Why is it so important to get all three of these pillars - Marketing & Sales, Business Systems, and Clinical Excellence - right for a well-running business?

In this 'sode we discuss the three main pillars of business.  If only one or two of these are working well, the business is going to take a lot of effort and always feel precarious.

Marketing & Sales - If you don't have a consistent source of leads and are always worried about getting the phone to ring, marketing your practice is vital.  We need a systematic plan for marketing to avoid dips and the 'rollercoaster' of business.

Business Systems - If you're feeling overwhelmed or burned out, it's usually because you're lacking systems.  We often make up for poor systems with more work and late nights, but that's not sustainable.

Clinical Excellence - If you're having patients drop off, space out their visits, or stop coming in because 'it's expensive', we aren't getting the clinical buy-in necessary.

When all three of these are working well, the business gets much easier to run, grows faster and is actually fun to work in!

We Were Wrong About Not Having a Mission Statement21 Nov 202400:24:03

If you're like us, you might have a negative perception of a 'mission statement'.  We certainly thought it was dumb for us as a small company.

We had both sat through long meetings to create, or explain, or pontificate about a mission statement before.  Usually something created by HR and long, boring and pointless.

But we were wrong to throw away the idea just because the execution is so bad in big companies!

Clarifying your message and mission can help you create content around what you really believe.  If you are clear on what you stand for, you'll never have trouble finding topics to talk about!  A lot of our struggles to make IG posts, newsletters, or other content come down to not being clear on this.

In this 'sode we talk about how we came to our missions statement for Rising:

"We coach entrepreneurs in authentically creating impactful patient care while building financial confidence and lifestyle freedom."

Each word is carefully chosen and means something to us (and communicates it to those thinking about working with us.

Does My Patient "Really" Have IC?18 Nov 202400:18:42

This is something I've heard when teaching and training about how to treat interstitial cystitis (IC): "I don't think they really have IC."

What we usually mean by this is we feel their symptoms are stemming from pelvic floor dysfunction and central sensitization.

The AUA Definition of IC is simply: "An unpleasant sensation (pain, pressure, discomfort) perceived to be related to the urinary bladder, associated with lower urinary tract symptoms of more than six weeks duration, in the absence of infection or other identifiable causes."

So this is really a description of symptoms.  If they have any kind of pelvic pain and urinary urgency/frequency without an infection or other medical cause, it is - by definition - correct to call it IC.

This is similar to how someone can have pudendal neuralgia....caused by pelvic floor dysfunction.
Vulvodynia....caused by pelvic floor dysfunction.
Coccydynia....caused by pelvic floor dysfunction.

Many of the diagnoses we treat are really just descriptors of the condition.  So just because pelvic floor dysfunction is the underlying cause, it doesn't mean they don't have the diagnosis!

Now, how you present this to your patient depends on their history with their symptoms and how they perceive the diagnosis.

But if they have those symptoms, among ourselves, we need to stop saying "I don't think this patient really has IC".

Challenges of a Growing Multi-Clinician Practice14 Nov 202400:27:30

We get really honest about the challenges and growing pains in a multi-clinician practice in this 'sode.  And we'll make sure to have some advice on how to mitigate them!

But you can really boil down the big challenges into three categories.

Time - You're going to have a huge increase in the demands on your time.  You'll be doing more admin work.  More marketing.  More leadership.  More logistics.  More training and onboarding.  All while still dealing with your own caseload.  

Role - The moment you hire, your role in the business changes forever.  But it can take time to recognize and assume your new role.  You're responsible for filling schedules, training and building systems.  Leading a team and providing mentorship.  And - as with any new role - we're usually not very good at it right out of the gate!

Financial - Many business owners are surprised when adding staff doesn't increase what they make in the business.  We've worked with many business owners who have employees making more than they do!  Your profit margin drops, and we often feel we're not making income commensurate with the extra time and patient volume the practice is seeing.

Especially when you're growing, you have a new business every six months.  What got you here won't get you to the next mountaintop.  We hope this 'sode helps you know you're not alone, and gives you some practical tips as you grow your business!

Why I Start Teaching Perineal Massage Way Earlier Than the Research Says11 Nov 202400:22:02

Why do we start teaching perineal massage (and all of our birth prep) earlier than most at PelvicSanity?  There are a few reasons.

