In this week’s episode, Dr. Nicole Rochester returns to the podcast. It’s been 5 years since she described how she was inspired to create her health navigation consulting business. Since then she has expanded her business and is now working with healthcare organizations to provide her services. She is also teaching other physicians to create their own consulting businesses.

Dr. Nicole Rochester obtained her undergraduate degree from Johns Hopkins University and her medical degree from the University of Maryland School of Medicine. She is a Clinical Assistant Professor of Pediatrics at the George Washington University School of Medicine.

During today’s interview, Nicole describes why she began working as a coach and consultant for patients with complex medical problems who needed help making decisions and navigating their healthcare journey. She also explains how she began providing these services on behalf of large health systems and hospitals, which is a much more sustainable model. She also shares her ideas for other healthcare coaching and consulting businesses to use a similar strategy.

In this week’s podcast episode, John addresses this question: Even if I’m burned out and frustrated, are there ways to stay in clinical medicine while achieving balance, satisfaction, and enjoyment? During this presentation, he discusses the following 5 options for remaining in clinical practice.

Take another stab at addressing the circumstances leading to dissatisfaction by aggressively renegotiating your employment contract. Review your contract and identify the major issues causing your frustration. Develop a strategy for renegotiating those issues with your attorney. These typically involve vacation time and covering for others’ vacations, work hours, salary, restrictive covenants, and inadequate pay for collaborating with midlevel providers.

If you’re looking for a new job, consider engaging an agent to represent you in contract negotiations with a new employer, often at no cost to you.

Start a new practice or join one that does not bill insurance companies. Direct Primary Care and Concierge practices, and narrow-focused practices often enable the physicians to work on cash-only, fee-for-service, or membership models, and to create a better lifestyle and income.

Almost any of us could start or transition to a narrow focused practice. John’s colleague was a full-time general pediatrician who transitioned to caring solely for patients with attention deficit disorder. He uses standard protocols for evaluation and management, and is an expert at billing for these services. He reduced his staff and overhead and his satisfaction and income have improved.

Finally, for those who want to achieve many of the above advantages, and others, without starting a practice, working as a micro-corporation may be a great choice. Incorporating and working as an independent contractor for a large group or hospital system provides more tax benefits, flexible investment opportunities, and off-loading the expenses for rent and staffing.

Today’s episode is Part 2 of my interview with Dr. Tod Stillson. Tod is a family physician who discovered that he could improve his autonomy and increase his income by incorporating and switching from employee to micro-corporation owner. In Episode 374 we learned that 10 years ago he discovered he could practice under a professional services agreement with the hospital system, so he incorporated and began working as a mini-corporation.

We pick up the interview with Tod, who explains how he shifted from employment to working using a professional services agreement. Then, he discusses a variety of benefits that you can access under this new model.

During this week’s episode, John describes why email marketing is an important tool for almost any business and how he has changed his approach to email marketing over the past year or so.

Unlike social media platforms, you own your list and it cannot be suddenly limited or deleted.

Email marketing is less expensive than other marketing approaches, and it provides you with customers who have demonstrated their interest in your products.

An email subscriber list must be nurtured over time. Subscribers will develop trust in you as you provide inspiration, encouragement, and education on a regular basis.

One strategy for doing that involves sending shorter, more frequent emails with personal stories that entertain and educate your subscribers, with a focus on increasing click rates and sales.

Dr. Laura Kaufman is a board-certified anesthesiologist who specializes in obstetrical anesthesia. She has always had an interest in patient safety and quality improvement, participating in and leading quality committees and teams.

She became frustrated when the focus on quality by those around her did not match their attention to the financial aspects of care. When her employer became part of a new organization, she decided to begin her transition from full-time practice to part-time practice and consulting while completing her master’s degree in patient safety and healthcare quality at the Johns Hopkins Bloomberg School of Public Health.

Dr. Armin Feldman practiced psychiatry and psychoanalysis for over 20 years and owned outpatient head injury rehabilitation clinics around the country.
