Wondered about expanding into a second location?  

It's one of the most common business questions we get.  I think we often get caught up in what's 'next.'  Or the fear of missing out on an opportunity.  But it it aligned with your goals for starting a practice?

We've had the opportunity to expand to a second location several times over the history of PelvicSanity, and have turned them down each time.  We'll explain why, and the lessons we've learned from helping other practice owners build into a second location.

We'll talk about the pros and cons of a second location (and there are a lot of cons you'll have to navigate).  

Sure, it can present exciting opportunities like reaching a new market or providing more room for your clinicians. But let's not ignore the challenges - increased costs, potential dilution of your practice’s culture, and facility issues. We go beyond the surface and consider the motivations that may be driving this desire: perceived opportunities, clinician preference, fear, or maybe even ego.

Lastly, we discuss factors that could influence the success of expansion. We stress the need for a comprehensive plan that takes into account everything from product control to potential increase in rent. But most importantly, we underscore the importance of maintaining practice integrity and family time while attempting to expand your business. Preventing Patient Flares18 Sep 202300:23:09

We've talked about managing flares, but can we prevent them in the first place?

In this 'sode we dive into what actually causes a flare, how to prevent them from happening and make sure your patient is prepared if symptoms do increase.

We also discuss the complicated balancing act between treating so timidly you aren't making a change vs. repeatedly flaring your patients.

In order to prevent flares you'll need to address two things: the irritability of the tissue and the irritability of the patient's nervous system.  Effective communication is one of your most important tools in preventing - or managing - symptom flares and building trust with your patient.  

Let's work upstream to prevent as many of these flares as possible, equip our patients so they don't come as a surprise and manage flares when they do arise!

7 Reasons to be On Social Media as a Pelvic Rehab Provider17 Aug 202300:32:25

The single biggest difference in our profession over the last ten years has been the rise of social media and the creators who educate patients there.

It hasn't been physicians who refer more patients.  It hasn't been insurance companies who allocate more visits.  It's been our colleagues and friends who let potential patients know how we can help.

And yes, there is criticism from 'dinosaurs' who feel we need to cite every study, make every caveat and ensure our posts meet their impossible standards.

But one of  the throughlines from our PelviCon speakers is the responsibility we have to educate the general population and help grow our profession.  Here are seven reasons to be involved in social media:

  1. It's fun!  Whether you're a 'lurker' or creator, being around others who are interested in moving our profession forward is great.
  2. Make connections.  You'll meet and connect with others who are passionate about pelvic health and continue to learn.
  3. Improve your teaching skills.  Brevity is truly the sole of wit, and the better we get at using the platform to teach the better we are at explaining what we do to our patients and clients.
  4. Learn what you believe (your Clinical Ethos).  As you put yourself out there online, you'll learn more about what you believe, what you want to teach and how pelvic rehab should be practiced.
  5. Life-long learning.  We've never been in a better time to learn from the leaders in our field online.
  6. Build your value.  Your personal brand, followers and content are valuable to potential employers (or yourself, if you choose to create your own practice).
  7. Expanding the field.  We all have a responsibility to do what we can to build our field, our profession, and raise awareness of how we can help.

You Need More Time27 Jul 202300:26:45

What if we didn't have to race against the clock to 'fix' a patient in set number of visits?  Or feel pressure to get them out of our office as soon as possible?

Ultimately, this pressure we feel to discharge as soon as possible comes from insurance companies.  We're taught to 'work ourselves out of a job.'  To make sure what we're doing is 'skilled' enough.  To need to show measurable improvements at each visit.  To have to justify our care.  And to fit our treatments within their allotted number of insurance visits - which is the same number for a sprained ankle and a 30-year history of pelvic pain!

This attitude has trickled into our PT schools, our 'productivity' standards in insurance-based practices, and has become the norm in our field.

But why?

What if instead of rushing to get someone out of the door, we acknowledge it takes time to provide meaninful, life-long change? 

We believe this attitude can help prevent burnout, reduce anxiety, and open up opportunities to benefit our patients in ways we've never explored.

Just Take That Vacation!03 Jul 202300:38:14

What vacation are you putting off?  Whether it's a week-long overseas trip or a long weekend getaway....here's the permission (that you absolutely don't need) to just take it!

Burnout is a huge problem in our pelvic rehab community.  And often we're playing 'catch-up', trying to deal with issues after they've come up.  But by being proactive we can avoid ever getting to that point!

We can be dissuaded from taking time off by our employer and their policies.  By our own sense of guilt in leaving our patients.  By the pressure of building our own business.

But ultimately we need to be taking care of ourselves and enjoying our own journey if we're going to show up the best for our patients.

