What if tamoxifen and aromatase inhibitors weren’t the only options?

For many people with hormone receptor-positive breast cancer, endocrine therapy can be lifesaving – but it can also be incredibly difficult to live with. Joint pain, sleep problems, sexual health changes, bone health, weight and metabolic changes can all have a significant impact on day-to-day life.

And for some, the question becomes: what else is there?

In this episode, Dani is joined by oncologist Dr Eleonora Teplinsky, author of Beyond the Pink: Navigating Life, Health, and Breast Cancer, to explore the changing landscape of breast cancer treatment and survivorship.

One of the big topics is SERDs – a newer generation of endocrine therapies that are creating a lot of interest in the breast cancer world. What are they? How are they different from tamoxifen and aromatase inhibitors? What did we learn from the LIDERA study? And could having more endocrine treatment options eventually make a meaningful difference for patients struggling with side effects?

But, as always, this isn't only about the science.

Dani also sits down with Rachel Bowman, breast cancer survivor and Director at Menopause and Cancer, to mull over what all of this actually means when you’re the person taking these treatments

Dr Teplinsky also shares the thinking behind her new book, Beyond the Pink, which brings together evidence-based, practical information about the questions people often don't have enough time to explore during an oncology appointment.

After more than 200 episodes of conversations with doctors, researchers, therapists, cancer specialists and, most importantly, people navigating menopause after cancer themselves, I’ve learned a lot.

Last week, a dream came true, and we hosted our first global menopause and cancer conference for healthcare professionals - and so for this episode, I’m doing something a little different.

I’m sharing five things I now know to be true (I know, a bold statement!) about menopause after cancer. Things I’ve learned at the conference and things I’ve learned over the years.

You might disagree with me - but here are mine (for today anyway!):

There is rarely one “right” answer.

Especially when it comes to HRT after cancer, risk and personal choice.

Treatment shouldn't mean simply putting up with everything.

We need to talk about the impact of treatments such as tamoxifen and aromatase inhibitors — and what can be done to help.

The things that might sound “small” can have an enormous impact.

Hair, vaginal and bladder symptoms, painful sex, body changes — these things matter.

Menopause after cancer isn't just physical.

Anxiety, sleep, confidence, relationships, identity and that feeling of “Who am I now?”

There is almost always something we can do.

We need to look beyond the medical and expand our thinking. From non-hormonal medications and symptom-specific treatments to movement, sleep, psychological support, lifestyle and community — often it's about finding your own combination of things that help.

Ah, this was a great start to such an important episode! I literally did no planning AT ALL! I just wanted to know from Dr Jordan Emont, our guest on today’s show, if there is a common question he gets asked by his patients in his clinic.

And after getting such lovely personal insights into Jordan’s life, we ended up having an evidence-based conversation about testosterone after cancer, especially for people navigating menopause after breast cancer.

We dig into why access in the US is getting harder not easier and how clinicians think about balancing quality of life with safety when the data is still limited.

This episode is so true to all the conversations we have on this podcast because it sits right in the middle of a real-world clinical grey area: patients want relief, oncologists want reassurance, and doctors are trying to make thoughtful decisions without perfect long-term trials.

Jordan also shares how this topic is evolving in guidelines, why he thinks testosterone can help some people stay on aromatase inhibitors, and where caution still makes sense.

Episode 234 - Veoza & Lynkuet: The New Non-Hormonal Drugs for Hot Flushes After Cancer09 Sep 202600:43:02

“Night sweats. I cannot go on like this. What are people taking who can’t take HRT?”

That was a recent post in our community and the replies came flooding in.

Venlafaxine. Oxybutynin. Acupuncture. HRT. Lifestyle changes. So many people trying different things to manage debilitating hot flushes and night sweats after cancer.

But increasingly, two new names are appearing in these conversations: fezolinetant (Veoza) and elinzanetant (Lynkuet).

These newer non-hormonal medicines have been specifically developed to treat menopausal hot flushes and night sweats. But what are they? How do they work? And, crucially for our community, can you take them after cancer?

I’m joined once again by the wonderful Dr Zoe Schaedel, NHS GP, menopause specialist and sleep expert, to give us a practical roadmap.

We discuss:

  • How fezolinetant and elinzanetant work and why they're different from some of the older non-hormonal options
  • What we currently know about their use following different cancers
  • Side effects, contraindications and the concerns you've perhaps heard about
  • How to actually access them
  • Why access can vary across the NHS
  • What your local formulary and its “traffic light” classification can tell you
  • Whether your GP can prescribe or whether you may need a menopause specialist or oncologist

And for our listeners outside the UK, we also talk about some of the barriers to accessing these medicines elsewhere, including the frustrating issue of insurance coverage in the US.

