43 REBROADCAST: You've Been Rucking Your Whole Life19 Aug 202600:18:32

In this episode of Mornings with Megan, rucking in perimenopause takes center stage: what it actually is, why it matters for hormones, muscle, and bone health, and how to start this week. I'm bringing back one of our most-loved episodes as I take some downtime before fall content kicks off.

Rucking is simply walking with a weighted pack, a term borrowed from the military. But as I explain in this episode, women have quietly been doing this our whole lives, carrying kids, groceries, water, laundry, without ever getting credit for it as exercise. I break down why declining estrogen and testosterone make load-bearing movement essential, not optional, at this stage of life; the Harvard hotel maid study proving that mindset alone can change body composition; the important distinction between maintaining and building bone density; and why rucking is one of the few cardio formats that doesn't spike cortisol.

By the end of this episode, you'll have a simple three-step plan to start rucking this week, and you'll understand exactly how this one practice touches all six pillars of the Perimenopause Matrix™.

42 REBROADCAST: I Tried 5 Diets So You Don't Have To: What Actually Works in Perimenopause12 Aug 202600:16:59

I tried five different diets over the years, made just about every mistake you can make, and I'm sharing all of it in this episode: vegetarian, paleo, keto, a candida protocol, and a short-term bulking phase. What worked, what backfired, and what I finally landed on.

This week I'm bringing back one of the most loved episodes in the Mornings with Megan archive while I take a little downtime before things pick back up this fall. It's a conversation about perimenopause diet and nutrition that still holds up: how hormones, blood sugar, and gut health all show up on your plate, and why chasing the "perfect" way of eating usually backfires.

In this episode we cover:

  • Why an unplanned vegetarian phase left me exhausted and under-fueled
  • What actually changed when I tried paleo, and why it wasn't the food elimination I thought it was
  • How years on hormonal birth control set up a gallbladder issue that keto made worse
  • What a candida protocol taught me about gut health and carb tolerance
  • Why a short-term bulking experiment reshaped how I think about calories and body size
  • What I eat now, and how comprehensive lab work replaced the guesswork

If you're dealing with energy crashes, bloating, stubborn weight, brain fog, or hormone chaos in perimenopause or midlife, this episode is for you. There is no single perfect diet for hormone health or menopause nutrition, there is only the one that fits your unique body, and I'll show you how data and functional lab work can help you find it.

33: What Actually Moves Your Cholesterol (And Why It's Probably Not Saturated Fat) Part 210 Jun 202600:20:05

Last week I told you that the 6% saturated fat guideline is not what it appears to be. This week we get into the part that actually matters: what does the research say really moves your cholesterol numbers?

This episode lives in the Fuel pillar of the Perimenopause Matrix. Fuel is about how your body uses energy, and for a lot of women in perimenopause, the lipid picture is one of the first places the Fuel pillar starts to signal.

I put every major lever side by side with real numbers: saturated fat restriction, psyllium fiber, oat beta-glucan, and statins. I walk through the two key trials in detail, including one where the same dietary change caused a 54-point LDL drop in some people and a 30-point rise in others. I explain why LDL-C and ApoB do not always tell the same story, and why that gap matters for 20 to 30% of women. And I share my own experiment, including the part where doing everything right made my LDL go up by over 30 points.

What you will learn:

  • The LDL-C versus ApoB distinction and why it changes how you read your lipid panel
  • How the RISSCI-1 and GET-READI trials actually performed, including the findings that rarely get mentioned
  • How fiber, exercise, and saturated fat restriction compare head to head
  • What the Fuel pillar framework tells us about lipid management in perimenopause
  • How to run a clean personal experiment to find out how your body actually responds

This is Part 2 of a two-part series. Listen to Episode 32 first.

32: I Tried to Eat 6% of My Diet From Saturated Fat for a Week and This Is What Happened Part 103 Jun 202600:19:06

I failed before lunch on day one. And I did not eat a single thing on the American Heart Association's high-saturated-fat restricted list.

This is Part 1 of a two-part series sitting in the Fuel pillar of the Perimenopause Matrix. Fuel is about how your body uses energy, and for a lot of women in perimenopause, the lipid picture is one of the first places the Fuel pillar starts to signal.

In this episode I walk through why the 6% saturated fat target is harder to hit than anyone tells you, where that number actually came from, and why almost all of the research behind it was conducted in men. I break down the AHA's own math using their own recommended foods, look at the two key trials that came closest to the target, and share the data on who was actually in the foundational research behind these guidelines.

👉 Full Episode Show Notes

What you will learn:

  • The difference between LDL-c and ApoB, in plain language, and why it matters for the 20% of women where they diverge
  • Why the 2013 AHA guidelines acknowledge in their own rationale that the studies behind the 6% target did not isolate saturated fat as a variable
  • How olive oil and walnuts can blow your daily budget before your first meal is finished
  • What the RISSCI-1 and GET-READI trials actually showed, including the findings that rarely get cited
  • Why the Women's Health Initiative, the largest dietary trial ever conducted in postmenopausal women, moved LDL by 3.55 points over 8 years with no significant cardiovascular effect

This is Part 1 of a two-part series. Part 2 drops next week with the full data comparison and my personal experiment results.

