S3 Ep143: Who Do You Believe When Experts Disagree? with Tamsen Fadal

Saison 3 · Épisode 143

jeudi 29 août 2024Durée 49:11

In a world where women are barraged with information when it comes to how best to manage menopause, it can be overwhelming and confusing. Between having their symptoms dismissed by their regular physician, and then being overwhelmed with information on social media, it’s hard to know what’s fact and hype.  And what do you do when the doctor you trust, tells you that the social media expert you also trust is wrong?
In this episode, I chat with Tamsen Fadal, a menopause advocate who has made it her mission to help perimenopausal and menopausal women navigate through the information and disinformation to get the correct information when it comes to maximizing their health.

  • Tamsen’s own experience when she entered menopause
  • Why Tamsen switched careers to become a menopause advocate
  • How do you know an expert is really an expert?
  • The best way to find a menopause expert
  • The impact of telehealth on finding a menopause expert
  • What to do if your own doctor is not a menopause expert
  • Why women put relatability over expertise
  • Hot to figure out if a social media expert is an expert
  • What to do when a social media expert disagrees with your own doctor
  • Do healthcare clinicians need to act professionally in their private lives?
  • What I think when it comes to being strictly evidence-based

It’s not just about making the midnight run for tampons, it’s that stomach-dropping fear that “blood equals cancer” that causes women to spend hours consulting Dr. Google for some reassurance. But, although most women imagine the worst, in most cases, postmenopausal spotting or bleeding is not, is not, an indication of anything serious.

S3 Ep133: Best Breast Advice When it comes to Hormone Therapy With Dr. Corinne Menn

Saison 3 · Épisode 133

jeudi 20 juin 2024Durée 57:07

There continues to be great confusion regarding the impact of hormone therapy on the breast. And despite the known benefits of hormone therapy, and reassuring data regarding the impact of hormone therapy on breast cancer, most clinicians are still reluctant to prescribe it, and most women are reluctant to take it. Even more so if a woman is high risk for breast cancer or has a breast cancer diagnosis.

In this episode, I am joined by Dr. Corinne Menn, a board-certified Ob GYN and menopause expert who herself had a breast cancer diagnosis when she was in her 20s. We will be reviewing the latest research on the impact of estrogen on the breast in several different scenarios.

  • PREGNANCY post breast cancer
  • Women who have a BREAST CANCER diagnosis
  • Women with a FAMILY HISTORY of breast cancer
  • Women with a BRCA MUTATION after prophylactic ovary removal
  • The approach to therapy for women with a NEW BREAST CANCER DIAGNOSIS
  • The use of local vaginal estrogen in women with a breast cancer diagnosis that are taking TAMOXIFEN or using an AROMOTASE INHIBITOR
  • If a LOCAL VAGINAL ESTROGEN needs to  be used at the lowest dose
  • What to do if your DOCTOR NOT WILLING to prescribe local or systemic estrogen (list of articles to share is below)
  • If a woman with a breast cancer diagnosis can use TESTOSTERONE for libido and other possible benefits

Saison 1 · Épisode 35

It used to be that the term “bioidentical hormone therapy” referred exclusively to compounded hormone products. But today, the term bioidentical includes FDA approved commercial products and compounded products. A legal loophole combined with aggressive marketing has resulted in women believing that compounded hormone creams and pellets not only work better but are safer than commercially produced products.

In this episode, Dr. Streicher discusses:
The following common MISCONCEPTIONS about compounded hormones:
  • Misconception #1 Compounded hormones are NATURAL but all commercial hormones are synthetic or made of horse urine
  • Misconception #2 The estrogen in compounded hormones is DIFFERENT from the estrogen in commercial pharmaceutical products
  • Misconception #3 Compounded hormones are SAFER than commercial hormones
  • Misconception #4 Compounded PROGESTERONE will PROTECT your uterine lining
  • Misconception #5 Compounded hormones undergo the same QUALITY CONTROL as commercial products
  • Misconception #6 Compounded HT is CUSTOMIZED based on your hormone levels
  • Misconception #7 PRESCRIBERS of compounded hormones are hormone experts

