1. Radical Neck Dissection (RND):

This is the most extensive type of neck dissection and involves the removal of:

• All lymph nodes from Levels I to V.

• The sternocleidomastoid muscle (SCM).

• The internal jugular vein (IJV).

• The spinal accessory nerve (cranial nerve XI).

This procedure is used when cancer has extensively involved these structures, and there is no option for their preservation.

2. Modified Radical Neck Dissection (MRND):

The goal of the modified radical neck dissection is to remove all lymphatic tissue from Levels I-V while preserving one or more of the following non-lymphatic structures that are removed in a radical neck dissection:

• Type I: Spinal accessory nerve preserved.

• Type II: Spinal accessory nerve and internal jugular vein preserved.

• Type III: Spinal accessory nerve, internal jugular vein, and sternocleidomastoid muscle preserved.

This approach is designed to reduce morbidity, particularly related to shoulder dysfunction, while still removing the affected lymph nodes.

3. Selective Neck Dissection (SND):

Selective neck dissection involves the removal of only certain groups of lymph nodes, depending on the location of the primary tumor and the risk of metastasis to specific levels. The following are the most common types of selective neck dissections:

• SND (I-III): Levels I, II, and III lymph nodes are removed (used for oral cavity cancers).

• SND (II-IV): Levels II, III, and IV are removed (used for oropharyngeal, laryngeal, and hypopharyngeal cancers).

• SND (I-IV): Levels I, II, III, and IV lymph nodes are removed.

• SND (Posterior Triangle or Level V): Lymph nodes in Level V are removed (often used for skin cancers or melanoma of the head and neck).

The selective approach is designed to minimize functional and cosmetic morbidity while still achieving oncologic control.

4. Extended Neck Dissection:

This involves removing lymph node groups or structures when cancer extends into areas beyond the typical boundaries of the neck:

• Retropharyngeal lymph nodes.

• Paratracheal nodes.

• Mediastinal nodes.

• Additional non-lymphatic structures like the hypoglossal nerve or vagus nerve, depending on cancer involvement.

A - Acetaminophen (Paracetamol):

1. Safety: Acetaminophen is considered safe for use during breastfeeding. It is one of the most commonly used pain relievers and antipyretics for nursing mothers.

2. Breastmilk Transfer: Acetaminophen passes into breastmilk in small amounts, but the levels are generally too low to cause harm to the infant.

3. Infant Concerns: There are no significant adverse effects reported in infants exposed to acetaminophen through breastmilk. It is typically well-tolerated by infants.

B - Ibuprofen:

1. Safety: Ibuprofen is also considered safe for breastfeeding mothers. It is a nonsteroidal anti-inflammatory drug (NSAID) commonly used for pain, inflammation, and fever.

2. Breastmilk Transfer: Ibuprofen passes into breastmilk in minimal amounts, and its short half-life and low levels in milk make it unlikely to affect the nursing infant.

3. Infant Concerns: No significant adverse effects have been reported in breastfed infants whose mothers take ibuprofen. It is preferred over other NSAIDs like aspirin due to its safety profile during breastfeeding.

C - Contraindications and Considerations:

1. Allergies: If the mother or infant has a known allergy to acetaminophen or ibuprofen, these medications should be avoided.

2. Chronic Use: While occasional use is safe, chronic or high-dose use should be discussed with a healthcare provider to ensure safety.

3. Infant Health Conditions: If the infant has certain health conditions (e.g., liver disease, in the case of acetaminophen), the use of these medications should be discussed with a healthcare provider.

In summary, both acetaminophen and ibuprofen are considered safe for short-term use in breastfeeding mothers, with no significant concerns for the nursing infant. However, it is always recommended to use the lowest effective dose and consult with a healthcare provider if there are any concerns.

• Sella (S): the central point of the Sella turcica, a saddle-shaped depression in the sphenoid bone at the base of the skull.

• Nasion (N): the point located at the intersection of the frontal bone and the nasal bones, at the bridge of the nose. It represents the most anterior point of the frontonasal suture in the midsagittal plane.

• A Point (A): also known as Subspinale, is the deepest concavity on the anterior surface of the maxilla, located between the anterior nasal spine and the alveolar ridge of the upper central incisors. It represents the most anterior point of the maxillary alveolar process.

• B Point (B): also known as Supramentale, is the deepest concavity on the anterior surface of the mandible, located between the infradentale (the tip of the mandibular alveolar process) and the pogonion (the most anterior point on the chin). It represents the most anterior point of the mandibular alveolar process.

SNA (Sella-Nasion-A Point):

• Normal Range: 82° to 84°

• Maxillary Hypoplasia (Retrognathism): SNA < 82°

• Maxillary Hyperplasia (Prognathism): SNA > 84°

SNB (Sella-Nasion-B Point):

• Normal Range: 78° to 80°

• Mandibular Hypoplasia (Retrognathism): SNB < 78°

• Mandibular Hyperplasia (Prognathism): SNB > 80°

Relation to Determining Prognathism vs. Retrognathism:

• Prognathism: A condition where the maxilla or mandible is positioned anteriorly (forward) relative to the cranial base. This is indicated by an SNA > 84° for maxillary prognathism or an SNB > 80° for mandibular prognathism.

• Retrognathism: A condition where the maxilla or mandible is positioned posteriorly (backward) relative to the cranial base. This is indicated by an SNA < 82° for maxillary retrognathism or an SNB < 78° for mandibular retrognathism.

These values are crucial in diagnosing skeletal discrepancies and planning orthodontic or surgical interventions.

The Plexiform Ameloblastoma:

Clinical Features:

• Presentation: Typically presents as a slow-growing, painless swelling in the jaw, most commonly affecting the mandible. It can cause facial asymmetry and, in some cases, displacement or loosening of teeth.

• Age and Gender: Often occurs in the third to fifth decades of life with no significant gender predilection.

Radiographic Features:

• Appearance: Radiographically, it appears as a multilocular or unilocular radiolucency. The borders are usually well-defined, often with a “soap bubble” or “honeycomb” appearance.

• Location: Primarily affects the posterior mandible, but can also be found in the maxilla.

Histopathological Features:

• Plexiform Pattern: Characterized by long, anastomosing strands of odontogenic epithelium within a loosely arranged connective tissue stroma. The epithelium is composed of a peripheral layer of columnar or cuboidal cells that resemble ameloblasts with reversed polarity (nuclei positioned away from the basement membrane).

• Stroma: The connective tissue stroma is typically loose, allowing for the characteristic anastomosing pattern of the epithelial strands.

• Basal Cell Layer: Cells in the basal layer are hyperchromatic and may show subnuclear vacuolization, which is a hallmark of ameloblastomas.

Treatment Options:

• Conservative Treatment: Includes enucleation or curettage. This approach is associated with a higher recurrence rate because the tumor’s plexiform pattern tends to infiltrate surrounding bone.

• Radical Treatment: Resection with wide margins is recommended, particularly for larger lesions or those located in areas where recurrence would be particularly problematic. This approach aims to minimize the risk of recurrence.

Factors for Treatment Selection:

• Size and Location: Larger lesions or those in critical anatomical locations (e.g., maxilla, where resection can lead to significant morbidity) may warrant more aggressive treatment.

• Recurrence Risk: Given the higher recurrence rate with conservative treatment, resection is often preferred, particularly in younger patients or when follow-up may be challenging.