31: The Conversation Your Doctor Never Had with You | Dr. Sarah Berg, OB-GYN27 May 202600:49:57

This is the conversation most women never get in a doctor's office.

I sat down with Dr. Sarah Berg, a board-certified OB-GYN and Certified Menopause Practitioner, to talk about what is actually happening in your body during the perimenopause transition, why your doctor may not have had the tools to help you even when they wanted to, and what changed when the FDA removed the black box warning from hormone therapy in November 2025.

In this episode we cover:

  • The slope shoulder moment: what Dr. Berg saw happening to her patients, and why it stopped her in her tracks
  • Why estrogen is not just a reproductive hormone, and how it affects your brain, joints, and genitourinary system
  • Why most OB-GYNs received fewer than four lectures on menopause during training, and what that means for your care
  • The Women's Health Initiative study: what it actually showed, and why the headlines got it so wrong
  • The difference between vaginal estrogen and systemic hormone therapy, and why Dr. Berg says vaginal estrogen is for every woman
  • The estrogen patch shortage: why it is happening, and what it says about how little the medical system has invested in women's health
  • Three things every woman navigating perimenopause needs to know right now

This episode connects to the Regulate and Connect pillars of the Perimenopause Matrix. Understanding the biology behind your symptoms is the foundation of breaking the shame spiral and getting the care you deserve.

30: IT Just Got a Whole Lot Better - Vaginal Estrogen, the FDA, and What Changes Now Part 3 20 May 202600:20:22

Episode 30: IT Just Got a Whole Lot Better -- Vaginal Estrogen, the FDA, and What Changes Now

This is the finale of my three-part series on sexual health in perimenopause, and the episode I have been most excited to record.

We are going deep on vaginal estrogen: what it is, what it does, the history that kept millions of women away from it, what changed in November 2025, and what is coming next with the FDA push to make it available over the counter.

What we cover:

  • The anatomy of the vulva and vagina, why both are estrogen-sensitive, and why knowing the correct vocabulary matters for your healthcare conversations
  • What vaginal discharge and cervical mucus are, why they are normal, and why your vagina does not need internal cleansing products
  • Why scented vaginal products are a problem, not a solution
  • What Genitourinary Syndrome of Menopause (GSM) is and why it is progressive if not addressed
  • Genitourinary Syndrome of Lactation and why postpartum people may also benefit from this conversation
  • Two client stories that illustrate very different entry points into the vaginal estrogen conversation
  • How vaginal estrogen works, what forms it comes in, and why it does not affect blood estrogen levels
  • Why local vaginal estrogen reduces UTI risk by more than 50 percent
  • The history of the FDA black box warning and why it was removed in November 2025
  • Who can use vaginal estrogen, including breast cancer survivors, and how to have the conversation with your provider
  • My personal story with vaginal estrogen and how quietly GSM can develop without you noticing

This episode lives in the Connect pillar of the Perimenopause Matrix.

The FDA removed the black box warning from low-dose vaginal estrogen in November 2025. The research is clear. The tools exist. You deserve a provider who knows about them.

29: Now Let's Do Something About IT - Your Sexual Health Toolkit for Perimenopause Part 213 May 202600:14:32

This is part two of my three-part series on sexual health in perimenopause, and this week we are getting practical.

Part one was about understanding what is happening in your body. This episode is the toolkit. We cover everything from solo sex as a health practice to the lubrication conversation most women have never had clearly enough, and everything in between.

What we cover:

  • Why solo sex is a body literacy tool and one of the most useful diagnostic practices in perimenopause
  • What chore play is and why the nervous system needs to feel safe and supported before it can access desire
  • The myth of spontaneous sex and why scheduling intimacy is more intimate than leaving it to chance
  • The 0-10 rating scale as a simple, pressure-free communication framework for desire
  • What osmolality is and why popular lubricants like KY Jelly and Astroglide may be making vaginal dryness worse
  • Which lubricants actually work with your tissue rather than against it
  • How toys can support exploration and partnered connection in midlife
  • A clear framework for knowing when to address changes yourself and when to involve your provider

This episode lives in the Connect pillar of the Perimenopause Matrix.

Your sex life does not have to wind down in perimenopause. Your body is changing. That is not the same thing as it being over. This episode gives you the tools to work with it.

Part 1: Let's Talk About IT, Baby is out now.Part 3: IT Just Got a Whole Lot Better drops next week.

28: Let's Talk About IT, Baby - What's Actually Happening in Your Body Part 106 May 202600:14:46

This is part one of a three-part series on sexual health in perimenopause, and it is one of the most important conversations we are going to have on this show.

I open with my own story: twenty years on birth control with essentially no libido, the practitioners who missed it, and the one sentence from a nutrition counselor that changed everything. From there we go deep on the hormonal biology of desire and what perimenopause does to it.

