| Innovating Community Fitness: Building Bridges from Research to Post-Stroke Rehab | 11 Feb 2024 | 01:03:54 | |
Chime In, Send Us a Text Message! Episode 67 - Innovating Community Fitness: Building Bridges from Research to Post-Stroke Rehab. With our continued theme of exercise as the best medicine for stroke recovery, in this episode we are diving deeper into this topic to find solutions for expanding the reach of rehab into the community and highlighting champions innovating for more solutions to make these new rehab guidelines a reality. Meet Our Guest: Dr Rachel Young PhD MSc, BSc Rachel is a senior research fellow at the Advanced Wellbeing Research Centre in Sheffield Hallam University. She is a chartered physiotherapist with expertise in neurological rehabilitation and exercise prescription. Rachel's research interests include the development and evaluation of rehabilitation technologies and accessible exercise solutions. In our discussion Dr. Young reminded us that the NICE guidelines of three hours are not the first to recommend extensive rehabilitation. The National Clinical Guidance for Stroke launched in March 2023 actually advised six hours of activity a day, which may include activities of daily living, communication and exercise. We dove into why these guidelines sent a ripple through the stroke community, including how providers don’t have the workforce to meet these new recommendations. We also discussed her research and how she believes technology, in the form of accessible exercise equipment, apps and wearables has to be part of the solution if we’re to have any hope of meeting the guidelines. Chime In, Send Us a Text Message! Episode 62: Justin Oakley, MSOTR/L CAPS of Oakley Home AccessPreparing your home for all of life's changes after a stroke can be overwhelming. In fact, the word discharge can be terrifying for both survivor, caregiver and treating therapists alike to provide access to continued safe mobility solutions. We’ve heard many times from past expert rehabilitation guests on our podcast that they predict ‘the future of stroke recovery will be in the home’. But that home must be safe and accessible first. So, how do we make it the best place to be? For the stroke survivor, this is not a nicety, it is a necessity and adapting for safe home access is one of the largest unseen issues in stroke recovery. In today’s episode we wanted to dive into this topic and offer solutions with a home access expert.[Did you know? Currently, according to AARP statistics, less than 5% of all US homes have features needed to support a safe and accessible (aging in place) home even though up to 90% of older adults surveyed want to stay in their home as long as possible.] About Our Episode 62 GuestWe welcomed our expert guest Justin Oakley, OTR/L Co-founder and Chief Development Officer at Oakley Home Access. Justin is a registered & licensed Occupational Therapist as well an Executive Certified Home Modification Provider. Oakley Home Access can assist a wide array of community members ranging from stroke survivors needing home access accommodations, children with special needs to the elderly striving to age-in-place. Oakley Home Access will provide free home safety assessments and full service home modifications, ranging from removing area rugs to providing a variety of adaptive equipment to assist mobility throughout the home, including complete remodels. Justin and his co-founding partner Mike Vieira (experienced contractor + CAPS certified) provide dual expertise (clinical + construction) that is unmatched and provides a unique educational piece that has become their secret sauce to success and foundation for plans for expansion. In this episode we explore how their education, empathy and energy are the key ingredients that keep their team’s core values aligned to help customers realize their goals for continued safe access and independence in their home. It is important for us to emphasize this interview would not have been possible without the great coordinated cooperation from Gina Cella Senior VP of Strategic Connections and Ann Mond Johnson CEO of the ATA, so we want to thank them both here as well along with all the great work their team does every day to support this common mission of advancing telehealth. We’ve learned lessons through the pandemic of telehealth’s ability to bridge care gaps, improve care coordination and fill healthcare workforce shortages. With all that now known, virtual care delivery could still be in danger of heading over the “telehealth cliff” without permanent policy changes and action as Kyle explains it well in this episode. Mike and I both believe strongly the impact of policy changes will be a critical piece in shaping future delivery of rehabilitative stroke care and recovery into the home with improved outcomes. So let’s dive in, listen and learn more from Kyle! Meet Our Guest Kyle Zebley is Vice President of Public Policy at the American Telemedicine Association (ATA) and Executive Director of ATA Action. | Restoring Arm Mobility with Spinal Cord Stimulation | 02 May 2023 | 00:51:29