The first is simple - 37 weeks is considered full term.  No one will try and stop it if you go into labor.  So we plan to have all of our birth prep complete by then.  Any time we get after 37 weeks is just a bonus.  We're keeping our people comfortable or reminding them of things we've already covered in those final weeks.

Next, most of our pregnant folks are coming in with other symptoms.  So if we're planning to wait until 34 or 35 weeks, our plan to teach perineal massage might get blown up if they have a flare-up of back pain or need other help.

As they get closer to delivery, our people (naturally) get more focused on the baby as well.  They are going to more OB appointments, which might get in the way of our plan of care.  They're trying to prepare for the arrival of the new addition, and have other stresses as they plan for delivery and bringing the new one home.

Finally, I think we usually oversimply perineal massage.  It's not that easy!  It's a difficult area to reach.  The tissue is often tender and needs to be worked on by us prior to teaching them to do it themselves.  They may not want a partner to help or might feel self-conscious about doing this.  Remember, not everyone is as excited about the pelvic floor as we are!

And speaking from personal experience, I'm guessing even many of us - as pelvic rehab providers - don't do much for our own perineal massage.  So if we're going to emphasize it to patients, we need to give ourselves time to do it!

Problems of a Busy Solopreneur07 Nov 202400:31:32

While many solopreneurs are struggling to fill their schedule, there is also a problem of being 'too busy.'  Remember, we are never out of problems as entrepreneurs.  Instead, we're just trading one problem for a better one!

So if you're a busy solopreneur, you're often feeling overwhelmed by the demand for your services.  Worried you aren't providing the best patient care because you can't offer regular appointments.  And dealing with a long wait list for people to get in to see you.  You're probably feeling guilty about not being able to provide the care you want for your people.

Remember, the answer isn't to 'panic hire'!  Sometimes you might want to stay a busy solopreneur.  

And if you do (or you can't find someone to hire), here are some suggestions to make life better:

  1. Honor why you started your practice and keep that in mind as you make decisions
  2. Increase your prices
  3. Set your ideal schedule
  4. Schedule your caseload further out so you can honestly assess your availability
  5. Work only with your ideal patients
  6. Buy back time with admin help

Being Happier as a Small Business Owner04 Nov 202400:23:52

Building an outwardly successful business doesn't mean much if we don't enjoy the journey!  In this 'sode we dive into how we can be more content and happy as business owners.

What we all get wrong is thinking we'll be happier in our business when we reach the next milestone.  The ideal amount of patients.  The next big revenue number.  When we hire.  We just keep chasing the next thing.

We certainly hit this in building PelvicSanity.  In 2019 we had grown from just Nicole to 5 full-time clinicians in just 3 years.  We had an admin team and a massage program.  But despite all that outward success we were still working constantly, stressing over finances, frustrated with our team, forgoing vacations and time off, and not being true to why we had started the business in the first place.

After working on it ourselves and with 600+ other business owners, we've learned a few things about being happier in business.

1.  Financial Confidence.  This is more than just financial success or security.  It's knowing your numbers.  Paying yourself regularly.  Setting aside money for taxes and building a buffer account for a rainy day in the business.  And being confident your business can provide for you.

2.  Time Freedom.  Many of us started our own business to have more time freedom.  But find ourselves working late, miss pickups, don't take vacation or sacrifice time we'd rather spend with our families, working out, or with friends.

3.  Crafting Our Job.  We want you to be able to spend more of your time doing what you love in the business and less doing things you don't enjoy.

Paying Your People (Part 3): Raises24 Oct 202400:17:45

Should we be giving raises?  How often?  And how much?  In Part 3 of the series, we dive into all your questions.

From our annual State of Pelvic Rehab survey, we found the following:

  • 41% of employees did not get a raise last year
  • 34% received a raise between 0-3%

So, what should we do as business owners about raises?  We believe (and teach) they should be RTF - Regular, Transparent and Fair.

Regular - We believe in an annual raise for your clinicians.  If you were an employee and didn't receive one, it was likely frustrating and a negative experience.  Importantly, we only discuss these at our annual review.  This means we aren't talking about it the other 364 days each year!

Transparent - Your people should know how it's done!  We start this conversation in our interviews, and then in our onboarding, 3-month and 6-month reviews.  There are no surprises.

Fair - We have the annual raise be a combination of cost-of-living increase (which everyone gets) and a performance-based raise.  Especially as your team grows, you want to make sure these raises are fair to each team member.