What we cover:

  • Why progesterone is often the first hormone to decline and how that drives up anxiety, disrupts sleep, and elevates cortisol
  • Why chronically elevated cortisol is one of the most powerful suppressors of sexual desire, especially in women
  • The full hormonal arc of desire across the cycle and what happens when perimenopause disrupts it
  • What Genitourinary Syndrome of Menopause (GSM) is, how it affects vaginal tissue, and why you should not wait until sex is painful to address it
  • The invisible load of midlife and how it compounds hormonal changes
  • The cultural erasure of women in midlife and what it does to desire and self-worth
  • The research connecting sexual health to cardiovascular health and longevity

This episode lives in the Connect pillar of the Perimenopause Matrix.

Female Sexual Dysfunction affects up to 75% of women between 40 and 50. That is not a coincidence. That is perimenopause. And most of it is addressable.

Fourth Trimester x Mornings with Megan: Postpartum Recovery After 3829 Apr 202600:41:25

Postpartum after 38 is metabolically different. Here's why, and what to do about it.

If your body isn't bouncing back after having a baby the way you expected, you're not imagining it. Recovery is genuinely harder in your late 30s and 40s, and for a growing number of women, the reason is the overlap between postpartum recovery and early perimenopause.

I joined Sarah Trott on the Fourth Trimester Podcast to talk about the physiology behind this, the labs that get missed during the six to 24 month postpartum window, and the practical interventions that help.

We cover the Fuel, Move, Recharge, and Regulate pillars of the Perimenopause Matrix and dig into:

  • Why insulin resistance increases as estrogen and progesterone decline, and how that affects postpartum recovery
  • How to tell whether your symptoms are typical postpartum recovery or early perimenopause
  • What MHT is and why the conversation looks different when you're also breastfeeding or trying to conceive
  • The testing gap: why fasting insulin shows metabolic changes up to 10 years before fasting glucose moves
  • What elevated triglycerides are signaling about your carbohydrate tolerance
  • The cortisol-blood sugar connection and what it means for sleep-deprived new mothers
  • Protein targets, plate building, and nutrient-dense food strategies for postpartum recovery
  • Strength training, pelvic floor timing, and how muscle is your best longevity investment
  • Circadian light, morning sunlight, and the 10-minute daily regulation habit that supports cortisol and melatonin balance

This episode is for you if you've had a baby after 38, if you're navigating both fertility and perimenopause, or if you're supporting women in this unique life overlap.

26: I Spent a Year Looking for a Doctor Who Would Believe Me. This Is What I Found.22 Apr 202600:09:20

This episode is personal.

I'm currently navigating an IVF cycle while in perimenopause, and finding support for both at the same time has been one of the most exhausting and illuminating experiences of my health journey. I spent a full year searching for a doctor in New York City who would hold both pieces. Every single one told me they couldn't help. They did fertility, or pregnancy, or perimenopause. No one did the crossover.

I share the moment one doctor finally looked me in the eyes and said "I believe you," why I ended up in bodybuilding forums researching my own hormone protocol, and what happened most recently with my IVF cycle that made me want to hit record immediately.

In this episode:

  • Why fertility and perimenopause absolutely coexist, and why medicine has cognitive dissonance about it
  • The real data on birth rates in women over 35 and 40 (it will surprise you)
  • What it took for me to advocate for hormonal support while navigating IVF
  • The reproductive endocrinologist training gap that reveals exactly how broken this system is
  • Why the labor of becoming your own citizen scientist is not a personal failing; it is a system failing

25: Is It Willpower or Is It Biology? What Protein Actually Does in a Perimenopause Body15 Apr 202600:14:10

Is it willpower or is it biology? What protein actually does in a perimenopause body.

Protein is having a moment. Again. But what does the research actually say, and why does it matter more in perimenopause than at almost any other time in your life?

In this episode of Mornings with Megan, I break down the protein conversation through the lens of the Perimenopause Matrix, specifically the FUEL pillar and how it connects to blood sugar regulation, muscle preservation, and hormone health.

In this episode:

  • Why perimenopause increases insulin resistance, even in women eating healthy diets, and what the SWAN study tells us about this
  • How protein supports blood sugar stability and stimulates natural GLP-1 production in your body
  • The difference between complete and incomplete proteins, and why serving size tells the real story
  • What the research says about protein in ultra-processed foods (yes, we talk about the protein PopTart)
  • How much protein to aim for, where to start, and why 90 grams is my clinical entry point
  • What women on GLP-1 medications need to know about lean mass loss and why protein is non-negotiable
  • The ethics and access conversation that gets consistently left out of the protein debate

I also share my own history with hypoglycemia and blood sugar chaos at 19, because I think it matters for you to know that I have lived this, not just studied it.

This is not a "eat more chicken" episode. This is the full picture.

24: The Thing Killing as Many Women as Breast Cancer That Nobody Is Talking About. Part 308 Apr 202600:28:39

Your bones are not just a structural frame. They are a living, metabolically active organ system that produces hormones, participates in glucose metabolism, and communicates with your immune system. And in perimenopause, the hormonal shifts that affect your mood, your sleep, and your body composition are also shifting the balance of your bone remodeling.

This is the final episode in the three-part Move pillar series, and it covers: what bone actually is, what happens to it in perimenopause, why osteoporosis and osteopenia are not inevitable, and exactly what the research says you can do about it right now.