Chime In, Send Us a Text Message! In Episode 51, we were joined by Marc Powell to speak about the ground-breaking neuro-technology he and his team at Reach Neuro are working on to restore arm and hand movement in patients who survived severe strokes.Globally, every fourth adult over the age of 25 will suffer a stroke in their lifetime, and 75% of those people will have lasting deficits in motor control of their arm and hand, severely limiting their physical autonomy. In April 2023, the FDA granted breakthrough device designation for Reach Neuro’s Avantis, a platform that delivers small electrical impulses to the spinal cord to restore shoulder, arm, and hand movement to post chronic stoke patients. The Avantis system is built to restore a patient's ability to control movement directly, giving them not only immediate relief but also allowing it to be possible for them to return to therapy. Early evidence has shown the effectiveness of the system in patients with chronic stroke up to 9 years poststroke, outside of the 6 month window where existing therapies are typically used.Connect with Marc on Twitter: @marcppowellVisit the Reach Neuro Website: https://www.reachneuro.com/Show Credits:Music intro credit to Jake Dansereau, connect at JAKEEZo on Soundcloud @user-257386777.Our intro welcome is the voice of Caroline Goggin, a stroke survivor and our first podcast guest! | Stroke Has No Age Limit | 07 Mar 2023 | 00:52:25
Chime In, Send Us a Text Message! In 2020, William Kyler Lewis, 15, was riding in the car with his mom in College Station, Texas, when he suddenly turned to her confused. His head hurt, he was sick to his stomach and his vision was blurred. Soon, he couldn’t remember anything for more than a few seconds. After his mom rushed him to the pediatrician, he got a shot and was told to go home. Instead, his mom drove 1½ hours to a children’s hospital in Houston. Nurses there thought Kyler had taken drugs, but his mom insisted no, he was not that type of kid, and something was very wrong. A CT scan revealed Kyler had an acute spontaneous intracranial hemorrhage from a brain arteriovenous malformation (AVM) that he unknowingly had when he was born. A brain AVM is a condition that is not inherited. AVMs are a random occurrence due to a gene malfunction in utero where a mass of arteries connect directly to veins.Kyler received lifesaving external ventricular drain (EVD) surgery to control the fluid build-up in his brain. After the stroke, memory and neuro fatigue became a big problem. Kyler has undergone radiation and chemotherapy and takes medication for epilepsy, headaches, dizziness, weakness and tremors.Yet, Kyler is inspiring many. He uses local news, community functions and social media to educate the public that a child can have a stroke, too. He also organizes walks to highlight the importance of recognizing the symptoms of a pediatric stroke.Kyler’s Facebook group, “A Cure for Kyler,” his CaringBridge page and YouTube channel educate and inspire pediatric stroke survivors by documenting his recovery. His video, “Being You after an Aneurysm/AVM Rupture,” and his family’s Facebook page, “AVM Alliance: A Cure for Kyler and Friends,” share stories of other hemorrhagic stroke survivors and educate people about symptoms and conditions.Links:Cxnadvisory.comhttps://www.f3metro.com/Freed to Lead CONTACT INFO:https://www.linkedin.com/in/tim-whitmire-7955145/Twitter: @trwhitmire_OBTShow Credits:Music intro credit to Jake Dansereau, connect at JAKEEZo on Soundcloud @user-257386777.Our intro welcome is the voice of Caroline Goggin, a stroke survivor and our first podcast guest! | A global community of Angels improving stroke patients’ chances of survival and a disability-free life. | 06 Feb 2023 | 00:50:46
Chime In, Send Us a Text Message! Episode 44: In this episode we sit down with Kim Malkin, Angels Initiative Lead for Australia to discuss the work she and her team are doing across the region to improve stroke care delivery. The Angels Initiative was launched in 2016 by Boehringer Ingelheim in partnership with the European Stroke Organisation (ESO), the World Stroke Organisation (WSO), the Stroke Alliance for Europe (SAFE), and many other national stroke societies, companies and health institutions. Their mission is aimed at reducing the burden of stroke for countless patients by working with hospitals, stroke teams, societies and individuals to build an innovative network of stroke-ready hospitals worldwide to reduce treatment delays and provide patients with the optimal standard of acute stroke care.About our guest:A move to Australia in 2004 provided not only more sunshine for Kim but also the opportunity to begin a long and diverse career with Boehringer Ingelheim – over a decade of which is in improving outcomes for stroke patients through the delivery of innovative preventative and acute stroke treatments. The company’s mission to improve the health and wellbeing of people and animals resonated strongly but more than that it is Boehringer Ingelheim’s inclusive culture and social responsibility which provides a source of great pride and motivation to Kim every day.In Australia, more than half of eligible stroke patients miss out on treatments that reduce disability and death. Although a developed country, Australia faces significant challenges when it comes to providing equity of access to optimal stroke care - particularly the tyranny of distance. Australia is a vast country, the sixth largest in the world, with a total area of 7,686,850 square kilometres and a population of over 25 million. Two-thirds of Australia’s population reside within metropolitan areas however the remaining live in regional or remote areas with poorer access to healthcare services.Leading the Angels Initiative in Australia, Kim is committed to establishing Angels as a sustainable model of quality improvement. With her dedicated team of Angels Consultants and over a decade of working closely with the Australian stroke community, they aim to achieve this through close collaboration with the many stakeholders across State and National health systems who are passionate about creating a better future for Australian stroke patients and their families.Show Resources:https://www.boehringer-ingelheim.com/cardiovascular/stroke-care/angels-initiativehttps://www.angels-initiative.com/https://fastheroes.com/Show Credits:Music intro crSupport Our Show! | Delivering stroke care on the roads and in the sky across Australia | 28 Jan 2023 | 00:56:23
