discovered that there were challenges to working remotely that caused negative mental and physical health consequences.

Those consequences led her to transition from remote clinical work to freelance medical writing. Then she leveraged her writing skills and her experiences to write a book called: Telecommuting Psychosis: From Surviving to Thriving While Working in Your Pajama Pants.

How to Secure a Captivating Career as an MSL29 Nov 202200:25:25

The Medical Science Liaison (MSL) in the pharmaceutical industry is a popular position for physicians who wish to leave clinical practice. This is true because it is possible to qualify for the job regardless of the extent of training (i.e., whether residency-trained or board certified, or not) or specialty; it does not require special certifications or degrees (beyond a medical degree); and it provides a moderate amount of flexibility to work part of the time from home. However, it often involves traveling to meet with clients up to 60% of the time.

Dr. Tod Stillson earned his medical degree at the Indiana University School of Medicine. He then completed a Family Medicine Residency and a one-year fellowship focusing on Obstetrics at the Roanoke Memorial Hospital in Roanoke, Virginia.

He has practiced Family Medicine in North Central Indiana for almost 30 years, initially as an employee of the local hospital. About 10 years ago he discovered he could practice under a professional services agreement with the hospital system, so he incorporated and began working as a mini-corporation. That opened up many opportunities for him resulting in more autonomy and a higher income.

He has now created several resources to help educate other physicians about incorporating and contracting with employers in this new way. How the Medical Matchmaker Provides a Solution to Burnout08 Nov 202200:29:45

Lara Hochman, MD is a Family Medicine physician and the Medical Matchmaker. She is an advocate for fellow physicians’ well-being in the face of rising burnout and dissatisfaction.   • find your first nontraditional job.

USUAL WAYS TO NETWORK

  • Face-to-face at a meeting or conference,
  • Online using Doximity and LinkedIn, and,
  • Online as a member of a Professional Organization (e.g., American Association for Physician Leadership, the MSL Society, the American Medical Writers Association, etc.).

This week, Dr. Debra Blaine returns to the show to discuss her latest book. She has appeared on the show twice before (Episode 115 and Episode 231) and returns to announce the release of Beyond the Pillars of Salt and to explain how she published it.

Dr. Debra Blaine is a family physician who has been practicing for over 30 years. She began writing fiction in earnest several years ago and released her first book, Code Blue: The Other End of the Stethoscope, in 2019.

Her second book, Undue Influences, was released less than a year ago. Since then, not only did she write her latest novel, but she self-published it too. 

Dr. Blaine explains why she loves to write novels, and how she managed to self-publish her third book so quickly after the release of her second book.

We also discover that Dr. Blaine is a certified coach who helps physicians with writing and publishing their own novels.

Why You Should Embrace the Powerful Mastermind Principle 9:20:22 9 16 AM27 Sep 202200:37:11

In this week’s solo episode John explains the power found in Mastermind groups. He defines what a mastermind is. And he recounts the history of the development of the Mastermind in 1939 which is attributed to Napoleon Hill, who wrote Think and Grow Rich.

John tells of the Mastermind sessions he had as a chief medical officer (CMO) of a community-based hospital during which members of his team would take turns taking the “hot seat” in order to help overcome a challenge or solve a problem that they were facing.

John started hosting mastermind sessions with his podcast listeners and email subscribers three years ago. And he currently has two groups going which follow his 10-minute mastermind process.

Members of his masterminds have experienced great success in selecting, pursuing, and landing a nonclinical jobs.

There are several benefits to participating in a Mastermind:

  • a source of encouragement and support,
  • accountability,
  • networking opportunities, and
  • brainstorming

And they've been shown to help entrepreneurs and business leaders advance their careers and improve the performance of their organizations.

John believes that everybody should participate in a mastermind if they are looking to accelerate the progress of their success in any endeavor.

Dr. Cherisa Sandrow is a Board Certified Family Physician. She received her medical degree from Philadelphia College of Osteopathic Medicine and practiced family medicine including obstetrics for 15 years.

Making the transition to telemedicine in 2015, she pivoted out of her busy office-based practice and created a thriving telemedicine practice, with the ability to prioritize her core values in life.

Dr. Peter Angood has led American Association for Physician Leadership (AAPL) since 2011, having previously provided senior leadership for all sizes and types of healthcare organizations.

During his career, Angood was the first chief patient safety officer and a vice president for The Joint Commission, senior adviser for patient safety to the National Quality Forum and National Priorities Partnership, and the former chief medical officer with the Patient Safety Organization of GE Healthcare.

With his academic surgery practice experience ranging from the McGill University hospital system in Canada to the University of Pennsylvania, Yale University, and Washington University in St. Louis, Angood completed his formal academic career as a full professor of surgery, anesthesia, and emergency medicine.

A fellow of the Royal College of Surgeons, the American College of Surgeons, and the American College of Critical Care Medicine, Angood has been an author in more than 200 publications and is a former president of the Society of Critical Care Medicine.

How to Be the Best Expert Witness Consultant - Interview with Dr. Gretchen Green23 Aug 202200:29:44

Dr. Gretchen Green has been a diagnostic radiologist since 2006. She completed her medical degree at Brown University Medical School. She then completed a residency in diagnostic radiology at Yale University School of Medicine and a Women’s Imaging Fellowship at Harvard Medical School.