This is episode 3 of 3 in the Move pillar of the Perimenopause Matrix, a six-pillar framework for understanding and navigating perimenopause.

41: What Actually Happens in a Pelvic Floor Therapy Session with Marlene Handler Part 205 Aug 202600:21:18

In this episode of Mornings with Megan, we are back with pelvic floor occupational therapist Marlene Handler for Part 2, and we are getting into what actually happens when you walk through the door of a pelvic floor therapy clinic for the first time.

If something from Part 1 landed for you: the leaking, the painful sex, the prolapse, the constipation, the shame of not asking for help sooner. This is the episode that tells you exactly what to do next.

Marlene is the founder of The Lifted Lotus in Williamsburg, Brooklyn. She holds certifications in sex and intimacy and trauma-informed care, and serves on the board of the New York State Occupational Therapy Association, where she helped write the position paper that gave OTs in New York legislative permission to perform pelvic floor rehab. Only two states in the country have that protection in place. She helped get one of them passed.

In the Perimenopause Matrix, the Regulate pillar is about how your nervous system holds tension and stress. The Connect pillar is about intimacy, pleasure, and sexual health. Marlene's trauma-informed approach sits at the intersection of both, and this conversation is the map for anyone who has been putting off the first phone call because they didn't know what to expect.

We cover:

  • How trauma-informed care shapes every moment of a session, from the intake to the assessment, and why safety and consent are built into every step
  • What a first pelvic floor therapy session looks like from start to finish: the 60 to 90-minute intake, the breathing and movement assessments, the external evaluation, and the internal assessment when and if appropriate
  • Why insurance does not reimburse for painful sex or difficulty orgasming, and what to do about it whether you have coverage or not
  • The advocacy work Marlene is doing at the state level, and the fact that only California and New York currently have legislative permission for OTs to perform pelvic floor rehab
  • Three things Marlene wants every person listening to do for their pelvic floor this week, no appointment needed

One of them is just to stop hovering over the toilet. Wipe the seat. Sit down. Let your pelvic floor do its job.

If any of this is landing, you do not have to live with it.

23: The Missing Piece for Women Who Already Work Out: Why Resistance Training Changes Everything in Perimenopause. Part 201 Apr 202600:19:57

If there is one thing I want every perimenopausal woman to do, it is this: lift weights.

Not for aesthetics. For your metabolism, your blood sugar, your bones, your cardiovascular health, and your longevity. The research on this is not ambiguous.

In this episode we cover:

  • Why resistance training is cardiovascular medicine, including the mortality data that should change how you think about strength training
  • The under-muscled woman: what she looks like in labs, why she exists, and how to make sure you don't become her
  • What happens to muscle stem cells when estrogen declines, and why mechanical load is one of the only signals strong enough to compensate
  • The heavy vs. light weight debate: what Dr. Lauren Colenso-Semple's research at McMaster University actually shows, where Dr. Stacy Sims' argument for heavy lifting fits, and how to apply it practically
  • Progressive overload explained simply, including all the levers you can use beyond just adding weight
  • A sex-based physiology note on fatigue and rest between sets that changes how you structure a session
  • How to start from absolute zero, including the three-minute workout that started my own training journey

This is part of a 3 part series on Movement:

Episode 22: Part 1

Episode 23: Part 2

Episode 24: Part 3 Coming April 8th

22: Moving Smarter in Perimenopause: The Full Framework Part 125 Mar 202600:24:24

Part of the Move Pillar of the Perimenopause Matrix

Doing everything right with exercise and still feeling exhausted, not recovering well, or holding weight around your middle?

This one is for you.

This is episode one of a three-part movement series. Today we cover the full cardio framework for perimenopause.

What we cover in this episode:

🧠 The stress cup model: why cortisol dysregulation (both too high AND tapped out) changes everything about how you should be moving

💪 Why your high-intensity class may not be your friend right now, and how to think about it

⏱ Dr. Martin Gibala's published protocols from McMaster University: the SIT protocol (10 minutes, 60 seconds of hard work) and the HIIT 10x1 (20 to 25 minutes total), with equipment options for every level

📚 How Dr. Stacy Sims (Next Level) and Gibala land in the same place from different research angles

❤️ Megan's personal story of cortisol burnout and why she needed someone else to hold her to the protocol she couldn't give herself

🚶 Why walking is genuinely medicinal: the blood sugar evidence, the 2025 Lancet longevity data (7,000 steps, 47% lower mortality risk), and the 2024 Lancet RCT on walking and pain

📊 Your real-world VO2 max test, no lab required

21: You've Been Rucking Your Whole Life18 Mar 202600:17:54

You've probably been rucking your whole life. You just didn't have the word for it.

In this episode, Certified Nutrition Specialist and perimenopause expert Megan Pfiffner breaks down why load-bearing movement - the kind you're already doing with groceries, kids, bags, and stairs - is one of the most powerful tools available for women in perimenopause. This episode focuses on the Move pillar of the Perimenopause Matrix™, and why it's the most underutilized lever for protecting your muscle, bone, blood sugar, and nervous system during this stage of life.