Way back in Episode 46, I spoke to Professor Waljit Dhillo, one of the researchers behind this new approach to treating hot flushes.

At the time, these medicines felt like an exciting glimpse into the future.

Well, the future is here.

Let's stop the toxic positivity! Just because active cancer treatment may have ended, we don't have to be endlessly positive and grateful for it all. What comes next can be hard.

That's why I invited Jessica Zucker, clinical psychologist, author of the bestselling book ‘Normalize It: Upending the Silence, Stigma, and Shame that Pervade Women's Lives’ and breast cancer survivor, to talk about the long silence that often follows women through miscarriage, breast cancer, tamoxifen and medical menopause.

Jessica and I talk about what it means to live with the aftermath of treatment, when the symptoms linger, the uncertainty stays, and the pressure to keep smiling can feel almost unbearable.

We also discuss the hidden toll of tamoxifen, the emotional impact of medical menopause, and why phrases like “you’re a warrior” or “at least” can sometimes do more harm than good.

Jessica was so open about how even she, as a psychologist, found the experience of tamoxifen and long-term side effects surprising and difficult.

We talk about exhaustion, brain fog, loss of creativity, grief, perfectionism and the illusion of control, and why so much of this still goes unsaid.

Episode 231 - Anal Cancer, Menopause & Breaking the Silence19 Aug 202600:49:03

Welcome to the first guest episode of Navigating Menopause After Cancer.

In the last two episodes, I shared why the podcast has entered a new chapter and reflected on the power of stories: how they help us navigate uncertainty, create connection and remind us that we're never alone.

Today's conversation is a really beautiful example of exactly that.

I'm joined by Michelle and Becky, two incredible women diagnosed with anal cancer back in 2024.

I learned on a previous episode on anal cancer (episode 209) that anal cancer is often surrounded by stigma, shame, embarrassment and silence. It's a cancer that simply isn't talked about enough. And treatment for anal cancer can also induce menopause - so we have to talk about it!

Yet despite receiving the same diagnosis at almost the same time, Michelle and Becky's experiences were very different.

Michelle openly shared her diagnosis but struggled to find someone else living with anal cancer who truly understood what she was going through.

Becky found support from others with anal cancer from the very beginning, but didn't feel comfortable telling many of the people around her she had anal cancer, choosing instead to say she had bowel cancer.

Together, they share an honest conversation about diagnosis, treatment, menopause, intimacy, identity, stigma and why sharing our true stories matters.

One of my favourite parts of this story actually happened away from the recording. After meeting Michelle in person, I introduced her to Ali who had previously shared her own anal cancer story on this podcast... and something rather special has grown from that connection.

In this special solo episode, I reveal the new name and look of the podcast and, more importantly, the reason behind it.

Your feedback made it very clear that the conversations, the experts, the survivor stories and the honesty are exactly what make this community so special. You asked me NOT to make any changes!

But, after reading an amazing book and a visit to the British Museum that followed, I knew I wanted to make a change regardless.

I have goosebumps writing this: I was standing in front of one of the oldest surviving stories ever written!! I mean - how cool is that! And so of course, I found myself reflecting on the power of stories to help us make sense of uncertainty. It reminded me that none of us simply "gets through" menopause after cancer.

We navigate it.

Together.

That's why this podcast has a new name, one that reflects the journey so many of us are on every single day. And of course it also reflects my book title ‘Navigating Menopause After Cancer’.

Episode 228 - 53 Kilometres, A Sex Toy & Lots of Laughter29 Jul 202600:44:48

This week's episode is a little different.

A few weeks ago, a group of us took on a 53km challenge across the Peak District to raise money for Menopause and Cancer. Somewhere between the hills, the aching feet, the snack stops and plenty of laughter, I had this feeling that I wanted to ‘bottle up’ some of the conversations and the energy we had - to bring it to you.

So, instead of an expert interview, I thought I'd invite you to come along for the post-walk chat with us.

You'll hear honest conversations, a few giggles, reflections on why community matters so much after cancer...and yes, the story of why I ended up feeling rather guilty about keeping hold of one of the prizes that had been donated for our walkers. Let's just say...it involved a rather unexpected gift from our lovely Dr Angela Wright!

This episode is a reminder that healing doesn't only happen in clinic rooms. Sometimes it happens when you're walking alongside people who simply understand.

Within just a year of being diagnosed with breast cancer, lawyer mum and so-called ‘midlife-hipster’ George made the deeply personal decision to restart HRT.