Like most things, taking time off starts with knowing yourself and what will refill your cup.  Do you need lengthy time away, or would you be better off with more frequent 3-day weekends?  Do you like a vacation where you relax by a beach or where you're active and learning?

Whatever it is, here's your encouragement to plan the rest of your year around your vacations and time off, and enjoy!

Constipation: A Missing Piece19 Jun 202300:25:09

In today's 'sode we tackle constipation.

This is far more common than most people realize.  In a study we conducted at PelvicSanity and published at IPPS, we found 69% of patients who came in for other symptoms (pelvic pain, prolapse, incontinence, etc.) had constipation as well.

Our missing piece for constipation?  Bringing it back to the basics.

Often we make this more complicated than it needs to be.  We suggest major dietary changes to our patients (which are hard to comply with).  We work on changing posture.  We jump to techniques like rectal balloon treatment.  All of which are good and helpful, but often overlook things that get us the most 'bang for our buck.'

So bring it back to basics:

  • Eat breakfast (something you can chew)
  • Water intake
  • Magnesium
  • Walking and movement, especially in the morning
  • Squatty potty

If a patient buys in and does these things, it's almost impossible not to see some changes in symptoms.  And then we can move on to more advanced techniques or bigger lifestyle changes if we need.

Male Pelvic Pain: A Missing Piece12 Jun 202300:27:45

A lot of times with male pelvic pain - particularly pain in the testicals, scrotum, or penile pain - we'll jump right to breathing and/or treating the pelvic floor.

But a missing piece for these patients is the spermatic cord.

Running along the inguinal canal, the spermatic cord carries nerves and blood vessels integral to the system.  Often this area is tender, inflamed or tight when dealing with male pelvic pain. 

This is definitely something we're assessing and treating within the first few visits with those dealing with penile, testicular, or scrotal pain.

Why IG & FB Ads Don't Work in Pelvic Health25 Jul 202400:27:50

We're told (ironically, often in paid ads!) paid advertising is the 'magic bullet' to getting the phone to ring in our clinics.  But is that true?

In this 'sode we dive into the challenges with IG and FB ads.  The fundamental problem?  No one is going on social media hoping to be hit with an ad to fix their incontinence!

And especially for a high-ticket item like a ~$200 initial evaluation.

So that leads to two options for ads.  One is the 'Group-On' model, where you offer an absurdly low initial evaluation price (think ~$40) and then try to 'upsell' when they get into your clinic.  For most clinicians this feels salesy, changes your business model, brings in the wrong type of clients, and certainly doesn't work if you have staff.

The other option is the 'Funnel Method.'  You get them to sign up for a free PDF.  Then you send a boatload of marketing emails in a sequence, trying to cultivate them, get them on a discover call, have them buy a $12 e-book, and finally ask them to come in for an evaluation.

It's really hard to make either of these strategies work!

And we often forget the opportunity cost - what could we have done with the same budget and effort in another area of marketing?  Usually the answer is we could have been a lot more effective and built a long-lasting marketing opportunity instead of relying on paid ads.

Tailbone Pain: A Missing Piece05 Jun 202300:24:25

What's something we overlook or miss when it comes to treating tailbone pain?

Remember that tailbone pain is a non-specific diagnosis.  It's just as description of symptoms!

So it's up to us to do a true differential diagnosis and understand the 'why' of what is going on.

But something I think we miss is treating rectally in quadruped. 

Why quadruped?  Well, it's the best position - in my opinion - to approximate the hip flexion and position where most patients experience their tailbone pain.  It gives us a lot of control over their positioning in non-weight bearing position.

Also, there are some major limitations with treating rectally in side-lying and supine that we get into within this 'sode.

Vulvodynia: A Missing Piece29 May 202300:22:28

What's something we often overlook or miss when it comes to vulvodynia?

Well, first it's important to recognize vulvodynia is just as non-specific as 'low back pain.'  It literally just means 'pain in the vulva.'

We could have gone a few different ways with a missing piece - rectal treatment, looking at the urachus and belly button, lumbar referral patterns, hip referrals, or more.  It's a complex diagnosis.

But a missing piece for vulvodynia is evaluation and treatment of the perineal fascia.

With the presentation of vulvodynia, it's almost impossible for the perineal fascia to not be involved.  Because of this, it's definitely an area we should be evaluating and treating early in our treatment progression with these patients.

Pudendal Neuralgia: A Missing Piece22 May 202300:23:55

What's something that's misunderstood or overlooked when treating complex patients with pudendal neuralgia?  

This is often one of the most intimidating diagnoses we work with.

It's easy to hyperfocus on the path of the pudendal nerve, but a missing piece is actually the posterior cutaneous femoral nerve.

In fact, it's almost impossible for 'pudendal neuralgia' to only involve the pudendal nerve!  This is why nerve blocks often don't address all symptoms or symptoms spread beyond the immediate pathway of the pudendal.