In this episode:

  • What rucking actually is and why women have been doing it for centuries
  • The Harvard study that shows your belief about exercise changes your body
  • Why estrogen and testosterone decline makes load-bearing movement urgent in perimenopause
  • The bone density truth: maintaining vs. improving, and why both rucking and lifting matter
  • Why rucking is one of the only cardio options that doesn't spike cortisol
  • The female endurance advantage under load — and the biology behind it
  • How to start this week with zero equipment
  • How rucking supports all six pillars: Move, Fuel, Recharge, Regulate, Connect, and Clean in the Perimenopause Matrix™

20: Reading Your Body: What a CGM Teaches You About Metabolic Health in Perimenopause11 Mar 202600:20:56

If you've ever crashed at 3pm, woken up at 2am anxious and sweating, or watched your cravings go completely off the rails in the week before your period - your blood sugar is involved. And in perimenopause, managing it gets harder. Not because of anything you're doing wrong. Because of what your hormones are doing.

In this episode I'm walking you through continuous glucose monitoring - what it actually shows you, how to use it as a learning tool rather than a source of anxiety, and what two weeks of my own personal CGM data revealed about food, stress, sleep, movement, and sex.

I also break down HbA1c, why it should never be read as a solo marker for insulin resistance, and how the Fuel pillar of the Perimenopause Matrix connects to every other pillar in the system - Regulate, Recharge, and Move.

19: Your Brain Isn't Broken: What Experts Want You to Know About Memory, Fog, and Perimenopause04 Mar 202600:11:58

Brain fog. Forgetfulness. The terrifying thought that you might be losing your mind. If you've experienced any of this in your late 30s or 40s, this episode is the one I've been wanting to make for a long time.

This week on Mornings with Megan, I'm sharing the story of a client who saw four specialists and had extensive neurological testing — because her symptoms were so significant she genuinely feared she had dementia. She's 39. What she actually has are early perimenopause signs driven by low progesterone, the hormone most responsible for sleep, mood stability, anxiety regulation, and self-regulation.

We also go deep on what is actually happening in the perimenopausal brain — drawing on the neuroimaging research of Dr. Lisa Mosconi (author of The XX Brain) and the mindfulness science of Ellen Langer (author of The Mindful Body) — to offer a reframe that changes everything: your brain isn't declining. It's reorganizing.

In this episode you'll learn why progesterone is usually the first hormone to become unreliable in perimenopause, how the Regulate and Recharge pillars of the Perimenopause Matrix™ are affected, why forgetfulness is often your brain triaging under stress, and three practical tools to help you work with your brain instead of fighting it.

18: You Have More Time Than You Think25 Feb 202600:14:13

"I don't have time" might be the most common thing Megan hears from clients. In this episode, she breaks down why time scarcity is almost never a time problem and how the same tools that power the Perimenopause Matrix can help you find breathing room you didn't know you had.

Drawing on the work of time management expert and NYT bestselling author Laura Vanderkam, Megan covers five strategies for making the most of the time you already have: tracking your time, setting a bedtime as a hormonal health tool, Friday priority planning, exercise snacks, and the universally controversial tip that gets the most pushback from everyone. (It's about hobbies.)

This episode connects to the Recharge, Regulate, Connect, and Move pillars of the Perimenopause Matrix - because for women in perimenopause, chronic depletion isn't just an energy problem. It's a hormonal one. Rest and joy are part of the protocol.

17: What Else You’re Missing: Thyroid, Nutrients & Reading Your Body’s Patterns Part 218 Feb 202600:17:29

What Else You're Missing: Thyroid, Nutrients & Reading Your Body's Patterns | Part 2

Your TSH is "normal." Your CBC is fine. But you're exhausted, your hair is falling out, you can't lose weight, and you can't think straight. What's going on?

In Part 1, we covered the metabolic and cardiovascular markers that shift during perimenopause. In Part 2, we're filling in the rest of the picture - because metabolic health doesn't exist in isolation. Your thyroid function, your nutrient status, and your inflammatory markers all work together. And when one piece is off, everything gets harder.

IN THIS EPISODE:

00:00Understanding Perimenopause and Thyroid Health02:48The Importance of Nutritional Status05:49Common Symptoms and Lab Testing08:11Interpreting Lab Results and Patterns10:40Proactive Health Management12:57Taking Action for Better Health

KEY TAKEAWAYS:

✓ TSH between 1.0-2.0 is the functional target - many women feel terrible at "normal" levels✓ Thyroid antibodies change your entire support strategy even when TSH is normal✓ Normal CBC doesn't mean adequate iron - ferritin is the most sensitive marker✓ Optimal ferritin is 45-79 ng/mL (Weatherby) - not the 15-20 conventional ranges flag✓ Ferritin, thyroid, and vitamin D show up in every single symptom cluster✓ These basics are your starting point before advanced functional testing✓ Prevention from a strong foundation beats starting treatment depleted

16: Why 'Normal' Labs Don't Mean You're Fine: Metabolic Health in Perimenopause Part 111 Feb 202600:20:01

Why 'Normal' Labs Don't Mean You're Fine: Metabolic Health in Perimenopause | Part 1

You're doing everything "right" but your doctor just mentioned statins. Your energy is terrible but your labs came back "normal." You can't lose weight despite no lifestyle changes. What's actually happening?