Remember, the 'raise' can include extra benefits you're giving that year as well!  Paid time off, 401k, health stipend, etc.!

We hope this 'sode eliminates some of the uncertainty when it comes time for an annual performance review and raise!

3 Things We Got Wrong in The IC Solution Book!21 Oct 202400:44:09

Not necessarily a fun 'sode to do, but we wanted to revisit The IC Solution - written almost 10 years ago now - to see what we got wrong or would change if we wrote another edition.

We talk about the book-writing process, how we wrote a good chunk of it in a oceanfront pool in Mexico, and what our goal was for patients.

The book has been the best-seller in it's Amazon category and reached nearly 25,000 patients.  And it's something we're incredibly proud of.

A lot of things in the book changed opinions or have aged really well.  The American Urological Association added our 'no kegels' rule for treating patients with IC to their 2022 Guidelines.  

We showed in the book the evidence for Elmiron helping patients was very weak and almost certainly didn't outweigh the side effects. 

There was a major emphasis in the book on 'nervous system upregulation', which we would now call more 'central sensitization', even before much of that research came out.

But three things we got wrong (or would do differently if we wrote it today):

  • Using more 'trigger point' language than we would today.  As it's a patient-focused book we might have left the term in, but our explanation of it would have been very different and focused more on global overactivity.
  • Vaginal estrogen.  If written today, with all the research that has been done, this likely would have it's own chapter or a long section.  We didn't have any mention of it in the book.
  • How to find a great pelvic therapist.  Our goal for the book was to get people with IC to go to a pelvic therapist.  We didn't put in resources for helping them find one who was a great fit or give them the right questions to ask. 

Hope this has been helpful to see how opinions evolve and change.  Anything you put out into the world won't be perfect in 10 years - just put it out!

Paying Your People (Part 2): Bonuses and Profit Sharing17 Oct 202400:24:34

Should you be 'bonusing' your employees?

Generally you'll find bonuses in other fields - lawyers, salespeople, etc.  And they can make sense.  You're trying to incentivize an employee to work longer or harder with a bonus. 

But that doesn't really track in pelvic health.  Do any of us believe a clinician is going to provide better clinical care because they get a bonus?

Clinicians provide better care when we provide clinical mentorship, onboarding, training, guide their con-ed and spend time with them as mentors.  Throwing money at them in the form of a bonus is never going to improve their clinical outcomes or buy-in.

And bonuses often create resentment if a clinician doesn't earn one, or the amount isn't as high as it was last year.  They can be a major profit leak; many business owners who give bonuses don't even know how much their employees are actually making per hour!

You have to implement metrics that really track performance and then be willing to not give the bonus if your people don't hit it - which is a difficult conversation! 

And it just complicates the whole thing.

So if you aren't giving bonuses, it's not something we recommend.

If you are, then decide if it's working!  Calculate out what you're actually paying on a per-hour or annual basis including all the bonuses.  Decide if it's worth changing in your business right now.  If so, it's going to be a difficult conversation, but it might need to happen for the long-term health of the business.

When Your Patient Wants to Talk Politics....14 Oct 202400:22:19

This 'sode is all about surviving this turbulent political season.  It's based on training we give our staff at PelvicSanity about how to navigate an election and patients who are passionate/anxious/fired up about the political environment.

Let's start with this - talking politics with your patients is never going to go well.

Even if you're on the 'same team', going back and forth about what the 'other side' is doing is only going to rile both of you up.

And if you have political disagreements, you aren't going to change anyone's mind.  Especially not while your finger is up their butt!

But with only a handful of days left until an election, it seems almost impossible to avoid this conversations.  So how do we navigate them?

1)  Be curious.  If your patient brings up politics, be curious.  Ask more about whether their family agrees with them.  How much political TV or news they are taking in.  Whether it's helping or hurting their symptoms or issues.

2)  Have a 'no politics' policy.  You can also just be a little more firm with a 'no politics' boundary.  You could say something like: "totally respect your opinion, but with things so polarized I think it's best to steer away from politics and focus on your pelvic health.  How have your symptoms been this week?"

3)  Shut it down.  If your patient says something you feel you can't just let slide - we give an example in the 'sode - it's shutting the conversation down in 1-2 sentences and then immediately redirecting back to them and their care.

We hope this helps navigate these conversations until the election!  Remember, it's not about you - while it's great to hold strong beliefs of your own, people on the 'other side' (whatever that is) deserve great care as well.