This episode isn't about advocating for or against HRT after breast cancer, and it certainly isn't about telling anyone what they should or shouldn't do. Instead, it's about understanding one woman's journey and exploring the thoughts, emotions and experiences that shaped her decision.

This conversation also goes far beyond HRT. George speaks beautifully about something I know many women will recognise: feeling invisible in midlife. Long before she made the decision to restart HRT, she had already begun finding ways to reconnect with herself. We talk about "Sparkly Fridays", bright lipstick, embracing colour, finding moments of joy and slowly rediscovering her confidence. It's a wonderful reminder that getting your mojo back isn't always about one big decision; sometimes it's the many little things that help you feel like yourself again.

When I first spoke to George, she was right in the messy middle. She had restarted HRT but, for the first time, found herself feeling anxious about that decision. I know so many people in our community are asking similar questions, often without straightforward answers.

My hope is that today's conversation helps us take some of the emotion out of these discussions and instead become a little more curious about one another. We don't all have to make the same decisions, but we can learn so much by listening without judgement.

Heart disease is one of the biggest long-term health concerns facing cancer survivors after menopause. But before you panic, there is a lot more within your control than you might think. Just recently, I’ve seen several social media posts talking about how our risk of heart disease increases during menopause, especially if you’re not taking HRT.

I totally understand how, for many cancer survivors, that can feel so frightening.

After all, what do you do if HRT isn’t an option, or if the decision is far more complicated because of your cancer history? Or because you’re super young, maybe?

That’s exactly why I wanted to bring in Dr Jordan Emont. Dr Emont is a board-certified OB-GYN with advanced training in menopause medicine, integrative hormonal care and sexual health.

Episode 223 - Four Years, Hundreds of Conversations and One Big Question: What's Next?24 Jun 202600:28:43

Four years ago, I sat down and recorded the very first episode of The Menopause and Cancer Podcast.

At the time, I had no idea what I was doing. I didn't know whether anyone would listen, whether I'd make it past a handful of episodes, or that four years later I'd still be here, talking to you every single week.

What began as a simple idea, to create the menopause resource I desperately wished I'd had after cancer, has grown into something far bigger than I ever imagined.

Over the last four years, we've shared hundreds of conversations with incredible experts, survivors, partners, advocates and healthcare professionals. Together we've explored symptoms, treatments, relationships, intimacy, identity, mental health, grief, hope and everything in between.

The podcast has opened doors I never expected. It has led to friendships, opportunities, a bestselling book, healthcare professional training, events, retreats and the growth of Menopause and Cancer into a global movement.

But perhaps the greatest gift has been you: your emails, messages and stories.

Hosting this podcast has been one of the greatest privileges of my life. And the truth is, I think the podcast has changed me just as much as it has helped others.

Over the last four years, life has changed, too.

My daughters have grown up. I've become a founder, an author and somehow a CEO. I've gained confidence in some areas and lost it in others. I've learned new skills, made mistakes, changed my mind, needed reading glasses and realised that growth rarely happens in a straight line.

Recently, I've been reflecting a lot on identity.

For years, I was a yoga teacher.

Then an advocate.

Then a founder.

Now a CEO.

And yet, if I'm honest, I don't always feel like any of those labels fully fit anymore.

Maybe you've felt that too? Are we constantly in transition?

Perhaps that's why this moment feels important. Not because we've reached four years, but because it feels like the beginning of a new chapter.

I've recently started a new project called Still Becoming: a kind of blog, and in many ways, this is how it all started for me. It's a space where I can share some of the conversations that don't always fit neatly into a Menopause and Cancer podcast episode: reflections on identity, motherhood, relationships, intimacy, purpose, building a charity and navigating all the messy, beautiful parts of being human.

And because this next chapter isn't just about me, I'd love your help.

Who controls what happens between women’s legs after menopause?

It’s a confronting question, but one that sits at the heart of this episode.

In this short, powerful conversation, I am joined by menopause specialist and sexologist Dr Angela Wright to explore why so many women — particularly those who have had cancer — are still being denied access to vaginal and vulval oestrogen treatments. And why we’re not routinely having the conversation about sexual health after cancer treatment.

Despite being widely recognised as one of the most effective and low-risk treatments for genitourinary symptoms of menopause (GSM), access remains incredibly low. Many women are left navigating pain, dryness, recurrent infections, loss of sexual function, and a deep impact on identity and relationships — often without being offered treatment. Or they’re told ‘no’.