In this episode, we break down the metabolic shift that happens during perimenopause - the one nobody warns you about. This isn't about your metabolism slowing down (it doesn't). This is about how estrogen decline changes the way your liver handles cholesterol and glucose, how insulin resistance develops silently, and why women catch up to men in cardiovascular disease risk faster than anyone realizes.

KEY TAKEAWAYS:

✓ Women catch up to men in CVD risk by end of menopause

✓ Fasting insulin is the #1 metabolic marker to request

✓ Your liver changes how it handles cholesterol when estrogen drops

✓ Visceral fat can increase 2-3x with NO lifestyle changes

✓ You're 13x more likely to die of metabolic disease than breast cancer

✓ Prevention beats treatment - catch patterns early

15: Fasting in Perimenopause: When to Try It (And When to Skip It)04 Feb 202600:14:51

Intermittent fasting is everywhere - but is it right for YOUR perimenopause? In this episode, nutritionist Megan Pfiffner breaks down when fasting can be a helpful tool and when it's just adding stress to an already overflowing cup.

KEY TAKEAWAYS:✓ Extended fasting can cause ½-⅔ of weight loss to be muscle (vs. ⅓ with GLP-1s)✓ You need 150-200 MORE calories during your luteal phase✓ Most fasting research is done on men or postmenopausal women✓ Exercise triggers autophagy without the fasting stress✓ Your stress cup matters more than any fasting trend

14: You Are Not Robin Berzin (And That's the Point)28 Jan 202600:10:54

What happens when even healthcare CEOs admit they're still figuring out their own health? In this raw, honest episode, I share why comparing yourself to people 10 years ahead is sabotaging your progress—and what to do instead.

Last week I spent 5 days on LinkedIn and ended up crying into my eggs. My partner had to remind me I wasn't a total failure. Sound familiar?

We're all trapped in the compare-and-despair cycle, measuring our chapter 1 against someone else's chapter 10. But here's the truth: even Robin Berzin, CEO of Parsley Health, is still adjusting her approach. If she's iterating, why are we expecting ourselves to be perfect?

In this episode:→ Why there is no "perfect" in perimenopause (or life)→ How to recognize when you're comparing yourself to people in completely different seasons→ Working with your inner critic ("Monica") instead of fighting her→ How to use the Perimenopause Matrix to meet yourself where you ARE, not where you "should" be→ Permission to take one small step instead of ten giant leaps

40: Your Pelvic Floor Is the Missing Piece in Your Perimenopause Care with Marlene Handler Part 129 Jul 202600:33:02

In this episode of Mornings with Megan, we are talking about one of the most overlooked pieces of perimenopause care: the pelvic floor.

Leaking, painful sex, prolapse, constipation. These are being dismissed in seven-minute appointments across the country. Women are being sent to three different specialists, and told it is just part of getting older. It is not. Most of it is treatable. Much of it is preventable. And there is an entire specialty dedicated to helping you with exactly this.

My guest is Marlene Handler, a licensed pelvic floor occupational therapist and founder of The Lifted Lotus in Williamsburg, Brooklyn. After being dismissed by a well-known OB-GYN in New York City despite being a healthcare provider herself, Marlene went back to school to specialize in pelvic floor rehabilitation. She came out the other side without pelvic pain, without leaking, with a sex life she loves, and she now helps other women do the same.

This is Part 1. In the Perimenopause Matrix, the Regulate pillar is about how your nervous system holds tension and stress in the body. The Connect pillar is about intimacy, pleasure, and sexual health. The pelvic floor sits at the center of both, and this episode is everything I wish someone had told me sooner.

We cover:

  • What the pelvic floor actually is and what it governs across bladder, bowel, sexual health, arousal, and even lymphatic flow
  • Why the OT lens is different from the PT lens, and why the biopsychosocial framework changes the care women receive
  • How perimenopause care has become fragmented across specialists who are not connecting the dots and not referring to pelvic floor therapy
  • Why leaking, painful sex, and prolapse have been normalized for generations and what it costs women who keep grinning and bearing it
  • The shame that keeps women from making that first phone call, and how Marlene uses her own story to meet her clients where they are

Part 2 drops next week. We are getting into what actually happens in a first pelvic floor therapy session, how to navigate insurance, and three things Marlene wants every woman to do for her pelvic floor this week.

If any of this is landing for you, you do not have to live with it.

13: What Your Digestion is Trying to Tell You in Perimenopause21 Jan 202600:16:23

What Your Digestion is Trying to Tell You in Perimenopause

Think your digestive issues are just about your gut? In this episode, we unpack why that gas, bloating, and "I can't digest salad anymore" feeling isn't just about what you eat - it's about HOW you eat and what perimenopause is doing to your stress resilience.

Using the Perimenopause Matrix framework, we explore how the anxiety creep (dysregulated HPA axis) affects every stage of digestion - from your brain's ability to signal saliva production to your stomach's capacity to extract B12. This episode covers the Regulate and Fuel pillars, showing you why you can't optimize one without understanding the other.