Interstitial Cystitis: A Missing Piece08 May 202300:25:12

For interstitial cystitis, a common 'missing piece' is neural tension of the sciatic nerve.

It's a testament to how difficult IC is to treat we considered a few other 'missing pieces.'  We thought about highlighting the 'IC Diet', superficial pelvic floor muscles, constipation, urachus, inner thighs, calves or ischiorectal fascia.

But we kept coming back to the sciatic nerve and the major role it plays in urinary urgency, frequency and/ or interstitial cystitis.

In this 'sode we discuss the evaluation and treatment of neural tension and how it can transform how you treat patients with IC and urinary issues.

Pros and Cons of a Cash-Based Practice04 May 202300:30:35

We're taking an honest look at the pros and cons of working in a cash practice.

As most of you know, PelvicSanity is a cash practice and we've been working without interacting with insurance companies for more than 7 years.

But that might not be the right decision for everyone.  So we want to lay out the pros and cons.  Whether you're a 'wannabe' considering your options or an already established business wanting to make sure you got it right, this should help!

Clinical - Cash

Cash practice gets insurance completely out of the picture.  Hour-long (or two-hour) treatment sessions, wellness, working with patients for a longer plan of care - as long as you and the patient believe it's worthwhile, you can do what you clinically believe!

Financial - Cash

In terms of profit margin and the financial metrics, a cash practice wins.  You have control over your profit margin, instead of insurance capping reimbursement.  There are no cash flow issues. 

Marketing & Sales - Insurance

This is always the biggest challenge for cash-based practices.  Being 'in network' literally means you're part of a referral network.  You might still have to do some work, but you should have a much easier time in an insurance practice of getting patients to call and schedule.  Your ability to do this in a cash practice is vital for success.

Ability to Grow - Tie

Both types of practices can grow, and have different issues.  An insurance-based practice often has the patient volume to grow, but with low profit margins it can be difficult to hire and expand while still paying yourself.  A cash practice can expand with good profit margins, but the marketing and sales need to be on point.

Lifestyle - Cash

With a cash practice you can work fewer hours.  You don't have insurance BS.  You don't have a biller.  And you can treat how you want.

Diastasis Recti: A Missing Piece01 May 202300:22:36

When it comes to diastasis recti, a missing piece is often the back.

We're often focused on the front....because, well, that's where the diastasis is!  We'll engage the transversus abdominus and strengthen the core muscles.

Many times we'll focus on the front until and unless things aren't going well, before finally looking for other contributing factors.

But the diastasis is often a result of a problem in the system, and originating in the back.  We can even ask patients to do things (ie: stack the ribcage over the pelvis) they may lack the ability to do!

At the initial evaluation we always want to check the obliques, paraspinal muscles and fascia of the back.  Often we'll focus our initial work here and see what a difference it makes prior to highly focusing on strengthening the abdominal muscles.

Abundance Mindset or Neurotic Mess - Your Choice!27 Apr 202300:29:24

How can we stop driving ourselves crazy by comparing to our competitors?  Worrying about what they are doing?  Feeling like they are 'taking our patients?'

It's not easy.  We're competitive.  We care.  But ultimately we need to cultivate an abundance mindset or drive ourselves crazy with comparison.

Here are five ideas that might help when it comes to cultivating an abundance mindset:

  1.  Do the math!  There are so many more people with pelvic health issues than we'll ever be able to see.  Even in a small town, if we get even 1% of those who could use our help in the door, we'll have a waitlist a mile long.
  2. Recognize patients are smart and deserve choices.  It's not a bad thing they can decide who they want to see.
  3. Being a neurotic mess isn't fun.   That's the other side of the coin, where you're constantly comparing and worried about others.  So for your own sanity, an abundance mindset is crucial.
  4. There's a place for competition.  We can use it as inspiration and fuel as we grow and provide better care.  What does it say if you believe you can only be successful if you're the only option in town?
  5. This is the future of pelvic health.  We're going to continue to grow as a specialty.  There will be options for patients.  We'll have smaller niches we occupy.  And that can all be good - for us and patients!

Ultimately, we want you feeling confident in your practice and being more authentically you in your business or clinical practice.

Pelvic Organ Prolapse: A Missing Piece24 Apr 202300:24:48

One thing we often miss when treating prolapse is a functional/standing assessment.

If an orthopedic patient came in complaining of pain with walking and running, would we do our entire examination with them sitting or laying down?  Absolutely not!

And yet, isn't that exactly what we're doing for our patients with prolapse if we aren't doing a standing assessment?

It's entirely possible a patient with prolapse will never be evaluated - by their OB or their pelvic rehab provider - in the position where they experience their symptoms.