This episode explores:

  • Why are women not allowed to make an informed choice about their intimate health?
  • The realities of genitourinary syndrome of menopause (GSM) — and why it’s more than “just dryness”
  • The added complexity for women on treatments like aromatase inhibitors (e.g. Letrozole)
  • The emotional and relational impact of untreated symptoms
  • How history, culture, and medical caution continue to shape women’s access to care
  • The growing body of guidance suggesting low-dose vaginal hormones may be appropriate for many women, including after breast cancer
  • The balance between clinical responsibility and patient autonomy

It’s an invitation — to think, to question, and to open up more honest, informed conversations between patients and healthcare professionals.

In this very special community conversation, I sit down with a group of incredible women from our Menopause and Cancer community to explore something so many of us struggle with after cancer treatment… identity. I’m so grateful the group allowed me to get this conversation out to you! Lucky us.

Who are we now?

Together, we talk openly about the physical and emotional changes that can leave us feeling disconnected from ourselves — from body image and confidence, to work, relationships, grief, loss, purpose and rebuilding.

Rather than bringing in an expert, I thought it might be best to just have an honest conversation, share experiences and learn from one another. Because the wisdom in our community is WONDERFUL!

What became clear throughout this discussion is that so many of us are living in what I call “the messy middle” — no longer who we were before cancer, but not yet fully arrived in who we are becoming.

I actually asked in our Facebook community if anyone’s partners would be willing to come on the podcast… and these two were put forward by their wives — earning themselves serious brownie points in the process.

This is a really honest conversation about what it feels like to be on the other side.

We talk about the overwhelm, the uncertainty, and that feeling of wanting to help but not always knowing how. From the emotional impact to the mood changes, this episode sheds light on a perspective we don’t hear enough about.

In this episode, I’m joined by the incredible Lisa, founder of Make 2nds Count and The House of Hope charity in Edinburgh, who is living with secondary breast cancer while continuing to support so many others.

This is an honest, heartfelt conversation about what it really means to live with ongoing cancer treatment. I ask Lisa if she feels excluded from other menopause conversations and even from the many cancer chats out there.

We talk about saying it as it is, the emotional reality of menopause and secondary cancer, and the strategies that help day-to-day, alongside the strength, purpose and perspective that can still exist within it all.

In this episode, I’m joined by Lindsey Lester, a British Menopause Society-accredited specialist and breast cancer survivor, for an essential conversation on bone health after cancer and menopause.

Bone health is one of the most common — and least talked about — long-term effects of cancer treatment. Whether your menopause was triggered suddenly by treatment or has developed over time, the impact on your bones can be significant.

We explore how treatments such as chemotherapy, endocrine therapies (including tamoxifen and aromatase inhibitors), steroids, and treatment-induced menopause can all affect bone density — across different types of cancer, not just breast cancer.

We also talk about who may be most at risk, how quickly bone density can change, and why bone loss is often called a “silent” condition — with many people not realising until much later.

A key part of this conversation is understanding DEXA scans — when to have one, how often to monitor your bone health, and what it really means if you’re told you have osteopenia or osteoporosis.

But most importantly, this episode is about what you can do.

From nutrition and movement to medications and monitoring, Lindsey shares clear, practical ways to protect your bones — and answers one of the biggest questions so many people have after cancer:

In this honest and judgment-free conversation, Dani is joined by cancer survivors Emily and Anita to explore their relationship with alcohol after cancer and during menopause.

Together, they talk openly about questioning drinking habits, attempts to moderate or stop altogether, the social shifts that come with drinking less, and the impact on identity and relationships.

We know that no amount of alcohol is completely safe. Large studies show that alcohol increases the risk of at least seven different cancers, and it can also worsen menopause symptoms such as hot flushes, poor sleep, and anxiety. But this episode isn’t about shame or blame.

It’s an invitation to reflect — gently and without judgement — on your own relationship with alcohol.

Episode 211- The PIONEER Trial: Rethinking Hormones After Breast Cancer?01 Apr 202600:42:52

In this episode of The Menopause and Cancer Podcast, I’m joined by Dr Susanna Unsworth, GP and menopause specialist at the Cambridge breast unit, to talk about why menopause after cancer can sometimes feel like such a complex and at times controversial area of medicine.

We explore why medical opinions can differ, why this space is not always black and white, and what happens when new research starts to challenge long-held assumptions.

A big focus of this conversation is the PIONEER trial, a Cambridge-led study looking at whether megestrol acetate — a drug that acts on the progesterone receptor — could be used alongside standard anti-oestrogen treatment in oestrogen receptor-positive breast cancer and how it may also help with hot flushes!

It’s a bit mind-blowing when you think that all we hear all the time is NO to any HORMONES after breast cancer!

So we talk about why that finding has caught so much attention.

We also talk about the very real emotional burden of endocrine therapy, the impact on quality of life, and why so many patients feel stuck between wanting to reduce recurrence risk and wanting to feel more like themselves again.