In this episode:✓ Why digestion and gut health aren't the same (and why it matters)✓ The Lucy & Ethel chocolate factory effect—your digestive conveyor belt in perimenopause✓ How modern eating habits are fundamentally at odds with digestive function✓ Why vacation is the only time you feel normal (and what that tells you)✓ The interconnected nature of the Perimenopause Matrix pillars✓ Simple practices to support digestive function starting today✓ When digestive enzymes and bitters are actually useful (and when they're band-aids)

"Your digestion in perimenopause isn't faster or slower - it's just unreliable."

12: You're Not Broken: Why New Year's Resolutions Are Designed to Fail14 Jan 202600:14:01

The wellness industry's "New Year, New You" model has a 50% failure rate by February 1st - and that's by design. In this episode, I'm breaking down why the system is rigged against you (especially during perimenopause) and sharing what actually works for sustainable change.

In this episode:→ Why the New Year's resolution model is a profit strategy, not a personal failing

→ How perimenopause depletes your bandwidth (and why January 1st pressure is cruel)

→ The research behind "fresh starts" (you have 52 this year, not just one)

→ My 3-minute workout story that changed everything

→ Identity, environment, and support strategies that actually work

→ How to protect your bandwidth and create autopilot defaults

This episode explores the Regulate pillar of the Perimenopause Matrix™ - protecting your nervous system and bandwidth is foundational to navigating perimenopause.

11: What to Actually Eat for Gut Health in Perimenopause (Part 2) 07 Jan 202600:15:14

Last week we talked about WHY your gut is changing in perimenopause. This week we're diving into what you can actually DO about it.

In this episode, I'm giving you the practical framework for supporting your gut through perimenopause:

  • The essential role of fiber (and why 25g/day is easier than you think)
  • The 40 plant foods per week framework—includes tea, coffee, spices, herbs, nuts, seeds, veggies, fruits, beans
  • Best gut-supporting foods: beans/legumes, dark leafy greens, polyphenols, fermented foods

10: Why Your Gut Went Rogue in Perimenopause (Part 1)31 Dec 202500:13:30

If you suddenly developed bloating, food sensitivities, or digestive issues in your 40s that you never had before - you're not imagining it. Your gut microbiome is actually changing because of your hormones.

In this episode, I break down the perimenopause-gut connection that nobody's talking about:

  • How estrogen and progesterone manage your gut (and what happens when they drop)
  • The estrobolome: bacteria that metabolize and recycle estrogen
  • Why gut and vaginal microbiomes are connected (the gut-vagina axis)
  • The real reason UTIs, yeast infections, and BV became more common
  • How gut health affects hot flashes, mood, sleep, and insulin resistance
  • Why over-sanitizing is destroying your protective bacteria
  • The critical connection between oral health and gut health in perimenopause

This is Part 1 of our gut health series. Next week we dive into the practical protocol - what to eat, fermented foods, the 40 plant foods framework, probiotics, and my exact antibiotic protocol.

Your gut isn't broken. Your perimenopause isn't in your head. Understanding what's happening is the first step.

If hosting the holidays suddenly feels overwhelming in perimenopause - even though you've done this a hundred times - this episode explains why.

When your hormones step down, your stress response gets heightened. Things that didn't used to be stressful suddenly become very stressful. What the Scale Isn't Telling You: Strength Training with Noelle McKenzie (Part 2)

Part of the Move and Regulate Pillars of the Perimenopause Matrix™

If you have ever felt frustrated that the number on your scale will not move, even though your pants fit differently and you feel stronger, this episode is going to change how you think about that number completely.

This is part two of my conversation with Noelle McKenzie, co-founder of Leading Edge Personal Trainers. We get into research out of the SWAN study, the largest long-term study on women in midlife, and why insulin resistance rises through the menopause transition independent of age or weight. Noelle breaks down exactly how building muscle helps your body handle that shift.

We get into:

  • Why hot flashes are linked to rising insulin resistance, independent of weight
  • How muscle acts as your body's own defense system for blood sugar
  • Why the scale can stay the same or go up while body composition improves
  • What training for the life you want, not the body you want to see actually means
  • Why runners and cardio lovers need strength training for bone density
  • How to know the difference between a normal off day and a sign something needs attention

This episode is part of the Move and Regulate pillars of the Perimenopause Matrix, a six-pillar framework for understanding and navigating perimenopause.

If you have not listened to part one yet, start there first. It covers muscle as a metabolic organ, why bulking up overnight is basically impossible, and how progressive overload actually works.

1: The BEST Diet for Perimenopause 05 Nov 202500:15:57

What's the "perfect" diet for perimenopause? Spoiler alert: it doesn't exist - at least not in the way you think. In this episode, Megan breaks down why the diet that works for your body might be completely different from what works for anyone else's, even during the same life stage.

Drawing on insights from the landmark SWAN study and her years of clinical experience, Megan explores how hormonal changes during perimenopause affect insulin sensitivity, metabolism, and how your body processes different foods. She shares her own journey through the "nutritionist gone wild" phase of trying every diet under the sun - paleo, vegetarian, keto, low-carb - only to discover that the real answer lies in listening to your own body's feedback.