Paying Your People (Part 1): Salary vs Hourly for Employees10 Oct 202400:28:42

In this 4-part series, we're doing a deep dive into how to pay your employees.  There's a lot of confusion here, and frankly a lot of bad advice.  So we're going to dive in over the next month to clear things up!

In this 'sode we talk about some things we shouldn't be doing: 

  • Paying clinicians as contractors instead of W2 employees
  • Paying a percentage instead of a wage
  • Having complex payment structures
  • Not leaving enough margin for the business

But the biggest thing we cover is the pros and cons of paying your employees a salary versus paying per patient.

Salary is most appreciated by our employees.  It provides steady income and reduces the number of conversations we have about money.  It's also very dangerous for a business that isn't 'full' - there's a huge issue if you're paying a clinician when they aren't working!

Paying per patient is safer for the business owner.  If the clinician isn't working, they aren't making money.  But it's more stressful for the clinician with a lot of fluctuation in their weekly schedule (and weekly pay).

There's a hybrid model as well, which we call "Base +".  This guarantees a 'base' per week while still paying per patient.  It can help protect the business while also providing more stability for your employees.

The Natural Fluctuations of Business03 Oct 202400:20:15

Are you feeling like you're on the business 'rollercoaster'?  A few good weeks or months, followed by a 'downturn' or a time when leads slow down?

This is not only normal, it's to be expected!  Why should business be perfectly consistent?  Some of the best business advice we got early in PelvicSanity was from another business owner - "respect the ebb and flow".

We're going to have fluctuations in the number of leads depending on hundreds of factors.  We're going to have turnover and fluctuations in our team and how many applicants we get for a position. 

There's also fluctuations within ourselves.  Our level of energy and motivation can change.  Our priorities can shift.  External issues with our parents, or our children, or a significant other will arise.

1.  Facts, Not Feelings

If you've ever been in any of our mentorship programs, you've heard us say 'facts, not feelings.'  Assess the situation objectively - is this really a 'downturn' or are you just stressed?

2.  Respond, Don't React

Assess the situation and respond accordingly, but don't jump to 'react' without thinking.  This is how we end up making poor business decisions like wasting money on FB Ads or panic-hiring someone to help with 'marketing'.

3.  Borrow Perspective From a Business Coach

To us, this is one of the most important things we can do as business coaches - give a longer-term perspective and help keep you on the right track!

Coaching Patients to Overcome Barriers30 Sep 202400:24:01

Finding that patients aren't finishing the homework you give or aren't following through on what they said in your appointment?  Try spending more time coaching them to overcome barriers.

Our job isn't to just dispense advice, ideas and exercises!  We need to make sure our patients can actually follow through and get them done.  As as rehab providers, we often don't realize how difficult the things we're asking our patients to do are FOR THEM.

We talk through examples we've seen in training our staff at PelvicSanity, including things like having patients do a bladder diary, see a mental health therapist, do their home exercise program, or get notes from a test from a physician.   

While all of these seem straightforward to us, that's only because we live this stuff every day!  We don't understand the barriers our patients have.  So we need to ask!

We might spend 30 seconds talking about an exercise and up to 10 minutes working through the challenges the patient would have in doing it themselves.  That's how we get compliance!

Too often we find ourselves rushing through our sessions - we actually need to slow down in order to help our patients overcome these barriers.

We're really about 3 things at PelviCon

  • BY and FOR pelvic rehab.  PelviCon was the first-ever conference both by and for pelvic rehab.  All of the speakers are clinicians.  It was the first conference to bring in both PTs and OTs under the 'pelvic rehab' banner.  Our specialty deserves it's own conference - one that leaves us feeling inspired!

3 Major Problems with "Discovery Calls"19 Sep 202400:24:02

Considering discovery calls, doing them now or thinking you might be missing out on them?  Check out the full 'sode to better understand the conversion process!

The first thing to recognize is where these 'discovery calls' - which are really sales calls - come from.  They come from the 'ortho bro' model and selling big packages. 

Here's why we aren't a fan:

1.  It's inconvenient for patients

You're creating an extra barrier for patients.  Instead of just filling out a contact form, they have to find a time on your schedule that works.  They have to decide to spend a half hour of their time with you and wait days/weeks to even know if you can help!   

What's actually happening is many patients are seeing this on your website, looking at the times and then disappearing instead of booking.

