Why is there a difference between you measured ETCO2 and PaCO2?24 Oct 202100:23:57

In this episode we talk about this very common phenomenon in anaesthesia, the difference between ETCO2 and PaCO2.

Essentially this all comes down to patient and equipment factors.
BUT the essential element that makes the largest difference is DEAD SPACE.

we discuss the factors effecting it, and the Bohr equation that measures dead space.

Effect of standing due to hydrostatic pressure

 Compensation is Global

-       Baroreceptor

o   A reduction in mean arterial pressure is sensed by the baroreceptors. They decrease their rate of firing which results in an increase in SNS discharge and decreased PNS discharge causing:

o   increased HR

o   increased myocardial contractility ® increased CO ® increased MAP

o   peripheral vasoconstriction ® increased TPR ® increased MAP (greater effect on resistance vessels than on capacitance vessels)

o   venoconstriction ® increased venous return ® increased CO

-       Atrial stretch

o   Over an extended period of time, decreased blood volume will be sensed by the cardiopulmonary receptors as decreased stretch and decreased firing of the A and B receptors will stimulate:

o   release of angiotensin, aldosterone and ADH ( Na+/H2O reabsorption, restoration of blood volume)

o   increased SNS activity in the kidney (vasoconstriction, ¯ GFR and ¯ urine flow)

In this episode we cover categorisation of core cardiovascular topics
Here is a brief summary:

Hypotension

Preload
Rate
Rhythm
Contractility
Afterload

Or Shock
Hypovolaemia
Cardiogenic
Distributive
Obstructive

O2 supply demand
Supply
= o2 flux or delivery to heart
CBF x o2 content (Hb, sats, PO2)

CVR and CPP
CVResistance = metabolic, myogenic, neural, physical compression, viscosity, pathology
CPP = Cor Pressure – RAP or LVP.

O2 extraction.
High basal extraction of 55-65%
other tissues only 25%
so O2 demand is met by increasing CBF

Demand
Depends on Metabolic rate and myocardial work
Wall tension – PL and AL
Contractility
HR
Rhythms

Wall thickness
Basal rate
Type of work (pressure > volume work)
Electrical activation

When this is unbalanced = ischaemia

Determinants of coronary vasc resistance

Resistance = pressure difference / flow = corPP/ CBF

= 8nl/Pi R4. (n = eta)

Radius
Neural
Physical – compression, HR,
Pathology
Autoregulation
            Metabolic
            Myogenic
Viscosity

Coronary blood flow

= CPP/CVR

Neural
Physical
-       Compression
HR
Viscosity
Autoreg
-       Metabolic
-       Myogenic: A myogenic response refers to the intrinsic tendency of vascular smooth muscle to contract in response to increased distending pressure and to relax in response to decreased distending pressure.

Live viva exam demo for the ANZCA part 1 exam - Pat v231 Mar 202100:32:46

Time for round 2 with Pat are very generous trainee who's offered to be quizzed on our podcast.

Here are the questions from this round:

Stem 5

1.     How does heparin work?

2.     How about low molecular weight heparin?

3.     Why do we monitor unfractionated heparin but not low molecular weight heparin?
Due to lower binding to plasma proteins and to cell surfaces, the plasma anti Xa activity generated by a given dose of LMWH is more predictable than for UH.

4.     How do we monitor unfractionated heparin’s effect?

5.     How is the clearance of unfractionated heparin different to that of low molecular weight heparin?

Stem 6

1.     What are the main differences between intracellular and extracellular ion concentrations?

2.     What is the function of potassium?

3.     What is the NERST potential?

4.     What are the ECG changes in hyperkalaemia?

5.     What is the treatment for hyperkalaemia?

Stem 7

1.     What are the functions of the placenta?

2.     How is the placenta different to the lung?

3.     How are substances transferred across the placenta?

4.     What are the factors that affect transfer of O2 across the placenta?

5.     What are the factors that affect placental blood flow?

Stem 8

1.     During anaesthesia, you notice a difference between the End Tidal CO2 and Arterial CO2. What can you do to minimise this difference?

2.     How do you measure physiological dead space?

3.     How do you measure anatomical dead space?

4.     What increases anatomical dead space?

5.     What is closing capacity?