Susanna also shares how the Cambridge breast unit is developing its menopause service, including group education sessions to help prepare patients earlier and improve access to support.

This is a really thoughtful conversation about evidence, uncertainty, quality of life, and why people affected by cancer deserve honest, nuanced menopause care.

Today, we’re exploring the evidence with medical acupuncturist Maria Vega.

Many cancer survivors experience menopause symptoms such as hot flushes, joint pain, insomnia and anxiety — often while navigating complex treatment plans or endocrine therapy. For some, HRT may not be an option, which leads many people to explore complementary approaches like acupuncture.

On this episode of The Menopause and Cancer Podcast, we’re marking Anal Cancer Awareness Day (21 March) by shining a light on a cancer that is often misunderstood, stigmatised, and rarely talked about.

Anal cancer is caused by HPV, just like cervical cancer, yet it doesn’t receive the same awareness or open conversation. It might not feel as “pink” or as socially comfortable to talk about as breast cancer. It’s not a cancer that gets glossy campaigns or big awareness drives. There’s often stigma. There’s embarrassment. There’s silence.

And that silence can leave people feeling incredibly alone.

I’m joined by Ali, who shares her honest experience of being diagnosed with anal cancer; what treatment involved, the physical and emotional impact, and how it affected her confidence, menopause and sexual wellbeing.

We also explore an important question: can treatment for anal cancer induce menopause?

Pelvic radiotherapy and chemotherapy can affect ovarian function, sometimes triggering sudden or early menopause. We discuss what that means, and whether HRT or local oestrogen may be options.

One of the hardest things about fatigue after cancer is not knowing what’s causing it. Is it the lasting impact of cancer treatment? Is it menopause? Is it hormone therapy — or all of it together? For many people, it’s impossible to tease apart, and that uncertainty alone can be exhausting. And please don’t tell me to ‘do less’ to feel better!

Fatigue is one of the most common and most misunderstood long-term effects of cancer and its treatment. Many people are told it’s “normal”, something they just have to live with. But for those experiencing it, this exhaustion can be overwhelming, persistent, and life-limiting.

In this episode, Dani is joined by Dr. Jessa Landman, a leading naturopathic doctor specialising in integrative oncology. Together they unpack what cancer-related fatigue really is — and how menopause can intensify it further.

Together, they explore why this fatigue is not the same as everyday tiredness, why rest alone often doesn’t help, and how hormonal changes, endocrine therapy, disrupted sleep, emotional load and identity loss can all layer on top of each other.

I’m joined by oncologist Claire Macaulay, a breast oncologist whose work focuses on what happens when patients fall off what she powerfully describes as the “treatment conveyor belt.” We’re also joined by a whole group of people from our community who will inform how this session goes.

Holly shares her lived experience of navigating menopause after cancer while facing complex and ongoing treatment decisions about her own care — a powerful reminder that menopause after cancer is rarely linear.

Instead, it’s a wriggly road. One that often feels like an uphill hike, with twists, setbacks, and moments where you’re not quite sure which way to turn next.

Being diagnosed with cancer is life-changing. But being told you have to stop HRT too? It’s a double whammy that can leave you feeling lost, frightened, and unsure where to turn.

In this episode, I speak with Sarah and Laura - two women who’ve lived through that moment. Laura is six months on, and Sarah is a year into life without HRT. They share their feelings on what it feels like, how they’ve coped, and what they’ve learned along the way.

This conversation isn’t about what’s right or wrong - it is my hope that you will be able to reflect a little on what has happed to you, or learn how a loved one or patient may experience this double whammy of a situation.

HRT can be transformative, even life-saving. So when it’s suddenly taken away, the grief and worry are real. With over 2.3 million women diagnosed with breast cancer worldwide every year, thousands will face this same painful dilemma. And currently, there is ZERO support!

Many people in our community say it has become virtually impossible to shift the weight since cancer and menopause. So of course we ask ourselves: What role do GLP-1 medicines play in cancer survivorship and what do we really know about their safety, risks, and benefits? 

In this episode, I’m joined by Caroline Tweedie, currently working as the lead cancer nurse in the North East of England. With over 17 years’ experience as a Clinical Nurse Breast Specialist, Caroline has dedicated her career to improving the lives of those affected by breast cancer and sits on the NICE committee shaping national breast cancer guidance.

We explore, with curiosity - and never with judgement:

- What GLP-1 medicines are and why they are so effective

- Safety concerns and what the evidence shows for cancer survivors

- Why these drugs matter in breast cancer survivorship

- The role of GLP-1s in weight, muscle, bone health, inflammation, and fatigue