2.  Gives the wrong impression of pelvic health

What other doctorate-level medical professional does a 'discovery call'?  Good luck getting your urologist on the phone to see 'if you're a good fit!'

If we're trying to normalize pelvic rehab, it's the wrong impression to be sending. 

3.  It's not sustainable for a growing business

This might be feasible when you're small and growing slowly....but if you're doing this, you're costing yourself the leads that could help you grow faster!

But once you're busy, these discovery calls take time away from patient care.  And if you're trying to outsource these to your clinicians, they are going to struggle and resent it. 

Patients can still feel heard and taken care of.  You just need a good phone script, NOT a discovery call!

Nervous About Seeing a Patient Who Has Been to Another Pelvic PT/OT16 Sep 202400:31:46

Ever feel a little worried or extra pressure when seeing a patient who has already been to another pelvic PT/OT?  

Here's the secret: 

  1. We care a lot about the patient's experience, thoughts and what they feel like was beneficial.
  2. We don't care at all about what the other provider thought or was working on

In fact, a lot of times these patients are easier because they know what has worked in the past and what they didn't like.  They're literally telling you exactly what they're hoping for and expecting from your session!

We ask early on in the session about their experience with their previous provider.  If you aren't asking about this on your intake form, make sure you are!

Now if the patient is coming from another provider, either they a) had a great experience, loved the person and aren't able to go to them for some reason outside their control, or b) weren't getting better and having great results.

Either way, you're not just going to duplicate what that other provider was working on! 

You're bringing your own experience, evaluation, skills and treatment philosophy to the person.

And this means you don't have to 'connect' or 'collaborate' or reach out to a past provider!

Discouraged by How Much to Learn29 Aug 202400:27:11

We know it can feel daunting and discouraging (especially as a newer pelvic rehab provider) how much there is to learn!  This 'sode came from a listener comment.

She was expressing frustration with how her entry-level courses didn't really prepare her for the way clinicians actually treat.

We talk about the challenges of doing an introductory course in pelvic health (it's certainly not easy).  How it can lay a foundation, but we also shouldn't expect to be 'ready to go' after a single weekend course.  And how those courses often don't acknowledge how much is missing or left out from just a basic course!

So if you're feeling a little discouraged by how much there is to learn, make sure to check out this 'sode!

The Gold Standard is Ongoing Mentorship

Ultimately, the gold standard is working somewhere you receive extensive onboarding and ongoing mentorship.  There's no substitute for being able to ask specific questions to a mentor as you grow in your career. 

So - if you can - prioritize working somewhere that will invest in mentorship for you.  If you've been promised mentorship and aren't receiving it, make sure you're standing up for what you were promised.

Other Suggestions

If you aren't able to work somewhere you're getting ongoing, specific and helpful mentorship, you aren't alone!  Here are some other suggestions to help.

  1. Use what you have!  When you know better, you do better.  The interviews we've had with PelviCon speakers are great examples of this.  All of them have grown and changed over the course of their careers!
  2. Utilize Free Resources.  There's no better time in the history of the world to be a pelvic rehab provider!  There are incredible podcasts, interviews, Facebook groups and other resources that didn't exist even 10 years ago.  Take advantage!  
  3. Choose Con-Ed Wisely.  There's a compound interest to learning early in your career, where you take that knowledge and apply it to every patient you work with.  Take con-ed early in your career.  Spend your own money if you have to (and if you can).  And don't just take the next in the basic series if that isn't serving how you want to treat patients!

Secrets of Delegation15 Aug 202400:25:48

Anyone else have a hard time with delegating to others?  Learning to lead a team is a whole new skill.

In fact, many of the traits that bring success as a solopreneur - a touch of perfectionism, wanting control over the business, being 'in the weeds' on every aspect of what you do - actually make growing beyond yourself more difficult!

What got you here won't get you there.

But this is a skill set that you can learn - it doesn't have to be trial and error.  And if you want to grow, you'll have to get better at this.  The three biggest mistakes we see with delegation:

  • Not delegating at all.  If you've found yourself saying "no one can do it as well as I can' or "Well, I'm used to doing it...' or 'It would be more trouble to find someone' you're often in this category.
  • Abdicating responsibility.  This is when you just throw a problem at someone else like a hot potato.  
  • Purgatory of micromanagement.  The worst of both worlds - you barely allow yourself to delegate, and then look over the shoulder of your person as they do the work.  Y

Instead, three things that make delegation work:

1.  Choose wisely what to delegate.  Spend time to decide what you can - and can't - delegate.

2.  Support your people!  This is where onboarding and training is so important.  Make sure you're giving someone the tools they need to be successful.