S1 Ep31: The Truth About Estrogen Therapy and Breast Cancer

Saison 1 · Épisode 31

mercredi 20 juillet 2022Durée 21:05

The number one reason women give for not taking estrogen is fear of breast cancer. This fear is based on the 2002 study which has been reevaluated and shown to be flawed in a number of ways. Despite solid proof that estrogen therapy does not cause breast cancer and other serious illness, most doctors continue to exaggerate the risks and undersell the benefits of hormone therapy. The goal of this episode is not to convince you to take estrogen. The goal is to present the data so that you can make an informed decision.  In this episode, Dr. Streicher discusses:
  • The original PURPOSE of the 2002 Women’s Health Initiative (WHI) and how the study was designed.
  • The INITIAL RESULTS of the WHI study
  • Why the design of the study was PROBLEMATIC
  • Why the results of the WHI were NOT REPRESENTATIVE of the women who take hormone therapy today
  • The results of the RE-ANALYSIS of the WHI
  • How the age of women in the study, the timing of therapy, and the use of statins altered CARDIOVASCULAR data
  • Why the use of PROGESTIN and the kind of progestin in the study changed the results.
  • Why TRANSDERMAL estrogens have less risk and who should use one
  • The FACTS about the relationship of estrogen and BREAST CANCER
  • The risk of hormone therapy COMPARED to other known risks for breast cancer
  • The medical impact of women NOT taking HT
  • GUIDELINES for taking hormone therapy safely

S1 Ep28: The BEST approach for BREAST Cancer Screening and Risk Reduction with Dr. Lisa Larkin

Saison 1 · Épisode 28

jeudi 30 juin 2022Durée 48:31

It used to be so simple. Women were just advised to get a mammogram every year. But now we have not just x-rays, but ultrasound, MRI, digital breast tomosynthesis, screening mammograms, and diagnostic mammograms. Women are confused, and frankly, a lot of doctors are also confused. I am joined by Dr. Lisa Larkin, a board-certified internist, a certified menopause practitioner of the North American Menopause Society, and a nationally recognized expert in breast cancer risk assessment and prevention.

In this episode, Dr. Streicher and Dr. Larkin discuss:
  • The LIKLIEHOOD of developing breast cancer in different populations
  • WHO is getting MISSED when it comes to early detection
  • Who should have genetic screening
  • The problem with using over-the-counter testing such as 23 and Me
  • When should someone start -and stop- breast cancer screening
  • What is TOMOSYNTHESIS
  • Who should have an MRI
  • Who should have an ULTRASOUND
  • Need to know info on DENSE BREASTS and why you should look at your report
  • If it matters WHERE someone goes to get their mammogram
  • RISK FACTORS for breast cancer that you can’t change
  • Risk Assessment models to PREDICT your lifetime risk of breast cancer
  • What it means when you are called back for ANOTHER VIEW
  • RISK REDUCING drugs many women do not know about
  • MODIFIABLE risk factors for breast cancer
  • Why ESTROGEN is not a risk factor for breast cancer
  • Why a PLANT-BASED DIET is beneficial
  • Dr. Larkin’s new model for patient care, MS. Medicine

S1 Ep23: The Impact of Systemic Racism in Mid-life Women's Health

Saison 1 · Épisode 23

jeudi 16 juin 2022Durée 46:38

Relatively minimal research is devoted to menopause even though menopause affects literally 100% of women that live past the age of 50. For women of color, the situation is far more dire.. Not only do we know less about the impact of medical conditions or therapies in mid-life black women but it is incredibly challenging for many black women to access appropriate care. My guest is Dr. Fenwa Milhouse, a urologist who is not only a talented surgeon, but uses social media to destigmatize pelvic floor & sexual conditions, as well as demonstrate the importance of representation in medicine.
Dr. Streicher and Dr. Milhouse discuss:
  • DISEASES that disproportionately affect Black women
  • How HYSTERECTOMY impacts cardiovascular disease in Black women
  • Why Black women are less likely to take HORMONE THERAPY
  • BLACK CELEBRITIES and increasing awareness of health conditions
  • BARRIERS to health care in Black underserved community
  • Micro and Micro AGRESSIONS in the health care system
  • The reason for and impact of IMPLICIT BIAS
  • The challenges of a BLACK WOMAN UROLOGIST practicing in a white hospital
  • The importance of DIVERSITY EQUITY and INCLUSION initiatives in our health system
  • What impacts racial disparity and potential action steps to improve health in the Black community
  • How SYSTEMIC RACISM impacts on health

Dr Fenwa Famakinwa Milhouse is a fellowship-trained, board certified urologist. Originally born in Nigeria, her family immigrated to the US when she was a young child.