Chime In, Send Us a Text Message! Episode 43: Interview with Professor Stephen Davis & Geoffrey Donnan of the Australian Stroke Alliance. The Australian Stroke Alliance is comprised of more than 30 organisations working together to transform stroke care across Australia and improve the lives of those affected by stroke. They are developing novel mobile imaging, digital health and emergency transport technologies to radically transform stroke outcomes and deliver commercial benefits for all Australians. Rather than transporting patients to hospital, they drive or fly Australian-designed life-saving brain scanning equipment to a patient during the Golden Hour for Stroke, transforming their chances of survival and recovery. For Australians living in rural, remote and Indigenous communities, access to stroke treatment during the Golden Hour for Stroke is limited or non-existent – creating inequity and a major gap in health outcomes. The Australian Stroke Alliance is about equity, world-first innovation and bringing time-critical stroke treatment direct to the patient.About our guests:PROFESSOR GEOFFREY DONNAN AOGeoffrey is a professor of Neurology at The University of Melbourne and former Director of The Florey Institute of Neuroscience and Mental Health. | "Houston, we have a problem" - Interview with world renowned stroke expert, Pamela Duncan PhD. | 16 Dec 2022 | 00:52:55
Chime In, Send Us a Text Message! Episode 41 In our final episode of 2022 we had the pleasure to be joined by Pamela Duncan. Pam is a leading expert in stroke, neurologic recovery, aging and patient reported outcome measures and widely recognized for her efforts to reengineer post-acute care services and recovery following stroke. She develops consumer-oriented assessments to improve patient outcomes by guiding real-time clinical decision-making and recommendations for self-management, medical and social services. The Compass CP platform was created based on insight gained from the COMPASS study, led by Wake Forest University School of Medicine and The University of North Carolina and conducted in 40 hospitals and with community partners across North Carolina. The study was funded through a $14 million, five-year award from the Patient-Centered Outcomes Research Institute (PCORI), an independent, nonprofit research funding organization authorized by Congress in 2010. COMPASS compared the health status of stroke patients who receive conventional post-hospitalization treatment to that of patients who receive comprehensive care based on a model developed by a team of physicians, nurses, therapists, pharmacists, health system and human services leaders, and patient and caregiver stakeholders. Because of her outstanding contributions to the investigation, management, mentorship and community service in the stroke field, Duncan is a 2020 recipient of the American Stroke Association’s David G. Sherman Lifetime Achievement Award. She is the first woman to ever be awarded this honor. Show Credits: Music intro credit to Jake Dansereau, connect at JAKEEZo on Soundcloud @user-257386777. | Interview with Kyle Zebley, Senior Vice President of Public Policy at the American Telemedicine Association and Executive Director, ATA Action | 15 Oct 2022 | 00:55:54
Chime In, Send Us a Text Message! Episode 36 Interview with Kyle Zebley, Senior Vice President of Public Policy at the American Telemedicine Association and Executive Director, ATA Action In this episode you’ll be hearing from the first policy expert we’ve had on our show and we don’t think we could have brought you a better one! For those not familiar with the American Telemedicine Association…The ATA is the only organization completely focused on advancing telehealth, and is committed to ensuring that everyone has access to safe, affordable, and appropriate care when and where they need it, enabling a wider reach and more access to care for more people. Kyle’s BioSketch Previously, Kyle was the Chief of Staff in the Office of Global Affairs (OGA) at the U.S. Department of Health and Human Services (HHS). He collaborated with senior leadership from HHS, the White House and other cabinet departments to develop, advise, and promote U.S. global health policy, including in such policy areas as drug pricing, medical devices, global health security, and non-communicable diseases. Prior to HHS, he worked in Congress as a Legislative Director, leading a legislative team in developing policy and drafting legislation, particularly on matters concerning the House Committee on Ways and Means, the House Committee on the Budget and the House Committee on Education and the Workforce. Kyle started his career in Washington, D.C.where he worked o Support Our Show! Show credits:

| Interview with Ralph Preston Founder of Stroke Buddies | 20 Aug 2022 | 00:50:22 | |