3.  Accountability.  You need to have a metric or way to determine if it's going well!  If you're just giving a task and hoping it gets done, you aren't really delegating.

We hope this helps!  Leading a team is a brand new challenge for most of us, but it can be fun, rewarding and certainly vital if we ever want to grow.

Should I Be Taxed as an S-Corp?08 Aug 202400:22:25

Business owners (or wannabes) - we're getting tactical today!  Don't skip this 'sode just because it doesn't sound sexy...it might save you tens of thousands of dollars in your business over the next few years.

As business owners, we actually get a ton of tax benefits that a regular employee does not.  One of those is the ability to choose how we are taxed.

This is all about the payroll taxes (~15%) we are all required to pay.  When you were an employee, your employer contributed half of those (7.5%) and you contributed the other half (7.5%).  Now that you're acting as both, you can owe 15% on every dime of profit you bring in if you're taxed as a sole proprietor (regardless of whether you're a sole prop, LLC, P-LLC, PC, or corporation).

However, if you elect to be taxed as an S-Corp you only pay payroll tax on what you actually put through payroll.  This just has to be 'reasonable'.  And the rest is 'pass-through' income, meaning you don't pay payroll taxes on it!

Generally our recommendation is if you're expecting $70-80k in profit, it absolutely makes sense to move to an S-Corp.

Just as an example.  If you bring in $100k in profit over the year and are taxed as a sole proprietor, you pay 15% of that in payroll tax (and THEN still have to pay income tax!).  That's $15,000 in taxes.

But as an S-Corp, you could pay yourself $50k through payroll.  That's taxed at 15% (the government always gets theirs), but the remaining $50k is 'pass-through' income.  This means you just saved $7,500 in taxes!

Now you might have to pay an additional $2k or so for tax prep for an S-Corp, but you're still saving more than $5,000 per year.  Added up over a decade, even if you aren't growing, it's more than $50,000 in tax savings!

Why IG & FB Ads Don't Work in Pelvic Health25 Jul 202400:27:50

We're told (ironically, often in paid ads!) paid advertising is the 'magic bullet' to getting the phone to ring in our clinics.  But is that true?

In this 'sode we dive into the challenges with IG and FB ads.  The fundamental problem?  No one is going on social media hoping to be hit with an ad to fix their incontinence!

And especially for a high-ticket item like a ~$200 initial evaluation.

So that leads to two options for ads.  One is the 'Group-On' model, where you offer an absurdly low initial evaluation price (think ~$40) and then try to 'upsell' when they get into your clinic.  For most clinicians this feels salesy, changes your business model, brings in the wrong type of clients, and certainly doesn't work if you have staff.

The other option is the 'Funnel Method.'  You get them to sign up for a free PDF.  Then you send a boatload of marketing emails in a sequence, trying to cultivate them, get them on a discover call, have them buy a $12 e-book, and finally ask them to come in for an evaluation.

It's really hard to make either of these strategies work!

And we often forget the opportunity cost - what could we have done with the same budget and effort in another area of marketing?  Usually the answer is we could have been a lot more effective and built a long-lasting marketing opportunity instead of relying on paid ads.

How Insurance Companies Dictate Care18 Jul 202400:29:32

How much of our daily lives as rehab practitioners is dictated by insurance companies?

Whether you're a current (or future) business owner or a clinician in an insurance-based world, it's important to realize how much insurance companies control.

Those of us in the cash-based world talk a lot about how insurance dictates care.  But what do we really mean by that?

In this 'sode we go through how insurance companies shape the landscape of pelvic rehab.

They shape the finances of rehab business.  They limit how much businesses can be paid.  They put a cap on how much employees can make.  And they incentivize practices to have aides, students or modalities reduce costs.

They shape how long we can see patients for by limiting the number of reimbursed visits.  Insurance also limits the length of the session (maxing at an hour).  And it dictates who we can see - no wellness, no healthy birth prep or anyone who doesn't have an impairment or issue.

Finally, they dictate how we document, which EMR we have to use and the hours and hours of extra paperwork each clinician needs to complete.

This isn't to say that working in an insurance-based practice is bad - it's absolutely necessary and important.  But we should realize what hoops we're jumping through just because an insurance company says so!