Gretchen began expert witness work in 2015 and has served as an expert witness in diagnostic radiology for hundreds of cases for both plaintiff and defense attorneys. This enabled her to transition to part-time clinical work in 2016 to create more time for her family and greater financial freedom. She now teaches an online course devoted to becoming an expert witness called Expert Witness Startup School.

In today's interview, she updates us on what's new in expert witness consulting work. She reminded us that as board-certified physicians with a few years of practice experience, we already meet the definition of "expert" as applied to this part-time consulting job.

And she shared 2 observations that make this side business such a compelling choice today:

  1. the demand for expert witnesses is high because the pandemic caused delays and a backlog of cases, and
  2. most of the work is done remotely, including depositions and testimony (which is rarely required).

Medical Science Liaison Is Still a Fantastic Pharma Job16 Aug 202200:24:54

Today I want to revisit one of the most popular nonclinical jobs for physicians: Medical Science Liaison.

It has been quite a while since I presented a podcast on this topic.
So, I want to update you on this popular career option.

An MSL serves as an educational and feedback link between a pharmaceutical, biotechnology, or medical device company AND healthcare professionals.

This role is strictly educational and collaborative in nature. Marketing and selling by MSLs is not permitted.

MSLs leverage their scientific background to learn about their products and communicate with influencers and key opinion leaders (KOLs). They use organizational skills to connect with these stakeholders. They use communication skills to teach, advise, and inform. 

Typically, an MSL meets with these KOLs, updates them on scientific studies, reports on their conversations to the employer, and attends both remote and in-person meetings.  Depending n the size of the territory covered, travel requirements may be extensive.

This career is attractive because it does not require any special certification. It is open to physicians at all levels of training, with or without board certification. There are ample jobs and the number of positions is growing.

Once employed as an MSL, it is quite possible to make a lateral job change or advance into more senior positions.

Landing the first MSL job can be challenging. And travel away from home for multiple days can sometimes be required.

REMEMBER THAT THE FIRST JOB IS THE MOST DIFFICULT TO LAND.

How Creative Writing Restores Vitality and Prevents Burnout02 Aug 202200:37:38

Claire Unis majored in creative writing and literature at Dartmouth College, where she wrote for and published a student newspaper. By senior year she had become a regular columnist in the daily school paper.

While in medical school at UCSF, she enrolled in the MFA (master in fine arts) program at the U. of San Francisco, where she focused on writing memoirs and narrative nonfiction. She completed both degrees simultaneously.

Now a practicing pediatrician, Claire is also a communication coach for clinicians in one of the largest medical groups in northern California. She teaches classes and mentors physicians as part of Sutter Medical Group's Clinician Wellbeing Program.

She writes a weekly blog about doctoring. In her role as Literature in Medicine Champion, she also leads monthly book groups, a biweekly writing group, and biweekly discussions.

And she has written a memoir about her experiences and feelings while becoming a physician. It would be a very nice resource for someone contemplating a medical career, as well as an enjoyable read. How to Overcome Adversity and Find Success in a Nontraditional Career21 Jun 202200:35:02

Dr. Anjani Mahabashya completed her medical training in India, then emigrated to the U.S. After participating in several elective experiences, she entered the McLaren Flint Internal Medicine Residency Program in Flint, Michigan. She then began her career as a hospitalist.

Her career was suddenly interrupted as the result of a disabling injury in 2020. This jeopardized her long-term plans and she had to develop a radical new career trajectory. She obtained additional health care management training at the Yale School of Management and became certified by the American Board of Quality Assurance and Utilization Review Physicians (ABQAURP).

During our conversation, Anjani describes how she overcame the challenges she faced, why she likes working as a hospital-based utilization management physician advisor, and the one step that made all the difference in her successful transition.

Are Career Ambitions Beyond Conventional Practice Within Reach17 May 202200:32:14

In 2014, Dr. Abeyna Bubbers-Jones was an NHS Urology Specialist Trainee working in Blackburn (in northern England). Having graduated from medicine at the age of 22 at Nottingham, she worked for several years as a surgical trainee in the NHS. But she was yearning for something more.

She moved to South Africa and worked as a trauma surgeon for nearly 2 years, revitalizing her love of medicine and realizing that a new challenge had helped her to gain the real satisfaction she needed to develop as a professional and individual.

She returned to the UK where she noticed that many doctors were experiencing similar issues she had faced. They were stressed and burned out, feeling doubtful about their careers with low self-worth, and considering leaving the profession.

That led Abeyna to create Medic Footprints with her school friend Sara Sabin. It was designed as a community and network for doctors exploring alternative careers and support.

Medic Footprints now helps doctors around the world overcome burnout and learn how to apply their passion and expertise in new ways. 

In this week's interview, Dr. Bubbers-Jones tells her story and describes the satisfaction she experiences helping her colleagues.

Today I want to provide an update on the part-time work providing remote collaboration and supervision. This is also known as working as a remote medical director. One of the attractive parts of this job is that you are paid for being AVAILABLE for collaboration. So, you can generate income while doing other things to some extent.

It’s something that I have been doing for the past 2 ½ years. I've been the medical director for a new urgent care network since 2014. Four years ago I began cutting back on my clinic hours and completely withdrew 2 years ago from direct patient care. However I still serves as remote medical director, collaborating as needed with APNs and PAs, updating policies, reviewing charts, and performing other medical director duties.

In today’s presentation, I focus on providing these services through third-party companies that find opportunities for collaborating physicians, connecting them with healthcare organizations that need help.