Chime In, Send Us a Text Message! Know Stroke Podcast Episode 31Interview with Ralph Preston Founder of Stroke BuddiesAfter a stroke, Ralph Preston leveraged over 40 years of professional video experience to continue to do good with his camera. He “turned the lens on himself “ and is developing high-quality videos for stroke survivors and for local non-profits. About our Guest:Preston is the founder of Stroke Buddies: By and For Stroke Survivors, a Facebook group for stroke survivors that currently has 8,000 members and is growing every day. Ralph built this community from the ground up so survivors can seek answers, learn with educational resources, and receive support. Last year, he launched a weekly virtual stroke survivor support group meeting in association with MedRhythms and is assembling a team of medical professionals, PTs, OTs, physicians, neurologists, and neuroscientists who have suffered stroke themselves and are interested in sharing their stories and knowledge at these support group meetings. Ralph also started a discussion show that is shared with the Stroke Buddies community and is called Stroke Roadmap. The purpose is to help those who are transitioning their recovery from PT to home therapy. Additionally, Ralph develops and shares dozens of home therapy exercise videos from his YouTube channel to improve at-home recovery. This year Stroke Buddies was granted non-profit status, so now he is off to raise money and see if he can fundamentally change stroke recovery.Show Resources:Stroke Buddies Group on FacebookRalph Preston Videos on YouTubeShow Credits:Music Credit and Podcast Production by Jake Dansereau, connect at JAKEEZo on Soundcloud @user-257386777Be sure to give the show a like and share, +follow and connect with us on social or contact us to be a guest on the Know Stroke Podcasthttps://www.know-stroke.orgSupport Our Show! | Interview with Dr. Heidi Schambra Director of Research Strategy in Neurology and Mobilis Lab NYU Langone | 04 Aug 2022 | 01:00:25
Chime In, Send Us a Text Message! Know Stroke Podcast Episode 30Stroke commonly damages motor function in the upper extremity (UE), leading to long-term disability and loss of independence in a majority of individuals. Rehabilitation seeks to restore function by training daily activities, which deliver repeated UE functional motions. The optimal number of functional motions necessary to boost recovery is unknown. This gap stems from the lack of measurement tools to feasibly count functional motions. Today's guest is Dr. Heidi Schambra from NYU Langone. She and her team at the Mobilis Lab, out of NYU Langone developed the PrimSeq pipeline to enable the accurate and rapid counting of building-block functional motions, called primitives. PrimSeq uses wearable sensors to capture rich motion information from the upper body, and custom-built algorithms to detect and count functional primitives in this motion data. They showed that their deep learning algorithm precisely counts functional primitives performed by stroke patients and outperformed other benchmark algorithms. The study also showed patients tolerated the wearable sensors and that the approach is 366 times faster at counting primitives than humans. PrimSeq provides a precise and practical means of quantifying functional primitives, which promises to advance stroke research and clinical care and to improve the outcomes of individuals with stroke. In our discussion we covered how this tool can translate into rehab happening in the clinical setting, a patient's home, and how insurers think about reimbursing stroke rehab. About our guest:Dr. Heidi Schambra is an Associate Professor of Neurology and Rehabilitation Medicine, Director of the Division of Neuro-Epidemiology, Director of Research Strategy in Neurology, and Director of the Mobilis Lab. Dr. Schambra received her BS in neuroscience from Brown University and MD from Emory University. She completed her training in neurology at Harvard-Partners and in neurorehabilitation at Burke Rehabilitation Hospital. She also completed a postdoctoral fellowship in motor learning and noninvasive brain stimulation with Dr. Leonardo Cohen at NINDS/NIH. Dr. Schambra was on faculty at Columbia University until 2016, when she joined NYU Langone. When not in the lab, she can be found taxonomizing clouds, cute-aggressing her pets and husband, and menacing New York on a Citibike.Watch this Episode on YouTube: https://youtu.be/PUGDDVrxjGgIn the news:Frequent napping may be a sign of higher risks of stroke, high blood pressureViz.ai, Hyperfine team up to add image-reading AI to bedside MRI scannersShow Credits:Music Show InSupport Our Show! Chime In, Send Us a Text Message! Season 3 Episode 6 -On The Road! Before we get to our "Stroke in the News" this week we would be remiss not to pause and reflect that this episode is being recorded asynchronously amidst the backdrop of war in Ukraine. As we all finally sense an end to the COVID-19 crisis, we find ourselves in the midst of another one, and the extent of which is still unclear. It is hard to put into words what it means to watch the invasion of a sovereign nation, and honestly we’ve all struggled with the immediate human costs and certainly more lives will be senselessly lost. Also realizing the potential implications of the Russian invasion of Ukraine, we have found hope in the strength of the Ukrainian people and we stand with them in support of democracy.