Learn about:

  • Why insulin resistance increases during perimenopause (independent of age)
  • How continuous glucose monitors (CGMs) can reveal surprising truths about your unique food responses
  • The difference between "just getting by" and actually thriving
  • Why the diet wars (high protein vs. low protein, high fat vs. low fat) miss the point entirely
  • Practical ways to tune into your body's signals about energy, sleep, digestion, and mental health

If you're experiencing brain fog, fatigue, anxiety, or metabolic changes during perimenopause, this episode offers a refreshingly personalized approach to finding what actually works for you.

Part of the Move Pillar of the Perimenopause Matrix™

If you have ever been afraid that lifting weights will make you bulky, or you have been doing the same workout for years without seeing changes in your strength, this episode is for you.

In this episode, I sit down with Noelle McKenzie, co-founder of Leading Edge Personal Trainers, to talk about strength training in perimenopause and midlife. This is part one of a two-part conversation covering why muscle is called the longevity organ, why you cannot accidentally build significant muscle, how to know when it is time to increase your weight, and why fueling your body matters as much as the workout itself.

We get into:

  • Why muscle is the longevity organ, and what it does for metabolic health, blood sugar, and bone density
  • Why bulking up overnight is simply not possible
  • The exact signs that tell you it is time to increase your weight or reps
  • What a mesocycle is and how long to stick with the same program
  • The real difference between getting stronger and building muscle
  • Why appetite alone is not a reliable guide for how much to eat

In this episode of Mornings with Megan, we are getting into the 2 a.m. wakeup and what is actually driving the sleep disruption so many women experience in perimenopause and midlife.

This is part two of my conversation with Morgana Lakatos-Hayward, certified perimenopause health coach, bestselling author, and founder of This Isn't a Diet. If you haven't listened to Episode 36 yet, start there.

What we cover:

  • Why sleep belongs at the top of the priority list, above nutrition and exercise
  • The cortisol and blood sugar mechanism behind 2 a.m. wakeups, and why it is so often mistaken for anxiety
  • What deep sleep is doing for the brain, hormones, and metabolism, and why protecting it is non-negotiable

36: Why Eating Less Is Making It Harder to Lose Weight Part 101 Jul 202600:18:06

In this episode of Mornings with Megan, we are talking about one of the most common and frustrating experiences in perimenopause: doing everything right and still watching your body change.

I sat down with Morgana Lakatos-Hayward, certified perimenopause health coach, bestselling author, and founder of This Isn't a Diet, to dig into the real biology of why midlife weight loss is so different, and why the thing most of us reach for first, eating less, is often making it harder.

What we cover:

  • Why eat less, move more was designed for male biology and backfires for women in midlife
  • The diet culture loop and why women keep cycling through the next thing
  • What restriction actually does to the female body hormonally
  • The three main drivers of stubborn belly fat in perimenopause: insulin, estrogen fluctuation, and chronic high cortisol
  • Why nervous system regulation belongs in the weight loss conversation
  • Practical tools for vagal tone, boundaries, and getting the body into a safety response

35: Put the List Down for a Second: A Summer Permission Slip for the Woman Who Keeps Forgetting to Put Herself On It24 Jun 202600:09:50

This one is a solo episode, and it is a little different.

No research roundup. No guest. Just me, pulling up a chair and actually seeing you for a minute before the summer disappears.

Before we fly through the next three months, I want to ask you something: what have you done this year that you feel good about? Not what's left on the list. What's already here.

In this episode we cover:

  • Why summer is genuinely hard for women in perimenopause, from navigating food sensitivities at someone else's cookout to the very real shift in how our bodies handle alcohol
  • Why your body is not broken. Perimenopause changes the way you metabolize alcohol, respond to sugar, and handle certain foods. That is information, not failure.
  • The permission slip you didn't know you needed: to say no, leave early, bring your own food, skip the wine, and feel whatever you are actually feeling this summer
  • The Recharge pillar of the Perimenopause Matrix in its simplest form: five minutes that belong to no one else
  • Why you are lovable right now, not once you fix the sleep, nail the protein, figure out the hormones, or get it all together

But before we get into the how, we need to talk about the story we have been told about menopause. In Japan, it is considered a renewal. In India, social liberation. In China, a second spring. In the West, we got loss. A systematic review of 13 studies found that women with negative attitudes toward menopause actually experience more symptoms during perimenopause. The story you carry matters, and we are rewriting it.

Then we get practical. This episode covers the Recharge pillar (sleep) and the Fuel pillar (blood sugar): why they are the most foundational pieces of the Matrix, how they feed each other when they are going wrong, and what one small change this week actually looks like.

What we cover:

  • Why your cultural narrative about menopause affects your symptom experience, and the research behind it
  • The six pillars of the Perimenopause Matrix and why Recharge and Fuel come first
  • The metabolic chain reaction that starts with one bad night of sleep
  • How estrogen fluctuations in perimenopause change how your body handles food and blood sugar
  • How to set a bedtime (yes, a bedtime) and use it as a hormonal health tool
  • What a protein and fiber breakfast actually looks like in real life
